전통적인 한의학(韓醫學)의 독성(毒性) 개념 (Toxic Concept in Oriental Medicine)

  • This treatise, after review recent data on Oriental Medical toxicity, gets a conclusion on toxic concept of Oriental Medicine. 1. In the oriental medicine, the concept of toxic character contains propensity which disposition is inclined, and general meaning covered with the effect of a medicine, a side effect, formation of a medicine Besides, the concept diversely is used in the cause of a disease, names, symptoms, how to treat, medicines, prevention name, etc. 2. Every herbal drugs has toxic character. Levels of toxicity are nonexistence(無毒), existence(有毒), a little(小毒), always(常毒), serious(大毒), fatal toxic(劇毒), whose concepts in the Oriental Medicine are divided relative and absolute at the same time. 3. The examples of the fatal events by poisoning up to now are more than 400 cases(in China). 4. The factors of toxicity are the amount used unsuitably, the combination, directions, and interactions between western and oriental medicine, etc. The reduction of toxicity and how to detoxicate is several methods. The toxic science in Oriental Medicine based on the formation of oriental medicine present principles which use herbal drugs safely and availablely, utilizing th dispositions and efficiencies. It has positive, learning spirits which prevent abuse of oriental medicine, and which exactly diagnose and use the herbal drugs in the treatment with absolute toxic medicine. However, I think that scientific, positive experimental research is necessary to setup dose-response relation, be in relative quantity of toxic character, operate on the reactive mechanism exactly.

  • 1. Objectives The purpose of this case is to report that a Soyangin Heat sensation in the chest patient Diagnosed as Hyperthyroidism was treated with 'Yangkyuksanhwa-tang' and Antithyroid drugs in conjunction and then her symptom and thyroid function test results improved. 2. Methods We diagnosed her Hypetthyroidism on Soyangin Syndrome. So we treated her with 'Yangkyuksanhwa-tang' and Antithyroid drugs in combination. 3. Results This patient's symptom and thyroid function test results were improved 4. Conclusions By a combined treatment on a Soyangin Heat sensation in the chest patient Diagnosed as Hypetthyroidism, her symptom and thyroid function test results were improved. This case study showed an efficient results by giving Yangkyuksanhwa-tang in treatment of Soyangin Heat sensation in the chest patient.

당뇨병성 말초 신경병증에 대한 RCT 분석 연구 (Review of Randomized Controlled Clinical Trials Targeting Treatment of Diabetic Peripheral Neuropathy)

  • Objective: This study aimed to produce an overview of RCT for treatments of diabetic peripheral neuropathy. Method: All RCT studies for treatments of diabetic peripheral neuropathy were selected via PubMed. The general trend of RCT number, subject and result, including Oriental medicine, were analyzed. Result: A total 147 RCT studies were found. The number of RCT has rapidly increased since 2003. 114 RCTs were conducted with western drugs, while RCTs with electrical stimulation and Oriental medicine were eight and ten respectively. All RCTs with acupuncture, moxibustion and herbal drugs showed positive results. Conclusion: Oriental medicine-derived RCTs for diabetic peripheral neuropathy are very rare, even their potential was sufficient. This study may provide valuable information for development of Oriental medicine against diabetic peripheral neuropathy in the future.

Prasaplai: An essential Thai traditional formulation for primary dysmenorrhea treatment

  • Affecting more than half of menstruating women, dysmenorrhea is a cramp which causes abdominal or lower back pain just before or during a menstruation. In western medicine, non-steroidal anti-inflammatory drugs (NSAIDs) are normally used to treat primary dysmenorrheal symptoms. Despite their rapidity in relieving pain, NSAIDs have many serious side effects on the liver, kidney, and gastrointestinal tract. Thai traditional medicines comprise many preparations for treating dysmenorrhea, especially Prasaplai preparation which has been listed in the Thai traditional common household drug list since 2006. The use of Prasaplai was originated about 100 years ago and is still being used in the present time to treat dysmenorrhea. This review focuses on the history of the preparation, active ingredients, and biological activities especially on cyclooxygenase inhibitor, artifacts occurred in the preparation, quantitative analysis, and clinical trial of Prasaplai formulation.

다제내성 결핵의 치료 성적 (Treatment Results of Multidrug Resistant Tuberculosis in a University Hospital in Korea)

  • Background : Multidrug-resistant tuberculosis(MDR-Tb) has been increased not only in Asia but also in Western society, which may cause public health problems and reduce the efficacy of treatment of tuberculosis. In Western society HIV infection is believed to do a central role in increasing incidence of MDR tuberculosis, but MDR-Tb in Korea may be somewhat different about clinical features, underlying disorders, and prognosis. Goble et al reponed that overall treatment failure rate in MDR-Tb including resistance to isoniazid(INH) and rifampin (RFP) was 44 %. The aim of this study is to find the treatment result in Korea and the factors determining the prognosis. Methods: A retrospective study of pulmonary tuberculosis cultured M. tuberculosis from sputum or bronchial washing fluid between 1986 through 1992 was conducted in the Seoul Paik Hospital, Inje University. We reviewed clinical courses of 141 patients, who had a tuberculosis with resistance to 2 or more drugs including isoniazid(INH) and rifampin(RFP). One hundred and 4 patients of 141 patients had completed treatment and followed up for more than one year. Results: Of 104 (mean age $43.6{\pm}16.7$, M: F=63 : 41) patients with sufficient follow-up data, 73(84.6%) patients responded which is defined as negative Sputum cultures for at least 3 consecutive months. Seven patients(6.7%) had a failure in negative conversion and 9(8.7%) of the patients who initially responded relapsed. Overall treatment failure rate was 15.4%, Patients who were treated for less than 12 months had a higher relapse rate(12.3%) than 18 months(4.9%). And there was a statistically significant correlation between the relapse rate and the number of drugs to which isolates wera resistant(p<0.05). Conclusion : The treatment failure rate of MDR-Tb in Korea was lower than previous studies in western Country and the major determining factor of prognosis was the number of resistant drugs to M. tuberculosis at drug sensitivity test. For reducing the relapse rate, we recommend more than 12 months of treatment for MDR tuberculosis.

경궐을 동반한 습온 치험 1예 (A Case report of Seupon (Shiwen, 습온) with Gyeonggwol (Jingjue, 경궐))

  • Objectives : We report an unusual case of a 57-year old woman with spasticity following fever of unknown origin (the origin of her fever was not certain by western medical diagnosis). Malignant hyperthemia with spasticity couldn't be cured by general western medical therapy, and furthermore the function of liver and renal system was worsened as a consequence of drugs. Methods : We diagnosed the watery state of the patient as seupon (습온) with gyeonggwol (경궐) through pattern identification (변증) of symptoms and signs. Results : This patient who had spasticity with malignant hyperthermia was treated by optical management of herb medicine and acupuncture, and should need long-term observation. Conclusions : We want to show that trial by febrile diseases can solve fever and spasticity.

한약 및 한약제제 관리와 연구개발 정책방향에 관한 연구 (Study on the Direction of Policies to Manage and Develop Herbs and their Products)

  • Objectives : To direct methods of management and development of herbs and their derived products, and at the end of this study to inform the process of making new herbal drug regulations in Korea. Methods : This study analyzed the regulations and laws of western medicine drugs. Results : We have got some herbal prescriptions which are not in the eleven books of oriental medicine now in use but which have proved effective. We need to establish standards for permissions to produce medical products through those prescriptions. Besides, we need the special permissions to produce herbal products through prescriptions, especially those which have been used to treat incurable diseases. And, we can contribute to globalization of oriental medicine and to well situating of herb products and medicinal acupunctures if we produce them according to international standards. It is thought that herbal medicine has no side effect in the medical society in Korea. But, it is getting popular to administer herbal medicine and western medicine together. Thus, the side effects of taking both medicines at the same time should be documented. Conclusions : Herbal medicines and herbal products should be under the control over the whole process of production, circulation, and sales. Now, it is time for herbal medicine to be known to all over the world. Therefore, herbal medicine must meet and adhere to the standards set by the western society and WHO.

그레이브스병 환자에 대한 안전백호탕의 임상적 유효성 안전성 연구 (The Clinical Study of Ahnjeonbaekho-tang on Patients with Graves' Disease)

  • Objective: Graves' disease, the most common cause of primary hyperthyroidism, is a thyroid specific autoimmune disorder. When resistance to medication is shown in spite of long term therapy with anti-thyroid drugs, radioactive iodine therapy would be chosen in Western medicine. However, this therapy has often been reported to cause patients have hypothyroidism, thus requiring them to take levothyroxine for the rest of their lives. In this study, we evaluate the clinical efficacy and safety of Ahnjeonbaekho-tang (AJBHT) on patients with Graves' disease. Methods: We prescribed AJBHT for 3 months to two groups: patients who had been taking antithyroid drugs were administered AJBHT after discontinuing the antithyroid drugs ($Com-Tx{\rightarrow}Single-Tx$), and patients who had not been taking antithyroid drugs were started with AJBHT (Single-Tx) immediately. We evaluated the thyroidal function test (TFT) and visual analogue scale (VAS) for clinical symptoms for 3 months. Results: Serum T3 and fT4 were significantly decreased in both groups and remission rate of thyroidal hormones were significantly improved in the Single-Tx group. The clinical symptoms of palpitation, fatigue, and heat intolerance were significantly improved in both groups. In the safety analysis, all patients were in normal range of liver, renal function blood test and common blood count. Conclusion: From these results, we suggested that AJBHT was effective on TFT and clinical symptoms of Graves' disease. The study supports that AJBHT may be a useful agent for patients with Graves' disease who are resistant to antithyroid medication or radioactive iodine therapy, and for patients at first diagnosis.

Anti-inflammatory Activities of Coumarins Isolated from Angelica gigas Nakai on LPS-stimulated RAW 264.7 Cells

  • Five kinds of coumarin compounds were successfully purified from Angelica gigas Nakai by using recyclingpreparative HPLC and identified as decursin (1), decursinol angelate (2), 7-demethylsuberosine (3), marmesin (4), and decursinol (5) by NMR analyses. None of the purified compounds in ethanol showed DPPH radical scavenging activity, while the A. gigas extract (AGNEX) displayed a significant level of activity. Interestingly, compounds 3 in phosphate buffered saline (PBS) showed good $ABTS^+$ radical scavenging activity ($IC_{50}=8.1{\mu}g$/mL) as did compounds 4 and 5. The anti-inflammatory activities of the purified compounds were evaluated and compared using the NO concentration assay and western blot analysis on LPS-stimulated RAW 264.7 cells. NO production was significantly suppressed by all the compounds in a dose-dependent manner among which compounds 1, 2, and 3 showed very good activities with $IC_{50}$ values of 7.4, 6.5, and $7.6{\mu}g$/mL, respectively. Treatment with compounds 1-5 effectively suppressed the expression levels of iNOS, IL-1$\beta$, and COX-2, which are responsible for promoting the inflammatory process. Thus, the ethanol extract and coumarin compounds of A. gigas Nakai hold promise for use as potential anti-inflammatory agents.
