An Elimination of False-Positive I-131 Sites in Esophagus for Thyroid Carcinoma; Using Water with Vitamin C Dissolved (갑상선암의 방사성옥소 치료 후 전신 스캔에서 비타민C 용액을 이용한 식도의 위양성 병소 제거에 대한 고찰)

  • Purpose: Whole body imaging with radioiodine can detect functioning metastases. Non-physiologic I-131 uptake detected on images usually is interpreted as suggesting functioning thyroid metastases. However, extra-thyroidal I-131 accumulation does not always imply thyroid cancer metastases and has been reported in many circumstances. In order to avoid unnecessary therapeutic interventions it is important to distinguish false-positive sites of I-131 localization. We study here to remove false-positive sites around esophagus region on I-131 whole body imaging in patients who were administrated thyroidectomy. Materials and Methods: From April to August in 2007, we had the patients who had visited our department after they received thyroidectomy due to thyroid cancer. They were given I-131, and performed radioiodine body scan after 41 to 50 hours. Patients were whole-body-scanned for the speed of 8 cm per minute. After that, we took anterior and posterior static images around the patients' neck measured by 300 thousand counts per image. We selected 44 patients who had hot spots around neck region, we divided the patients into two groups. One group was given 0.5 L of water and the other group was given 0.5 L of water with 1 g of Vitamin C dissolved. The patients were asked to drink the fluid for one minute in sitting position and after that, we measured 300 kilo counts per image again. We compared prior anterior, posterior static images with anterior, posterior images after the patients had water or water that Vitamin C resolved. Results: In using water, both observer 1 and 2 interpreted 6 patients were washed out. In the water with Vitamin C resolved, observer 1 and 2 interpreted 9 and 8 patients were washed out. Observer 1 and 2 interpreted 6 and 5 patients had 'indeterminate' when they used water. Both observer 1 and 2 interpreted 6 patients had 'indeterminate' when they used water with Vitamin C resolved. When they used water, observer 1 interpreted 10 patients had 'unchanged' and observer 2 interpreted 11 patients had 'unchanged'. Differently, when they used water with vitamin C resolved, observer1 had 8 patients having 'unchanged'and observer 2 had 9. Conclusion: As a result, by making patients drink 0.5 liter of water which has vitamin C resolved helped getting rid of false-positive sites in esophagus. Therefore, based on this study, we believe that drinking water with vitamin C dissolved is very in terms of reducing false-positive hot spot around the esophagus for the iodine-131 whole body scan.

Evaluation of Radiation Shielding Rate of Lead Aprons in Nuclear Medicine (핵의학과에서 사용하는 납 앞치마의 방사선 차폐율 평가)

  • Considering that the X-ray apron used in the department of radiology is also used in the department of nuclear medicine, the study aimed to analyze the shielding rate of the apron according to types of radioisotopes, thus ${\gamma}$ ray energy, to investigate the protective effects. The radioisotopes used in the experiment were the top 5 nuclides in usage statistics $^{99m}Tc$, $^{18}F$, $^{131}I$, $^{123}I$, and $^{201}Tl$, and the aprons were lead equivalent 0.35 mmPb aprons currently under use in the department of nuclear medicine. As a result of experiments, average shielding rates of aprons were $^{99m}Tc$ 31.59%, $^{201}Tl$ 68.42%, and $^{123}I$ 76.63%. When using an apron, the shielding rate of $^{131}I$ actually resulted in average dose rate increase of 33.72%, and $^{18}F$ showed an average shielding rate of -0.315%, showing there was almost no shielding effect. As a result, the radioisotopes with higher shielding rate of apron was in the descending order of $^{123}I$, $^{201}Tl$, $^{99m}Tc$, $^{18}F$, $^{131}I$. Currently, aprons used in the nuclear medicine laboratory are general X-ray aprons, and it is thought that it is not appropriate for nuclear medicine environment that utilizes ${\gamma}$ rays. Therefore, development of nuclear medicine exclusive aprons suitable for the characteristics of radioisotopes is required in consideration of effective radiation protection and work efficiency of radiation workers.

Minimized Radiation Dose of Patients Receiving High Dose Radioiodine(I-131) Therapy (고용량 방사성옥소(I-131) 치료환자의 피폭선량 저감화 연구)

  • The number of thyroid diseases treated with radioiodine(I-131) is increasing steadily. The sharp increase in patients who require high dose radioiodine therapy greatly increased the need for new therapy rooms. Accordingly, interest in radiation exposure is rising as well, and is a major psychological stress factor for the patient and those who come in close contact with the patient. This study aimed to minimize the radiation exposure on discharge. Based on various previous reports, the decision for discharge should be individualized depending on many factors related to the patient's living or working environment. Educating patients repeatedly on the importance of sufficient oral hydration, while the adequate amount was relative to the patient's individual condition, greatly lowered the detected radiation measurement within the same admission period. In some cases, the period of admission could be abbreviated.

Survey Study on Radioactivity of Domestic Fishery Product (국내 시중 유통 수산물에 대한 방사능 농도 조사)

  • Samples of fishery products were tested for radioactivity by using the intake frequency data from Korea Health Statistics. The radioactivity of $^{40}K$, $^{137}Cs$, $^{134}Cs$, and $^{131}I$ was analyzed using gamma spectrometry with a simplified sample pre-treatment procedure. The radioactivity range for $^{40}K$ was 21.9-3050 Bq/kg, whereas the radioactivities of $^{137}Cs$, $^{134}Cs$, and $^{131}I$ were under minimum detectable activity which were in the range of 0.140-1.97, 0.0900-1.89 and 0.124-1.94 Bq/kg, respectively, for the three species. The results suggest that the Fukushima accident did not have a significant impact on domestic fishery products, which were analyzed during the period from 2013 to 2015. Additionally, there seemed to be no significant impact of additional exposure dose by the analyzed radionuclides.

  • The generation amount of radioactive waste has been rapidly increased by increase of the usage of radioisotope source in medical field. Especially, the use of the radioactive source of I-131 with short half-life of 8.02 days used in treatment of thyroid has been increased, and all of the wastes concerned have been disposed by means of the self-disposal method. IAEA proposed criteria for clearance level of waste which depends on the individual (10 ${\mu}Sv/y$) and collective dose (1 man-Sv/y), and concentration of each nuclide (IAEA Safety Series No 111-P-1.1, 1992 and IAEA RS-G-1.7, 2004). In this study, various radioactive wastes in medical fields are collected and measured for establishing the disposal methods and procedures of radioactive wastes. In addition, comparison evaluation of decay storage period between the half-life which was calculated by attenuation curve based on real measurement and analytical half-life is considered. With comparing the theoretical half-life and the effective half life(7.72 days) which was based on the decay equation of measured data, it is resulted in the theoretical half-life is longer than effective half-life. The storage period of radioactive waste for self-disposal may be curtailed. The result of this study will be proposed as ISO standard.

An Assessment of Radiological Consequences of I-131 Atmospheric Release by the System Analysis Method (계통해석법에 의한 I-131대기방출의 영향평가)

  • The annual individual and collective doses to the thyroids of four age-dependent groups due to the in-take of I-131 released from the Younggwang nuclear power plant NU-1 & 2, Korea, are estimated using the model presented in ICRP 29. Sensitivity and robustness of the model are analyzed. In case of 0.12% fuel defect during normal operation, the collective dose is founded to be 3.05${\times}10^{-3}$man-thyroid-Sv, which is higher than the value calculated by the GASPAR code, 2.3${\times}10^{-3}$man-thyroid-Sv. The maximal individual annual doses resulting from an acute release are higher than those calculated under the assumption of continuous release by $1.4{\sim}1.7$ times. The most important pathway to the infant is milk and, in contrast, that to child, teen and adult is ingestion of crops. The model used is the calculation appears to be influenced by the variables such as roubstness-index. The weighted committed dose equivalent obtained by the ICRP 29 model is slightly higher than that calculated by the three-compartment model.

Clinical Observation on Hyperthyroidism (갑상선기능항진증(甲狀腺機能亢進症)의 임상적(臨床的) 관찰(觀察))

  • A clinical analysis was made on 161 cases of hyperthyroidism seen at the Radioisotope Laboratory of Kyungpook National University Hospital. This series consisted of 144 cases of diffuse goiter and 17 cases of nodular goiter. 1) Hyperthyroidism was most prevalent in the 4th decade and male to female ratio was 1:4.6. 2) Cardinal symptoms in the order of frequency were weakness, easy fatigability, palpitation, weight loss, nervousness, perspiration, heat intolerance, increased appetite, insomnia and dysmenorrhea. 3) Major physical findings in the order of frequency were goiter, fine tremor, tachycardia, wide pulse pressure, emaciation, warm moist skin, exophthalmos, systolic hypertension and atrial fibrillation. 4) The complications were ophthalmopathy (34.2%), thyrotoxic heart disease (5.6%), thyroid crisis (1 case), pretibial myxedema (1 case) and thyrotoxic myopathy (1 case). 5) Mean values of the six hour and twenty-four hour $^{131}I$ uptakes by the thyroid glands were 67.5% and 71.6%, respectively, in diffuse goiter and 64.5% and 65.0%, respectively, in nodular goiter. 6) Mean values of twenty-four hour $PB^{131}I$ conversion ratio were 76.3% in diffuse goiter and 70.2% in nodular goiter and those of the basal metabolic rate was +51% in the former and +41% in the latter. Mean serum cholesterol level was 152mg% in diffuse goiter and that in nodular goiter was 175mg%. 7) Among the 134 cases treated with $^{131}I$, 66 cases (49.3%) were successful1y controlled with single dose and in the majority of the cases the initial therapeutic dose required was $4.1{\sim}5.0mC$ in diffuse goiter and $5.1{\sim}6.0mC$ in nodular goiter. 8) With $^{131}I$ treatment the symptoms improved in the following order: heat intolerance, emaciation, nervousness, insomnia, easy fatigability, weakness, fine tremor, goiter, perspiration, exertional dyspnea and palpitation. And in a few cases improvement of even exophthalmos was seen. 9) Following $^{131}I$ treatment myxedema occurred in 4 cases (3%) and reccurrence in 9 cases (6.8%).

  • Radioactive iodine($^{131}I$) treatment reduces recurrence and increases survival in patients with differentiated thyroid cancer. However, it is important in terms of radiation safety management to measure the radiation dose rate generated from the patient because the radiation emitted from the patient may cause the exposure. Research methods, it measured radiation dose-rate according to the elapsed time from 1 m from the upper abdomen of the patient by intake of radioactive iodine. Directly comparing the changes over time, high dose rate sensitivity and efficiency is statistically significant, and higher chamber than GM counter(p<0.05). Low dose rate sensitivity and efficiency in the chamber had lower levels than gm counter, but not statistically significant(p>0.05). In this study confirmed the characteristics of calibrated ionization chamber and GM counter according to the radiation intensity during high-dose radioactive iodine therapy by measuring the accurate and rapid radiation dose rate to the patient explains, discharged patients will be reduced to worry about radiation hazard of family and others person.

Establishment of I-131, Tc-99m Labeling Methods to In-house Anti-CEA Antibodies and Evaluation of the Immunological Characteristics (국산 항 CEA 항체의 I-131, Tc-99m 표지법 확립 및 면역학적 특성 분석)

  • Cancer cells have several tumor-associated antigens on the cell surfaces, and antibodies against these antigens have been developed by many investigators. Radiolabeled antibodies have been used as new methods to diagnose and treat malignant tumors. Especially anti-carcinoembryonic antigen (CEA) is the most popular antibody for these purposes. In this investigation, we tried to label $^{131}I$ and $^{99m}Tc $ to anti-CEA monoclonal antibodies which were developed in the Seoul National University College of Medicine. We found CEA-79 and CEA-92 antibodies had the better immunological characteristics among 8 anti-CEA monoclonal antibodies. And radioiodination of CEA-79 could be performed by chloramine-T method, while radioiodination of CEA-92 by iodogen method. To label these antibodies with $^{99m}Tc $, we used pretargeting transchelation as direct labeling method. At first, $^{99m}Tc $ was bound to glucaric acid, and monoclonal antibody was reduced by $\beta-mercaptoethanol$. When these were incubated together. $^{99m}Tc $ bound to glucarate was switched to monoclonal antibody because of higher affinity. We established conditions of several steps in this method. Anti-CEA monoclonal antibodies labeled with $^{131}I$ and $^{99m}Tc $ are expected to be used valuably in the detection and treatment of malignant tumors.
