Smoking-cessation Model for Male Patients with Coronary Heart Disease (남성 관상동맥질환자의 금연모형 구축)

  • purpose : The purpose of this study was to find out the influencing factors of smoking-cessation behavior of patients with coronary heart disease and to suggest the model of smoking-cessation behavior which was based on the relationship between influencing factors and then to test its fitness empirically. method : This study was based on the Theory of Reasoned Action and a hypothetical model was constructed with fifteen paths in consideration of main predictive factors of smoking-cessation behavior such as biological factor, disease-related characteristics, self-efficacy, supportive factor, environmental factor, disease-related perception factor, intention-to-quit, and psychological factor. The validity of a smoking- cessation model was tested to 264 patients with coronary heart disease by using SPSS 8.0 and Window LISREL 8.12a. results : 1. Seven of the 15 paths of smoking-cessation behavior proved to be significant. 2. The final model excluded three paths in the hypothetical model was demonstrated to be improved by $x^2$=44.31 (df=38, p=.22), Goodness of Fit Index (GFI)=.98, Adjusted Goodness of Fit Index (AGFI)=.96, Non-Normed Fit Index(NNFI)=1.00, Normed Fit Index(NFI)=1.00, and Root Mean Square Residual(RMR)=.24. 3.The smoking-cessation behavior was influenced directly by biological factor, self-efficacy, supportive factor, environmental factor, intention-to-quit, and psychological factor. The smoking-cessation behavior was accounted for 82% of variance by these factors. conclusion : although the adolescents' smoking behavior can be predicted by only smoking intention, it is hard to predict the adults' smoking-cessation behavior by only this factor. Therefore, intention-to-quit, self-efficacy, supportive factor should be improved because these are promotive factors for smoking-cessation behavior. Biological factor, environmental factor, and psychological factor are inhibitive factors, so nicotine replacement therapy is helpful to the high nicotine-dependents, and ex-smokers avoid other smokers in their environment and also patients should learn and practice the stress coping-skills.

  • A 26-year-old man with a one-year history of asthma and sinusitis presented with bilateral pleural effusions, patch basilar infiltrates on a chest x-ray and a pericardial effusion on an echocardiogram. The peripheral blood showed marked eosinophilia. An obstructive pattern was also observed during the pulmonary fuction test, which was responsive to bronchodilator inhalation. Nerve conduction studies showed right sural neuropathy. Thoracentesis yielded an acidotic exudative effusion with low glucose, low $C_3$ and eosinophilia. An open lung biopsy revealed an eosinophilic interstitial pneumonitis associated with a necrotizing eosinophilic vasculitis, and granulomatous inflammation foci. In the literature, pleural effusions were reported in 29 percent of Churg-Strauss patients, but the number of effusions was low and their characteristics have not been well described. This report describes the characteristic findings of pleural fluid and its histologic features in a case of classical Churg-Strauss syndrome.

A Case of Neuroblastoma Presenting with Sudden Blindness (실명을 주소로 한 신경아세포종 1예)

  • Neuroblastoma is the most common extracranial solid tumor of childhood which presents various clinical symptoms depending on the primary and metastatic sites. However, it has been rarely reported that sudden onset of blindness was the chief complaint of neuroblastoma. A four years old boy was admitted to the Yeungnam University Hospital with the chief complaint of a sudden onset of blindness due to a distant metastasis of abdominal neuroblastoma to the sphenoid sinus. On admission, both side pupils were dilated without light reflex, fundoscopy showed pale optic disk, electroretinogram was subnormal and visual evoked potential showed no response. The liver was palpable in $3{\frac{1}{2}}$ finger breadth from the right costal margin and adult fist sized mass was palpable in the right flank. Skull X-ray showed destructed sphenoid bone and clinoid process and brain CT scan showed tumor mass in the sphenoid sinus and left orbit. Ultrasonogram and CT scan of the abdomen showed large tumor masses around the right kidney and para-aortic and retropancreatic lymph node. IVP showed displaced right calyceal system with preserved contour. Left supraclavicular lymph node which appeared after admission was biopsied and it showed poorly differentiated neuroblasts. He was treated according to the multiagent chemotherapy schedule for stage IV neuroblastoma patient of children's cancer study group. Abdominal tumor masses and sphenoid sinus mass were markedly reduced after 2 courses of the combination chemotherapy of cyclophosphamide, vincristine, DTIC, adriamycin and VM-26. Eventhough the blindness was not improved, the patient has been in good clinical condition.

Factors Affecting Active Early Detection Behaviors of Breast Cancer in Outpatients (외래내원 여성의 적극적 유방암 조기검진행위 영향 요인)

  • Purpose: This study was done to evaluate factors affecting active early detection behaviors of breast cancer and performance rate of breast self examination (BSE), physical examination and mammography. Methods: The participants were 264 women from an outpatient breast clinic of a university hospital and materials were collected from March 2007 to February 2008 using a structured questionnaire. The data were analyzed using $x^2$ test, logistic analysis. Results: The rate for BSE was 58.3%, for physical examination, 55.3% and for mammography experience, 63.4%. Women with all of these active early detection behaviors accounted for 31.8% of the participants. Various factors such as age, income, marital status, and menopause showed increased significant performance rate. The explanation power of logistic model was 48.5%, and was significant for age, income and health belief. Factors related to high performance rate were being over 40 years of age, high income and high health belief score. Conclusion: Active early detection behaviors were not high in spite of marked increases in breast cancer incidence. Encouragement for women practicing early detection behavior is important, but there is also a need to develop interest and support for the low performance group. More sustained education and public relations are needed to further improve active early detection behavior.

Overexpression of CXCL2 inhibits cell proliferation and promotes apoptosis in hepatocellular carcinoma

  • C-X-C motif chemokine ligand 2 (CXCL2) is a small secreted protein that exhibits a structure similar to the proangiogenic subgroup of the CXC chemokine family. Recently, accumulating evidence suggests that chemokines play a pivotal role in cancer progression and carcinogenesis. We examined the expression levels of 7 types of $ELR^+$ CXCLs messenger RNA (mRNA) in 264 clinical samples. We found that CXCL2 expression was stably down-regulated in 94% of hepatocellular carcinoma (HCC) specimens compared with paired adjacent normal liver tissues and some HCC cell lines. Moreover, CXCL2 overexpression profoundly attenuated HCC cell proliferation and growth and induced apoptosis in vitro. In animal studies, we found that overexpressing CXCL2 by lentivirus also apparently inhibited the size and weight of subcutaneous tumours in nude mice. Furthermore, we demonstrated that CXCL2 induced HCC cell apoptosis via both nuclear and mitochondrial apoptosis pathways. Our results indicate that CXCL2 negatively regulates the cell cycle in HCC cells via the ERK1/2 signalling pathway. These results provide new insights into HCC and may ultimately lead to the discovery of innovative therapeutic approaches of HCC.

Occurrence of Fusarium wilt and Twospotted Spider Mite under Plastic Mulched and Non-Plastic Mulched Bed in Hydroponic Culture of Strawberry (딸기 수경재배에서 베드 피복유무에 따른 시들음병과 점박이응애 발생 양상)

  • Hydroponic strawberry culture system is increasing annually. Most of strawberry farmers use mulched bed in hydroponic culture and strawberry plants were transplanted in early September. After transplanting, Fusarium wilt caused by Fusarium oxysporum f. sp. fragariae and twospotted spider mite (TSSM), Tetranychus urticae, can increase their occurrence under high temperature condition. Therefore, we conducted for comparison occurrence of Fusarium wilt and TSSM on mulched with green polyethylene film and non-mulched bed. Occurrence of Fusarium wilt on mulched bed was started from early October and more increase than non-mulched bed. Damage rate of TSSM on mulched bed was shown higher than non-mulched bed. Temperature of substrate in mulched bed increased than non-mulched bed, but relative humidity near plants was decreased. As a result, use of non-mulched bed should be effective for reducing of Fusarium wilt and TSSM on strawberry plants.

Evaluation of health screening data for factors associated with peri-implant bone loss

  • Purpose: Systemic health has a profound effect on dental treatment. The aim of this study was to evaluate peri-implant bone loss and health screening data to discover factors that may influence peri-implant diseases. Methods: This study analyzed the panoramic X-rays of patients undergoing health screenings at the Health Promotion Center at Seoul St. Mary's Hospital in 2018, to investigate the relationship between laboratory test results and dental data. The patients' physical data, such as height, weight, blood pressure, hematological and urine analysis data, smoking habits, number of remaining teeth, alveolar bone level, number of implants, and degree of bone loss around the implant, were analyzed for correlations. Their associations with glycated hemoglobin, glucose, blood urea nitrogen (BUN), creatinine, and severity of periodontitis were evaluated using univariate and multivariate regression analysis. Results: In total, 2,264 patients opted in for dental health examinations, of whom 752 (33.2%) had undergone dental implant treatment. These 752 patients had a total of 2,658 implants, and 129 (17.1%) had 1 or more implants with peri-implant bone loss of 2 mm or more. The number of these implants was 204 (7%). Body mass index and smoking were not correlated with peri-implant bone loss. Stepwise multivariate regression analysis revealed that the severity of periodontal bone loss (moderate bone loss: odds ratio [OR], 3.154; 95% confidence interval [CI], 1.175-8.475 and severe bone loss: OR, 7.751; 95% CI, 3.003-20) and BUN (OR, 1.082; 95% CI, 1.027-1.141) showed statistically significant predictive value. The severity of periodontitis showed greater predictive value than the biochemical parameters of blood glucose, renal function, and liver function. Conclusions: The results of this study showed that periodontal bone loss was a predictor of peri-implant bone loss, suggesting that periodontal disease should be controlled before dental treatment. Diligent maintenance care is recommended for patients with moderate to severe periodontal bone loss.

Effect of Cold Plasma on Total Polyphenol Content and Anti-Inflammatory Activities of Peanut (Arachis hypogaea L.) Hull

  • In recent studies, cold plasma has been used to induce exudation of polyphenols and flavonoids from food materials, leading to enhancement of functional properties. And it is known that polyphenols interact with inflammation related metabolism. The objectives of this study were to investigate the effects of cold plasma treatments on the increase of total phenolic content (TPC), total flavonoid content (TFC), and anti-inflammatory activities of 'Sinpalkwang' peanut (Arachis hypogaea L.) hull. Plasma treatments were carried out using a dielectric barrier discharge gas exchange system at different radicals and temperatures (O3-25℃, O3-150℃, NOx-150℃). Significant differences in TPC, TFC, and inflammatory mediator such as nitric oxide (NO) and tumor necrosis factor a (TNF-α) in lipopolysaccharide stimulated Raw 264.7 macrophages were observed between treated and non-treated peanut hull samples (p < 0.001). Cold plasma treated samples showed higher content (TPC: 2.87-2.93 mg/g sample, TFC: 0.96-0.98 mg/g sample) than non-treated sample (TPC: 2.47 mg/g sample, TFC: 0.78 mg/g sample). Cold plasma treated samples showed lower content of NO (3.3-5.0 uM) and TNF-α (141.4-162.2 ng/mL) than non-treated sample (NO: 11.1 uM, TNF-α: 210.2 ng/mL). This study suggests that cold plasma has potential to improve functionalities of food materials and that cold plasma treated peanut hull can be used as immune enhancing materials.

The Effect of Dietary Protein and Calcium on Urinary Calcium in Young Men (식이단백질(食餌蛋白質)과 칼슘이 인체(人體)의 뇨중(尿中) 칼슘양에 미치는 영향(影響))

  • Studies were carried out on seven young, adult caucasian males to determine the short-term effects of protein(animal or plant)and calcium intakes on the excretion of urinary calcium. The subjects were studied on a self-selected diet for a period of seven days. Mean daily protein and calcium intakes were $103{\pm}31\;g$ and $1237{\pm}594mg$ respectively. Variation among subjects in the mean urinary calcium excretion per 24 hour was from $121{\pm}40$ to $258{\pm}104mg$. When the protein intake of all aubjcts was divided in to four levels : low (x=53g), medium (87g), intermediate (117g) and high(153g), The mean urinary calcium was $179{\pm}53$, $189{\pm}73$, $184{\pm}55$ and $264{\pm}84mg$, respectively. Urinary calcium increased significantly with an increase in protein intake. The calcium excretion was seriously increased with the protein intake above the intermediate level. Animal protein intake was more closely related to urinary calcium excretion than plant protein. There was a significant difference in the urinary calcium excretion when calcium intakes increased from low(x=544mg) to interne-diate levels (1232 mg). However, the difference between intermediate and high levels (1834mg) was not significant. Urinary calcium was $169{\pm}46mg$ on the low calcium diet, $196{\pm}71mg$ on the medium, and $222{\pm}21mg$ on the intermediate calcium intake, Calcium excretion was more closely related to changes in protein intake than to changes in calcium intake. Some nutritional implications can be drawn from this research. Protein intakes above the intermediate level (117g) are not recommendable. An over intake of calcium, however, may not be a serious problem for the calcium balance.

Effects of Activated Carbon Types and Service Life on Removal of Odorous Compounds: Geosmin and 2-MIB (활성탄 재질과 사용연수에 따른 Geosmin과 MIB 흡착특성)

  • Adsorption performance of odorous compounds such as geosmin and 2-MIB on granular activated carbon were evaluated in this study. The coal-based activated carbon was found more effective than other carbons in adsorption of geosmin and 2-MIB. The wood-based virgin activated carbon was less effective than coconut- and coal-based carbon in adsorption nevertheless having larger pore volume and specific surface area than others carbons. The maximum adsorption capacity(X/M) of coal-based activated carbon for geosmin and 2-MIB was $1.2\sim1.9$ and $2.1\sim2.6$ times larger than coconut- and wood-based virgin activated carbon, respectively. Carbon usage rate (CUR) of coal-, coconut- and wood-based virgin activated carbons for geosmin and 2-MIB were 1.72 and 1.44 g/day, 1.72 and 2.05 g/day and 2.12 and 1.90 g/day, respectively. In the evaluation of adsorption isotherm of geosmin and 2-MIB for coal-, coconut- and wood-based virgin activated carbons, k value of 2-MIB was lower than geosmin, It menas 2-MIB is more difficult to remove by activated carbon adsorption than geosmin. The relationship of max. adsorption versus total pore volume of coconut- and wood-based virgin and used activated carbon for geosmin and 2-MIB were $y=264,459\times-79,047(R^2=0.95)$, $y=319,650\times-101,762(R^2=0.93)$.
