Clinical Study on Weak Children (허약아클리닉에 내원한 환아들에 대한 임상적 연구)

  • The frequency of weak children, derived from constitutional and functional factors, has increased recently, but their symptoms have never been observed and analysed. A survey was conducted to evaluate and understand the characteristics of weak children. 311 children who had visited in the weak child clinic of Kyunghee Oriental Medical Hospital from January, 1999 to August, 1999, were the respondents of the Weak Child Questionnaire. They were divided into 4 groups by age and birth weight, and statistical significances were tested by one way analysis of variances and bivariate correlation analysis among groups. The results obtained were as follows: 1. Distribution of sex : male 191 cases, female 120 cases 2. The distribution of age showed the highest in $2{\sim}6$ years(45.3%), followed by $7{\sim}12$ years(31.8%), $0{\sim}1$ years(18.0%), over 13 years(4.8%) in order. There was a statistical difference of all symptoms' mean, except respiratory symptoms, among groups(p<0.01). 3. The distribution of birth weight showed the highest in $2.5{\sim}3.3kg(43.4%)$, followed by $3.3{\sim}4.1kg(39.5%)$, under 2.5kg(5.1%), over 4.lkg(2.1%) in order. There was a statistical difference of digestive symptoms' and total score mean among groups(p<0.01, p<0.05 respectively). 4. The correlation coefficient between general symptoms' mean and other symptoms´ is the highest(p<0.01). From the above results, it was suggested that general symptoms are representative of weak children's characteristics.

A Study on the Reliability and Validity of the Five-Visceral Weak Children Questionnaire for Children with Anorexia (식욕부진 소아 대상 오장허약아 설문지의 신뢰도 및 타당도 연구)

  • Objectives This study aimed to assess the reliability and validity of the Five-Visceral Weak Children Questionnaire (FWCQ) in childhood anorexia and to propose optimal cutoff values. Methods We conducted two surveys, spaced one month apart, targeting the parents of 366 children aged 2-9 years who had been diagnosed with anorexia for at least one month. To evaluate the reliability of the FWCQ, we calculated Cronbach's alpha coefficient and employed the test-retest method. Additionally, correlation analysis was performed between the FWCQ and each visceral question, and discriminant validity was assessed by comparing responses from the anorexic and normal groups. Furthermore, we determined the cutoff value of the spleen weak score, which is closely associated with appetite, for the anorexic group. Results The first survey yielded responses from 336 participants, followed by 171 responses in the second survey conducted one month later. The FWCQ demonstrated high internal consistency (Cronbach's alpha = 0.776), and test-retest results were consistent. The correlation coefficients between the FWCQ and each visceral question were all statistically significant. Moreover, significant discriminant validity was observed between the anorexic and normal groups in the FWCQ score and the heart, spleen, and kidney weak scores. The optimal cutoff value for the spleen weak score in the anorexic group was found to be 5.5 points (with a sensitivity of 73.44% and specificity of 66.47%). Conclusions The FWCQ exhibited strong reliability and validity in children with anorexia.

The Characters of Autonomic Nervous System in Heart Weak Children through Analysis of Heart Rate Variability (심박변이도 분석을 통한 심계 허약아의 자율신경계 특성)

  • Objectives The purpose of this study is to evaluate the characteristics of autonomic nervous system in heart-weakened children via analyzing heart rate variability (HRV) compare to healthy children. Methods Among the children who have visited the department of pediatrics at OO oriental medicine hospital, the subjects were composed of 62 elementary school students without cardiac disorder, who have yet develop secondary sexual characteristics. Results 1. Mean HRT and SDNN of the heart-weakened group of children were lower than the healthy group, but with no statistical significance. 2. Heart-weakened children had higher LF norm and LF/HF ratio, but lower HF norm than healthy children. The rest of the Frequency Domain Index have no significant differences. 3. Heart-weak score showed a positive correlation with Mean HRT and LF/HF ratio. Conclusions Heart-weakened children had an imbalance in autonomic nervous system due to increase of sympathetic nerve activity and decrease of parasympathetic nerve activity.

  • Objectives : Osteoporosis is generally occurred in old age, especially postmenopausal women therefore, it is relatively overlook osteoporosis in children, but the authors have paid attention to bone density in children because maximizing peak bone mass of childhood is advocated as a way to prevent osteoporosis and environmental factor of childhood play role in the regulation of bone density. the present study was performed to measure bone density of children and to assess the influence of age, sex, body size, exercise, weak symptoms on bone density during the period of bone growth. Methods : We have measured bone density of calcaneus bone in 283 children (male 144 female 139, ranged from3 to 18) who visited in growth clinic of Pediatrics, Dongguk University Bundang Oriental Hospital, using quantitative ultra sound densitometry and they were divided into 3 groups by age(3-10, 11-15, 15-18 years). Pearson's correlation was used to assessed the influence of age, body size and Student's t-test was used to detect differences in sex, exercise, weak symptoms between the groups on bone density Results : The bone density correlated with age, height, weight, body mass index in this study. Bone density were significantly higher in males than in female. Digestive weak children's bone density were lower than normal children. Conclusion : Weight-bearing exercise and food rich in nutrition are necessary to increase maximizing peak bone mass in children.

  • The pediatrics has other diagnosis and treatment compared with an adult. It is therefore, the children have different pathogenesis, pathology from adult's. The special feature of the children is two. Firstly, children are the body of the pure Yang[純陽之體], which means the growth and development of them are very fast. Secondly, they have weak muscle, skeletal and digestive function. So we must consider health-preserving method in different physiology and pathology aspects of the children. The children have weak digestive functions, so they should be careful to avoid eating greasy food and overeating. They must eat the nourishing Eum[滋陰] food. The children should wear thin clothes as possible. If the children wear clothes so thick, it promote the Yang (陽), so the Eum(陰) is hurted. The children are to be lack of Eum[陰不足], so they need to grow and raise the Eum(陰) in this way which the lower body is cool. Sleep is important ways to generate Eum Essence[陰精]. So the children should take a bed as change of the season. Children's muscles and skeleton are growing so incomplete that they are hard to handle Wind-Cold pathogen[風寒邪]. Therefore, the children should be training vital Essence and Gi(氣) with appropriate exercise. We should be careful treatment the Hwayeoljeung(火熱證) with purgation[瀉下] - bitter in taste and cold in nature[苦寒藥], aromatic herb[芳香性藥物], sudorific herb[發汗藥] In addition, child has weak digestive function, so doctor should not use a lot of nourishment[滋補].

A Survey on Children's Magazines in Korea (최근발간된 한국 어린이 잡지의 분석)

  • The magazine in mordern society is a powerful mass-media. Children are generally interested in magazines, which are relatively in expensive, readily and reguarly, and easy to read. The research chose 5 children's magazines published in Korea in 1973 for close analysis. The Sonyon (Youth) was found to be closer to a pure literary reading for children; the Sae Sonyon (New youth) and Sonyonsegeh (World of the youth) were purposes oriented toward popular amusement purposes carrying a great number of catoons; the Sonyon Chungang (Chungang youth) carried too many advertisements; and the Okae Dong Mu (Rear group) was rather weak in contents in fine contrast with its colourful appearance. The following characteristics were generalized from analysis of the five children's magazines published in Korea: (1) The function of periodic information dissemination was very weak, feared in danger of being wiped out. : (2) The educative and guidance function was found rather weak. (3) Economic motives seemed stronger in spite of the expressed zeal for missions of magazine. (4) In terms of contents variety was lackey, with little individuality for each magazine. (5) Generally emphasis was given to pictoral contents rather than to reading articles.

A Study for Corelation between Action Type and Physical Symptoms in Children (소아(小兒) 행동양식(行動樣式)과 소증(素症)의 관련성(關聯性)에 관한 탐색적(探索的) 연구(硏究))

  • Objective : This study was designed for find corelation between behavior and physical symptoms in child. Physical symptoms appearing related with weak viscera are selected. And behaviors related to selected symptoms are compared that of children with character type defined by jung, c. G. Method: 497 children ( 3-11 years old ) who have visited two oriental clinic ( one is in Gang-Nam Gu Seoul (282 children), the other is in Joong-Rang Gu Seoul (215 children) ) from Jan. to Jul. 2006 were invested through the questionnaires, which were filled by their parents. These questionnaires have 61 yes-no type questions about behaviors on 12 situations, 36 yes no type questions about physical symptoms in 15 categories. 6 questions expected positive answer in child with weak spleen are categorized sp-q and 6 related weak lung, lu-q, 6 related weak kidney, ki-q. Numbers of positive answers in sp-q, lu-q and ki-q are counted so-index, lu-index and ki-index each. Corelation between behavior questions and 3 symptom indexes are measured and questions shown low significance level ( under 5% in pearson corelation, two-tailed ) are selected. Result: 15 behaviors related sp-index, 12 related lu-index, 12 related ki-index are selected and analysed. Conclusion : 1. Behaviors related sp-index is similar to that of Thinking Type children Grouped by jung, c. G.. 2. Behaviors related lu-index is similar to that of Sensation Type. 3. Behaviors related sp-index is similar to that of Feeling Type.

Study on Weak Children Information Collection Using Personal Health Record (PHR) (개인건강기록 (PHR)을 이용한 허약아 정보 수집 연구)

  • Objectives The Korean Medicine (KM) PHR platform is a personalized healthcare service which allows individuals to keep and manage their own health records. When parents are reporting for their children from their memories, there is high possibility of recall errors. In these cases, it could be useful for doctors to collect the patient's symptoms through PHR platform. In this study, we aimed to investigate the clinical significance of the PHR by using the KM PHR platform in the pediatric clinic. Methods The PHR platform was used to collect child health information from parents and child care teachers. The collected data were analyzed in comparison with the results of screening by pediatrician. Results A total of 58 children were recruited, 44 of which health information were collected from their parents and their child care teachers. The remaining 14 children's health information were collected from their parents only. As a result the parents tended to perceive their children weaker than the child care teacher. Compared to other organs, there was a only significant difference in the heart weakness score and spleen weakness score in the comparison of the weak and healthy children. Conclusions Although the study was conducted on a small group of subjects, and used PHR platform developed specifically for adults to indirectly input child's symptoms, and analyzed their health information, there was a difference in health records between information providers. Development of PHR platform for children is needed to collect more reliable information.

Clinical study for inspection of face color and tongue state on Weak Children (허약(虛弱)을 주소(主訴)로 내원한 환아(患兒)의 임상적 특징과 진단법(診斷法)에 관한 연구 -안색(顔色)과 설진(舌診)을 중심으로-)

  • We have plenty of study of adults diseases, but not much has been said about children. 79 children who had visited in the weak children of Dongeui Oriental Medical Hospital from March, 2002 to October, 2002, were the respondents of the Weak Child Questionnaire. The results obtained were as follows: 1. Their symptoms have been observed with sequences of respiratory diseases, Digestive diseases, Psycho-neurological diseases. 2. Distribution of sex and ages : male 38 cases, female 41 cases. $0{\sim}6$ years 60 cases, $7{\sim}12$ years 17 cases, $13{\sim}18$ years 2 cases. 3. The texture of complexion was resulted in face color and the region of the face : whitish-2/ cases. yellowish-27 cases, darkish-22 cases, blue-2 cases, flushed face-l case. Sangeun(山根)-blue : 59 cases, a lower eyelid- dark or light brown : 54 cases. the rests of color on face region didn't mean. 4. The texture of tongue was ended in tongue state : tongue form-not plump not haggard patten(57 cases), a pricky patten(12), a map patten(7), a fissured patten(3). tongue color- pink (57 cases), red(16), light white(6). coating color- thin white(61 cases), thin yellow(l0), lack(6), deep and white(2). tongue coating substance-thin(72 cases), moisten(5), dry(1), deep(1).

  • A clinical observation by the tongue change as a diagnostic method was made on 36 week children visited to Dongeui Oriental Medical Hospital from August. 2001 to December 2001 and the observed results were summarized as follows. 1. Their symptoms have been observed with sequences of Resparatory diseases, Digestive, Neuromotor, Psycho-neurological, Urogenital diseases 2. Distribution of sex : male 22 cases, female 14 cases 3. Distribution of age : 0-6 years 24 cases, 7-18 years 12 cases 4. The texture of tongue was resulted in tongue color in pink-28 cases, red-8 case, and shape in thin-18 cases, swelling-6 cases, a fissured patten-5 cases, a prickly patten-4 cases, and a map patten-3 cases, and fur of the tongue in thin-26 cases, moisten-8 cases, deep-2 cases, and coating color of the tongue in thin and whitish-15 cases, thin and yellowish-9 cases, lack-8 cases, deep and whitish-4 cases. 5. The texture of tongue in connection with weak children was come out mostly normal state and the rest red color, a fissured patten, a prickly patten, deep coating, thin and yellowish etc appeared to cause of slight damage due to exogenous pathogenic fators and due to internal injury.
