Nonalcoholic fatty liver disease (NAFLD) is recognized as the most common liver disease with an estimated prevalence of 20-30% in the Western world and 16-33% in Korea. NAFLD encompasses a broad spectrum of hepatic dysfunction ranging from nonalcoholic fatty liver (NAFL) to nonalcoholic steatohepatitis (NASH), cirrhosis, and hepatocellular carcinoma. The increasing prevalence of NAFLD is particularly worrying because patients appear to have higher non-liver-related and liver-related death, as compared to the general population. Given its well-known association with metabolic comorbidities, NAFLD is commonly associated with obesity, type II diabetes, dyslipidemia, and metabolic syndrome. The natural history of NAFLD remains unclear due to its indolent clinical course and the lack of well-designed prospective studies. The prognosis of NAFLD depends on the histological subtype, while NASH may be associated with liver fibrosis and cirrhosis and may progress to hepatocellular carcinoma. The overall and liver-related mortality are increased in patients with NASH, as compared to NAFL and the general population. NAFLD is strongly associated with cardiovascular disease and type 2 diabetes, so it should also be considered a metabolic liver disease. Further long-term studies of the natural course of NAFLD are warranted.

Nonalcoholic fatty liver disease (NAFLD) is composed of nonalcoholic fatty liver (NAFL), nonalcoholic steatohepatitis (NASH) and NASH related cirrhosis and these are defined by histological findings. These diseases have absolutely typical and different clinical findings and natural courses respectively. NAFL generally has benign process however; NASH and NASH related cirrhosis have an increased risk for liver-related morbidity and mortality. So, the accurate diagnosis and differentiation of steatohepatitis or advanced fibrosis are essential in the management of patient and the prediction of prognosis. The gold-standard of diagnosis of NAFLD is liver biopsy however; it has some limitations in clinical practice. So many studies have been done to find out noninvasive diagnostic methods. These methods can be divided into 2 groups: those that predict the presence of NASH and that predict the presence of advanced fibrosis. This paper provides an overview of various noninvasive methods for detecting NAFLD and their clinical meanings in clinical practice.

Non-alcoholic fatty liver disease (NAFLD) is the most common form of chronic liver disease. The prevalence of NAFLD is growing gradually worldwide with increases in obesity, sedentary lifestyles, and an unbalanced diet. NAFLD ranges from simple steatosis without inflammation to steatohepatitis that can progress to cirrhosis. There is no single effective treatment that has widespread effects in NAFLD. The cornerstone of treatment is lifestyle modification, including weight reduction, diet, and physical activity. An approximately 7-10% weight reduction via diet or physical activity can improve the liver histopathology. Risk factors for NAFLD include a high-calorie diet, high-lipid diet, high-carbohydrate diet, saturated fatty acids, trans fatty acids, cholesterol, high fructose intake, and low-choline diet. Factors that protect against NAFLD include a low-calorie diet, low-carbohydrate diet, low-lipid diet, monounsaturated fatty acids (MUFA), polyunsaturated fatty acids (PUFA), dietary fiber, coffee, green tea, and light alcohol consumption. Physical activity also helps to manage NAFLD with or without weight reduction. Recent reports found that resistance training is as effective as aerobic training. Lifestyle modification has very low compliance. To maintain a treatment program, a multidisciplinary team approach is required that includes physicians, dietitians, physical trainers, and psychologists.

Weight loss via lifestyle modification remains the most efficient treatment for NAFLD. Weight loss and exercise are the cornerstones of therapy, but achieving long-term lifestyle modification is not free from difficulties. Pharmacologic therapy should be considered for patients with NAFLD unable to achieve or maintain lifestyle-induced weight loss. Unfortunately, there is no approved drug for NAFLD currently. Current treatment methods for NAFLD can be divided roughly into those methods that target components of metabolic syndrome using weight reduction and insulin sensitizers (pioglitazone) and those that use antioxidants (Vitamin E) to benefit the liver. Pioglitazone has been shown to improve steatosis, hepatocellular ballooning, and inflammation and also to reduce the risk of fibrosis progression in several randomized-controlled trials (RCTs). In a large RCT, large doses of vitamin E improved all histological lesions except for fibrosis. Compared with a placebo, Metformin lowered ALT, but did not improve liver histology. Recently, novel anti-diabetic agents (GLP-1 analogues, DPP IV inhibitors) and probiotics that alter the gut microbiome were shown to mildly benefit ALT and liver histology. In this report, we systemically review current pharmacologic therapies and other promising agents that were not considered in the most recent guidelines for the treatment of NAFLD.

Emphysema is a category of chronic obstructive pulmonary disease characterized by chronic airflow limitation and hyperinflation. Several decades ago, it was found that the surgical resection of emphysematous lung improved lung function and the long-term survival in patients with severe advanced emphysema. However, it was associated with substantial postoperative morbidity, such as persistent air-leak and mortality. Therefore, bronchoscopic lung volume reduction (BLVR) techniques have been developed to avoid these risks associated with surgery. Of these, endobronchial valves have been subject to the largest number of clinical trials. Endobronchial valves are unidirectional valves placed in the bronchus of the most hyperinflated lobe to block regional inflation, while allowing exhalation, leading to atelectasis of the target lobe. BLVR using endobronchial valves improves lung function, exercise tolerance, and symptoms. To obtain the maximum clinical benefits, it is important to select patients who have a complete lobar fissure and heterogeneous emphysematous lung involvement. Other bronchoscopic approaches are being developed to overcome these limitations.

열대 아열대 지역 국가를 여행한 후 의사를 방문하게 되는 가장 흔한 원인 중 하나가 발열이다. 최근 질병관리본부 자료에 의하면 우리나라 국민의 해외여행 후 감염증은 뎅기열 34.1%, 세균성 이질 24.3%, 말라리아 15.8%의 순이었다. 해외여행지에서 흔히 발생하는 여러 감염병 정보와 여행력에 대한 체계적인 문진, 감염원에 대한 노출 정보를 평가하고 신체검사를 시행한다. 열대열 말라리아는 신속히 진단하여 치료를 해야 하는 중증 감염증으로 말초혈액도말검사, 신속 진단검사와 PCR 검사로 정확하게 진단할 수 있다. 의료인은 열대 아열대 지역 국가에서 발생하는 감염병에 대한 최신 정보를 습득하여, 해외여행을 계획하고 있는 여행객에게 여러가지 감염병 발생정보의 제공과 예방접종, 말라리아 예방약을 처방하여 안전한 여행이 될 수 있도록 해야 한다.

Recent studies have identified a new family of ammonia transporters, Rh B glycoprotein (Rhbg) and Rh C glycoprotein (Rhcg). Rhbg and Rhcg are expressed in the collecting duct system of the kidney. Although it remains controversial whether Rhbg contributes to renal ammonia transport, Rhcg expression parallels ammonia excretion in a variety of experimental models. Kim et al. made an animal model of unilateral ureteral obstruction and demonstrated a decrease in urinary ammonia excretion. Immunohistochemistry with quantitative morphometric analysis revealed that total intensity of Rhcg expression significantly decreased in the obstructed kidney, but did not change in the non-obstructed kidney. These results suggest that decreased Rhcg is likely to cause for the impaired renal ammonia excretion in unilateral ureteral obstruction.

목적: 일측 요로폐쇄는 요중 산배설이 감소하는 질환으로 대부분 요 암모니아 배설의 감소에 의한 것으로 알려져 있다. 본 연구는 일측 요로 폐쇄 흰쥐에서 산배설의 변화와 암모니아 운반체인 Rh C Glycoprotein (Rhcg)의 발현 변화를 확인하여 신세뇨관산증의 병인에 암모니아 운반체의 역할을 규명하고자 하였다. 방법: Sprague-Dawley계 웅성 흰쥐를 사용하였으며, 실험군은 24시간 일측 요로폐쇄를 유발시켰다. 대조군과 실험군은 대사케이지에 mineral oil을 처리하여 24시간 동안 요를 수집하였다. Rhcg의 면역조직화학염색을 시행하였고 세포크기, 총 Rhcg 발현, 자유세포면의 Rhcg 발현에 대하여 면역조직화학염색법을 정량화 분석하였다. 결과: 일측 요로폐쇄군에서 혈중 tCO2 농도는 대조군 $27.5{\pm}2.8mEq/L$에 비하여 실험군은 $25.2{\pm}1.0mEq/L$로 감소하였다(p < 0.05). 24시간 요 암모니아 배설은 대조군 0.38{\pm} 0.08 mmol에 비하여 실험군에서 $0.19{\pm}0.03mmol$로 요 암모니아 배설이 감소하였으며(p < 0.05), Urine pH는 양 군 간에 차이가 없어 일측성 요로폐쇄에 의해 신세뇨관산증이 유발되었음을 확인하였다. 반정량적 immunoblot 검사에서 Rhcg의 단백 발현은 양군 간에 유의한 차이가 없었다. 면역조직 화학염색과 정량화 분석 결과 일측 요로폐쇄군에서 Rhcg의 면역반응성은 비폐쇄부위 콩팥에서는 차이가 없었으나 폐쇄부위 콩팥에서는 감소하였다. 결론: 일측 요로폐쇄 흰쥐에서 암모니아 배설 감소와 폐쇄부위 콩팥에서 Rhcg의 면역반응성의 감소가 동반되었다. 따라서 일측 요로폐쇄의 산성화 장애 병인에 암모니아 운반체 Rhcg가 중요한 역할을 할 것으로 생각된다.

목적: 울산 지역 호흡기 알레르기 질환자의 꽃가루를 포함한 주요 흡입항원에 대한 감작률을 조사하고 지역적 꽃가루 분포 특성과 그 임상적 의의를 조사하고자 연구를 시행하였다. 방법: 비염 및 천식으로 진단되었던 환자를 대상으로 꽃가루를 포함한 주요 흡입 알레르겐에 대한 피부시험을 시행하였으며 울산의 3개 지역에서 연중 지속적으로 대기 중 꽃가루 종류와 농도를 측정하였다. 결과: 개별 항원에 대한 감작률은 집먼지진드기에 대해 높았으며 자작나무 15.8%, 오리나무 15.3%, 개암나무 14.2%, 참나무 13.5%, 너도밤나무 10.8%, 쑥 9.6% 순의 감작률을 보였다. 울산에서는 연중 2차례 꽃가루 비산 절정기가 있었으며 봄철에는 소나무, 참나무, 오리나무가 가을철에는 환삼덩굴과 쑥 꽃가루가 높게 측정되었다. 결론: 최근의 기후 및 환경 변화와 함께 알레르기 질환이 증가되고 있으며 흡입 알레르겐에 대한 종류나 감작률도 지속적으로 변하고 있다. 향후 꽃가루 항원에 대한 지속적인 모니터링으로 알레르기 질환을 체계적으로 관리하는 것이 필요하다.

국내에서 내시경역행성췌담도조영술 시행 중 간문맥 삽관이 보고된 증례는 없었다. 황달이 발생한 췌장 두부암 환자에서 내시경역행성췌담도조영술 중 유도선을 따라 삽관 캐뉼라를 삽입하였으나 간문맥 내 삽관이 발생하여 시술을 중단하고 보존적 치료를 통하여 회복된 증례를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

자가면역성 췌장염에 대한 이해가 넓어짐에 따라 대부분의 경우에 수술적 절제 없이 진단되고 있다. 최근에 수술 후 진단된 자가면역성 췌장염은 거의 모두 수술 전 검사에서 췌장의 일부에만 이상 소견을 보이는 국소형으로 영상 검사에서 췌장암으로 오인되었던 경우이다. 저자들은 췌관내 유두상 점액종양과 그 주위로 국소 종괴를 형성하는 자가면역성 췌장염을 수술 전에 낭성 종양에서 발생한 췌장암으로 진단하고 수술한 증례를 경험하여 췌관내 유두상 점액종양과 병발한 자가면역성 췌장염을 국내 최초로 보고한다. 본 증례는 췌장에 국소 종괴형으로 발생한 자가면역성 췌장염이 췌관내 유두상 점액종양 주위로 특히 심한 국소적인 림프형질세포의 침윤 및 섬유화를 일으켜 수술 전 영상 검사에서 낭성 종양을 동반한 췌장암과 감별이 어려운 증례였다.

자발성 근육 내 혈종은 간경변 환자에서 드물지만 치명적인 합병증이다. 저자들은 알코올 간경변 환자에서 자발적으로 발생한 근육 내 혈종 3예를 경험하여 보고하는 바이다. 출혈 혈관에 대하여 반복적인 경동맥 색전술을 시행하였음에도 불구하고 지속되는 출혈과 간부전으로 모두 사망하였다. 현재까지 보고된 간경변 환자에서 발생한 자발성 근육내 혈종 14증례에서 알코올 간경변 환자에서 가장 많이 발생하였으며 빠른 진단과 치료에도 불구하고 사망률이 높았다. 따라서 간경변 환자에서 발생하는 자발성 근육 내 혈종은 드물지만 예후가 매우 불량하여 이의 발생과 치료에 주의를 요한다.

심부정맥혈전증은 폐동맥색전증의 주된 선행 질환이고 폐동맥색전증은 발생 시 환자에게 치명적일 수 있으므로 심부정맥혈전증의 올바른 진단과 치료는 임상적으로 매우 중요하다. 대부분의 심부정맥혈전증은 하지에서 발생하나 약 10%에서는 상지에서 발생하는 것으로 알려져 있으며 상지심부정맥혈전증은 최근 중심정맥관의 사용이 증가하고 영구심장박동기 이식이나 제세동기 삽입 등과 같은 중심정맥으로 접근하는 시술이 늘어나면서 과거보다 유병률이 증가하는 추세이다. 따라서 저자들은 고도비만 남성에서 상지 심부정맥혈전증과 폐동맥색전증이 발생한 예를 경험하였기에 보고하는 바이다.

관상동맥 조영술을 시행받은 75세 여자 환자에서 좌측 대퇴동맥 천자 부위의 가성동맥류가 발생하였다. 초음파 유도압박법으로 가성동맥류의 치료가 실패하였고 가성동맥류의 경부가 짧고 넓어 누출의 위험이 높아 풍선카테터를 통하여 대퇴동맥을 일시적으로 폐쇄하고 경피적 트롬빈 주입요법을 시행하여 치료한 증례를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

Here, we report a case of gastropericardial fistula associated with the treatment of acute pericarditis using non-steroidal anti-inflammatory drugs (NSAIDs) in the presence of gastric cancer. The patient presented with acute pericarditis with pericardial effusion that had progressed into definite gastropericardial fistula with pneumopericardium after the administration of NSAIDs. The patient improved after conservative management. Based on the current case, we advise caution when using NSAIDs to treat acute pericarditis in the presence of gastric cancer or possible gastropericardial fistula.

베체트병에서 발생하는 복부 가성 대동맥류는 그 빈도는 드물지만 주요 사망 원인 중의 하나로 임상적 중요도가 높은 혈관염 합병증이다. 만성적인 혈관의 염증상태로 인하여 수술적 치료 후 재발 및 가성 대동맥류의 형성 빈도가 높아 스텐트 이식편을 이용한 비침습적 방법이 대안으로 제시되고 있다. 저자들은 베체트병으로 진단받은 46세 남자 환자에서 발생한 거대 복부 가성 대동맥류를 수술 위험도와 재발의 위험을 고려하여 스텐트 이식편 삽입으로 3년간 효과적으로 치료된 1예를 경험하였기에 이를 문헌고찰과 함께 보고하는 바이다.

폐에서 발생한 CCAM은 전형적으로 신생아에서 나타나는 드문 선천성 기형이다. 성인에서는 type I CCAM이 다른 호흡기 이상들과 관련하여 극히 드물게 보고되고 있다. 하지만 만성 기침과 같이 서서히 발생하는 증상들이 있거나 영상의학검사에서 낭종 병터가 있는 경우에는 악성 변형 가능성이 있는 선천성 이상 병터일 수 있기 때문에 추가적인 검사가 필요하다. 이러한 병터의 진단과 치료를 위해서 전산화 단층촬영과 수술적 생검이 고려되어야 한다. 저자들은 수술적 절제한 CCAM에서 국내 처음으로 악성 변형을 관찰하였기에 문헌고찰과 함께 보고하는 바이다.

대장내시경 검사 시 경구용 인산염은 폴리에틸렌 글리콘에 비하여 적은 용량으로 더 효과적인 장세척이 가능하나 전해질 이상 및 칼슘-인 결정의 침착에 의한 급성 인산염 신병증이 보고되고 있다. 급성 인산염 신병증의 경우 많은 수의 환자들이 만성 콩팥병으로 진행하고 일부는 말기 신부전으로 진행한다고 알려져 있다. 저자들은 조직 검사에서 진단된 급성 인산염 신병증에서 완전 회복된 환자를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

본 증례는 중증재생불량성빈혈 환자에서 동종 조혈모세포이식 후 발생한 기증세포백혈병에 관한 보고이다. 기증세포백혈병은 드문 질환이지만 동종 조혈모세포이식 후 발생할 수 있는 합병증 중의 하나로 잘 알려져 있으며 진단에 있어서 세포유전적 분석 외에도 최근 short tandem repeats (STR) 혹은 variable number of tandem repeats (VNTR) 등의 분자적 분석기술의 발달로 진단이 용이해질 수 있다. 국내에서는 중증재생불량성빈혈 환자에서 동종골수이식 후에 발생한 기증세포기원의 급성 림프구성 백혈병이 보고된 바가 없고 저자들이 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

Haemophilus aphrophilus는 HACEK군에 속하는 구강정상세균총으로 침습성 감염, 특히 감염심내막염을 주로 일으키고, 또한 골관절 감염을 일으킨다고 알려져 있다. 전 세계적으로 H. aphrophilus에 의한 감염심내막염 및 드물지만 골관절 감염에 대한 보고가 있었으나 H. aphrophilus에 의한 골관절 감염에 대한 국내 보고는 없었다. 저자들은 69세 여자에서 감염심내막염을 동반하지 않는 H. aphrophilus에 의한 흉추 부위 척추 골수염과 경막외 농양을 진단하여 항생제 투여와 수술적 처치를 통해 성공적으로 치료한 사례를 경험하였기에 문헌고찰과 함께 보고하고자 한다.

본 저자들은 문헌에 보고된 바가 없는 패혈증 비브리오균에 의한 위장관염에 이은 급성 화농성 척추염을 경험하였기에 문헌고찰과 함께 보고하는 바이다.

피부근염은 특징적인 근생검 결과와 함께 피부증상과 근위부 근력의 대칭적 약화 및 특징적 근전도 소견 및 CK의 상승 등을 특징으로 하는 질환이다. 드물게 조혈모세포 이식후에 발병하는 경우도 보고되었다. 저자들은 동종 조혈모세포 이식 후 만성 이식편대숙주병이 있는 환자에서 발생한 피부근염의 증례를 경험하여 보고하는 바이다.
