Objectives : The aim of this study is to suggest approach of oriental medical management and necessity by research of eastern and western medical literatures for Complex Regional Pain Syndrome(CRPS). Methods : We reviewed the clinical and experimental literatures of eastern and western concerned with CRPS which is related causation, signs and symptoms, diagnosis, etiology and management. Results : 1. CRPS is divided into type I and II by nerve injury. Two types of CRPS have been recognized: type I, corresponds to RSD and occurs without a definable nerve lesion, and type II, formerly called causalgia refers to cases where a definable nerve lesion is present. These conditions can be charaterized clinically by the sensory abnormalities, vascular abnormalities, oedema, sweating abnormalities, motor or trophic changes. 2. CRPS are well known to patients and physicians relatively, but the pathophysiology, causation and treatments are still unclear. 3. CRPS is needed to take the early diagnosis and multidisciplinary approach for significant effect. 4. CRPS can be regarded for obstruction syndrome of Ki and blood(痺證), blood stasis(瘀血), Wei symptom(痿證), numbness(痲木) in the oriental medical management of CRPS. Conclusions : Above the results, it is suggested that further studies and active approach of management of CRPS will be conducted precisely in oriental medicine.
Objectives : Anxiety is a symptom that many psychiatric patients experience. And in the traditional Korean medicine, anxiety is often categorized as a Gyeong Gye(驚悸) and Jung Choong(怔忡). In this case report, we will show the effective method in treatment of anxiety. Methods : First, We treated the patient with anxiety by the differential diagnosis of blood stasis(瘀血) that is summarized as stasis of blood, pollutant of blood and external blood of vessel. And we gave him Hyeolbuchukyeo-tang(血府逐瘀湯) whose effect is to eliminate blood stasis. And Second, we treated the patient with anxiety in the viewpoint of analytic psychology that claims anxiety has a meaning that makes patient be aware of the problem of himself. Results : The patient's symptom improved significantly. And this patient also can insight himself via the symptom of anxiety. It also help the patient improve. Conclusions : In the treatment of anxiety, the approach by the differential diagnosis of blood stasis(瘀血) can be helpful to improve patient's symptom. And treatment should include the insight of patent by the meaning of anxiety.
Purpose: Cigarette smoking causes many kinds of cancer, and it is more closely related with lung cancer, rather than other cancers. Smoking is the leading cause of lung cancer and ninety percent of the smokers are male in China, but there is little published data concerning the psychological responses in the male smokers with lung cancer and its influence on the symptom burden. The aim of the study was to verify the hypothesis that male smokers with lung cancer have more positive attitude and less symptom burden, comparing to male non-smokers. Methods: A total of 194 men with cancer in West China Hospital, Sichuan, China, were assessed by self-administered questionnaire. Psychological response was measured by the Chinese version of Mini-Mental Adjustment to Cancer scale (Mini-MAC), and symptom burden was measured by the physical symptom distress scale from the Rotterdam Symptom Checklist (RSCL). Results: We found that smokers with lung cancer got higher scores in positive attitude and a smaller symptom burden than non-smokers. Patients with education lower than high school got higher scores of positive attitude compared to college graduate patients (p=0.038). Smokers with lung cancer who knew the potential carcinogenicity of cigarette showed less negative emotions (p=0.011). The psychological response was not affected by age, clinical stage, cell type, smoking duration and amount. Conclusions: Male smokers with lung cancer have a more positive attitude and fewer symptoms, comparing to male non-smokers. Appropriate psychological intervention for non-smokers with lung cancer deserves more attention.
Conversion disorder is a diagnosis based on a psychological construct that currently has no known neurobiologic substrate. But It is not easy to differentiate a real conversion disorder from a neurological disease or other medical disease. A patient in this case report had been diagnosed as conversion disorder but later it was found that the conversion disorder was misdiagnosis. During the 7 days of treatment, this patient was treated by herb medication, acupuncture and TENS and bad a little improvement. Both way, this patient was evaluated and evantually was diagnosed as Guillain-Barre Syndrome Miller-Fisber Variant, suggesting that the former diagnosis bad been a mistake. In this case report, we will present this patient's case and review the misdiagnosis of conversion disorder.
In Oriental Medical theory, origin of kidney's weakness or atrophy is shen qi(腎氣) and function of san jiao(三焦) deteriorate, it result in a passage of evacuation is blocked. - In Oriental Medicine, Shen(腎) take charge of storing and evacuating function, by taking qi(氣) of the five viscera and the six bowels. - The cause of reducing of shen qi and san jiao's evacuative function is xu han(Emptiness and Coldness) of the five viscera and the six bowels' activity. So we do not treat only kidney, but we also must focus the five viscera and the six bowels' organic function and ying wei's function. A Renal Failure is similar in symptom to Kwan-kyuk(關格), oliguria or anuria, edema, Hu-son(虛損), Sin-pung(腎風) and Yuk-kuk(六極) in chenxiang(沈香). We grasp symptom of 7 cases of chronic renal failure, and diagnose its pathology based on Sa-jin(四診), and prescribed herbal medicines. And in the point of the chenxiang, we separate two group, Ater one is taken herbal medicine with chenxiang and the other is only taken herbal medicine with no using chenxiang, we compared the rate of treating with only herbal and herbal compounded chenxiang. We repeat medical examination for continuation of effective result, report clinical progress and result which based on this examination.
Hwabyung is a Korean culture bound syndrome and translated as anger syndrome at DSM-IV. And it is caused by the suppression of anger and has characteristics of chest discomfort, burning up as physical symptoms, and mortification, chagrin as psychological symptoms. It is very common in the field of psychiatry in Korea with Major Depressive Disorder (MDD). * Objective : 95 patients with MDD or Hwabyung, who visited the department of psychiatry in Korea University Medical Center or oriental medical hospital of Kyunghee Medical Center from May 2003 to August 2003, were selected for the study to compare between Major Depressive Disorder and Hwabyung. * Methods : The Structured Clinical Interview Schedule for DSM-IV (SCID) and Hwa-Byung Diagnostic Interview Schedule (HBDIS) were conducted for diagnosis. And Symptom Check List-90-Revised (SCL-90-R) was carried out for comparison between MDD and Hwabyung. * Result : Among the 95 patients, MDD group was 23 persons, Hwabyung group was 36 persons, and MDD plus Hwabyung group was 36 persons and it occupied 37.9%. MDD plus Hwabyung group had higher scores than any other groups in the whole dimensions of SCL-90-R. Hwabyung group had higher scores than MDD group in somatization, anxiety, hostility, Global Severity Index(GSI) and positive symptom total (p<.05) of SCL-90-R, but two groups had no difference in obsessive-compulsive, interpersonal-sensitivity, depression, phobic anxiety, paranoid ideation, psychoticism, positive symptom distress index of SCL-90-R. * Conclusion : These result showed that Hwabyung is similar to MDD at the point of depression and psychoticism but more severe at somatization, anxiety and anger than MDD.
Objective : This is a clinical report about the 77-years-old man patient with polymyositis treated by oriental medicine. Methods : The patient was treated by acupuncture and herb medication. The improvement of the patient was judged by MRC scale and CPK, LDH. Results & Conclusions : 1. Patient's symptoms(paraplegia) rapidly improved. 2. The level of CPK, LDH and Potassium are recovered to the normal range. 3. Associated with respiratory symptoms are getting better.
Objective: Diabetic neuropathy is a common complication of diabetes mellitus. Patients with this disease suffer from various peripheral sensory symptoms that are not easily alleviated. We report on a case of a patient with diabetic neuropathy whose symptoms were alleviated after traditional Korean medical treatment.Methods: A patient with hand and foot numbness and coldness diagnosed with diabetic neuropathy was treated with Palmijihwang-tang (Bai Wei Di Huang Wa, 八味地黃湯) three times a day. The progress was evaluated with the numerical rating scale (NRS), neuropathy disability score (NDS), and neuropathy symptom score (NSS).Results: The improvement of symptoms appeared to be faster and more effective than that of conventional treatment. The improvement was observed from the second day of the treatment.Conclusions: This case study shows that Palmijihwang-tang may be an effective treatment for diabetic neuropathy.
Objectives : The aim of this study was to investigate the correlation between the effectiveness of Brief Qigong-based Stress Reduction Program(BQSRP) and personality. Methods : Twenty nine women and nine men from the community with symptoms of distress(mean age 39.4 years, SD=11.4) were randomized into a group receiving a 4-week intervention of BQSRP(n=19) or a waiting-list control group. All subjects completed the temperament and character inventory (TCI) before the intervention. Before and after the intervention period, questionnaires were completed on perceived stress scale(PSS), anxiety, and quality of life. Results : 1. Those who have higher temperament of harm avoidance or lower character of self-directedness showed higher PSS, Hwa-Byung character, Hwa-Byung symptom, STAI and lower WHOQOL-BREF scores. 2. In the BQSRP training group, a correlation analysis demonstrated an association between higher temperament of harm avoidance and greater improvement of perceived stress and an association between lower character of self-directedness and greater improvement of perceived stress. 3. Those who have high Hwa-Byung symptom scores prefer the breathing meditation and there were no differences in effects of BQSRP group by the preference Brief Qigong-based Meditation(BQM) component. Conclusions : These results suggest that a BQSRP training may be effective for those with higher harm avoidance trait and low self-directedness.

The effect of acupuncture was evaluated by clinical grmptomi hematological findings and blood chemical values. Acupuncture and Iaserpunctore group revealed rotatively fast recovery compared with other sloops. For the pancreas study, sixteen dogs were divided into four groups(each of 4 in dogs): laserpuncture, electroacupuncture, acupuncture and control group. Pancreas disease was brought by experimentally induced pancreas trauma, and then treatment was done once daily for 4 days The acupoints used were Wei-shu and Pi-shu. The effect of acupuncture was evaluate6 by clinical symptom, hematological findings and blood chemical values. Laserpuncture revealed relatively frost recovery and acupuncture group revealed secondly fast recovery after treatment. For the urinary bladder study, sixteen dogs were divided into four groups(each of 4 in dogs): laserpuncture, electroacupuncture,acupuncture and control group. Spleen disease was brought by experimentally induced urinary stoner and then treatment was done once daily for 4 days. The acupuncture used were Pang Guang-shu and Shen-shu. The effect of acupuncture was evaluated by clinical symptom, hematological findings, and blood chemical values. Laserpuncture and acupuncoure group revealed relatively fast recovery compared with other groups.
