The Status of Infection for Infectious Diseases and Status of Infection Control in Occupational Therapy - Focusing on Busan (작업치료실의 감염 실태와 감염예방 관리 실태 : 부산지역을 중심으로)

  • Objective : The aim of this study was to investigate the status of infection for infectious diseases and infection control in occupational therapy. Methods : This study was implemented through the survey composed of the status of infection for infectious disease and the infection control and awareness of the infection control. The duration of survey was a month, June, 2016. 111 occupational therapists in 23 room of occupational therapy completed a survey. Results : 34.8% of occupational therapy room experienced the infectious diseases. This study showed the occupational therapists have high performance in almost items of infection control. But the occupational therapist showed the low performance in the items including 'Washing hands over than 15 seconds', 'Washing hands before and after wearing gloves', 'Wearing mask during treatment', 'Throwing away mask after using' and 'Washing your uniform separate from other cloths'. Almost occupational therapists think infection control is important and the education for infection control is needed in occupational therapy. But most of the education is implemented in only medical institution. Conclusion : To improve performance in infection control, individual effort of occupational therapists and institutional strategies are needed. This study will use as basic data for the education about infection control aimed at the occupational therapists.

Development of Firefighting Performance Test Drills while Wearing Personal Protective Equipment (소방방화복을 착용했을 때의 소방진압 업무 적합도 평가 프로토콜의 개발)

  • A firefighting simulation was developed in order to assess the physical work capacity of firefighters. The simulation consisted of eight common firefighting tasks, including walking with radiant heat for 3 min while wearing full personal protective equipment (PPE). Nine professional firefighters performed the test a total of three times with a 5 min rest interval between each session. The entire series of tests took approximately 30 min to complete ($381{\pm}30s$). Rectal temperatures were found to increase from $37.4{\pm}0.3^{\circ}C$ to $38.5{\pm}0.4^{\circ}C$, while heart rates were found to increase from $92{\pm}18bpm$ to $185{\pm}13bpm$ during testing. Time to completion of the test drills and non-modified physical fitness values showed a significantly negative correlation (r = -0.728, p < 0.05). Firefighters who had longer periods of firefighting service had longer duration time and also recorded higher scores using an integrated scoring system of time to completion (TTC) and physiological strain index (PhSI). The results indicated that the determination of TTC alone can be a misrepresentation of capability, as it neglects to account for accumulated heat strain. The simulated firefighting test provided a useful insight into physical fitness level, but also the comprehensive work capacity of the firefighters when assessed based on TTC and PhSI.

Study on Compliance in Sleep Life Log: Observational Cohort Study (수면 라이프로그 순응도에 대한 연구)

  • Objectives : This study collected sleep information by wearable device in the Korean medicine Daejeon citizen cohort (KDCC). It was measured based on the sleep record information measured by wearing a Fitbit, and the possibility of clinical use was examined for compliance with objective sleep collection. Based on compliance, the possibility of clinical use was examined. Methods : After surveying personal information and PSQI(Pittsburgh Sleep Quality Index), sleep information was collected by Fitbit for 14 days. Compliance was measured based on sleep record information by Fitbit. Compliance was analyzed by sex, age, BMI(Body Mass Index), and sleep group(deep/poor). Results : The number of participants was 730, and the compliance was 94.3%, and the compliance group was 675(92.5%). The age of the participants varied from 30 to 60 years old, and the average age was 46±6.7 years. There were 218 males and 512 females. Young people have high compliance. Males are more compliance than females. As the BMI score decreased in the 30s, the compliance was higher. The underweight group in all age groups had 100 compliance. The underweight group was all female. The low compliance groups were that 30 years males (obesity level2), 50 years females (overweight group), and 50 years females (obesity level2). There was no significant difference in compliance between deep sleep group and poor sleep group. In deep sleep group, females showed higher compliance. In poor sleep group, males showed higher compliance. The average duration of Fitbit usage among participants was 20.1 days. The compliant group wore the device for an average of 21.3 days, while the non-compliant group wore it for only 5.2 days. Of the compliant group, 86.9% (73.8% of all participants) continued to wear the Fitbit after the recommended 14-day period, and 50.8% wore it for more than 20 days. Conclusions : This study showed the possibility of adaptation for wearing a Fitbit for collecting objective sleep information. It is judged that the compliance is high because it was worn for more than 13.2 days out of the 14 days required. It is considered meaningful because the compliance was measured based on the sleep information by Fitbit, not the questionnaire. As the data on objective sleep time is collected automatically, we believe that the burden on participants after the study period is not significant for a certain period. Compliance may be even higher for cohorts related to illnesses and with doctor's orders, rather than for the general population.

A Study on Diagnostic Criteria of Noise-Induced Hearing Loss among Workers in an Iron Foundry (철강공장 근로자를 대상으로 살펴본 소음성 난청 진단기준에 관한 조사)

  • This study was carried out to evaluate diagnostic criteria of noise-induced hearing loss (NIHL) among-workers in an iron foundry. Of 1,093 workers under the observation of noise-specific health examination, 184 workers were selected by way of first and second screening audiometric examination. A questionnaire survey, otological examinations, Rinne test and audiometric test were performed and the results were as follows ; The degree of hearing impairment in the left ear was more severe than in the right ear (p<0.05). The difference between hearing threshold of the first and the second hearing test at 1,000 Hz was about 5 dB with a narrow range of deviations while the difference at 4,000 Hz was about -7 dB with a wide range. Of the total study workers, 84.8% were tested within 15 hours away from noise exposure, and the rest after 16 hours. This study has identified that mean hearing loss at 4,000 Hz showed a significant statistical difference among the two study groups while mean hearing loss by 4-divided classification did not. The same phenomena were observed between the group with and without tinnitus and between the group with and without difficulty in hearing (p<0.05). Among 184 workers, 10 workers (5.4%) diagnosed as NIHL by old diagnostic criteria in contrast to 150 workers diagnosed as NIHL by the new diagnostic criteria. There was a significant difference between the two groups in the average hearing loss at 4,000 Hz and 4-divided classification (p<0.01), but there were no significant differences in age, the duration of employment, blood pressure and the duration wearing the personal hearing protector (p>0.05). If we apply Early Loss Index (ELI) method, some workers in younger age group diagnosed as NIHL by the new diagnostic criteria were fallen into within the normal range. In the mean time older age group show reverse results in contrast to the above finding. It is too early to confirm the value of the usage of the new diagnostic criteria in hearing examination. Further study is called for to verify the value of this criteria.

  • In the clothes industry, there are lack of experts including technical designers who can analyze the fit of clothes. This study is to provide practical data available for fit analysis education by distinguishing the differences in standards and aspects of garment fit between experts and novices, through the eye-tracking technology to quantify the sense of fit. For this study, two groups were organized; one composed of 7 experts with over 15 year-experience including technical designers and patternmakers, and the other composed of 7 novices who are students majoring in clothing. Wearing the goggle type eye-tracker Tobii Pro Glasses 2, the participants in the experiments were required to conduct fit analyses for a pair of pants on a live model. After those experiments, they were required to check the items for fit analysis and assess the importance level of them on a questionnaire. The differences between the two groups in the ratios of total visit count and total visit duration by each AIO(Area of Interest) of clothes were analyzed through non-parametric statistical test. The results of eye tracking experiments showed that experts focused on center front and back line, crotch area, and side seam, while novice's fixation points were dispersed around the pants. The survey results showed that the experts put importance on the center line position and its verticality, front-back proportion of side seam line, and front-back proportion of waist line, 71.4~100% of whom checked them, while 14.3% of the novices checked them.

A Study on the Therapeutic Shoes for Diabetic Patients (당뇨병 환자의 치료용 신발에 대한 연구)

  • Purpose: To investigate the characteristics of the patients and therapeutic shoes for diabetic patients. Materials and Methods: Forty two diabetic patients who had their own therapeutic shoes which were prescribed somewhere else were studied from March 2003 to December 2003. There were 27 males and 15 females, and the mean age was 62.1 years (range, 49-72 years). Duration of diabetes was average 14 years (range, $6{\sim}30$ years), all had type 2 diabetes. Sensation was examined with 5.07 nylon monofilament. The route of purchasing the shoes, compliance to the prescribed shoes were investigated by interview. The shape of shoe, stiffness of upper, conformity of insole to the shape of the foot were recorded. In-shoe plantar pressure was measured in 15 patients. Results: Eighteen patients were insensate to the monofilament. Seven patients did not wear the therapeutic shoes, and only 18 of 35 patients were wearing the therapeutic shoes more than 6 hours a day. The shoes of 17 patients were prescribed by medical doctor and the rest were purchased by the recommendation of acquaintances or advertisement. Ulcer recurred in four of five patients to whom the shoe was prescribed by medical doctor and the cause of three recurrences were evident by just observing the foot and shoe. The therapeutic shoes were made from 11 different makers. Eight shoes were adequate for diabetic patients with respect to the material, shape of insole, type of shoe. In-shoe plantar pressure was examined in 15 patients and was less than 300 kPa in all patients. Conclusion: The therapeutic shoes for the diabetic patients need to be prescribed by medical doctor for selective patients with neuropathy or previous history of ulcer and follow-up examination is important to monitor the compliance of the patients and adequacy of the shoes.

  • this study was designed to discuss the pattern of Confucian funeral ceremony and its costumes in Chosun dynasty thereby exploring the ideas and symbolic meanings expressed on them. 1. Male's official mourning dresses were composed of Choieui symbolizing sadness Choisang(최상), Dukun Sangkwan Sujil Yojil Kyodai and mourning stick Female's official mourning dresses consisted of Daersoojangkon and Kaedoo which were of simple kinds as compared to those of males. Duration of wearing mourning dreses was the longest to the death of father husband and eldest son The forms of mouring dresses were one and the same irrespective of the noble and the mean which expressed the sameness of the sorrow full of losting blood relative. Symbol expressed on funeral ceremony costumes were something like "eldestson-oriented" "male-centered" "distinction of the sexes" and "principle of yin and yang" to reinforce vitality to the costumes. 2. In the other funeral ceremony costumes in a royal fami8ly were those Euijangsu in red and blue colors Yeosakon in purple and white colors. Bandsman in Danryung and Guards in Kukonbok Changeui and Hoeui with five colors while the generally of people woere white dress in the ceremony Hoeui with five colors symbolized the principle of Yin and Yang costumes of military uniforms the dignity of the royal family: white dresses grife symbolizing the dignity of a royal family and Confucian sorrow as well 3. Unlike the mouring dresses there were great differences relative to socio-ecpmpomic status in funeral goods for dercoration Soyeo and daeyeo Among the goods the instrument to drive out evil spirites symbolized shamanism the distinction of Soyeo and Daeyeo separation of body and soul; the sculpture of the Lord of Hades and messenger philosophy of buddhistic netherworld: the engraving of blue and yellow dragons in bier and Juksanma the principles of Yin and Yang : elaborate burial accesaries realistic viewpoint of the hereafter.

A comparative study of contact lens wearer with dry-eye patient on tear function tests (콘텐트렌즈 착용자와 건성안 환자의 눈물 검사에 대한 비교 연구)

  • This study was performed to compare the relationship between contact lens wearer and dry-eye patient. In this study, TBUT, SIT, TTT, Rose bengal staining and McMonnies dry eye symptom questionnaire were performed as a baseline. With the base data, the subjects were classified to 3 groups : 3S patients who have dry eye signs and symptoms, 38 patients who are wearing soft contact tenses, 35 subjects who have health eyes and never worn on a contact lenses as control subjects. Contact lens wearers were divided into 3 groups according to the duration of contact lens wear. There were no significant differences in TBUT, STT, TTT, Rose-bengal staining and McMonnies dry eye symptom questionnaire result between contact lens wearer group and dry eye patients group. We suggest that there are similarities in tear function tests between the dry-eye patient and the contact lens wearer.

  • The reduction of risk within the workplace has long been focus of attention both through industry initiatives and legislation. Exposure matrices according to industries and processes treated DMF (N,N-Dimethylformamide) were constructed based on KOSHA (Korean Occupational Safety and Health Agency)'s 2005 exposure database which were gathered from Korean agencies of workplace hazards evaluation for business place. These exposure matrices were assessed by danger value (DV) that was calculated from combination of hazard rating, duration of use rating, and risk probability rating of exposure to chemical hazardous agents in accordance with Hallmark Risk Assessment Tool. The results of risk assessment is divided four kinds of control bands which were related with control measures. The applicability of risk assessment using exposure matrices was performed by field study and survey for high matrices group. This study found that more attentions should be paid to two industries, manufacture of sewn wearing apparel and manufacture of textiles, among 19 industries, and to 3 processes, coating, processing & mixing, and lab, among 80 processes because those were regarded as having the highest risk.

Preliminary study of Korean orthodontic residents' current concepts and knowledge of cleft lip and palate management

  • Objective: A national survey was conducted to assess orthodontic residents' current concepts and knowledge of cleft lip and palate (CLP) management in Korea. Methods: A questionnaire consisting of 7 categories and 36 question items was distributed to 16 senior chief residents of orthodontic department at 11 dental university hospitals and 5 medical university hospitals in Korea. All respondents completed the questionnaires and returned them. Results: All of the respondents reported that they belonged to an interdisciplinary team. Nineteen percent indicated that they use presurgical infant orthopedic (PSIO) appliances. The percentage of respondents who reported they were 'unsure' about the methods about for cleft repair operation method was relatively high. Eighty-six percent reported that the orthodontic treatment was started at the deciduous or mixed dentition. Various answers were given regarding the amount of maxillary expansion for alveolar bone graft and the estimates of spontaneous or forced eruption of the upper canine. Sixty-seven percent reported use of a rapid maxillary expansion appliance as an anchorage device for maxillary protraction with a facemask. There was consensus among respondents regarding daily wearing time, duration of treatment, and amount of orthopedic force. Various estimates were given for the relapse percentage after maxillary advancement distraction osteogenesis (MADO). Most respondents did not have sufficient experience with MADO. Conclusions: These findings suggest that education about the concepts and methods of PSIO and surgical repair, consensus regarding orthodontic management protocols, and additional MADO experience are needed in order to improve the quality of CLP management in Korean orthodontic residents.
