The Effect of Nutrition Education and Iron Supplementation on Iron Status of High School Girls (여고생의 철영양상태 개선을 위한 영양교육과 철보충제 효과 연구)

  • This study was designed to investigate the effects of iron supplementation and nutrition education on the iron status and anemia of high school girls. The subjects resided in Ulsan city in Korea and were already diagnosed as having anemia or iron deficiency. Over a period of three months, one iron tablet (80 mg Fe as ferrous sulfate/day) was administered to the iron deficient subjects and two tablets (160 mg Fe as ferrous sulfate/day) were administered to the anemia subjects. The average height and weight of anemia subjects were 161.24 $\pm$ 4.50 cm and 50.87 $\pm$ 5.86 kg, respectively. The average BMI (kg/$m^2$ )was 19.58 $\pm$ 2.03 and the PIBW(percent ideal body weight) were 92.52 $\pm$ 9.84%. Except for vitamin A and vitamin C intakes, the intake levels of all other nutrients were below the RDA. Total calorie intakes of anemia subjects were 73.5% of RDA. The iron intakes of subjects from food were 69. 1% of RDA and the Ca intakes were 59.1% of RDA. The basal hemoglobin(Hb) concentration of anemia subjects averaged 10.77 $\pm$ 1.33 g/dl, and this increased significantly (p < 0.001) to 12.12 $\pm$ 1.08 g/dl, after iron supplementation. The basal ferritin, and transferrin saturations {TS (%)}of anemia subjects were 12.51 $\pm$ 15.19 ng/$m\ell$ and 8.43 $\pm$ 7.56%, respectively, and these significantly increased to 20.59 $\pm$ 22.39 ng/$m\ell$ and 15.56 $\pm$ 12.87%, respectively. The level of total iron binding protein (TIBC) significantly decreased from the initial 486.80 $\pm$ 70.16 $\mu\textrm{g}$/dl to 417.86 $\pm$ 67.73 $\mu\textrm{g}$/dl (p < 0.001) after iron supplementation. For the iron deficiency subjects, the ferritin, iron and TS(%) levels were increased significantly (p < 0.001) and the TIBC levels were significantly (p <0.001) decreased after iron supplementation. Anemia symptoms such as 'Feeling blue (p<0.05)', 'Decreased ability to concentrate (p<0.001)' and 'Poor memory (p<0.05)' improved significantly after iron supplementation in the anemia subjects. The number of tablets administered was positively correlated with changes in serum hemoglobin (t=0.194, p< 0.01), serum ferritin (t=0.181, p<0.01), TS(%) (t=0.141, p<0.05), and hematocrit (t=0.254, p<0.01), and was negatively correlated with changes in TIBC (t=-0.143. p<0.05) and red cell distribution width (RDW, t=-0.140, p<0.05). In conclusion, daily iron supplementation was effective in improving the iron status and reducing symptoms of anemia in high school girls. (Korean J Nutrition 35 (9) : 943~951,2002)

Factors Associated with Self-Rated Health among Poor Glycemic Control Group with Diabetes Mellitus: The 4th-6th Korea National Health and Nutrition Examination Survey (2007-2015) (당뇨병 유병자 중 혈당 미조절군의 주관적 건강인식 관련 요인: 제4-6기 국민건강영양조사 자료(2007-2015)를 중심으로)

  • Background: This study aimed to properly manage diseases such as blood sugar control so that patients with diabetes can benefit from both medication and health activities. Also, these health practices are greatly influenced by self-rated health, a subjective assessment of health status. Because self-rated health does not necessarily match the objective health status, it is important to identify which factors affect self-rated health. Methods: For the study, the data was gathered from the 4th-6th National Health Nutrition Survey (2007-2015). Out of the total 73,353 participants in the survey, 2,303 patients with uncontrolled blood sugar with an HbA1c level of more than 7% were selected for the final study. Dependent variables fell into two categories depending on how the participant reported whether he or she was in good health or not. Independent variables included socio-demographics, health behavioral, and health status factors. This study performed logistic regression analysis. Results: Out of 2,303 participants, 18.1% reported that their heath was 'good,' despite the fact that their blood sugar level was not controlled. After running a logistic regression model, the odds ratio of groups that perceive subjective health awareness as good was higher in the groups of people as below: in the people over 60 years old; in the people who graduated from a junior college or higher than those who had a level of education of primary school completion or less; in the people living in Chungnam than those living in Seoul; and in the group with hypertriglyceridemia. Conclusion: The study identified factors associated with those failed to perceive the blood sugar level as a severe health problem despite of the fact that blood sugar was not controlled. To improve public health, diabetes management policies need to be addressed to population groups with these problems above.

Effects of ${\beta}-Glucan$ from Agaricus blazei Murill on Blood Glucose and Lipid Composition in db/db Mice (db/db 마우스에서 아가리쿠스 버섯 ${\beta}-Glucan$이 혈당과 지질성분에 미치는 영향)

  • Obesity and diabetes mellitus are associated with common pathogenic mechanism, and ${\beta}-glucan$ of Agaricus blazei Murill is potent inhibitor of intestinal ${\alpha}-glycosidase$ and inhibit the digestion of starch and sucrose in the small intestine. In this studies, there was observed the anti-hyperglycemic effect in obese diabetic mice(C57BLKsJ db/db), which were supplied Agaricus and Acarbose for 5 weeks. In db/db mice, food intake and body weight gain were decreased significantly in Agaricus groups(p<0.05). Also these group exhibited lower fasting serum glucose level compared with control group. HbA1c level, triglyceride level, total cholesterol level, HDL cholesterol level, LDL cholesterol level and VLDL cholesterol level were lowered in db/db mice. The activity of disaccharidases on proximal and distal segments of small intestine was decreased. In conclusion, it was assumed that ${\beta}-glucan$ of Agaricus blazei Murill has anti-hyperglycemic and anti-obesitic effects by reducing food intake and body weight gain, and also decreasing serum glucose and lipid level through inhibiting the activity of small intestinal disaccharidases.

Relationship Survey Study between Diabetic Control Education and Diabetic Retinopathy: data from the Korea National Health and Nutrition Examination Survey V (당뇨병 환자에서 당뇨관리 교육과 당뇨병성 망막병증 발생간 관련성 조사연구: 제5기 국민건강영양조사를 중심으로)

  • Background: Diabetes is accompanied by complications. One of the chronic complications, diabetic retinopathy is the most common cause of the loss of eyesight and thus has enormous impacts on the quality of life to the patients. It has been reported that thorough glucose regulation can prevent or postpone the outset of diabetic retinopathy in diabetic patients and that the patients who received anti-diabetic manage & care education would be capable of more thorough glucose-level regulation than those who did not. Method: This study set out to investigate the current state of education on anti-diabetic manage & care in South Korea and connections between anti-diabetic manage & care education and occurrence of diabetic retinopathy in diabetic patients based on the Korea National Health and Nutrition Examination Survey of 2011. Results: Of the 410 diabetes patients, 74 received anti-diabetic manage & care education, which means that only 15% of diabetic patients benefited from the education in the nation. The occurrence rate of diabetic retinopathy was 28% in the education group and 24% in the non-education group with no significant differences between them. The anti-diabetic manage & care education group recorded a higher occurrence rate of diabetic retinopathy, one of the chronic diabetic complications, than the non-education group contrary to the hypothesis. One of the reasons was that the educated group had a significantly longer duration of diabetes and significantly higher HbA1c than the noneducated group, which indicates that anti-diabetic manage & care education is provided to those who have progressed farther along the course of diabetes instead of the early stage and cannot regulate their glucose-level well in the nation. Conclusion: Those findings raise a need for active educational policies in order to provide anti-diabetic manage & care education under the goals of preventing complications through anti-diabetic education for many patients in early stages of diabetes.

Effect of JiaoTeng-Yuan(交藤圓) on Oxidation Stress Caused by D-galactose in Sprague-Dawley Rats (교등원(交藤圓)이 백서(白鼠)의 산화유발(酸化誘發)을 방어(防禦)하는 작용(作用)에 관(關)한 연구(硏究))

  • Objectives : JianTeng-Yuan(交藤圓) is said to be a prescription for preservation of health in ${\ulcorner}$HuaTuo ZhongZangJing(華陀 中藏經)${\lrcorner}$. It is known to have the effect of Bu-Shen(補腎: strengthening kidney) and Yi-Shou(益壽: prolonging the span of one's life). This study investigates whether JTY is effective on inhibition of oxidation stress. Methods : Sprague-Dawley Rats(12-week-old, weight $300{\pm}20g$) were divided into 3 groups. Normal group(n=8) was injected PBS(1ml/body, s.c) at the back neck's skin. Control group(n=8) was injected D-galactose(50mg/kg, 1ml PBS/body, s.c) to induce pathological animals. JTY group was injected the same treatment for the Control group, and fed containing JTY(10%). The whole groups were treated 1 time per day for 6 weeks. After rats were sacrificed and anti-oxidant enzyme(SOD, CAT, G-px) activity, GSH quantity of RBC and tissue(heart, liver and kidney), plasma Vit-C quantity were examined. Besides, the MDA levels of liver and kidney, lipofuscin of heart and endurance of erythrocyte membrane were measured. Results : In the JTY group, RBC's SOD activity decline was halted by 21% of the normal level, compared to the control group ; G-px activity(unit/g of Hb) increased significantly, compared to the normal group ; and the level of Vit-C in plasma increased by 16%. Heart's SOD activity was kept at the same level as that of the normal group ; and CAT activity decline was halted by 26%. Kidney's CAT and G-px activities were kept at the same level as that shown in the normal group, implying the existence of halting effect. Liver also showed a slight halting effect against the decline of anti-oxidant ability, but the effect was not significant(a=0.05). A comparison between the levels of peroxide in SD rats showed that the level of TBARS in plasma increased significantly in the control group and that it was normal in the JTY group. The livers in the JTY group, compared to those in the control group, showed 36% halting effect of the normal level while their kidney's indicated the level significantly lower than the normal level. Heart's lipofuscin increased significantly in the control group, but was alike in both the JTY and the normal groups. Endurance of erythrocyte membrane(%) decreased significantly in the control group while it was kept at the similar level in both the JTY and the normal groups, indicating the halting effect. Conclusions : This study suggests that JTY is effective to defend oxidation stress caused by D-galactose in the animals. It showed that the anti-oxidant ability was maintained and strengthened. On the other hand, it reduced the level of peroxide in animals. In sum, JTY appeared to have the equilibrium normal physiological function in SD rat.

Consumption of Dairy Foods and Risk of Pre-Diabetes in Subjects that Visited Health Examination Center in Gwangju (광주 지역 건강검진센터를 내원한 수진자의 유제품 섭취와 전당뇨병 위험 인자와의 연관성)

  • The prevalence of diabetes is increasing worldwide, particularly in Asian regions such as Korea. It is estimated that the number of diabetic patients will exceed 5 million by 2030 and that about 30% of people with diabetes are unaware of their condition. A comparative analysis was conducted to determine the relationship between dairy food intake and pre-diabetes risk factors in subjects that visited the health examination center of Sunhan Hospital, Republic of Korea. Subjects were classified into two groups according to fasting blood glucose concentrations: 1) normal (fasting blood glucose <100 mg/dL, n=57) and 2) pre-diabetes (100 mg/dL${\leq}$ fasting blood glucose ${\leq}125mg/dL$, n=56). Total dairy food intake tended to be lower in females and subjects with pre-diabetes compared with males and normal subjects, respectively; however, differences between groups were not significant due to a wide range of variations. HbA1c, systolic and diastolic blood pressures were significantly associated with the prevalence of pre-diabetes. Odds ratios for pre-diabetes were significantly reduced per incremental increases in dairy food intakes after adjusting for confounding factors. Findings in the current study suggest that appropriate intake of dairy foods may need to be considered for the prevention of pre-diabetes.

A Study on the Circadian Blood Pressure Rhythm of Diabetic Patients (당뇨병 환자의 혈압 일주기 리듬에 관한 조사연구)

  • This study was performed to investigate the relationship between reversed circadian blood pressure and risk factors of peripheral vascular disease in non-insulin-dependent diabetes mellitus (NIDDM) subjects. The subjects in this study were 18 NIDDM patients who were hospitalized in a medical unit of an university medical center located in Incheon, Korea, between November, 1998 and March, 1999. Blood pressure was measured with a mercury sphygmomanometer by 2 trained examiners every 2 hours during 24 hours. NIDDM subjects were divided into a dipper group and non-dipper group. Dippers are defined as those who show a mean nighttime blood pressure(BP) drop of more than 10% compared with daytime BP. Non-dippers are defined as those who show a mean nighttime BP drop of less than 10%, or an elevation in BP compared with daytime BP. Daytime BP included values obtained between 6 a.m. and 10 p.m. Night time BP included values obtained between 10 p.m. and 6 a.m. Data was analyzed by SPSS/PC package. Chi-square( $^2$) test was used for the comparison of sex between The dipper group and non-dipper group. Mann-Whitney test was used for comparisons of values of the risk factors of peripheral vascular disease and the frequency of complications of diabetes between the dipper group and non-dipper group. The results are as follows. There were no significant differences in daytime systolic, diastolic, and mean blood pressures between the dipper group and non-dipper group. However, night time systolic, diastolic, and mean blood pressures in the non-dipper group were significantly nigher than those in the dipper group (p=.021). There were no differences in sex, age, body, weight, duration of diabetes, serum lipid levels, BUN and HbA1c between the two groups. On the contrary, 87.5% of non-dipper group subjects showed having hypertension, 30% of dipper group subjects showed having hypertension and this difference was statistically significant (p=.018). All of the non-dipper group subjects (N=8) showed having at least one diabetic complication. However, 40% of the dipper group subjects (N=10) showed having no diabetic complication at all and this difference was also statistically significant (p=.049). There were no significant differences in frequency of nephropathy, neuropathy and retinopathy between the dipper group and non-dipper group.

  • This study investigated the effects of the diet with Cucurbita moschata (CM), adlay seed (AS), and Cudrania tricuspidata leaf (CTL) mixed-powder on the visceral fat, fecal amount, and serum lipid profiles in rats fed a high-fat diet (HFD). The mixed ratio of CM, AS, and CTL as a functional food ingredient was 1:1:1 on a weight basis. Male Sprague-Dawley rats were fed a high-fat diet (D12492; 45 kcal fat) for 5 weeks during the first phase. In the second phase, which lasted 5 weeks, the rats were divided into six experimental groups. The groups were: a high-fat diet group as a control during the 10 weeks of experimental period; and a high-fat diet with 5, 10, 15, 20, and 30% mixed-powder supplement group. The adipose tissue (RFP, EFP) weights were significantly decreased in the 20% and 30% mixed-powder supplement groups than in the HFD group (p<0.05). The fecal weight produced by the 15%, 20%, and 30% mixed-powder supplement groups was significantly increased during the second phase compared to the first phase (p<0.05). In terms of serum lipids, TG (triglyceride) content was decreased in the 10% and 15% mixed-powder supplement groups. The blood glucose levels were significantly decreased in all experimental diet supplement groups than in the HFD group (p<0.05). The findings show that the high supplement groups could improve the visceral fat accumulation, fecal amount, and blood glucose content compared to the HFD group.

The Hypoglycemic Effect of Supungsunkihwan on Impaired Glucose Tolerance & Mild NIDDM Patiens (수풍순기환(搜風順氣丸)이 내당능장애 환자와 경증 당뇨병환자의 혈당조절에 미치는 영향)

  • Purpose : Impaired glucose tolerance (IGT) was standardized in 1979 by the National Diabetes Data Group and the World Health Organization as a risk factor for type 2 diabetes, replacing groups such as 'borderline' and 'chemical' diabetes. The main clinical significance of IGT is as a risk factor for type 2 diabetes and as a risk factor for cardiovascular disease and as a component of the metabolic syndrome. In 1997 the American Diabetes Association(ADA) was proposed the new classification and diagnostic criteria for diabetes, which is strict with the diagnostic baseline of Diabetes from 140mg/dl to 126mg/dl. And it's main purpose is to prevent chronic complications by early diagnosis and treatments. In the oriental medicine, Supungsunkihwan has been used in treatments of Diabetes including IGT & NIDDM, however there is not enough studies about the its objective hypoglycemic effect. so in order to investigate whether there is hypoglycemic effect of Supungsunkihwan, clinical studies were performed with IGT and mild NIDDM patients. Methods : Prior to the study, fasting blood sugar(FBS) and postprandial 2hrs(PP2hrs) glucose were checked. In addition, ECG, cholesterol, TG, HbA1c levels were measured, and the dinical characteristics of patients that can be influence to the blood sugar level such as age, gender, rest type, diet type were surveyed. Supungsukihwan was administered for 7 days, and FBS & PP2hrs were measured again after the therapy. Result : PP2hrs glucose levels were decreased by the administration of Supungsunkihwan. And It's hypoglycemic effects has the correlation with age. That is, the more age increase, the more hypoglycemic effect decrease. Conclusion : Supungsukihwan has hypoglycmic effects on IGT & mild NIDDM patients and it is more effective when administrated to young patients relatively.

Antiglycemic Effect of Carnosine in Diabetic Mice (당뇨 마우스에서 카르노신의 혈당강하 효과)

  • Carnosine is a dipeptide ($\beta$-alanyl-L-histidine) found in mammalian brain, eye, olfactory bulb and skeletal muscle at high concentrations. Its biological functions include antioxidant and anti-glycation activities. The objectives of this study were to investigate anti-diabetic effects of carnosine as determined by blood glucose levels in glucose tolerance test (GTT) and insulin tolerance test (ITT), insulin level and serum biochemical and lipid levels in male C57BL/6J db/db mice. There were five experimental groups including normal (C57BL/6J), control (vehicle), and three groups of carnosine at doses of 6, 30, and 150 mg/kg b.w. Carnosine was orally administered to the diabetic mice everyday for 8 weeks. There was no significant difference in body weight changes in carnosine-treated groups compared to the control. The treatments of carnosine significantly decreased the blood glucose level in the diabetic mice compared with the control (p < 0.05) after 5 weeks. The treatments of carnosine also significantly decreased the blood glucose levels in GTT and ITT and glycosylated hemoglobin (HbA1c), compared with the control (p < 0.05). Carnosine at the dose of 6 mg/kg significantly decreased the serum insulin level compared to the control (p < 0.05). Carnosine significantly increased total proteins but significantly decreased lactate dehydrogenase and blood urea nitrogen compared with the control (p < 0.05). Carnosine also significantly decreased glucose, LDL, and triglyceride in the serum of diabetic mice compared to the control (p < 0.05). These results suggest that carnosine has a hypoglycermic effect resulting from reduction of glucose and lipid levels and that high carnosine-containing diets or drugs may give a benefit for controlling diabetes mellitus in humans.
