지역 특성과 친구의 흡연이 청소년의 습관적 흡연 시작에 미치는 영향 (The Effect of Neighborhood Characteristics and Friends' Smoking Status on the Habitual Smoking Onset in Adolescents)

  • Purpose: The aim of this study was threefold, to longitudinally examine the risk of habitual smoking onset in adolescents, to delineate the effects of neighborhood characteristics and friends' smoking status on the habitual smoking onset, and to investigate whether the association between friends' smoking status and habitual smoking onset was moderated by neighborhood characteristics. Methods: This study conducted multilevel discrete-time survival analysis, using cohort data from the 3rd to 6th waves of the Korean Child and Youth Panel Survey, which excluded habitual smokers, matched with 2010 census data on respondents' residence. Results: Habitual smoking onset risk increased from the 8th to the 11th grade, and then slightly decreased from the 11th to the 12th grade. Friends' smoking status (B = 0.60, p < .001), smoking rate (B = 0.06, p = .038), and the number of tobacco outlets in the respondents' neighborhood (B = 0.51, p = .003) were positively associated with habitual smoking onset risk. Furthermore, the association between friends' smoking status and habitual smoking onset risk was moderated by the number of tobacco outlets in the neighborhood. Specifically, the association was stronger in neighborhoods with more tobacco outlets (B = 0.58, p = .048). Conclusion: Friends' smoking status and living in neighborhoods that are more susceptible to smoking increase the risk of habitual smoking. The number of tobacco outlets in the neighborhood enhances the peer effect of adolescent's smoking behavior. Therefore, policies or interventions designed to reduce youth's tobacco use should focus on not only on reducing peer smoking, but also restricting smoking by adults and the number of neighborhood tobacco outlets.

청소년의 성경험 관련요인 (Factors related to Adolescents' Sexual Experience)

  • Purpose: This study aimed to understand the current situation of Korean adolescents' sexual experience and its characteristics by school level, gender, and related factors to their sexual experience. This could be the basic evidence for the development of various sex education programs for the youth. Methods: This study used the 11th online survey data on youth health behavior (2015) and all 68,043 students who participated in the study were included for the analysis. The binominal logistic regression model was used to identify the characteristics and relevant factors of sexual intercourse experience. Analysis was performed by complex sample design using SPSS/WIN 18.0. The statistical significance was defined as p<.05 with a two-sided test. Results: The factors related to sexual experience of adolescents included grade, sadness and hopelessness, smoking, Internet addiction, and sex education in school, for middle school boys. For middle school girls, grade, residence area, sadness and hopelessness, smoking, drinking, and sex education in school were related. For high school boys, the factors were grade, living with parents, economic situation, sadness and hopelessness, smoking, drinking, and Internet addiction. For high school girls, grade, sadness and hopelessness, smoking, and drinking were found to be related factors. Conclusion: The results of this study suggest that a practical program considering adolescents' level and characteristics should be developed and applied.

Trends in Cigarette Use Behaviors Among Adolescents by Region in Korea

  • Objectives: Understanding recent trends in cigarette smoking among adolescents is important in order to develop strategies to prevent cigarette smoking. The aim of this study was to compare recent trends in cigarette smoking for adolescents living in rural areas, small towns and metropolitan cities in Korea. Methods: The raw data from the Korea Youth Risk Behavior Web-based Survey (KYRBWS) from 2005 to 2009 were used. Data were analyzed by using the method of complex survey data analysis considering complex sampling design. Logistic regression models were used to evaluate significant linear time trends in cigarette smoking. The indicators of cigarette use behaviors were 'current smoking rate', 'frequent smoking rate', 'heavy smoking rate' and 'smoking experience rate before 13 years of age'. All analyses were conducted according to gender. Results: Statistically significant increasing trends in current smoking rate and frequent smoking rate were observed and borderline significant increasing trends in heavy smoking rate were shown among rural boys. Among metropolitan city boys, statistically significant increasing trends were also seen for frequent smoking. Statistically significant decreasing trends in current smoking rate were observed among small town and metropolitan city girls. Smoking experience rate before 13 years of age for rural girls decreased significantly. Conclusions: Cigarette smoking prevalence among adolescents in the rural areas has increased in the last five years especially among boys. Our findings suggest that anti-tobacco program for adolescents should be conducted primarily for those in rural areas.

우리나라 청소년에서 흡연 및 음주상태와 치은증상의 관련성 (The association of smoking and drinking status with gingival symptoms among the adolescents in Korea)

  • Objectives: The purpose of this study was to evaluate the association of smoking and drinking status with gingival symptoms among the adolescents in Korea. Methods:We used the data from the web-based survey of the National Korean Youth Risk Behavior of Korean Center for Disease Control in 2016. The study subjects were 65,528 adolescents from middle- and high schools. Data were analyzed by SPSS Ver. 19.0. Multiple regression analysis was used to assess the factors related to gingival symptoms. Results: We observed 1.52 of odds ratio (OR) for gingival symptoms for those who smoked only without drinking in middle schools. In addition, we had ORs of 1.29 or 1.69 for those who drank only without smoking, or who smoked and drank in the middle schools, respectively, compared to nonsmoking nondrinkers. Risk of gingival symptoms for those who drank only or who smoked and drank in high schools were 1.18 or 1.17 times higher than nonsmoking nondrinkers. Conclusions: Smoking and drinking status among the adolescents are the risk factors to develop gingival symptoms. The problem of smoking and drinking in the adolescents should be considered when establishing the policies and institutions to improve oral health targeting the adolescents.

Factors Related to Increasing Trends in Cigarette Smoking of Adolescent Males in Rural Areas of Korea

  • Objectives: Cigarette smoking prevalence among adolescent males in rural areas of Korea has increased in recent years. The aim of this study was to explore the factors related to increasing trends in cigarette smoking among adolescent males living in rural areas. Methods: The raw data from the Korea Youth Risk Behavior Web-based Survey from 2006 to 2009 were used. Data were analyzed by using the method of complex survey data analysis considering complex sampling design. Multiple logistic regression models were used to explore the factors affecting cigarette smoking. We evaluated the linear time trends in the prevalence of factors that were related to current smoking status and the linear time trends in cigarette smoking in groups stratified by the exposure to each factor using logistic regression models. Finally, we examined the contributions of the factors to the time trends in cigarette smoking by adjusting for each of those factors in the baseline regression models and changes in the adjusted odds ratio by survey year. Results: A statistically significant increasing trend in smoking was observed after adjusting for the factors affecting cigarette smoking. Significant factors related to cigarette use were perceived stress, experience with depression, current alcohol drinking, exposure to secondhand smoke, and academic performance. The factor related to increasing trends in cigarette smoking was academic performance. Conclusions: Stress about academic performance is an important factor affecting the increase in cigarette smoking among adolescent males in a rural area of Korea.

Intention to Quit Smoking among Intermediate and Secondary School Students in Saudi Arabia

  • Background: Smoking is one of the most preventable causes of disease and death, including cancer, and quitting at an early age can reduce smoking-related morbidity and mortality. This study aimed to estimate the prevalence and to identify factors affecting the "intention to quit" among intermediate and secondary school current cigarette smoker students in Al Madinah city, Saudi Arabia. Materials and Methods: This study cohort included 307 current smoker students in a school-based survey. The intention to quit and its related determinants were assessed using a self-administered questionnaire. Results: More than half of the participants were ${\geq}17$ years, and of male gender (54.7%, 77.9% respectively). An intention to quit smoking was reported in 71.7% of participants, and was been significantly associated with: male gender (OR=3.25, 95% CI=1.65-6.41): age at 1st trial of smoking. 10-15 years (OR=2.11, 95% CI=1.03-4.32) along with age of ${\geq}15$ years (OR=3.10, 95% CI=1.20-7.88); days of smoking in the past 30 days (days <10 (OR=2.31, 95% CI=1.23-4.35) along with days ranging from 10-19 days (OR= 3.42, 95% CI=1.18-9.91); knowing that smoking is hazardous to health (OR=3.04, 95% CI=1.42-6.47); and finally, supporting smoking bans in public places (OR=1.89, 95% CI=1.11-3.25). Conclusions: A substantial number of participants were willing to quit smoking. Effective interventions focusing on providing information about the hazards of smoking and prohibiting smoking in public places could help initiate the intention to quit among youth smokers.

  • Viet Nam is a country with the highest rate of adult male smokers in the world. In 2010, the percentage of adult male smokers was 47.4%. Each year in Viet Nam, there are 40,000 lives lost from tobacco-related diseases. The Global Tobacco Youth Survey is an international standardized survey that has been conducted in more than 140 countries. To provide evidence for tobacco control, Viet Nam already conducted two rounds of GYTS in 2003 and 2007. The GYTS in 2014 is the survey's third iteration. This paper aims to document key methodological details and socio-demographic characteristics of the respondents included in the 2014 GYTS in Viet Nam. This survey followed international protocol and was conducted in 13 provinces representative of 6 geographical regions. A total of 3,430 school children, aged 13 to 15 years used a standardized answer sheet to answer 76 questions about seven tobacco-related topics, including prevalence of tobacco use, environmental tobacco smoke, access and availability, media and advertisement, cessation, knowledge and attitudes, tobacco-related school curriculum. This GYTS provides valid and reliable data for monitoring tobacco use among youth in Vietnam and is recommended to be regularly repeated.

Is Exposure to Tobacco Advertising, Promotion and Sponsorship Associated with Initiation of Tobacco Use among Current Tobacco Users in Youth in India?

  • Background: The rise in consumption of tobacco products among youth is a public health concern in India. Several studies have shown that advertisements promoting tobacco products influence decisions and behaviour of youth towards smoking. Objective: To ascertain which method of Tobacco Advertising, Promotion and Sponsorship (TAPS) was more influential for initiating tobacco use in youth in India. Materials and Methods: The secondary data of youth (15-24 years) from nationally representative Global Adult Tobacco Survey (GATS) conducted in 2009-2010 was analyzed. Odds ratio and p-value were used to know the association between TAPS and initiation of use of tobacco products among youth. Logistic regression was used to determine the most significant means of TAPS altering the youth's behaviour towards tobacco products. Results: Out of 13,383 youths, 1,982 (14.7%) used smokeless forms of tobacco and 860 (6.38%) used smoke forms. Logistic regression reveals that promotional activities mainly through cinemas (p<0.05) and providing free samples of tobacco products (p < = .001) were most influential means of initiating consumption of tobacco products among youth. Conclusions: The smoking in youth is associated with watching advertisements particularly in cinema and promotional activities like distribution of free samples, coupons and sales on the price of tobacco products. Stronger legislative measures should be enforced to curb promotional advertisements in cinemas and distribution of free samples.

  • 본 연구의 목적은 '제11차(2015년) 청소년건강행태온라인조사'를 이용하여 흡연을 경험한 청소년의 흡연특성과 흡연유형에 따른 특성을 파악하여 복합흡연에 관련된 요인을 알아보고자 하였다. '제11차(2015년) 청소년건강행태온라인조사'에 응답한 68,043명 중 일반담배 및 전자담배 흡연 경험이 있는 중 고등학생 5,347명을 대상으로 분석하였다. 사용된 데이터는 가중치가 적용된 복합표본분석을 이용하여 빈도분석, 카이제곱 검정, 로지스틱 회귀분석으로 분석하였다. 전체 흡연 청소년의 20%가 복합흡연자였으며 전자담배 이용자 85.6%가 니코틴을 사용하였다. 다중 로지스틱 회귀분석 결과 남학생인 경우(OR 1.63; 95% CI 1.15-2.31), 대도시에 거주하는 경우(OR 1.76; 95% CI 1.32-2.33), 하루에 패스트푸드를 1-2회 먹는 경우(OR 2.48; 95% CI 1.33-4.64), 최초흡연 경험 시기가 고등학교인 경우(OR 0.76; 95% CI 0.58-0.98), 간접흡연 경험이 있는 경우(OR 1.24; 95% CI 1.07-1.44), 음주경험이 있는 경우(OR 2.03; 95% CI 1.65-2.48), 일주일간 용돈수준이 높을수록 복합흡연의 위험이 높았다. 본 연구는 청소년의 전자담배와 복합흡연에 대한 기초자료를 제공하고 금연 프로그램 및 정책 개발에 도움이 되리라 판단된다.

청소년 흡연중재 교육프로그램의 효과평가에 관한 설명적 문헌고찰 (Descriptive literature review on the evaluation of the youth smoking prevention program effectiveness)

  • The age of the very first smoking is a significant indicator of life-long health status. Smoking prevention intervention was actively conducted based in middle and high schools with the support of the Korean Act for Health Promotion since 1998. These governmental supports became large and various smoking prevention programs were developed and conducted in diverse standards and perspectives. This study made a comprehensive descriptive literature review on smoking prevention educational programs for youth in Korea to identify the intervention quality and effectiveness of them for future smoking prevention program development. A total of 28 peer-reviewed journal articles published between May, 1995 and May 2005 (for the last 10 years) were finally included in this review process. The study participants should be adolescences in middle or high schools in Korea and the intervention types of the review studies should be smoking prevention or cessation educational programs. The outcomes of the reviewed studies were reanalyzed by the participants' characteristics, theory basement, evaluation design, intervention period, evaluation outcomes, and program effectiveness Most smoking intervention programs were not specified by sex and smoking status. Largely most intervention programs focused on male students although female students' smoking percent is increasing including both smokers and nonsmokers. Based on the school grades, the large percents of smoking intervention were conducted to the first grade of middle school and the first grade of high school. Almost 70% of the reviewed studies did not apply any health behavior change theories and the Transtheoretical model and social cognitive theory were utilized in 7 studies among the last reviewed 30%. The theory-based intervention studies had greater effectiveness than the non-theory based studies. More than 90% had quasi-experimental evaluation design and the effectiveness of the non-experimental designed study seemed over estimated than the quasi-experimental or experimental designed studies. More than 60% of the reviewed studies made their education for less than j days, over a short period and the evaluation factors were knowledge and attitude in general which can be obtained in short intervention period. Therefore, smoking intervention programs for youth in Korea need to be modified in terms of research design such as the intervention period, intervention-evaluation design, theory-based approach, and population-focused intervention specification.
