Clinical efficacy and safety of lamotrigine monotherapy in newly diagnosed pediatric patients with epilepsy (간질 환아에서 Lamotrigine 초기 단독 요법의 효능 및 안정성에 대한 연구)

  • Purpose : To verify the efficacy and safety of lamotrigine (LTG) monotherapy in newly diagnosed children with epilepsy. Methods : We prospectively enrolled 148 children who had undergone LTG monotherapy at our institution between September 2002 and June 2009. Twenty-nine patients were excluded: 19 due to incomplete data and 10 were lost to follow up. The data of the remaining 119 patients was analyzed. Results : We enrolled 119 pediatric epilepsy patients (aged 2.8-19.3 years; 66 males and 53 females) in this study. Out of 119 patients, 29 (25.2%) had generalized epilepsy and 90 (74.8%) had partial epilepsy. The responses of seizure reduction were as follows: Seizure freedom (no seizure attack for at least 6 months) in 87/111 (78.4%, n=111) patients; partial response (reduced seizure frequency compared to baseline) in 13 (11.7%) patients; and persistent seizure in 11 (9.9%) patients. The seizure freedom rate was in 81.6% in patients with partial seizure (75.9% for complex partial seizure and 90.9% for benign rolandic epilepsy) and 44.8% in patients with generalized epilepsy (30.0% for absence seizure, 35.7% for juvenile myoclonic epilepsy patients, and 100.0% for idiopathic generalized epilepsy patients). Adverse reactions were reported in 17 (14.3%) patients, and 8 patients (6.7%) discontinued LTG because of rash and tic. No patient experienced severe adverse reaction such as Stevens-Johnson syndrome. Conclusion : LTG showed excellent therapeutic response and had few significant adverse effects. Our findings report may contribute in promoting the use of LTG monotherapy in epileptic children.

Detection of Lily symptomless virus, Lily mottle virus, and Cucumber mosaic virus from Lilium Grown in Korea by RT-PCR (RT-PCR법을 이용한 백합 바이러스 LSV, LMoV, CMV의 검출)

  • Leaf samples and bulbs showing characteristic symptoms of virus infection were collected from Gang-won, Chung-nam, and Jeju Province of Korea in 2008-2009. Three viruses, Lily symptomless virus (LSV), Lily mottle virus (LMoV), and Cucumber mosaic virus (CMV) were detected by RT-PCR. Virus-infected plant samples were identified; 12 plants with LSV, 20 plants with LMoV, and 1 plant with CMV. Of the twelve LSV infected samples, seven samples were found to be mix-infected with LMoV and LSV. Symptoms of LMoV and LSV mixed infection were fairly severe, like as vein clearing, leaf curling, leaf mottling, leaf mosaic, and yellow streaking. Mixed infection with LMoV and LSV was also found in lily bulbs which have been stored under unfavorable environmental conditions. LMoV predominated in our tests, whereas spread of Lilyvirus X (LVX) was not found. The nucleotide sequences of coat protein (CP) region of seven isolates (4 LMoV, 2 LSV, and 1 CMV) were compared with the corresponding regions of LMoV (AJ564636), LSV (AJ516059) and CMV(AJ296154). The nucleotide sequence homologies between reference viruses and seven isolates were 95-99%. Complete sequencing of seven isolates is necessary to obtain more information on the molecular characteristics of these viruses as well as to increase sensitivity and rapidity of viral detection.

Effect of Fermented Brown Seaweed Waste (FBSW) on in vitro Rumen Microbial Fermentation (발효 미역부산물이 반추위 발효특성에 미치는 영향)

  • This study was conducted to investigate the effects of brown seaweed waste (BSW) fermented with DS-01 microbe on in vitro rumen microbial fermentation. In in vitro trial, three different diets supplemented with 2%, 4%, 6% BSW fermented with DS-01 either for one month or two months was tested at 3 h, 6 h, 9 h, 12 h, and 24 h incubation. The chemical composition (CP, EE, CF, and ash) between brown seaweed waste (BSW) and fermented BSW (FBSW) were not different. The contamination of pathogenic microbes was not detected in FBSW. The pH value tended to be higher with 6% level of supplementation of FBSW for one month than other treatments. The pH at 24 h was significantly higher in FBSW than that of treatments without FBSW (p<0.05). In FBSW for two months, the pH value in 6% FBSW at 3 h in vitro fermentation tended to be higher than 2% or 4% FBSW treatments (p=0.0540), but there were no differences in other fermentation times. Although the concentration of $NH_3$-N of BSW fermented for one month was higher than control at 3 h (p<0.05), the volatile fatty acid values were significantly increased in 4 and 6% FBSW fermented for one month at 6 h incubation (p<0.05). In BSW fermented for two months, the volatile fatty acid values were significantly decreased in 6% treatment at 9 h (p<0.05). As a result of in vitro trial, it was recommended that the 2~4% supplementation level of brown seaweed waste fermented with DS-01 microbe for two months could be utilized for in vivo trial in ruminants.

Effect of Forage Species, Mixed Pastures and Mesh on Forage Yields and Botanical Composition for Rapid Establishment of Grassland at Cutting Area in Middle Region of Korea (중부지역 절개지에서 신속한 초지조성을 위한 목초의 초종, 혼파조합 및 그물망 설치가 생산성과 식생에 미치는 영향)

  • The objective of this study was to investigate the suitable method for rapid establishment of grassland according to forage species, mixed pastures and installation of mesh at 35 degree angle of cutting area in the middle region of Korea. In agronomic characteristics after wintering, vegetation coverage of Tall fescue(TF) was 74% in monoculture, which was the highest among 3 forage crops. Meanwhile average vegetation coverages of monoculture and mixed pasture were 67 and 92% in treated of mesh, which were 4 and 18% higher than untreated of mesh, respectively. In botanical composition, TF of monoculture and Orchardgrass(OG) and TF oriented mixed were indicated over 94% forage coverage. Meanwhile forage coverages Kentucky bluegrass(KBG) and Red top(RT) of monoculture and Perennial ryegrass(PRG) oriented mixture were 89, 81 and 92% in treated of mesh, which were 7, 6 and 5% higher than untreated of mesh, respectively. In forage productivity, dry matter yield(DMY) of TF(12,537kg/ha) and KBG(11,897kg/ha) of monoculture were significant(p<0.05) higher than RT(9,604kg/ha). Meanwhile DMY of OG(12,227kg/ha), TF(12,823kg/ha) and PRG(11,871kg/ha) oriented mixed were not significant difference(p>0.05). In forage quality of monoculture, in the first year, crude protein of KBG was 13.6%, which was the highest among 3 forage species. Also neutral detergent fiber(NDF) of TF was 56.5%, which was the lowest among 3 forage species. In mixed pasture, in the second year, NDF and acid detergent fiber of PRG oriented mixed were 56.5 and 34.3%, respectively, which was the lowest among 3 mixed pasture. In conclusion, forage species TF was more suitable on initial rootage and continuous forage coverage, OG and TF oriented mixed were more suitable on continuous forage coverage. Also the installation of mesh showed positive effects on initial rootage and maintain forage ratio.

Effect of Mixed Pasture Using Domestic Varieties Orchardgrass 'Kodione' and Tall fescue 'Purumi' on Forage Yields and Botanical Composition in Middle Region of Korea (중부지역에서 국내육성 목초 오차드그라스 '코디원' 및 톨 페스큐 '푸르미' 이용 초지조성 혼파조합별 사초생산성 및 식생에 미치는 영향)

  • This study was carried out to determine the effect of mixed pasture using domestic varieties orchardgrass 'Kodione' and tall fescue 'Purumi' on forage yields and botanical composition from 2013 to 2015 in middle region of Korea. The following mixed pastures were used in this study: treatment A (orchardgrass oriented mixture with imported varieties), treatment B (orchardgrass oriented mixture), treatment C (orchardgrass and tall fescue oriented mixture), treatment D (tall fescue oriented mixture), treatment E (tall fescue oriented mixture without orchardgrass), treatment F (only orchardgrass), and treatment G (only tall fescue). In botanical compositions at last cutting time in 2015, treatment A had orchardgrass at 7.1%, tall fescue at 47.9%, kentucky bluegrass at 15.2% and white clover at 25%. Meanwhile, treatment B had orchardgrass at 25.9%, tall fescue at 47.0%, kentucky bluegrass at 4.9% and white clover at 18.3%. Consequentially, in botanical composition from 2014 to 2015, treatment A was changed more compare to other treatments B, C, D and E. The average of dry matter (DM) yield for 2 years of treatment E (18,369 kg/ha) was the highest among the seven treatments, but there was no significant difference among other treatments except treatment A (p>0.05). The crude protein (CP) and in vitro dry matter digestibility (IVDMD) of treatment A were 14.5% and 74.8%, and treatment E were 14.1% and 73.0%, respectively. The content of neutral detergent fiber (NDF) and acid detergent fiber (ADF) were lower in treatment E and A than other treatments, and treatment E showed high content of total digestible nutrient (TDN). Therefore, tall fescue oriented mixture using domestic tall fescue variety Purumi had good forage productivity, quality and botanical composition in middle region of Korea for the establishment of grassland.

Evaluation of Characteristics, Winter Survival and Forage Production for Warm Season Grass in the Mid-Southern Regions of Korea (중남부지역에서 난지형 목초의 생육특성, 월동성 및 사초생산성 평가)

  • Field studies were conducted from the years 2009 to 2012 in order to determine the cultivation limit as well as to evaluate the characteristics and forage production of warm season grass in Korea. Two bermudagrass [Cynodon dactylon (L.) Pers.] cultivars, two bahiagrass (Paspalum notatum Flugge) cultivars and a Kleingrass [Panicum coloratum L.] cultivar were compared for forage production and quality in the mid-southern regions of Korea. The experimental design was a randomized block design (RBD) with three replications. The number of days to seedling emergence for bremudagrass and bahiagrass was observed as approximately 12 days and 28 days after seeding, respectively. In Kwangju, the heading dates of bahiagrass and kleingrass were 21 August and 10 July, respectively,. Warm season grass did not winter in the mid-regions (Kimjea, Cheonan) of Korea. All of the Bermudagrass cultivars had higher dry matter (DM) than bahiagrass at the first harvest. The dry matter yield of kleingrass was usually greater than the other entries at all study sites. Peak forage DM production of bermudagrass and bahiagrass cultivars occurred in June and July, respectively. The contents of crude protein (CP) and total digestibility nutrient (TDN) for bermudagrass cultivars were usually greater than the other entries at all study sites. Further, acid detergent fiber (ADF) and in vitro DM digestibility (IVDMD) were similar across all cultivars.

Evaluations of Growth and Forage Quality of Sesbania Accessions Adaptable to Korean Environments (국내적응 세스바니아의 생육특성 및 사료가치 평가)

  • Sesbania, an annual herb, is known for its high forage value and salt tolerance. It has merits as a forage crop that is adaptable to reclaimed land in the Republic of Korea. Therefore, we collected Sesbania genetic resources from the Republic of Korea and other countries, and conducted experiments to evaluate their potential as a forage crop in Korean climate and soil conditions. In the preliminary experiments, 15 genetic resources which were able to set seeds in Korean environment were selected out of a total of 46 collected genetic resources. Among 15 genetic resources, SL13 was the tallest and it was followed by that of SC04, SR01 and SE07. The accessions with the earliest flowering started flowering 101 days after sowing and set seed in early August. Fifteen accessions were evaluated for their salt tolerance at germination stage based on germination rate and growth of germinated seedlings at 0 mM, 150 mM and 300 mM NaCl concentrations. Five genetic resources like SC04, SL13, SS20, SS24 and SR01 were selected to be tolerant to NaCl treatment. Forage value was evaluated based on crude protein, acid detergent fiber, neutral detergent fiber and in vitro dry matter digestibility. The forage value of leaves was significantly higher than that of stems, and the forage value of the stem was slightly better than that of rice straw. The forage value of leaves of all the genetic resources was higher than grade 1 by the American Forage and Grassland Council grade. Among five selected genetic resources, the relative feed value of SC04 was the highest and it was followed by that of SS20, SL13, SS24 and SR01.

Effects of Gypsum on Dry Matter Yield and Chemical Composition of Alfalfa in Reclaimed Tidal Land with Soil Dressing (객토 간척지에서 석고처리가 알팔파 건물수량 및 사료성분에 미치는 영향)

  • The objective of this study was to investigate the effect of gypsum on the dry matter yield and the chemical composition of alfalfa in reclaimed tideland with soil dressing. The experimental site was Sukmoon reclaimed tideland. The tideland was reclaimed approximately 17 to 33 years ago and the 70 cm of soil was top-dressed. The soil that covers the reclaimed tideland brought from the island did not treat di-salinized. Treatments were consisted of three groups; control group where no gypsum (G0) was applied and two experimental groups where 2 ton/ha (G2) and 4 ton/ha (G4) of gypsum were applied, respectively. The first harvest was conducted when the alfalfa reached early flowering (open the flower 10%), and after that subsequent harvest was conducted at approximately 35 days intervals. The dry matter yield of the alfalfa showed that G2 was significantly higher in the first year than G0 and G4, and G2 tended to be higher in the second year than G0 and G4, although there were no significant differences between treatments. The reason for the high dry matter yield in G2 was that the soil pH and EC of the soil were at marginal and ideal levels and the coverage and alfalfa botanical composition were also high. In both years, there were no differences in the crude protein, neutral detergent fiber and acid detergent fiber contents and relative feed value between gypsum treatments. Meanwhile, the results in the first and second years showed that the alfalfa dry matter yield were negatively affected by droughts stress in spring and concentrated precipitation in summer. Therefore, this study suggests gypsum treatment in reclaimed tidal land could increase the dry matter yield of alfalfa, and 2 ton/ha of gypsum was the optimum rate.

Changes in Fermentation Characteristics and Bacterial Communities of Whole Crop Rice Silage during Ensiling Period (저장기간에 따른 사료용 벼 사일리지의 발효특성 및 미생물상 변화)

  • Understanding changes in fermentation characteristics and microbial populations of forage silage during ensiling is of interest for improving the nutrient value of the feed for ruminants. This study was conducted to investigate the changes in fermentation characteristics and bacterial communities of whole crop rice (WCR) silage during the ensiling period. The chemical compositions, pH, organic acids and bacterial communities were evaluated at 0, 3, 6, and 12 months after ensiling. The bacterial communities were classified at both the genus and species levels. The dry matter content of WCR silage decreased with the length of storage (p<0.05), but there was no significant difference in crude protein and NDF contents. Following fermentation, the pH level of WCR silage was lower than the initial level. The lactic acid content remained at high levels for 3 to 6 months after ensiling, followed by a sharp decline at 12 months (p<0.05). Before fermentation, the WCR was dominated by Weissella (30.8%) and Pantoea (20.2%). Growth of Lactiplantibacillus plantarum (31.4%) was observed at 3 months after ensiling. At 6 months, there was a decrease in Lactiplantibacillus plantarum (10.2%) and an increase in Levilactobacillus brevis (12.8%), resulting in increased bacteria diversity until that period. The WCR silage was dominated by Lentilactobacillus buchneri (71.2%) and Lacticaseibacillus casei (27.0%) with a sharp reduction in diversity at 12 months. Overall, the WCR silage maintained satisfactory fermentation quality over a 12-month ensiling period. Furthermore, the fermentation characteristics of silage were found to be correlated to bacterial microbiome.

Tracheal gas insufflation (TGI) in patients with increased deadspace fraction: the effect and its determining factors (사강호흡율이 증가된 환자에서 기관내 가스주입법(Tracheal Gas Imsufflation)이 가스교환에 미치는 효과와 그 결정인자)

  • Background : Tracheal Gas Insufflation (TGI) is one of the newer ancillary measures in mechanical ventilation employed to enhance carbon dioxide elimination. TGI exerts its effect through reduction of deadspace ventilation, but the factors determining its effect are not well studied yet. Method : The subjects were seven mechanically-ventilated patients ($58.8{\pm}10.6$ yrs) who showed increased physiologic deadspace greater than 60%. After 30 nun of stabilization with 100% oxygen on pressure control ventilation, continuous flow TGI was administered via the insufflation lumen of Hi-Lo Jet Tracheal Tube (Mallincrodt, USA) for 15 min at 3 L/min and 5 L/min each. Results : $PaCO_2$ was decreased ($51.4{\pm}17.6$ at baseline, $49.1{\pm}18.9$ at TGJ 3 L/min $45.0{\pm}14.9$ mm Hg at TGI 5 L/min, p=0.050), and pH was increased ($7.37{\pm}0.12$, $7.38{\pm}0.13$, $7.39{\pm}0.12$, respectively, p=0.037) while mixed expired $CO_2$ ($P_ECO_2$) was not changed significantly from baseline (p=0.336) by TGI. Physiologic deadspace(Vdphy) was decreased ($73.0{\pm}7.9$% at baseline, $69.8{\pm}10.0$% at TGI 3 L/min, and $67.1{\pm}10.1$% at TGI 5 L/min, p=0.015). $AaDO_2$(p=0.147), Vt(p=0.2140), Pmean(p=0.7788) and mean arterial pressure(p=0.4169) were not changed. The correlation between % maximal decrease of Vdphy were r=0.790 with the ratio of baseline Vdana/Vdphy(p=0.035) and r=-0.754 with baseline Vdalv(p=0.050). Conclusion: TGI was effective in reducing $PaCO_2$ and deadspace, and the deadspace-reducing effect was best correlated with baseline anatomic/physiologic deadspace ratio.
