Effects of Squat Exercise according to Weight Support on Balance and Gait in Patients after Total Hip Replacement: a Pilot Study

  • Purpose: The purpose of this pilot study is to identify the problems and stability of a study to investigate "Effects of Squat Exercise according to Weight Support on Balance and Gait in Patients after Total Hip Replacement." before proceeding with the study. Methods: Twenty-two rehabilitation patients after THR surgery who met the selection criteria participated. The study subjects were randomly assigned to a squat group using a slider or a squat group using a reformer. The interventions were applied for two weeks. The patients were assessed using Berg balance scale (BBS), Timed up and go test (TUG), and 10-meter walking test (10MW). Results: Although twenty-two study subjects participated in this study, eight study subjects participated dropouts occurred during the study period. There was a significant difference within the group in BBS and TUG in two groups (p<0.05). The difference between the two groups was not significant in all outcome measures (p>0.05). The largest effect size was 1.21 and the smallest effect size was 0.39, all from the BBS. Conclusion: This pilot study suggest that it is feasible with minor adjustment to conduct a larger scale, powered RCT to examine the efficacy of squat exercise according to weight support with patients after THR.

Morphological Changes in Quadriceps Muscles through 3-Week Combined Exercise using a Wearable Robot (EX1) in Young Adult

  • Objective: This study aims to analyze the effect of regular exercise through the combined walking-oriented aerobic and resistance exercises using EX1 in young adults. Design: Experimental one group pre and post test Methods: Participants comprised17 healthy young adults. All subjects performed a combined exercise program for 10 times using EX1. We measured quadriceps muscle thickness using ultrasound. Additionally, the hand grip strength test, and sit and reach test were performed before and after the exercise. Through paired t-test, we investigated whether there was a statistically significant difference in the measurement results after exercise program. Results: The rectus femoris muscle contraction ratio showed significant difference after exercise(P< 0.01). In the sit and reach test, flexibility showed significant difference after exercise(P < 0.01). The hand grip strength test also showed significant difference after exercise(P < 0.05). Conclusions: Healthy young adults can effectively perform various exercises commonly performed in daily life using EX1.

Comparison of Bag Weight and Applied Location on Plantar Pressure and Gait in the Twenties Student

  • Objective: The purpose of this study is to confirm a comparison of bag weight and applied location change on plantar pressure and gait parameters (stride time, stride length, cadence) of the 20s students. Design: One group cross-sectional design Methods: The subjects were measured in each condition. Each condition was no carrying a bag, carrying a 3kg bag, and a 6kg bag on the right shoulder, and walked on the gait analyzer. After the location of the bag was changed, measured when a 3kg backpack and a 6kg backpack was each carried on the back while walking. Results: There was a statistically significant difference in plantar pressure according to bag weight when carried on the right shoulder (p<0.05). As a result of the post-hoc analysis, carrying a 6kg bag was a significant difference between no carrying a bag and carrying a 3kg bag (p<0.05). There was no statistically significant difference in changes in stride time, stride length, and cadence. There was no statistically significant difference in plantar pressure and gait parameters according to backpack weight. Conclusions: As a result of this study, it is thought that it will be possible to present a method for the application of the correct bag on location and weight in 20s students, and to prevent and reduce muscle musculoskeletal system diseases caused by incorrect bag wearing.

Associations among Physical Activity, Comorbidity, Functional Capacity, Peripheral Muscle Strength and Depression in Breast Cancer Survivors

  • Physical inactivity may an important outcome in the prognosis of breast cancer. Physical activity levels decrease significantly for breast cancer patients following the treatment and remain low after oncology treatment is completed. The aim of this study was to determine physical activity levels and to examine associations among physical activity, comorbidity, functional capacity, peripheral muscle strength, and psychosocial status in breast cancer patients. Forty breast cancer survivors were included in this study. Demographic and clinical characteristics were recorded. Physical activity was assessed with a short form of the International Physical Activity Questionnaire (IPAQ). We defined comorbid diseases according to the Charlson Comorbidity Index (CCI). Functional capacity was evaluated with a six-minute walk test (6MWT). Peripheral muscle strength was measured for quadriceps femoris muscle with a hand-held dynamometer. Psychosocial status was measured with the Hospital Anxiety Depression Scale (HADS). Forty percent of patients were inactive. The IPAQ total score was significantly related with quadriceps muscle strength (r=0.492; p<0.001) and HADS depression score (r=0.341; p<0.05). Marked correlations were also observed between IPAQ walking score and quadriceps muscle strength (r=0.449; p<0.001), HADS depression score (r=0.341; p<0.05), and CCI (r=-0.433; p<0.001). The CCI score was also markedly associated with quadriceps muscle strength (r=-0.413; p<0.001). 6MWT distance was not significantly correlated with any of the parameters. Regression analyses revealed that psychosocial status and peripheral muscle strength were significant predictors of physical activity estimated with the IPAQ short form and when combined, they explained 35% of the variance. Comorbidities, peripheral muscle strength and psychosocial status partially explain the variability of physical activity level in breast cancer survivors. These results suggest that physical inactivity contributes to worsening health in breast cancer survivors.

Analysis of Food Intake and Physical Activity in Randomized Controlled Trials on Herbal Medicine for Treatment of Human Obesity (비만 치료 한약 무작위 대조 임상시험에서의 음식 섭취량과 운동량 실태분석)

  • Objectives: The objective of this study was to analyse the methods being used to control food intake and physical activity in RCTs of human obesity. Methods: A total of 21 randomized controlled trials (RCTs) were investigated. Nine of which were domestic studies from "http://oasis.kiom.re.kr" and the other of which were foreign studies from systematic reviews of RCTs on herbal medicine for treatment of human obesity. Results: According to domestic studies, "low calorie diet" were recommended in five cases of the domestic studies, "maintain current dietary habit" were recommended in two and no information on diet was two. Considering the seven cases where the information on diet was available, patients' food intake were checked at every visit in six cases. Only two cases among the six had been dropped owing to the violation of dietary habit by patients. Exercises were prohibited in two cases, "maintain current level of phisical activity" were recommended in three cases and, from the rest, no information was available. The level of physical activity were not strictly controlled by any means hence no drop out. According to foreign studies, "low calorie diet" were recommended in two cases, "very low calorie diet (less than 700 kcal/day)" in one case, "maintain current dietary habit" in two cases, "do not eat fat" in two cases and no information was available in the rest five cases. Exercises which concerns spending about 300 kcal/day was recommended in one case, "moderate exercise" were recommended in three cases, "maintain current level of physical activity" were recommended in three cases and no information available in the rest five cases. Conclusions: In order to improve the accuracy of RCT, for the dietary side, researchers should record patient food intake at every visit by means of 24-hour dietary recall methods. This can be supplemented by multiple choice survey that are designed to help patients to diagnose themselves more accurately leading to less bias. For the exercise side, it is highly recommended to confine the exercises to walking only so as to quantify the amount of physical activity more easily by using pedometer.

The Effects of Ohyaksungi-san(Wuyaoshunqi-san) on Cognition and Motor Function Recovery after Ischemic Brain Injury in Rat (오약순기산(烏藥順氣散)이 허혈성 뇌손상 흰쥐의 인지 및 운동기능회복에 미치는 효과)

  • Objectives : Ohyaksungi-san(Wuyaoshunqi-san) has been used for many years as a treatment for cerebrovascular diseases in Oriental medicine. This study was designed to evaluate the effects of Ohyaksungi-san(Wuyaoshunqi-san) on cognition and motor function recovery after ischemic brain injury, and also the expression of BDNF in hippocampus. Methods : This study was designed with 4 subgroups to evaluate the effects of Ohyaksungi-san(Wuyaoshunqi-san). As control groups, group I has no treatment during 1 week after ischemic brain injury and group II has no treatment during 2 weeks after ischemic brain injury. As experimental groups, group III has been treated with Ohyaksungi-san(Wuyaoshunqi-san) during 1 week after ischemic brain injury and group IV have treated with Ohyaksungi-san(Wuyaoshunqi-san) during 2 week after ischemic brain injury. Each group has been examined by tests as follows, neurological motor behavioral tests, cognitive motor behavior test and histological test. Neurological motor behavior tests consisted of limb placement test, beam-walking test and horizontal wire test. Cognitive motor behavior test was performed by using Morris water maze. In the histological test, TTC(2,3,5-triphenylteterazolium chloride) staining, hematoxylin & eosin staining, and immunohistochemical staining were used. Results : 1. The tests for motor function recovery change had significantly good result in the experimental groups as compared with control groups(p<.05). 2. The Morris water maze test on cognition also had significantly good result in the experimental groups as compared with control groups(p<.05). 3. In the immunohistochemical staining for the expression of BDNF in hippocampus, more immune reaction was investigated in the experimental groups as compared with control groups. Especially group IV has the greatest immune reaction. Conclusions : Ohyaksungi-san(Wuyaoshunqi-san) has good effects on cognition and motor function recovery after ischemic brain injury, and also the expression of BDNF in hippocampus.

The Relationship Between Self-Concept and Self-Care Activities of the Cerebral Palsied Adolescents (뇌성마비 청소년의 자아개념과 자가간호수행의 관계)

  • The purpose of this study was to investigate the relationship between self-concept and self-care activities of the cerebral palsied adolescents and to gain the baseline data for development of effective rehabilitation nursing intervention program of the cerebral palsied adolescents. The design of this study was a descriptive correlational study. The subjects of the study were 160 cerebral palsied adolescents attending at special schools located in Seoul and Kyonggi and rehabilitation centers located in Seoul, Kyonggi and Kyongnam province. The data was collected from May 20 to July 20, 2000. The instrument used for this study were the self-concept scale(50items 4point scale) and self-care activities scale(29items 4point scale). Self-Concept Scale had developed by Fitt(1965), which was standardized by Chung(1968) and modified by Kim(1984). Self-Care Activities Scale developed by the researcher through out the consulting of expert and pilot study on the basis of ADL check list developed by Kang(1984) and LDSQ-3(Lambeth Disability Screening Questionnair-3) developed by Na et al. (1995). The data was analyzed by the SPSS/PC+program using frequency, percentage, mean, standard deviation, t-test, ANOVA & Scheffe test and Pearson's correlation coefficient. The results of this study were as follows; 1. The mean score of self-concept was $138.55{\pm}17.20$(range: 50-100), which the item mean score was $2.77{\pm}0.34$(range: 1-4). The score of subarea of the self-concept was the highest score in family self ($3.01{\pm}0.54$) and the lowest score in physical self ($2.52{\pm}0.42$). 2. The mean score of self-care activities was $95.25{\pm}21.69$ (range: 29-116), which the item mean score was $3.28{\pm}0.75$ (range: 1-4). The score of subarea of the self-care activities was the highest score in feeding($3.75{\pm}0.59$) and the lowest score in walking($2.64{\pm}1.21$). 3. There were statistically significant difference in the score of self-concept according to the age (F=3.24, P=.04), the grads (F=4.36, P=.01), and types of cerebral palsy (F=2.42, P=.03). 4. There were statistically significant difference in the score of self-care activities according to the age (F=8.29, P=.00), the grads (F=16.05, P=.00), types of living place (F=6.46, P=.00), types of cerebral palsy (F=48.92, P=.00), whether or not receiving a rehabilitation therapy (t=-3.64, P=.00), whether or not receiving a vocational training (t=2.14, P=.03), and whether or not using a device (t=-7.42, P=.00). 5. There was not significant correlation between self-concept and self-care activities (r=.081, P=.311).

Neuroprotective Effects of Sopung-tang(Shufeng-tang) on Cognition and Motor Function Recovery after Ischemic Brain Injury in Rats (소풍탕이 허혈성 뇌손상 흰쥐의 인지 및 운동기능회복에 미치는 효과)

  • Objectives : Sopung-tang(Shufeng-tang) is a famous herbal prescription that treated ischemic brain injury. This study was designed to evaluate the effects of Sopung-tang(Shufeng-tang) on congnition and motor function recovery after ischemic brain injury in rats. Methods : Male rats were divided into 4 groups. Those rats caused ischemic brain injury by occlusion of MCA as Longa method. Control group I was per os normal saline for 7 days after ischemic brain injury. Control group II was per os normal saline for 14 days after ischemic brain injury. Experimental group I(Ex I) was taken with Sopung-tang(Shufeng-tang) for 7 days after ischemic brain injury. Experimental group II(Ex II) was taken with Sopung-tang(Shufeng-tang) for 14 days after ischemic brain injury. The author carried out neurological, cognitive motor behavior tests and histological assessment. Neurological motor behavior tests consist of limb placement test, beam-walking test and horizontal wire test. Morris water maze test was used for cognitive motor behavior test. In the histological assessment test, TTC(2,3,5-triphenylteterazolium chloride) staining, Hematoxylin & Eosin staining and immunohistochemical staining were experimented. Results : 1. In neurological motor behavior tests, motor function recovery was significantly increased in the experimental groups as compared with control groups(p<0.05). Especially Ex II was significantly increased as compared with Ex I(p<0.05). 2. In Morris water maze test, congnitive motor function recovery was significantly increased in the experimental groups as compared with control group(p<0.05). Especially Ex II was significantly increased as compared with Ex I(p<0.05). 3. In the immunohistochemical staining for the expression of BDNF in hippocampus, more immune reaction was investigated in the experimental groups as compared with control groups. Especially most immune reaction was experimented in the EX II. Conclusions : According to the above results, Sopung-tang(Shufeng-tang) can treat on the congnition and motor function recovery after ischemic brain injury in rats. And it is effective method in expression of BDNF in hippocampus.

The Effect of Golf Exercise through Rehabilitation Training for Middle-aged Women (중년여성의 재활트레이닝을 통한 골프운동의 효과)

  • The purpose of this study is to verify the effect of golf exercise through rehabilitation training for middle-aged women and to suggest the right golf activities. To achieve the purpose of this study, the subjects were 40-50 year old middle-aged women in Jinju, Gyeongnam Province in February 2020. The subjects of this study were 8 women who were controlled by the subjects who needed to be corrected in golf swing orbit. For the accurate measurement test, the program was conducted for 10 days after explaining the purpose and utilization plan of the study. The data collected by testing level of physical strength and distance before and after the experiment were finally analyzed and used. The statistical processing of the collected data was conducted using SPSS win18.0 program, and the statistical techniques were calculated by means of frequency analysis, average(M) and standard deviation(sd), and t-test, one-way ANOVA and multiple regression analysis were conducted. The results of this study through these methods and procedures are as follows. First, rehabilitation training of general characteristics showed a high difference in golf exercise. Second, there was a high difference in the level of rehabilitation training and physical fitness in swing orbit and distance. Third, rehabilitation training and physical fitness level had a high effect on swing orbit and distance.

A Study of Influence of Asymmetrical Weight-Bearing on the LOS of Independent Ambulatory Hemiparetic Patients on Standing (편마비 환자의 비대칭적 체중지지가 기립균형 안정성 한계에 미치는 영향)

  • Decreased equilibrium in standing and walking is a common problem associated with hemiparesis secondary to cerebral vascular accident. In patients with hemiplegia, postural sway is increased and often displaced laterally over the non-affected leg, reflecting asymmetry in lower extremity weight bearing during standing balance. Human balance is a complex motor control task, requiring integration of sensory information, neural processing, and biomechanical factors. Limits of stability (LOS) is a one of the biomechanical factors. The purposes of this study were to establish the influence of asymmetrical weight-bearing on the LOS of independent ambulatory hemiparetic patients. The subjects of this study were 29 hemiparetic patients (18 males, 11 females) being treated as admitted or out patients at Young-Nam University Hospital and Taegu Catholic University Hospital, all of whom agreed to participate in the study. Participants were asked to lean and displace their center of gravity (COG) as far as possible in directions to the sides and front of the body. The LOS and weight-bearing ratio were measured with a Balance Performance Monitor (BPM) Dataprint Software Version 5.3. In order to assure the statistical significance of the results, the independent t-test and a Pearson's correlation were applied at the .05 and .01 level of significance. The results of this study were as follows: 1) There were statistically significant differences in anteroposterior LOS according to the cause of brain demage (p<.01). 2) There were statistically significant differences in mediolateral LOS according to the hemiparetic side (p<.05). 3) There were statistically significant differences in anteroposterior and mediolateral LOS according to the brain operation (p<.01). 4) The mediolateral LOS significantly correlated with weight-bearing ratio (p<.01).
