Ginsenosides Attenuate Formalin-Induced Pains Through Spinal and Supraspinal Sites

  • In previous studies we have demonstrated that several individual ginsenosides such as Rc, Rd, Re and Ri relieves formalin-induced pain following systemic treatment. But it is unknown where these single ginsenosides induce antinociception. We investigated the antinoiceptive effect of four individual ginsenosides on formalin-induced pain after intrathecal (i.t.), intracereventricular (i.c.v.), or subcutaneous (s.c.) administration using mice. We found that ginsenoside Rc, Rd, and Re except Rf attenuated both acute and tonic phase of pain. Ginsenoside Rf attenuated only tonic phase of pain after i.t. administration. The ED$\_$50/ was 1.0 (0.55∼l.75 mg/kg) for Rc, 1.15 (0.6∼2.25 mg/kg) for Rd, and 8.9 (3.9∼20.5 mg/kg) for Re in acute phase of pain. The ED$\_$50/ was 0.3 (0.1∼0.85 mg/kg) for Rc, 0.6 (0.35∼l.1 mg/kg) for Rd, 2.45 (1.25∼4.65 mg/kg) for Re, and 1.9 (1.5∼4.25 mg/kg) for Rf in tonic phase of pain. We also found that ginsenoside Rc, Rd, Re, and Rf after i.c.v. administration attenuated both acute and tonic phase of pain. The ED5o for acute phase of pain was 0.9 (0.55∼l.4mg/kg) for Rc, 0.9 (0.45∼1.7 mg/kg) for Rd, 0.93 (0.5∼l .75 mg/kg) for Re, and 1.85 (0.95∼3.5 mg/kg) for Rf. The ED$\_$50/ for tonic phase of pain was 0.7 (0.45∼1.05 mg/kg) for Rc,1.25 (0.7∼2.2 mg/kg) for Rd, 0.85 (0.45∼1.6 mg/kg) for Re, and 0.8 (0.4∼1.45 mg/kg) for Rf. Thus, the order of the analgesic potency was Rc$\geq$Rd>Re>Rf in both i.t. and i.c.v. administration routes. However, s.c. pretreatment of four ginsenosides did not reduce formalin-induced pain. These results suggest that analgesic effect of ginsenosides is achieved through spinal or supraspinal site(s) in formalin test.

Development of Korean Version of the Dementia Eating Evaluation Tool based on Behavioral Observation (행동관찰 기반 치매 식이 평가 도구의 한국판 개발)

  • Objective : This study introduces domestic and overseas systematic assessment tools that can identify eating problems of dementia patients based on abnormal behavior observations and turns them into Korean through the verification of content placement by expert groups. Methods : Three types of assessment tools were selected for final development in Korean version through several meetings based on a wide range of relevant literature searches. The 3 selected assessment tools were first translated by the researchers, and a 9-person expert team was used to verify the Content Validity Index. Results : The EBS content equivalence calculation shows that all 6 questions and 1 response item had a CVI value 0.9, and all items were included in Korean EBS without modification. The EdFED content equivalence calculation showed that all 11 questions had CVI value 0.9, which was included in the Korean edition of EdFED without modification. The content equivalence calculation of the FDI showed that all 19 questions had a CVI of 0.8 or higher, and all items were included in the Korean version of the FDI without modification of the item. Conclusion : Korean versions of the EBS, EdFED and FDI, which are based on behavioral observation and diet tools for people with dementia, have been developed. Early determination of problems related to diet in dementia patients and providing proper intervention through observational Korean version assessment tools is vital in terms of strengthening patient nutrition and reducing caregivers' burden.

Increased Readmission Risk and Healthcare Cost for Delirium Patients without Immediate Hospitalization in the Emergency Department

  • Objective: Hospitalization of patients with delirium after visiting the emergency department (ED) is often required. However, the readmission risk after discharge from the ED should also be considered. This study aimed to explore whether (i) immediate hospitalization influences the readmission risk of patients with delirium; (ii) the readmission risk is affected by various risk factors; and (iii) the healthcare cost differs between groups within 28 days of the first ED visit. Methods: Using the National Health Insurance Research Database, the data of 2,780 subjects presenting with delirium at an ED visit from 2000 to 2008 were examined. The readmission risks of the groups of patients (i.e., patients who were and were not admitted within 24 hours of an ED visit) within 28 days were compared, and the effects of the severities of different comorbidities (using Charlson's comorbidity index, CCI), age, gender, diagnosis and differences in medical healthcare cost were analyzed. Results: Patients without immediate hospitalization had a higher risk of readmission within 3, 7, 14, or 28 days of discharge from the ED, especially subjects with more severe comorbidities ($CCI{\geq}3$) or older patients (${\geq}65years$). Subjects with more severe comorbidities or older subjects who were not admitted immediately also incurred a greater healthcare cost for re-hospitalization within the 28-day follow-up period. Conclusion: Patients with delirium with a higher CCI or of a greater age should be carefully considered for immediate hospitalization from ED for further examination in order to reduce the risk of re-hospitalization and cost of healthcare.

Ohmic Characteristics of Ni/3C-SiC Interface (Ni/3C-SiC 계면의Ohmic 특성)

  • 본 연구에서는 3C-SiC의 ohmic 접합에 대하여 그 전기적 특성과 미세구조의 상관관계에 대하여 분석하였다. 표준사진식각 공정을 통하여 ohmic접합 금속으로서 Ni을 진공증착시켜 일련의 TLM패턴으로 열처리에 따르는 전류-전압 특성을 조사하였고 TEM, SEM, AES, EDS를 사용하여 Ni/SiC 계면에 대한 미세구조, 화학적 특성을 분석하였다. 열처리 온도와 시간을 통한 thermal budget이 증가함에 따라서 접촉저항이 감소되었으며 그 값은 $10^{-2}$-$10^{-4}$$\textrm{cm}^2$의 범위에 속하였다. EDS와 AES를 통하여 7$50^{\circ}C$이상의 열처리 후 silicide(NiSi$_{2}$)의 주변에 carbon층이 형성되는 것을 확인하였으며, 열처리 온도가 증가함에 따라서 island형 silicide의 크기가 조밀해지며 SiC와의 접착성이 향상됨을 알 수 있었다. Ni/3C-SiC ohmic 접합의 전기적 특성은 계면에 생성되는 silicide와 carbon의 형성거동에 의하여 결정되는 것으로 믿어진다.

Cromakalim 및 Pinacidil에 의한 개의 관상 동맥평활근 세포의 이완반응기전에 대한 연구

  • 최근 $K^{+}$ channel openers인 cromakalim과 Pinacidil이 새로운 종류의 혈관확장제로서 항고혈압 치료제로 소개되었다. 그러나 이들 약물의 이완 효과와 새포내 작용기전은 분명치 않다. 따라서 본 연구에서는 개의 관상 동맥에서 얻은 평활근 세포에서 $K^{+}$ channel openers에 의한 이완의 세포내 기전을 규명하고자 시도하였다 혈관 평할근 세포(dispersed smooth muscle cells)를 collagenase를 이용한 효소소화(enzymatic digestion)에 의해 분리하고, saponin을 이용하여 permeabilize되게 하였다. frypan blue exclusion과 전자 현미경적 관찰에 의하여 세포의 viability를 확인하였다. Dispersed intact cells은 phenylephrine (PE)에 의하여 용량의존 수축반응을 보였고 ED$_{50}$는 2.3 $\times$ $10^{-12}$ M이었다. 이러한 PE에 의한 수축반응은 cromakalim과 pinacidil (ED$_{50}$, 1.2 $\times$ $10^{-12}$ M)의 용량에 의존하여 억제되었다.

The Effect of N-Substituted Alkyl Groups on Anticonvulsant Activities of N-Cbz-$\alpha$-amino-N-alkylglutarimides

  • In order to examine the effects of N-substituted alkyl group on the anticonvulsant activities of N-Cbz-$\alpha$-aminoglutarimides as novel anticonvulsants with broad spectrum, a series of (R) or (S) N-Cbz-$\alpha$-amino-N-alkylglutarimides (1 and 2) were prepared from the corresponding (R) or (S) N-Cbz-glutamic acid and evaluated for the anticonvulsant activities in the maximal electroshock seizure (MES) test and pentylenetetrazol induced seizure(PTZ) test, including the neurotoxicity. The most potent compound in the MES test was (S) N-Cbz-$\alpha$-amino-N-methylglutarimide($ED_{50}$=36.3 mg/kg, PI=1.7). This compound was also most potent in the PTZ test ($ED_{50}$=12.5 mg/kg, PI=5.0). The order of anticonvulsant activities against the MES test as evaluated form $ED_{50}$ values for (R) series was N-methyl > N-H > N-ethyl > N-allyl ; for the (S) series N-methyl > N-H > N-ethyl > N-alkyl > N-isobutyl compound. Against the PTZ tests, the order of anticonvulsant activities showed similar pattern ; for the (R) series, N-methyl > N-H > N-ethyl > N-allyl ; for the (S) series N-methyl > N-H > N-ethyl > N-allyl > N-isobutyl compound. From the above results, N-substituted alkyl groups were though to play an important role for the anticonvulsant activities of N-Cbz-$\alpha$-amino-N-alkylgutarimides.

  • Cinnamomum Iners (Darchini, 쟈 바 계피), Elettaria Cardamomum (Elachi, 백두구, 소두구, 카다몬, 카도몬), Punica Granatum (Anar Dana, 석류), Curcuma Longa (Haldi, 터메릭, 투메릭, 심황, 울금, 울금은, 컬쿠마), Piper Nigrum (Kali Mirch, 페퍼, 페퍼블랙, 후추, 블랙 페퍼), Syzygium aromaticum (Loung, 클로브, 정향, 정향나무,), Capsicum Annum (Laal Mirch, Red Pepper), Mentha Arvensis (Pahari Podina), Curcuma Zedoaria (Kachur, 가쥬츠, 봉출, 아출, 커큐머 제도) Zingiber officinale (Adrak, 건강, 생강, 진저) and Garam Masala (Hot Spice, a mixture) have been analyzed in the present work for their metallic components with the help of SEM-EDS and ICP-AES. Minor concentrations have been determined with ICP-AES and major concentrations with the help of SEM-EDS.
