Effect of Acupuncture on Regional Cerebral Blood Flow at Acupoints GV 20, GV. 26, LI. 4, ST. 36, SP. 6 Evaluated by Tc-99m ECD Brain SPECT (Tc-99m ECD 뇌혈류 SPECT를 이용한 백회, 인중, 합곡, 족삼리, 삼음교에서 체침의 뇌혈류에 대한 효과)

  • Purpose: To evaluate the effect of acupuncture on regional cerebral blood flow (rCBF) at acupoints suggested by oriental medicine to be related to the treatment of cerebrovascular diseases. Materials and Methods: Rest/acupuncture-stimulation Tc-99m ECD brain SPECT using a same-dose subtraction method was performed on 54 normal volunteers (34 males, 20 females, age range from 18 to 62 years) using six paradigms: acupuncture at acupoints GV. 20, GV. 26, LI. 4, ST. 36 and SP. 6. In the control study, needle location was chosen on a non-meridian focus 1 cm posterior to the right fibular head. All images were spatially normalized, and the differences between rest and acupuncture stimulation were statistically analyzed using SPM$^{(R)}$ for Windows$^{(R)}$. Results: Acupuncture applied at acupoint GV. 20 increased rCBF in both the anterior frontal lobes, the right frontotemporal lobes, and the left anterior temporal lobe and the left cerebellar hemisphere. Acupuncture at GV 26 increased rCBF in the left prefrontal cortex. Acupuncture at LI. 4 increased rCBF in the left prefrontal and both the inferior frontal lobes, and the left anterior temporal lobe and the left cerebellar hemisphere. Acupuncture at ST. 36 increased rCBF in the left anterior temporal lobe, the right inferior frontal lobes, and the left cerebellum. Acupuncture at SP. 6 increased rCBF in the left inferior frontal and anterior temporal lobes. In the control stimulation, no significant rCBF increase was observed. Conclusion: The results demonstrated a correlation between stimulation at each acupoint with increase in rCBF to the corresponding brain areas.

  • We investigated the rate of hepatitis G virus infection among 50 patients who were not infected with the hepatitis C virus but showed symptoms of hepatitis. Viral RNA was extracted from the patients' sera and cDNA was synthesized and amplified by RT-PCR (reverse transcription-polymerase chain reaction) using random hexamer and 5 primers (470-20-1-77F, 470-20-1-211R, 470-20-1-211R-biotin, GV57-4512MF, GV57-4657MR). The amplified PCR products were confirmed by electrochemiluminescence (ECL), liquid hybridization (LH) and Southern blotting (SB). Among the 50 PCR products, by means of ECL, we found 4 samples to be positive and 5 samples to be indeterminate. The GV45-89M probe (5'-CYCGCTGRTITGGGGTGTACfGGAAGGC-3') was end-labelled with gamma-$^{32}P$ ATP and used for liquid hybridization with the PCR products. By using liquid hybridization, we detected specific bands from 4 positive sera and also from one indeterminate serum as determined by ECL. An 1.5% agarose gel electrophoresis of the 9 PCR products which were HGV positive or indeterminate as determined by ECL showed a 160bp band from 4 positive and one indeterminate serum. The 5 PCR products proved to be positive when SB was applied with the GV45-89M probe as well as when LH was applied. LH and SB were shown to have higher sensitivity and specificity than ECL. Two cases among 5 positive cases had relatively high SGOT, SGPT, ALP values when compared with other 48 cases. In summary, we confirmed hepatitis G virus infection in 5 cases among 50 Korean patients showing symptoms of viral hepatitis.

A Review of Clinical Studies for Treatment of Multiple System Atrophy Using the CNKI Database (다계통 위축증의 치료에 대한 임상연구 동향 - CNKI검색을 중심으로)

  • Objectives: The aim of this research was to investigate and review the trends in recent clinical studies of multiple system atrophy (MSA) in China. Methods: We searched for clinical studies discussing Oriental medicine-based treatments for MSA in the China National Knowledge Infrastructure (CNKI) database. The search focused on the authors; publication year; type of study; method and duration of treatment; evaluation criteria; and results of the selected articles. Results: Among the articles retrieved from the database, eight case series, two randomized controlled trials (RCTs), and one non-randomized controlled trial (nRCT) were analyzed. Eight articles used acupuncture; ten used herbal medicine. The most frequently used acupoints were GV20, GV24, GB20, PC6, Hyeopcheok Acupoint, CV4, EX-HN3, and GV17. Jihwangeumja was the most commonly used herbal prescription. All 13 studies confirmed the efficacy of Oriental medicine treatments. Conclusions: More varied and scientifically designed clinical studies are required to develop treatments for MSA. The results of this study may provide guidance for further studies of MSA.

Traditional Korean Medicine Treatment of a Traumatic Brain Injury Patient with Aggressive Behavior (외상성 뇌손상 환자의 공격성에 대한 치료 1례)

  • Objective: To describe the effectiveness of traditional Korean medicine (TKM) on a patient showing aggressive behavior after a traumatic brain injury. Methods: We used complex TKM treatments. The acupoints used were as follows: Soyangin Taegeuk acupuncture (HT3, KI3, and SP3) and acupoints frequently used in hemiparesis (GV20, GV24, ST4, LI11, LI4, ST36, GB34, SP9, and LR3). Moxibustion was also applied to GV20, and herbal medicine (Samulanshin-tang-gami, 四物安神湯 加味方) was administered. The efficacy was evaluated at baseline, week 8, and week 16 using the Modified Overt Aggression Scale (MOAS). Results: After 8 weeks, the total MOAS score was decreased from 13 to 5, and this score was maintained until week 16. The average sleep time was increased and waking up during the night was decreased. Conclusions: We suggest that TKM could be effective for reducing aggressive behavior and alleviating sleep disturbances. Further studies are needed.

A Philological Study on the Pathology, Acupuncture and Moxibustion of Alopecia (탈발(脫髮)의 병인병기(病因病機) 및 침구치료(鍼灸治療)에 관한 문헌고찰)

  • Objectives : The purpose of this study is to analyze literature on oriental medicine to investigate the pathology, acupuncture, and moxibustion therapy applicable to alopecia Methods : We searched for the causes of alopecia in the bibliographic data from 41 sources of literature and the acupunctural treatments for alopecia in the bibliographic data from 49 sources of Chinese and Korean literature and from the journals of the acupunctural treatments for alopecia which have been published since 2000. Results : 1. The main causes of alopecia are blood deficiency, blood heat, static blood, depletion of kidney's water, wind excess, seven emotional damage, dietary damage, etc. 2. The acupuncture points frequently used are $GV_{20}$, $GB_{20}$, $ST_{36}$, $SP_6$, $SP_{10}$, $BL_{17}$, $BL_{18}$, $BL_{23}$, $GV_{23}$, $GV_{14}$, $CV_6$, $ST_8$, $KI_3$, $LR_3$, $HT_7$, $LI_{11}$, EX-$HN_1$, and ouch point. 3. New acupuncture therapies, including herbal acupuncture therapy, ear acupuncture therapy, dermal needle therapy, needle-embedding therapy and scalp acupuncture therapy are frequently used to treat alopecia. Conclusions : The acupuncture points frequently used for treating alopecia have characteristics such as tonifying blood, tonifying the liver and kidney, extinguishing wind, activating blood, clearing heat and draining dampness, so they are highly associated with the causes and the mechanisms of alopecia.

A Basic Study on the Analysis Method of Thermography (Thermography의 분석방법에 관한 기초연구)

  • Background: The basic concept of thermographic interpretation is the thermologic equality of both side in normal person. But both sides diseases were limited diagnostic values by thermographic interpretation, and this interpretation does not apply to the case in thermal temperature of each part of body. Nevertheless, the measurement conditions are not standardized. So, for its clinical applications are extended, we think that the measurement conditions are considered the individual variations. Objectives: The purpose of this study is to examine the optimum conditions thermal temperature of the time period and region are not effected by internal and external variables. Methods: After the subjects took off their clothes, the filming were repeatedly five times made on duration of 5minutes during 20minutes. We selected nine regions around acupoints including Yin dang[印堂, HN1], Sugu[水溝, GV26], Ch’ondol[天突, CV22], Chonjung[CV17], Chung-wan[中脘, CV12], Ch’onch'u[天樞 S25], No-gung[勞宮, P8], and calculated based on the utility of R.O.I.(Region of Integer) in our system these points temperature. We measured the optimal time period and region that has little variation of thermal temperature. Results: The results shows that the optimal time period is 20minutes after undressed, and the optimal region is the region around acupoints including Sugu[水溝, GV26]. Conclusions: we obtained the measurement conditions were considered the individual variations. And also, this study offers basic sources for that the measurement conditions would be standardized. Furthermore, based on this results, we expect that clinical applications using thermography would be extended.

A Case Report of a Tremor Patient with Stenosis of the Left Carotid Artery (머리 떨림 환자에 대한 한방 치험 1례)

  • The carotid artery is one of the main vessels supplying blood to the brain. Carotid artery stenosis is mostly caused by atherosclerosis, a disease where cholesterol is deposited in the arterial blood vessels. Tremor refers to rhythmic shaking of a body part. Tremor is a symptom of many diseases, including Parkinson's disease, essential tremor, orthostatic tremor, cerebellar disease, peripheral neuropathy, and alcohol withdrawal. Tremors may be classified as postural, rest, and action tremors. Tremor of a patient with stenosis of the left carotid artery decreased with acupuncture treatment. The acupoints were GB20, TE17, GV8 and GV11. This case shows that the acupuncture treatment is effective against tremor.

Application of Traditional Acupuncture on Canine Intervertebral Disc Disease (개에서 추간판 질환에 대한 침술치료 증례)

  • A four-year-old female Pekingese dog was presented with intermittent ambulatory hindlimb lameness and severe back pain. The course of the disease was acute and static and 5 days passed by until presentation. In neurological examination, severe back pain and slightly decreased proprioception in right hindlimb were shown. In radiographic examination, radiopacity was increased at T10-11 intervertebral disc space. According to the history taking, neurological examination and radiographic examination, it was tentatively diagnosed as intervertebral disc disease (IVDD). Conservative treatment was performed including cage rest, medication with prednisolone and traditional acupuncture. Acupoints were GV-6, GV-7, BL-18, BL-19. BL-40 and GB-34. Dry needlings rested for 20 minutes. Just after the first acupuncture treatment back pain was significantly decreased. With additional acupuncture treatment, all the clinical signs were not observed. It is considered that conservative therapy including traditional acupuncture, medication and cage rest might be the first choice of therapeutic method for Grade I or II IVDD in dogs.

cyclic GMP Mediated Inhibition of Spontaneous Germinal Vesicle Breakdown Both with and without Cumulus in Mouse Oocyte

  • Intact germinal vesicle (GV) arrest and release are essential for maintaining the fertility of mammals inducing human. Intact germinal vesicle release, maturation of oocytes is maintained by very complex procedures along with folliculogenesis and is a critical step for embryonic development. Cyclic guanosine monophosphate (cGMP) has been suggested a key factor for meiotic arrest but so far its mechanisms are controversy. In this study we examine the effects of cGMP on germinal vesicle breakdown in cumulus-enclosed oocytes and denuded oocytes. Spontaneous maturation was inhibited by a cGMP agonist, 8-Br-cGMP with concentration dependent manners both in cumulus-enclosed oocytes and denuded oocytes. The inhibitory effect was more severe in denuded oocytes than cumulus-enclosed oocytes. The Rp-8-Br-cGMP and Rp-pCPT-8-Br-cGMP did not severely block GVB compared to 8-Br-cGMP. The spontaneous GVB inhibitory effects were different by the existence of cumulus. Based on them it is suggested that the cumulus modulates the role of cGMP in GV arrest.
