Pharmacological Therapies of Obesity: A Review of Current Treatment Options

  • Over the last decade, the incidence of overweight and obesity has nearly doubled in many countries and is considered a pandemic. Obesity was identified as a major risk factor for cardiovascular disease as the same level as smoking and diabetes. Visceral fat is considered one of the key contributors to outcome and certain ethnic groups such as Asians seem to be more affected than others. Weight reduction through lifestyle changes was found to be impactful to improve overall health, but weight loss and maintenance thereof is limited and difficult to sustain. Surgical intervention demonstrated a greater weight loss in the severely obese and was associated with improved all-cause mortality. Despite numerous pharmacological targets and a high medical need, only few drugs have been successfully developed. Earlier studies with amphetamine-derived compounds showed significant weight loss but their critical safety profiles led to market withdrawals and disappointment. More recent compounds; orlistat - a lipase inhibitor, rimonabant - a cannaboid-1-receptor antagonist, and sibutramine - a combined serotonin/norepinephrine re-uptake inhibitor, all demonstrated similar significant efficacy; however, they carry specific safety profiles making them unsuitable for every patient. The main limitation of pharmacotherapy is the absence of clear benefit-risk assessments through outcome studies. Such a study - the SCOUT trial - was designed to compare sibutramine versus placebo and the effect on morbidity and mortality in nearly 10,000 obese patients with additional risk factors. Such studies could provide new scientific evidence for obesity treatment and may support future pharmacological approaches.

Perinatal and Postnatal Study of KTC-1, a New Semisynthetic Rifamycin Derivative, in Rats (새로운 반합성 Rifamycin 유도체 KTC-1의 랫트 주산기 및 수유기 시험)

  • A perinatal and postnatal study of KTC-1, a new semisyntheitic rifamycin antituberculous drug, was conducted in Sprague-Dawley rats. Dosages of KTC-1 0, 12, 27.6, and 63.5 mg/kg/day were administered to dams orally by gavage from day 17 of gestation to day 21 of lactation. All pregnant rats were allowed to deliver naturally for postnatal examination of their offspring. At 63.5 mg/kg/day, weakness, dark-red discharge around eyes, a loss in body weight, and a decrease in food and water consumption were observed in dams. An increase in the weight of adrenal gland and spleen, and a decrease in the weight of kidney and heart were also found. An increase in neonatal deaths during the lactation period, a loss in body weight, a delay in physical development, a decrease in traction ability, an increase in the number of errors and the time required for the multiple T-maze trial were found in F1 offspring. In addition, an increase in the incidence of visceral variations and retarded ossification were observed in F1 4 day old rats. An increase in the incience of skeletal anomalies was seen in F2 fetuses. There were no sings of maternal toxicity or embryotoxicity at 12 and 27.6 mg/kg/day. From the results mentioned above, it can be concluded that the no-effect dose levels(NOELs)for dams, F1 offspring, and F2 fetuses are 27.6 mg/kg/day.

Performance of non-prismatic simply supported prestressed concrete beams

  • Prestressing is the most commonly employed technique in bridges and long span beams in commercial buildings as prestressing results in slender section with higher load carrying capacities. This work is an attempt to study the performance of a minimum weight prestressed concrete beam adopting a non-prismatic section so that there will be a reduction in the volume of concrete which in turn reduces the self-weight of the structure. The effect of adopting a non-prismatic section on parameters like prestressing force, area of prestressing steel, bending stresses, shear stresses and percentage loss of prestress are established theoretically. The analysis of non-prismatic prestressed beams is based on the assumption of pure bending theory. Equations are derived for dead load bending moment, eccentricity, and depth at any required section. Based on these equations an algorithm is developed which does the stress checks for the given section for every 500 mm interval of the span. Limit state method is used for the design of beam and finite difference method is used for finding out the deflection of a non-prismatic beam. All the parameters of nonprismatic prestressed concrete beams are compared with that of the rectangular prestressed concrete members and observed that minimum weight design and economical design are not same. Minimum weight design results in the increase in required area of prestressing steel.

Alkaline Softening of $TiO_2$ Delustered Polyester Fabrics ($TiO_2$로 소광가공된 폴리에스테르 직물의 알칼리 유연가공에 관한 연구)

  • Polyester yarns and fabrics containing three levels of $TiO_2$ delusterant were hydrolyzed with NaOH and examined for physical and morphological changes. The mechanical propertis and hand values of alkaline hydrolyzed polyester fabrics were measured using KES-FB system. Also, the relationship between the morphology and the mechanical property of alkaline hydrolyzed polyester fabrics was analyzed. The results are as follows: 1. At an initial stage of alkaline treatment, the concentration of $TiO_2$ did not affect the weight loss of the treated yarns. But by increasing treatment time, the effect of the concentration of $TiO_2$ on the weight loss of the fiber became more pronounced. The weight loss were increased in the following order; fulldull> semidull> clear 2. The effect of hydrolysis on yarn tensile strength seems to be more related to the size of the pits on the fibers rather than the number of pits. 3. Axially oriented pits occurred along the hydroyzed, delustered fiber surfaces, while such pitting was absent on hydrolyzed fiber containing no $TiO_2$. The number of voids across the surface of a fiber increased with an increase in the amount of TiOa incorporated into the fibers. The size of the voids depended on the treatment time of hydrolysis rather than the concentration of TiOa. 4. The mechanical properties and hand values of polyester fabrics were changed by alkaline treatment but were identical regardless of the concentration of TiOa. While the mechanical properties of polyester fabrics depended on the structural change of the fibers and the yarns within the fabrics as the fiber diameter became progressively smaller rather than the size and number of pits.

The improving effect of Zizyphus jujube on dextran sulfate sodium-induced colitis in mice (대추의 궤양성 대장염에 대한 개선 효과)

  • Objectives : Zizyphus jujube (ZJ) has been used as a traditional medicine for various diseases. However, the inhibitory effect of ZJ on intestinal inflammation has not been fully understood, yet. The aim of this study is to investigate anti-colitis activity of ZJ in dextran sulfate sodium (DSS)-induced colitis mouse model. Methods : To investigate the protective effects of ZJ,the colitis mice were induced by drinking water containing 5% DSS for 7 days. Mice were randomized into groups receiving ZJ (500 mg/kg), sulfasalazine (SFZ) (150 mg/kg) as a positive control, or water as a negative control. We assayed the effects of ZJ on DSS-induced the clinical signs, measuring weight loss, colon length and disease activity index (DAI). Additionally, to find a possible explanation for the anti-inflammatory effects of ZJ, we evaluated the effects of ZJ on the production of prostaglandin $E_2$ ($PGE_2$) and expression of cyclooxygenase (COX)-2 in colitis tissue. Results : The results showed that mice treated with DSS showed considerable clinical signs, including weight loss, and reduced colon length. However, administration of ZJ significantly reduced the weight loss, shortens colon length, and improved DAI as clinical symptoms. Moreover, ZJ inhibited the $PGE_2$ production and COX-2 expression levels in DSS-treated colon tissues. Conclusions : Collectively, the findings of this study provide us with novel insights into the pharmacological actions of ZJ as a potential molecule for use in the treatment of intestinal inflammation including ulcerative colitis.

Dyeing Properties Resin Treatment Effects of the Lyocell Fabrics (리오셀 직물 수지가공 효과와 염색성)

  • The effects of resin finish and dyeabilities of four kinds of Iyocell fabrics that were manufactured by four kinds of pulps were investigated. The dyeabilities of Iyocell fabrics were similar, but differed from cotton fabric. In early stage of 30 minutes, cotton fabric was shown higher dye exhaustion ratio than Iyocell fabrics, however after then cotton fabric did not increase dye exhaustion, Iyocell fabrics increased continuously. At last, the dye exhaustion ratio of Iyocell fabrics were about 75% and that of cotton fabric was 65%. Two kinds of experimental procedures were applied for Iyocell fabrics. One was what the fabrics were treated with resins and washed with cellulase, and then dyed with reactive dyes. The other procedure was the fabrics were dyed with reactive dyes, and then applied the resin treatments and cellulase washing. After fibrillation and washing the undyed Iyocell fabric and the Iyocell fabric that was dyed with C.I.Reactive Red 120, their weight loss ratios were 3.5% and 2.8%, respectively. Dyeing with reactive dyes caused the crosslinking between cellulose and dyes and the crosslinking decreased fibrillation. The weight loss by enzyme washing of Iyocell fabrics decreased by the glyoxal and melamine resin treatments. The reduction of weight loss can be caused by fibrillation decrease. Dyeing and resin treating can be showed the synergic effect on the reduction of fibrillation. The effect of glyoxal resin on the reduction of fibrillation was a little better than that of melamine resin.

Effect of Packaging Method on the Quality of Strawberry, Tomato, and plum during Storage

  • To examine the effect of packaging method on strawberry, tomato, and plum quality, the rate of weight loss, Hunter a value, decay rate, anthocyanin contents, and microbial (total bacterial counts, mold and yeast, and pseudomonas) changes were determined during storage. Strawberry was packaged with low density polyethylene (LDPE). Tomato and plum were packaged with high density polyethylene film (HDPE). Strawberries, tomatoes, and plums were then stored at 4$^{\circ}C$ and 20$^{\circ}C$, respectively. LDPE package was the most effective on the decrease of decay rate of strawberry and the rate of weight loss for packaged strawberry was lower than that of the non-packaged. HDPE package was the most effective on the rate of weight loss during storage of tomatoes and plums regardless of storage temperature. Hunter a value increased during storage. Anthocyanin contents of plums increased overall with increasing storage time, and plums stored without package were changed more than those with package. Microbial changes of strawberry, tomato, and plum stored at 4$^{\circ}C$ and 20$^{\circ}C$ were monitored during storage. Packaging method did not affect the microbial change, yet temperature did affect the microbial change significantly. These results indicate that storage of these commodities at 4$^{\circ}C$ should be recommended in terms ,of microbial safety as well as quality and shelf-life.

Effect of film liner packaging with chlorine dioxide on the storage quality of containergrown or bare-root seedlings at cold or frozen temperatures in sawtooth oak and Japanese larch

  • This study was carried out to discover suitable packaging methods during the cold or frozen storage of sawtooth oak and Japanese larch seedlings, which are major afforestation tree species in Korea. The weight loss rate of sawtooth oak was lower under frozen storage at -2℃ (maximum, 33.7%) than under cold storage at 2℃ (maximum, 61.0%). A film liner treatment inside the packaging box effectively suppressed the increase in the weight loss rate. For the sawtooth oak containerized seedlings, when stored at -2℃, the rate of weight loss was 17.9 to 25.4% in the untreated group and less than 1% in the film liner treatment group. Similar results were observed with Japanese larch. After two months of freezing at -2℃, both species maintained a low index of 1.0 - 1.1 with regard to the degree of shoot desiccation of containerized seedlings, and there was no significant difference between the treatments. On the other hand, the chlorine dioxide treatment could more effectively reduce the occurrence of mold and shoot desiccation than a PE single treatment in bare-root seedlings. According to these results, it was concluded that frozen storage is generally suitable as a means of long-term storage compared to cold storage of sawtooth oak and Japanese larch. A chlorine dioxide treatment at 20 ppm was effective for maintaining seedling quality levels during storage, demonstrating the good effect of suppressing the occurrence of shoot desiccation and the occurrence of mold.

The Effect of Weight Control on Obese Women (비만 여성의 체중조절 실시를 통한 효과 측정)

  • This study was conducted to investigate the effect of weight control by use of commercial formula diet and nutrition education on 36 obese women over a period of 6 months. During the study it motivated ways for women to control their weight and improve their health, thus analyzing variables that influence weight control. To help weight control, for the first 3 weeks, along with 1 regular mea, 2-3 liquid formula diets of 135㎉ per pack a day were provided within the 1,200-800㎉ range. For 6 months since then, all 3 meals were taken regularly and it was recommended that energy which is 500㎉ less than energy requirement be taken. To evaluate the effect of weight change by the weight control program, anthropometric measurements, biochemicl parameter, and food behavior are measured. The mean body mass index(BMI) was 30.1$\pm$3.8(25.0-43.6) and it was in the upper 5 percentile of the mean BMI of Korean women. The percent of body fat was significantly reduced from 29.1$\pm$2.4 to 26.4$\pm$2.3, showing that the weight control program induced not only a change in weight but also a change in body composition. The body circumference such as waist and hip circumference and skinfold thickness measurements of biceps, triceps, subscapular and surprailiac were reduced significantly, but the mean waist-to-hip ratio didn't change. Singnificantly improved food behavior score measured by the questionnaire indicated that the weight-contorl program induced the change of the food behavior, as well. Weight loss also induced biochemical parameters of the subject, thus total cholesterol, LDL-Cholesterol, and triglycerol which were in the high range of the normal values were reduced to 20.2%(p<0.01), 22.2%(p<0.01), 25.8%(p<0.01), repectively. HDL-cholesterol didn't change significantly, but the ratio of HDL to total cholesterol increased(p<0.01).

  • Honeycomb structures have advantages in weight reduction with stiffness increment. As far as noise is concerned, however a light aluminum structure, instead of a steel frame, should have an equivalent mass density in order to maintain sound insulation performance.
