The Differences of Clinical Aspects in Children and Adolescents Poisoning (소아 및 청소년 중독 환자에서 중독 양상의 차이)

  • Objectives: To analyze the general characteristics and clinical differences of poisoning in children and adolescents and to take precautions of occasions. Methods: We retrospectively evaluated poisoning children and adolescents (less than 19 years) visiting to the emergency medical center of tertiary hospital in urban area. We collected demographic data, substance exposure data (materials, causes of poisoning and amount), and clinical outcome of poisoning for the past 2 years and 9 months. Results: 189 cases were reported of concerning poisoning in children and adolescents. The age groups were divided into four categories. (1) Infants group:<2 year, (2) Preschool age group: $2{\sim}5year$, (3) Children group: $6{\sim}12year$ and (4) Adolescents group: $13{\sim}18year$. The most vulnerable age group was the infants group. There were two-peaks of age distribution in poisoned patients on the whole. Various types of materials belonged to classes of druqs (56.6%). household products (34.4%) and industrial solvents (9.0%). On adolescents group, the frequency of drug poisoning was significantly high, in comparison with infants, preschool age, and children group (p=0.001). Most of the patient groups had been poisoned accidentally(73.5%), while most cases of adolescents poisoning had been intentional. 63% of the adolescents group had a suicidal purpose. Conclusion: The incidence of poisoning was most highly due to drugs. The cause of poisoning is most commonly accidental. while in adolescent group, intentional poisoning is mostly common. Special cares, like keeping children away from drugs, will be needed to prevent children poisoning, and psychiatric consultation and supportive cares can reduce the adolescents poisoning cases.

Relationship between Nutrient Intake Ratio and Sedentary Time of Female Adults by Dehydration Estimated with Blood Urea Nitrogen to Creatinine Ratio: Based on the 2016~2018 Korea National Health and Nutrition Examination Survey (혈중 요소질소와 크레아티닌 비율에 근거한 성인 여자의 탈수 여부에 따른 영양소 섭취 정도와 앉아서 보내는 시간과의 관련성: 2016~2018년 국민건강영양조사를 이용하여)

  • There is a dearth of Korean studies on dehydration, one of the health risks for adult women. This study analyzed the subjects of the 2016~2018 National Health and Nutrition Examination Survey (KNHANES) to investigate the relationship between nutrient intake ratios and sedentary time in dehydrated women. Body mass index and waist circumference in the dehydrated group (DG) were significantly higher than the normal group (NG). Intake of other liquids in the DG was significantly higher than in the NG, but total water intake in the DG was significantly lower than in the NG. Compared to the 600 sedentary minutes or more per day group, the odds ratio of dehydration was significantly higher in the less than 300 sedentary minutes per day group (1.871 [95% CI: 1.579~2.215], P<0.001). Compared to the drinking over 6.0 cups of water per day group, the odds ratio of dehydration was significantly higher in the 3 cups or fewer per day (OR [95% CI] is 1.198 times [1.019 to 1.408], P<0.05), and 6.0 cups or fewer group (OR [95% CI] is 1.162 times [1.018 to 1.326], P<0.05). The results of this study showed that the total water intake was relatively lower in the ≥600 sedentary minutes per day group compared to the 300 sedentary minutes per day group, making them more vulnerable to dehydration. The nutrient intake ratios were higher in the dehydration group than in the normal group. In conclusion, emphasis should be placed on the importance of hydration and nutrition education for adult women in the workplace.

The Effects of a Fall Prevention Program on the Low-Income Elderly at Risk of Falls (저소득층 낙상위험 노인을 대상으로 한 낙상예방 프로그램의 효과)

  • Purpose: This study was conducted to determine the effects of a fall prevention program on knowledge, efficacy, and prevention behavior of falls among the low-income elderly. Methods: This study used a nonequivalent control group pre and post test quasi-experimental research design. The study was conducted from August to October, 2008 with 20 subjects in the experimental group and 22 in the control group who were registered at the public health center of S District in D City. Results: Hypothesis 1 "The experimental group that participated in the fall prevention program will show a higher degree of knowledge about falls than the control group" was supported. Hypothesis 2 "The experimental group will have a higher degree of self-efficacy of falls than the control group" was not supported. Hypothesis 3 "The experimental group will report fall prevention behavior more frequently than the control group" was supported. Conclusion: It was verified that the fall prevention program in this study was an effective intervention to improve knowledge about falls and fall prevention behaviors among the low-income elderly. The results can be used as part of an intervention to prevent falls for the vulnerable elderly such as the low-income elderly.

Dehydration Risk from Age, BMI, and Disease Exposure (연령, BMI, 질병노출로 인한 탈수 위험)

  • Objective : The purpose of this study was to investigate the risk factors of dehydration from the subjects who underwent anthropometric and blood parameters testing during a comprehensive health screening. Methods : For the study analysis, 5,391 samples with valid data of the levels of Sodium($Na^+$), BUN (Blood Urea Nitrogen) and FBS(Fasting Blood Sugar) were selected to calculate a dehydration indicator of plasma osmolality. The study data was collected from the health screening examinees who visited Sahmyook Medical Center Seoul Adventist Hospital Comprehensive Health Check-up Center from 2014.01.01 to 2015.12.31. The relationship between dehydration and age group, BMI, disease exposures(hypertension, diabetes mellitus, dyslipidemia, kidney disorder) were analyzed by gender. Results : The odds ratio of dehydration showed statistical significance from age ${\geq}50$ in both male and female, respectively. The female obese group was vulnerable to dehydration while the male study group showed no statistical significance in the BMI difference. The disease exposed groups(hypertension, diabetes mellitus, dyslipidemia, kidney disorder) were vulnerable to dehydration. Also, the more types of disease carried by the exposed patients, the higher odds ratio and susceptibility to dehydration. Conclusions : Aging, increasing BMI, and exposed to diseases were found to be the risk factors for vulnerability to dehydration. To prevent dehydration, special caution to be taken for those in the ${\geq}50s$ group, along with controlling BMI and chronic diseases. Further studies are suggested to investigate the risk factors of dehydration that may affect increasing plasma osmolality as a potential stimulus mechanism in disease outbreaks.

Anti-apoptotic and Neuroprotective Effects of Gastrodiae Elata Pharmacupuncture at $GB_{20}$ on Focal Brain Ischemic Injury Induced by Intraluminal Filament Insertion in Rats (풍지(風池)($GB_{20}$)의 천마약침(天麻藥鍼)이 Intraluminal Filament 삽입술에 의하여 유발된 백서(白鼠)의 뇌허혈에 미치는 영향)

  • Objectives : Delayed neuronal death(DND) of pyramidal neuronsin the CA1 regions of the hippocampus has been extensively studied following global brain ischemia, whereas little is known about DND in this highly vulnerable brain region after focal brain ischemia. The aim of the present study was to investigate the effect of Gastrodiae Elata(GA) pharmacopuncture on hippocampal neuronal apoptosis in rats with focal brain ischemic injury. Materials and methods : The neuroprotective effects of water extracts of GA were investigated in middle cerebral artery occlusion(MCAo) of Sprague-Dawley(SD) rats. Seventy-five healthy SD ratswere randomly divided into five groups following MCAo : control group with focal ischemia, saline injection group, pharmacopuncture group GA-1($0.0007mg/m{\ell}/g$), pharmacopuncture group GA-2($0.00035mg/m{\ell}/g$), pharmacopuncture group GA-3($0.00014mg/m{\ell}/g$). Results : The intensity of mGluR5 increased in the GA-1 group. The intensity of Bax and the Bax/Bcl-2 ratio decreased in the GA-1 group. The intensity of Bcl-2 increased in all the GA groups. The density of neurons stained by Cresyl violet and ChAT increased in the GA-1 group. Conclusions : Our study suggests that GA pharmacopuncture at $GB_{20}$ showed anti-apoptotic and neuroprotective effects on cholinergic neuronsin focal cerebral ischemia caused by stroke in SD rats.

  • Group oriented multicast service is a prerequisite for the current application system for remote lecture and customer service. IP multicast is used to be generally accepted as an internet standard. IP multicast which is designed to support network based replication model can efficiently use host and network resource, however it has some weak points that it has to support IP multicast in the internet by adding multicast-capable internet infrastructure such as router and is vulnerable to security by using public IP address for group identifier. Therefore we propose the trigger based application level multicast model that can enhance both scalability and security by separating the functions, which send and receive message to solve these problems. Our suggested model is expected to ensure the promotion of quality of service and reliability.

Susceptibility of rat hippocampal neurons to hypothermia during development

  • Purpose: This study evaluated the extent of damage due to hypothermia in the mature and immature brain. Methods: Hippocampal tissue cultures at 7 and 14 days in vitro (DIV) were used to represent the immature and mature brain, respectively. The cultures were exposed at $25^{\circ}C$ for 0, 10, 30, and 60 minutes (n=30 in each subgroup). Propidium iodide fluorescent images were captured 24 and 48 hours after hypothermic injury. Damaged areas of the cornu ammonis 1 (CA1), CA3, and dentate gyrus (DG) were measured using image analysis. Results: At 7 DIV, the tissues exposed to cold injury for 60 minutes showed increased damage in CA1 (P<0.001) and CA3 (P=0.005) compared to the control group at 48 hours. Increased damage to DG was observed at 24 (P=0.008) and 48 hours (P=0.011). The 14 DIV tissues did not demonstrate any significant differences compared with the control group, except for the tissues exposed for 30 minutes in which DG showed less damage at 48 hours than the control group (P=0.048). In tissues at 7 DIV, CA1 (P=0.040) and DG (P=0.013) showed differences in the duration of cold exposure. Conclusion: The immature brain is more vulnerable to hypothermic injury than the mature brain.

The Effects of a Personality Development Program on Ego-Resilience and Maladaptive Behavior in Children Using a Community Child Center (심성계발프로그램이 지역아동센터 이용 아동의 자아탄력성 및 부적응행동에 미치는 효과)

  • Purpose: This study examined the effects of a personality-development program on ego-resilience and maladaptive behavior of school age children. Method: This study used a non-equivalent control group pre-post design. The data was collected from Apr. 2007 to Nov. 2007. The subjects consisted of 33 children. The experimental group (n=17) participated in a self-development program that consisted of 8 sessions during an 8 week period, each session lasted for 45 minutes. To examine the effects of the self-development program an ego-resilience scale and behavior problem scale (BPPS)of K-YSR were measured before and after the program. Result: The results of analyses indicated that the score of ego-resilience increased but not significantly in the experimental group. There was significant change in anxiety/depression, social problems, and aggressive behavior of BPS of K-YSR after the intervention. Conclusion: This study showed that the 'Personality-Development Program' was effective on maladaptive behavior. Therefore, it is advisable to use it in a community-based child center for maladaptive behavior in vulnerable children.

Geographical Distributions of the Aged population in Korea - Rural-specific approach with GIS - (노인복지 수요예측을 위한 노인 인구의 공간적 분석 - GIS를 이용한 농촌인지적 접근 -)

  • The aged population in Korea, especially in rural areas, has been growing rapidly. The welfare for the rural elderly has become a major concern, however we don't have enough information about that population; we don't even have detailed demographics of it. The present research is aimed at; 1) investigating the tendency and changing geographical distributions of the rural elderly, and 2) introducing GIS(Geographic Information Systems) as a useful tool in analyzing geographical distributions of the aged. The General Census Data from 1960 to 2000 was used to carry out this study. The major findings are; 1) There has been a gradual decrease in the rural population over the past 40 years, but this tendency has slowed down more recently; 2) It was observed that the 'oldest-old' group aged 85 years old and over has actuality increased in rural areas faster than any other age group; 3) The changing patterns of the elderly population were different in metropolitan areas and rural areas. In brief, there are far more aged people, especially of the 'oldest-old' group and females in rural areas than those in urban areas. These population, the 'oldest-old' or females, are the most vulnerable and have the greatest need for social welfare and social services of different kinds.

Analysis and Comparison of Labor Market Stability by Business Categories in Urban and Rural Areas : Industrial Group, Employment Size, and Survival Duration (도시 및 농촌지역 사업체 유형별 노동시장 안정성 비교분석: 산업군, 종사자규모 및 존속기간별 유형을 중심으로)

  • Stability of labor market in rural areas was analyzed in this paper with categories of industrial group, employment scale, surviving period, and founder group. The stability of each classified labor market was compared with each other to figure out the stable business class and the unstable class in rural areas. The results of rural areas were compared with those of urban areas. The stability was analyzed with average and coefficient of variation (C.V.) of annual total employees' change rates. It was revealed that labor market of 'primary industry', including agriculture, is unstable. Especially, labor market of 'mid-size' and 'primary industry' businesses founded as 'incorporated company' in rural areas is vulnerable. While labor market of 'large-size' is proved to be unstable, it is confirmed that 'small-size' or 'mid-size', and 'over-ten-year survived' businesses have positive contribution to the stable labor market in rural and urban areas. The results show that the stability of labor market is different in each category of business and in each region of rural or urban area. It is expected that the results can be utilized for the regional development policies, of labor and industry part.
