선부자를 적용한 하악골 골절환자의 치주조직에 기능성 치약이 미치는 영향 (THE EFFECT OF GINGIVAL GEL ON PERIODONTIUM IN MANDIBULAR FRACTURE PATIENTS APPLIED BY ARCH BAR)

  • For many years, intermaxillary fixation using arch bar has been operated in treatment of mandibular fracture patients. But it has many complications including injury of operators and assistants cause by wire, inflammation of periodontium. For that reasons alternatives are required; osteosynthesis technique using mini plate, intermaxillary fixation using IMF screws have been available. Treatment by arch bar fixation, however, is still valuable to treat craniomaxillary fracture patients. The purpose of this study is to know effect arch bar on periodontium and influence gingival gel on periodontium applied by arch bar. 40 mandibular fracture patients are monitored. 30 patients were applied by arch bar, 10 patients were not. And the former were classified by 3 categories; Nano vitamin and Mastic gel were applied to 10 patients respectively and any gingival gel was not used to 10 patients. Clinical attachment level, bleeding on probing and periodontal depth of each group were measured and compared before operation and on 2 weeks and 6 weeks after operation. Mann-Whitney U test was used to analyze result which leads to this conclusion. 1. Whether arch bar is applied or not, treatment of mandlbular fracture gave rise to gingivitis, but 6 weeks after operation, gingivitis is restored to the same level as the state before operation. 2. More severe gingivitis appeared when arch bar is applied to mandibular fracture than when it is not. 3. Both gingival gel used in this study can reduce gingivitis which can be caused by arch bar. 4. In this study, Mastic gel is more effective for prevent gingival inflammation cause by arch bar than nano vitamin. In regard to this result, gingivitis is considered to be available because it is reversible and does not induce periodontal disease. Gingival gel is regarded to be helpful for patients applied by arch bar to feel less discomfort.

악간고정 없는 하악골 골절의 관혈적 정복술: 후향적 연구 (Open reduction of mandibular fracture without maxillomandibular fixation: retrospective study)

  • Introduction: Maxillomandibular fixation (MMF) is essential before surgery under general anesthesia in maxillofacial trauma patients. MMF is used basically to reconstruct the occlusion and occlusal stability to recover the facial shape and oral functions. The arch bar and wire is a traditional method for MMF, but it can not only bring pressure to the periodontal ligaments and teeth but also cause a penetrating injury to the surgeons. Materials and Methods: In this study, 198 patients with an open reduction using a manual reduction without MMF from September 2005 to May 2010 in Dankook University Dental Hospital were subjected to a follow-up evaluation during the postoperative 4 months periods. This study evaluated the incidence of complications according to the condition of the patient (gender, age), the state of bony union of the fracture sites and a numeric rating scale evaluation for postoperative pain scoring. Results: 1. The complications were classified into major and minor according to the seriousness, and the major complication rate was as low as 2.02%. Only 2 cases of re-operations (1.01%) were encountered. In the classification according to the fracture line, plate fracture was observed in both cases of mandibular symphysis fracture, and angle fractures and loosening of two screws were noted in the case of mandibular angle fracture. 2. The complication rate was similar regardless of gender and age. 3. The degree of bony union was satisfactory, and the complication rate was reduced as the bony union improved. 4. More patients complained of pain as the operation time was increased. Conclusion: The use of MMF is not always necessary if a skilled assistant is provided to help manually reduce the fracture site. Compared to other studies of mandibular fracture surgery using MMF, the complication rate was similar using only manual reduction and the patients' discomfort was reduced without MMF.

악간고정용 스크류를 이용한 위턱뼈 이마돌기 골절편의 효과적인 정복 (Effective Reduction of the Frontal Process of the Maxillary Fracture Segment Using Intermaxillary Fixation Screw)

  • Purpose: It is relatively unusual that infraorbital rim fracture is accompanied by nasal bone fracture. In order to correct effectively, subciliary approach and intranasal manipulation are applied simultaneously. But if reduction is not successful, intranasal manipulation may become aggressive and this often causes complications. We introduce a method using intermaxillary fixation screws for decreasing such complications and effective reduction of fracture. Methods: Total seven patients with fracture of frontal process of maxilla were treated with this method. The fracture site was exposed through the subciliary approach, and one or two screws were inserted into the displaced fracture fragment. During the traction of the screws using the wire, the fracture fragment was pushed upward from the intranasal side using an elevator supplementarily and fixed with a plate and the screws. Results: In all patients, the fracture fragment was reduced successfully and no complication occurred during one year's postoperative follow-up. Conclusion: When reduction cannot be attained through a bone hook or an elevator alone, reduction of fracture fragment can be done easily using intermaxillary fixation screws. This method is less likely to cause a mucosal injury because intranasal manipulation is not aggressive. Furthermore, as the screw can be inserted and removed easily, this method is considered effective not only for fracture of frontal process of maxilla but also for fractures in other regions.

Bipolar Hemiarthroplasty in Unstable Intertrochanteric Fractures with an Effective Wiring Technique

  • Purpose: Bipolar hemiarthroplasty has recently been acknowledged as an effective option for treatment of unstable intertrochanteric fracture. Trochanteric fragment nonunion can cause postoperative weakness of the abductor muscle and dislocation; therefore, reduction and fixation of the fragment is essential. The purpose of this study was to perform an evaluation and analysis of the outcomes of bipolar hemiarthroplasty using a useful wiring technique for management of unstable intertrochanteric fractures. Materials and Methods: A total of 217 patients who underwent bipolar hemiarthroplasty using a cementless stem and a wiring technique for management of unstable intertrochanteric femoral fractures (AO/OTA classification 31-A2) at our hospital from January 2017 to December 2020 were included in this study. Evaluation of clinical outcomes was performed using the Harris hip score (HHS) and the ambulatory capacity reported by patients was classified according to Koval stage at six months postoperatively. Evaluation of radiologic outcomes for subsidence, breakage of wiring, and loosening was also performed using plain radiographs at six months postoperatively. Results: Among 217 patients, five patients died during the follow-up period as a result of problems unrelated to the operation. The mean HHS was 75±12 and the mean Koval category before the injury was 2.5±1.8. A broken wire was detected around the greater trochanter and lesser trochanter in 25 patients (11.5%). The mean distance of stem subsidence was 2.2±1.7 mm. Conclusion: Our wiring fixation technique can be regarded as an effective additional surgical option for fixation of trochanteric fracture fragments during performance of bipolar hemiarthroplasty.

Mersilene tape와 K-강선을 이용한 제 II형 원위부 쇄골 골절의 수술적 치료 (Surgical Treatment for Type 11 Distal Clavicle Fracture using Mersilene tape and K-wire)

  • 목적: 제 II형 원위부 쇄골 골절은 불유합과 지연 유합의 비율이 높아 수술적 치료의 대상이 된다. 저자들은 제 II형 원위부 쇄골 골절에 대해 Mersilene tape과 K-강선을 이용한 수술적 치료를 하여 좋은 결과를 얻었기에 보고하는 바이다. 대상 및 방법: 1999년부터 2003년까지 시행한 제 II형 원위부 쇄골 골절 환자 11예를 대상으로 Mersliene tape을 이용한 골절의 정복과 오구 쇄골 인대 봉합 그리고, 골절 부위에 K-강선을 이용한 고정을 시행하였다. 모든 환자에서 최소 12개월 이상의 추시관찰을 하였으며 최종 추시 상 기능적, 방사선적 평가를 시행하였다. 방사선적 평가는 최종 추시 상 전후면 사진에서 골유합 유무로, 기능적 평가는 UCLA scoring system과 Constant scoring system을 사용하였다. 결과: 전례의 환자에서 수술 후 11 $(10\sim13)$주에 골유합을 얻을 수 있었다. 11예에서 UCLA scoring system상 평균 33 $(31\sim35)$점, Constant soring system에서 평균 96 $(91\sim100)$점으로 만족할만한 결과를 얻었다. 결론: 이 술기는 비교적 간단하며, 견고한 고정을 할 수 있고, 조기 재활을 통하여 환자의 만족도를 높일 수 있는 술식이라 생각되며 이의 임상적 적용을 추천하는 바이다.

Benoist씨 장치를 이용한 소아의 하악 과두 골절의 치험례 (MECHANO THERAPY OF PEDIATRIC CONDYLAR FRACTURES USING BENOIST'S APPLIANCCE : A CASE REPORT)

  • 소아에서의 하악 골절은 성인에서보다 드물다. 그러나, 어릴수록 강직과 성장장애의 가능성이 크며 성인만큼 악간고정을 잘 견뎌내지 못한다. 반면에, 골절은 소아에 있어 더 빨리 치유되고 합병증도 적다. 소아에서 하악 골절은 종종 하악의 acrylic splint therapy 단독 또는 eyelet wire 와 악간 고정을 같이 사용하여 성공적으로 치료될 수 있다. 발생할 수 있는 심각한 합병증으로는 유착과 성장장애가 있다. 이러한 합병증의 빈도와 심각성은 좀더 짧은 기간의 악간고정과 긴밀한 술 후 관리에 의해 감소될 수 있다. 특히 소아 환자가 악간고정을 잘 견디지 못하는 경우, 악간고정의 기간이 길어지는 경우, 환자의 자발적인 운동요법이 어려운 경우 악간고정이 불필요하고 개구장애 만을 일으킨 미약한 증상의 경우에는 동기부여에 의한 물리적 하악 운동요법이 사용될 수 있다. 본 종례에서는 하악정중부의 외상으로 인하여 양측성 과두 골절이 일어난 6세 여자 환아를 대상으로 상, 하악에 elastic을 이용하여 하악의 개구, 측방, 전방운동에 대해 훈련을 가능하게 하는 Benoist씨 장치를 이용한 능동적 하악 운동 요법으로 7개월 간의 정기 검진을 통하여 하악 운동량의 증진 과두 유착의 방지, 골개조의 치료효과를 보였다.

소아 원위지골 기저부에서 발생한 Seymour씨 골절의 치험례 (Seymour's Fracture of the Base of the Distal Phalanx in a Child)

  • Purpose: Prior to closure of the epiphysis of the distal phalanx, fracture usually occurs through the growth plate, Salter-Harris type I or II, or through the juxtaepiphyseal region 1 to 2 mm distal to the growth plate. The terminal tendon of extensor inserts into the epiphysis only, while insertion site of the flexor digitorum profundus spans both the epiphysis and metaphysis. Because of the difference between these tendon insertions, this injury mimics a mallet deformity. But, this type of injury does not involve a tear or avulsion of the extensor, unlike mallet finger of adults. Seymour was the first to describe this type of injury in children and called after his name, Seymour's fracture. This fracture is prone to infection or remain the residual deformity unless adequate treatment. Methods: We report a case of Seymour's fracture. A 9-year-old boy presented a laceration of the nail matrix, with the nail lies degloved from the nail fold on the right middle finger gotten from an impact against a door. An X-ray examination showed the fracture line lying 1 mm distal to the growth plate. The injury was treated with debridement and the fracture was reduced by applying hyperextension force. Under the C-arm, a single 0.7 mm K-wire was used to immobilize the distal interphalangeal joint. Intravenous antibiotics were applied for 5 days after surgery. Results: The K-wire was removed in the 3rd week. No infection or significant deformity was found until follow-up of 12 months. Conclusions: Seymour's fracture may be at first classically mallet deformity by its appearance. But it is anatomically different and more problematic injury. If it isn't corrected at the time of injury, derangement of the extensor mechanism, and growth deformity of the distal phalanx may occur. The fracture site should be debrided, removed of any interposed soft tissue, and the patient should be given appropriate antibiotics. Reduction should be maintained by K-wire fixation. We experienced no infection or premature epiphyseal closure.

Successful Treatment Using Wire-reinforced Interdental Splint for a Puppy with Rostral Mandibular Fractures

  • A five-month-old toy poodle was referred for treatment of bilateral mandibular fractures. Preoperative skull and dental radiographs showed rostral mandibular fractures and permanent teeth that had not yet erupted. In addition to providing stability for the healing of fractures, it is very important to focus on restoring normal occlusion. The mandibular fractures were repaired using interdental wiring in a dentate area where the deciduous canines and fourth premolars were stable and could be used to anchor the interdental fixation. In the absence of an erupted permanent mandibular first molar, the modified Risdon wiring was performed using the deciduous mandibular fourth premolars as anchor teeth. An intraoral splint was placed on the mandibular teeth using self-curing composite resin. Five months after surgery, the patient had experienced no complications, and the permanent teeth had erupted normally. In this case, which involves a small-breed puppy with deciduous dentition, treatment with a wire-reinforced interdental splint using intact deciduous fourth premolar teeth as anchor teeth can be considered as a suitable method for rostral mandibular fracture restoration.

외과적 하악 후퇴술 후 악간고정기간 중의 골격성 재발과 치열의 변화 (Skeletal relapse and dental change during intermaxillary fixation after mandibular setback)

  • 하악 후퇴술 후의 골격성 재발과 치열 변화는 악간고정(intermaxillary fixation, IMF)제거 후 뿐만 아니라 악간고정기간 중에도 일어난다고 보고되고 있다. 악간고정기간 중에 일어나는 골격성 재발에 따른 골편간 견고한 융합의 방해, 보상성 치아이동, 전치부 개방교합 등과 같은 여러 가지 부작용은 술후 교정치료에 많은 영향을 미칠 수 있으므로 임상적으로 중요한 의미를 갖는다. 본 연구는 악간고정기간 중에 일어나는 골격성 재발과 치열의 변화를 알아보고자 시행하였으며, 하악전돌증의 치료를 위해서 양측성 하악지 시상분할골절단술(bilateral sagittal split ramus osteotomy, BSSRO)과 강선고정 (wire fixation), 악간고정을 시행한 28명을 대상으로 하였다. 본 연구에서는 측모 두부 방사선 사진을 통하여 수술에 의한 골편들의 이동량과 방향, 악간고정기간 중의 골격성 재발과 치열 변화의 양과 방향을 측정하였다. 또한 수술에 의한 골편들의 이동과 골격성 재발과의 상관관계, 골격성 재발과 치열 변화간의 상관관계를 평가하였다. 본 연구를 통하여 다음과 같은 결론을 얻었다. 1. 수술에 의한 골편의 이동방향은 원심골편은 후상방으로, 근심골편은 시계방향으로 회전하였다. 2. 악간고정기간 중에 원심골편의 전방부는 후방 변위, 후방부는 상방 변위하였으며 근심골편은 상방 변위와 함께 근심골편의 gonion(p-Go)부위는 전방 변위를 하였다. 악간고정기간중에 근심골편의 gonion(p-Go)의 전방 변위는 수술에 의한 근심골편의 gonion (p-Go)의 후방으로의 이동량과 유의한 상관관계를 보였다. 즉, 수술에 의한 근심골편의 gonion(p-Go)의 후방으로의 이동이 많을수록 악간고정기간 중의 근심골편의 gonion(p-Go)의 전방 변위는 증가하였다. 3. 악간고정기간 중에 수평피개, 수직피개는 변화가 없었다. 상악 전치부는 후방경사(retroclination), 하악 전치부는 후방경사, 정출이 일어났으며, 이러한 보상성 치아이동은 악간고정기간 중의 근심골편의 gonion(p-Go)의 상방 변위와 유의한 상관관계를 보였다. 즉, 악간고정기간 중의 근심골편의 gonion(p-Go)의 상방 변위가 클수록 보상성 치아 이동량도 증가하였다.

단측성 외고정 기구를 이용한 개의 대퇴골 신연에 있어서 골수강내 K-강선 삽입의 효과 (Effects of Intramedullary K-wire Insertion on Femoral Lengthening in Canine with Monolateral External Fixator)

  • During long bone lengthening, there are many disadvantages including axial deviation, malalignment and re-fracture which are commonly encountered inspite of its proven abilities. To study the effects of intramedullary K-wire application on the lengthening of long bone, ten skeletally mature mongrel dogs were separated into two groups(Group I, II). Right femurs of group I(5 dogs) were fixed with only monolateral external fixator after subperiosteal osteotomy. Right femurs of group II(5 dogs) were fixed with mono lateral external fixator and intramedullary K-wire after subperiosteal osteotomy. Lengthening was started at 7 days after the surgery with the rate of 0.5 mm per day for 5 weeks and the dogs were sacrificed after 15 weeks postoperatively to examine histologic differences and evaluate bone mineral density. Radiographic examination at an interval of two weeks was done to evaluate the type of callus formed and to analyze complications including instability of external skeletal fixation and axial deviation. Bone mineral density at the lengthened area and contralateral nonlengthened area were measured using quantitative computerized tomography. Histological examination of regenerated bone was performed using Masson's trichrome stain method. The radiographs demonstrated poor callus formation, higher incidence of axial deviation and screw loosening in the group I compared to the group II. The bone mineral density at the lengthened area in the group II was higher than that of the group I(P<0.05). Histological examination showed that the new bone trabeculae in the group II were greater than that of the group I. In conclusion, the combination of monolateral external fixator and intramedullary K-wire can prevent pin loosening, axial deviation and reduce healing period in dogs.
