Full-mouth rehabilitation of a patient with severe wear using digital facebow transfer and virtual articulator

디지털 안궁이전과 가상교합기를 이용한 광범위한 치아 마모 환자의 완전구강회복 증례

Abstract

The progressive wear of teeth is a physiologic process of aging, but excessive wear can lead to occlusal disharmony, tooth sensitivity and pulp exposure, decreased masticatory efficiency, and aesthetic problems. It should be treated by evaluating the cause of tooth wear and establishing a stable centric relation with appropriate vertical dimension of occlusion and occlusal plane. The present case report successfully rehabilitated a full-mouth case of a severely worn dentition with the use of digital facebow transfer and virtual articulator.

치아의 점진적인 마모는 노화에 따른 생리적 과정 중 하나이나 과도한 마모는 교합의 부조화, 치아 과민 및 치수 노출과 저작효율감소, 그리고 심미적인 문제를 유발할 수 있다. 본 증례의 환자는 75세 남환으로 전반적인 치아 마모로 인해 본원 보존과에서 의뢰되었고, 임상검사 결과 전 치열에 걸친 심한 치아 마모와 구외 계측 값 기준 5 mm 가량의 수직 고경 소실이 관찰되었고, 예비 인상 채득 후 중심위 상태로 반조절성 교합기에 마운팅하여 진단 분석을 실시한 결과 역시 5 mm 가량의 수직 고경 소실이 관찰되어 심미적 문제와 저작기능을 개선하기 위해 전악 수복치료를 계획하였다. 전악 왁스업한 모형을 스캔하여 CAD 프로그램 상에서 임시보철물을 디자인한 후 밀링하여 임시 보철물을 사전에 제작하였고, 전악 치아 삭제 후 임시 보철물 장착하였다. 임시 보철물 장착 후 경과 관찰한 결과 별다른 불편감 없어 전악 최종 인상 채득 및 교차 마운팅을 시행하였다. 안정된 기존 임시보철물의 형태 및 교합관계를 참고하여 디지털상에서 최종보철물을 디자인하였다.
