개에서 건삭파열로 유발한 급성 이첨판 폐쇄부전 모델의 혈류역학적 평가 (Hemodynamic Evaluation of Acute Mitral Valve Insufficiency Model induced by Chordae Tendinae Rupture in Normal Dogs)

  • 본 연구는 개에서 건삭파열로 유발한 급성 이첨판 폐쇄부전 모델의 혈류역학적 변화를 관찰하는 것이다. 이 연구는 10마리의 정상 심장기능을 가진 비글견에서 실시되었다. 직접혈압측정법과 Swan-Ganz 카테터를 통해 건삭을 실험적으로 파열시키는 기간동안 혈류역학적 지표 변화를 진행하였다. 이첨판 폐쇄부전 모델을 만들기 위해 5번 늑간으로 접근하여 관절경에 사용하는 작은 훅나이프를 사용하여 건삭을 파열하였다. 수술 중 칼라 도플러 영상 검사를 통해 이첨판 역류를 확인하였다. 혈류역학적 지표를 측정한 결과 폐모세혈관쇄기압은 유의적으로 증가하였지만, 평균동맥압, 정맥압, 폐동맥압, 심박출량, 심박출 지수는 건삭 파열 후에 유의적으로 감소한다는 것을 발견하였다. 이것은 건산파열로 인해 좌심실로 부터 역류된 혈액이 좌심방에 과부하를 일으킨다는 것을 나타내는 것이다. 본 연구를 통해 개에서 급성 이첨판 폐쇄부전 모델을 만드는데 있어 건삭파열방법이 효과적인 것을 알 수 있었고, 이첨판 역류 유발 후 시간이 지나면 만성 이첨판 부전증으로 진행할 수 있을 것으로 생각되며, 차후 판막의 만성적 변화 양상을 연구하는데 도움을 줄 수 있을 것으로 생각됩니다.

The Effect of Prism Adaptation Following Traumatic Brain Injury: A case report

  • Background: The presence of visuospatial impairment can make patients slow functional recovery and impede the rehabilitation process in TBI patients. Objective: The aim of this study is to investigate effects of prism adaptation treatment for functional outcomes in patients following traumatic brain injury. Methods: The subject received prism adaptation treatment for 2 weeks additionally during traditional rehabilitation for 4 weeks. The Patient has prism adaptation treatment while wearing wedge prisms that shift the external environment about $12^{\circ}$ leftward. The patient received 10 sessions, 15-20min each session. Outcome measures were visuospatial deficit(line bisection, latter cancellation), Visual and spatial perception(LOTCA-visual perception and spatial perception), motor function of upper extremity(FMA U/E; Fugl-Meyer motor assessment upper extremity, ARAT; Action research arm test), balance(BBS; Berg Balance Scale), mobility(FAC; Functional ambulation classification) and functional level(FIM; Functional independent measure). All Assessments took place on study entry and post-treatment assessments were performed at discharge from the hospital. Results: After prism adaptation, the visuospatial impairment scores improved as indicated in the line bisection(-15.2 to -6.02), latter cancellation(2 to 0) and LOTCA- spatial perception scores(7 to 9). The upper motor function improved as indicated in the scores of affected FMA U/E(21 to 40) and ARAT(4 to 22). Ambulation and balance improved as indicated in the BBS scores(25 to 38) and FAC scores(0 to 4). ADL function improved as indicated in the FIM total scores 54 to 70(motor 34 to 61, cognition 20 to 29). Conclusion: Prism adaptation did improve functional level such as motor functions and ADL abilities in TBI patient. Further research is recommended.

복합 불연속면을 갖는 포텐셜 문제 해석을 위한 확장된 MLS 차분법 (Extended MLS Difference Method for Potential Problem with Weak and Strong Discontinuities)

  • 본 논문은 복합 불연속면을 갖는 포텐셜 문제의 해석을 위해 확장된 MLS(Moving Least Squares) 차분법을 제시한다. 계면경계를 따라 해(solution)와 수직방향, 접선방향 미분들이 모두 불연속 특이성을 나타내는 복합 불연속면을 묘사하기 위해 계단함수, 쐐기함수, 가위함수와 같은 불연속 특이함수를 추가하여 기존의 MLS 차분법을 개선했다. 계면경계조건은 기지의 조건으로서 지배방정식의 이산화과정에서 추가의 미지계수를 발생시키지 않는다. 포아송 방정식 형태의 지배미분 방정식을 풀기 위해 내부영역과 경계에 절점을 배치하고 차분식을 구성한다. 차분식을 조립한 계 방정식을 직접 풀기 때문에 계산효율성이 매우 우수하다. 수치예제는 제시된 해석기법의 우수성을 잘 보여주며, 균열전파, 이동경계, 상호작용 문제 등 다양한 불연속 문제로의 확장이 기대된다.

Omega-3 Polyunsaturated Fatty Acid for Cholestasis due to Bile Duct Paucity

  • Omega (${\omega}$)-3 polyunsaturated fatty acids appear to be effective in preventing and treating parenteral nutrition-associated liver disease, and several mechanisms were proposed for this observation. An 8-week-old male infant with cholestasis and acholic stool was diagnosed non-syndromic intrahepatic interlobular bile duct paucity by open-wedge liver biopsy. Initially he was treated with usual supportive medical therapy, including ursodeoxycholic acid. However, the clinical status and laboratory tests did not improve. Omega (${\omega}$)-3 polyunsaturated fatty acids (initially intravenous administration and oral administration later), were started and his liver function, including aminotransferase level and bilirubin levels normalized, and the ivory stool color turned green. We report the possible effectiveness of ${\omega}$-3 polyunsaturated fatty acids as a potent choleretic agent for non-syndromic intrahepatic interlobular bile duct paucity, a very rare structural pediatric hepatic disease.

초음속 연소기 내부의 추력 분포 계산 (Determination of Thrust Distribution in the Supersonic Combustor)

  • 본 논문에서는 이상적인 추력 방정식을 이용하여 초음속 연소기의 국부 추력을 계산하였다. 측정압력으로부터 추력을 결정하는 방법을 마하수 2.5의 초음속 연소기에 적용하였다. 추력계산을 위해서 미시간대학에서 실험한 압력측정치를 사용하였다. 초음속 연소기 내부의 국부추력을 계산하여 연소와 쐐기의 존재가 연소기 상류 부문의 추력에 많은 영향을 주었음을 발견하였다. 측정된 압력으로부터 추력을 계산하는 방법은 특히 고가의 추력특정 장치가 없는 경우에도 사용이 가능한 간단하고 실용적인 방법이 될 수 있을 것이다.

엔진속도 변화에 따른 연소실내 Spark Plug 주위의 유동특성 고찰 (Characteristics of in-cylinder flow near the spark-plug for different engine speeds)

  • Flows in the combustion chamber near the spark plug are measured using LDv.A single cylinder DOHC S.I. engine of compression ratio 9.5:1 with a transparent quartz window piston is used. Combustion chamber shape is semi-wedge type. Measured data are analyzed using the ensemble averaged analysis and the cycle resolved analysis which uses FFT Filtering. Turbulent intensity and mean velocity are studied in the main flow direction and the normal to main flow direction as a function of engine speeds. The results shows that the turbulent intensity obtained by the ensemble averaged analysis is greater than that calculated by the cycle resolved analysis. Especially, the ensemble averaged analysis shows increase in turbulence at the end of compression stroke although the cycle resolved analysis shows increase only in the cycle-by-cycle variation with no noticeable increase in turbulence. The mean velocity in the main flow direction increase as engine speed increase. But the mean velocity normal to the main flow does not show such increase. Turbulent intensity in both direction increase in proportion to engine speeds. The magnitude of turbulent intensity is about 0.3 ~ 0.4 times the mean piston speeds at the end of the compression stroke.

시추 및 야외조사 자료의 절취면 투영 분석 시스템 Fracjection (The Fracjection: An analytical system for projected fractures onto rock excavation surface from boreholes and outcrops)

  • Surveying rocks for engineering aims for prediction of geological feature of the construction site. Conventionally, survey information at outcrops and bore holes are projected to the construction sites, such as tunnel and slopes, and rock properties of the sites are predicted by interpretations of specialists. This system, the "Fracjection", aims to assist the specialist for visualization of the projected fractures from borehole and outcrop survey. The Fracjection accepts the BIPS and outcrop survey data to its database and allows plotting them in AutoCad map. The software also reads elevation data from contours of the topographic map and constructs DEM of the construction sites. With user's guide, it generates 3D excavation sites such as slopes and tunnels at the topographic map. The s/w projects borehole and outcrop surveyed fractures onto the modeled excavation surface and allows analysis of failure criteria, such as plane, wedge, and toppling failures by built-in stereonet function. Projected fractures can further be analyzed for structural homogeneities and rock mass quality. Moving window style correlation comparison of stereonet plots are used for formal analyses, and RQD type counts of the projected fractures are adopted for the latter analyses.

링 전단시험기를 이용한 암석절리의 잔류강도 특성에 관한 연구 (A Study on Residual Stress Characteristics for Joint of Rock in Ring Shear Tests)

  • Residual stress is defined as a minimum stress with a large displacement of specimens and the residual stress after peak shear stress appears with displacement volume but there is no provision to select the residual stress. In the previous study, residual stress was recorded when the change of shear load is small in the condition of the strain more than 15%. But, in this study, hyperbolic function((No Abstract.see full/text), b=experimental constant) of soil test is adapted to joint of rock and the propriety is investigated. In a landslide and landsliding of artificial slope, wedge failure of tunnel with a large displacement, tests are simulated from peak stress to residual stress for safety analysis. But now. direct shear stress and triaxial compressive tests are usually performed to find out characteristics of shear stress about joint. Although these tests get a small displacement, that data of peak stress and residual stress are used for safety analysis. In this study, we tried to determine failure criteria for joints of rock using ring shear test machine. The residual stress following shear behavior was determined by the result of ring shear test and direct shear test. In conclusion, after comparing the results of the two test, we found that cohesion(c) and internal friction angle(ø) of ring shear test are 30% and 22% respectively of those of the direct shear test.

전국 위암 환자 진료 현황에 관한 설문조사 결과 (Current Status of Clinical Practice for Gastric Cancer Patients in Korea -A Nationwide Survey-)

  • This nationwide survey was conducted to evaluate the current status of clinical practice for gastric cancer patients in Korea. The Information Committee of the Korean Gastric Cancer Association (KGCA) sent questionnaires containing 45 items about the preoperative diagnosis, medical and surgical treatment, and postoperative follow-up for gastric cancer patients to all 298 KGCA members in 108 institutes. Response rates were $32.6\%$ (97/298) for individuals and $59.3\%$ (64/108) for institutes. Most university hospitals responded (response rate of university hospitals: $71.6\%$, 48/67). The preoperative staging work up was performed primarily by abdominal CT, followed by bone scans, abdominal ultrasound, endoscopic ultrasound, and so on. Gastric cancer patients with stages II, III, and IV usually received adjuvant chemotherapy after a curative operation. About half of the surgeons regarded 2 cm as a safe resection margin in early gastric cancer and 5 cm in advanced gastric cancer. More than half of surgeons usually performed a D2 lymph node dissection in early gastric cancer and D2+$\alpha$ lymph node dissection in advanced gastric cancer. About $20\%$ of surgeons performed less invasive surgery and/or function-preserving surgery, such as a pylorus-preserving gastrectomy, a laparoscopic wedge resection, or a laparoscopy-assisted distal gastrectomy.

Surgical Extent for Ground Glass Nodules

  • As diagnoses of small ground glass nodule (GGN)-type lung adenocarcinoma are increasing due to the increasing frequency of computed tomography (CT) screening, surgical treatment for GGN-type lung adenocarcinoma has rapidly become more common. However, the appropriate surgical extent for these lesions remains unclear; therefore, several retrospective studies have been published and prospectively randomized controlled trials are being undertaken. This article takes a closer look at each clinical study. Convincing evidence must be published on 2 issues for sublobar resection to be accepted as a standard surgical option for GGN lung adenocarcinoma. In the absence of such evidence, it is better to perform lobar resection as long as the patient has sufficient lung function. The first issue is the definition of a sufficient resection margin, and the second is whether lymph node metastasis is conclusively ruled out before surgery. An additional issue is the need for an accurate calculation of the total size and solid size on CT. Given the results of clinical studies so far, wedge resection or segmentectomy shows a good prognosis for GGNs with a total size of 2 cm or less. Therefore, sublobar resection will play a key role even in patients who can tolerate lobectomy.
