Imaging Assessment of Visceral Pleural Surface Invasion by Lung Cancer: Comparison of CT and Contrast-Enhanced Radial T1-Weighted Gradient Echo 3-Tesla MRI

  • Objective: To compare the diagnostic performance of contrast-enhanced radial T1-weighted gradient-echo 3-tesla (3T) magnetic resonance imaging (MRI) and computed tomography (CT) for the detection of visceral pleural surface invasion (VPSI). Visceral pleural invasion by non-small-cell lung cancer (NSCLC) can be classified into two types: PL1 (without VPSI), invasion of the elastic layer of the visceral pleura without reaching the visceral pleural surface, and PL2 (with VPSI), full invasion of the visceral pleura. Materials and Methods: Thirty-three patients with pathologically confirmed VPSI by NSCLC were retrospectively reviewed. Multidetector CT and contrast-enhanced 3T MRI with a free-breathing radial three-dimensional fat-suppressed volumetric interpolated breath-hold examination (VIBE) pulse sequence were compared in terms of the length of contact, angle of mass margin, and arch distance-to-maximum tumor diameter ratio. Supplemental evaluation of the tumor-pleura interface (smooth versus irregular) could only be performed with MRI (not discernible on CT). Results: At the tumor-pleura interface, radial VIBE MRI revealed a smooth margin in 20 of 21 patients without VPSI and an irregular margin in 10 of 12 patients with VPSI, yielding an accuracy, sensitivity, specificity, positive predictive value, negative predictive value, and F-score for VPSI detection of 91%, 83%, 95%, 91%, 91%, and 87%, respectively. The McNemar test and receiver operating characteristics curve analysis revealed no significant differences between the diagnostic accuracies of CT and MRI for evaluating the contact length, angle of mass margin, or arch distance-to-maximum tumor diameter ratio as predictors of VPSI. Conclusion: The diagnostic performance of contrast-enhanced radial T1-weighted gradient-echo 3T MRI and CT were equal in terms of the contact length, angle of mass margin, and arch distance-to-maximum tumor diameter ratio. The advantage of MRI is its clear depiction of the tumor-pleura interface margin, facilitating VPSI detection.

  • This paper focuses on a systematic configuration of steam reforming fuel processor, particularly designed for small and medium sized hydrogen production application. In a typical integration of the fuel processor, there exist significant temperature gradients over the entire system which has negative effect on both catalyst life-time and system performance. Also, the volumetric inefficiency should be avoided to obtain the possible compactness for the commercial purpose. In the present work, the computational analysis will be performed to gain the fundamental insight on the transport phenomena and chemical reactions in the reformer consisting of preheating, steam reforming (SR), and water gas shift (WGS) reaction beds in the flow direction. Also, the fuel processing system includes a top-fired burner providing necessary thermal energy for endothermic catalytic reactor. A fully two-dimensional numerical modeling for a integrated fuel processing system is introduced for in-depth analysis of the heat and mass transport phenomena based on surface kinetics and catalytic process. In the model, water gas shift reaction and decomposition reaction were assumed to be at equilibrium. A kinetic model was developed and then computational results were compared with the experimental data available in the literature. Finally, the case study was done by considering the key parameters, i.e. steam to carbon (S/C) ratio and temperature. The computer-aided models developed in this study can be greatly utilized for the design of advanced fast-paced compact fuel processors research.

A Study on the Prediction of the Strength and Axial Strain of High-Strength Concrete Columns Confined by Tie Reinforcement (띠근 보강 고강도 콘크리트 기둥의 강도 및 축변형 특성 산정에 관한 연구)

  • The use of high-strength concrete which permits smaller cross sections, reduced dead loads, and longer spans has been getting more popular in tall buildings. However, there has been little research on behavior of high-strength concrete columns laterally reinforced with square ties and subjected to compressive loading. With the addition of transverse reinforcement which lead to triaxial compressive state, ductility behavior of high-strength column member shall be increased. In this study, rational quality and quantity evaluations were made to investigate the ultimate strength and strain ductility by confinement effect of tie reinforced high-strength concrete columns subject to uniaxial loads. Concrete failure theory at the triaxial compressive state and statistical results based on conventional experimental data were applied for this propose. Up to 185 columns, tested under monotonically increasing concentric loading, were evaluated in terms of strength and strain ductility. Analytical results show that confinement stress, maximum compressive strength, and increase of strain equations were developed with the consideration of concrete strength, yield strength, spacing, volumetric ratio, and configurations of tie reinforcement.

Micro-computed tomography analysis of changes in the periodontal ligament and alveolar bone proper induced by occlusal hypofunction of rat molars

  • Objective: To three-dimensionally elucidate the effects of occlusal hypofunction on the periodontal ligament and alveolar bone proper of rat molars by micro-computed tomography (micro-CT). Methods: Occlusal function in the molar area was restricted by attaching an anterior bite plate on the maxillary incisors and a metal cap on the mandibular incisors of 5-week-old male Wistar rats for 1 week. The periodontal ligament space and alveolar bone proper around roots of the mandibular first molar were assessed by histology and micro-CT. Results: The periodontal ligament space was narrower and the alveolar bone proper was sparser and less continuous in the hypofunction group than in the control group. Further, both the volume of the periodontal ligament and the volumetric ratio of the alveolar bone proper to the total tissue in the region of interest were significantly lower in the hypofunction group (p < 0.05). Conclusions: Occlusal hypofunction induces atrophic changes in the periodontal ligament and alveolar bone proper of rat molars.

Relationship between chin deviation and the position and morphology of the mandible in individuals with a unilateral cleft lip and palate

  • Objective: In this study, we aimed to examine the relationship between chin deviation and the positional and morphological features of the mandible and to determine the factors that contributed to chin deviation in individuals with a unilateral cleft lip and palate (UCLP). Methods: Cone-beam computed tomography (CBCT) images of 28 adults with UCLP were analyzed in this study. Segmented three-dimensional temporomandibular fossa and mandible images were reconstructed, and angular, linear, and volumetric parameters were measured. Results: For all 28 individuals, the chin was found to deviate to the cleft side by 1.59 mm. Moreover, among these 28 individuals, only 7 showed distinct (more than 4 mm) chin deviation, which was toward the cleft side. Compared to the non-cleft side, the mandibular body length, frontal ramal inclination, and vertical position of the condyle were lower and inclination of the temporomandibular fossa was steeper on the cleft side. Furthermore, the differences in inclination of the temporomandibular fossa, mandibular body length, ramus length, and condylar volume ratio (non-deviated/deviated) were positively correlated with chin deviation. Conclusions: UCLP individuals show mild chin deviation to the cleft side. Statistical differences were noted in the parameters that represented positional and morphological asymmetries of the mandible and temporomandibular fossa; however, these differences were too small to indicate clinical significance.

Combustion and Emission Characteristics of Biodiesel Blended Fuel by EGR Rate in a 4-cylinder CRDI Diesel Engine (4실린더 커먼레일 디젤엔진에서 바이오디젤 혼합연료와 EGR율에 따른 연소 및 배기특성)

  • This study describes the effect of EGR rate on the combustion and emissions characteristics of a four cylinder CRDI diesel engine using biodiesel (soybean oil) blended diesel fuel. The test fuel is composed of 30% biodiesel and 70% ULSD (ultra low sulfur diesel) by volumetric ratio. The experiment of engine emissions and performance characteristics were performed under the various EGR rates. The experimental results showed that ignition delay was extended, the maximum combustion pressure and heat release gradually were decreased with increasing EGR rate. Comparing biodiesel blended fuel to ULSD, the injection quantity of biodiesel blended fuel was further increased than ULSD. The emission results showed that $NO_x$ emission of biodiesel blended fuel becomes higher according to the increase of EGR rate. However, in the case of biodiesel blended fuel, HC, CO and soot emissions were decreased compared to ULSD.

Orbital Wall Restoring Surgery in Pure Blowout Fractures

  • Background Restoring orbital volume in large blowout fractures is still a technically challenge to the orbital surgeon. In this study, we restored the orbital wall using the combination of transorbital and transnasal approach with additional supports from the paranasal sinuses, and we compared the surgical outcome to that of a conventional transorbital method. Methods A retrospective review of all patients with pure unilateral blowout fractures between March 2007 and March 2013 was conducted. 150 patients were classified into two groups according to the surgical method: conventional transorbital method (group A, 75 patients, control group), and the combination of transorbital and transnasal approach with additional supports from the paranasal sinuses (group B, 75 patients, experimental group). Each group was subdivided depending on fracture location: group I (inferior wall), group IM (inferomedial wall), and group M (medial wall). The surgical results were assessed by the Hertel scale and a comparison of preoperative and postoperative orbital volume ratio (OVR) values. Results In the volumetric analysis, the OVR decreased more by the experimental groups than each corresponding control groups (P<0.05). Upon ophthalmic examination, neither the differences among the groups in the perioperative Hertel scale nor the preoperative and postoperative Hertel scales were statistically significant (P>0.05). Conclusions Our surgical results suggest that orbital volume was more effectively restored by the combination of transorbital and transnasal approach with additional supports from the paranasal sinuses than the conventional method, regardless of the type of fracture.

Effects of Elutriating Rates for Elutriated Acid Fermentation of Food Waste (음식물쓰레기 세정산발효공정에서 세정율의 영향)

  • The korean government started to ban the sanitary landfill of food wastes as of 2005. The radical change of policy is primarily due to the limited landfill site, but aimed to promote not only to reduce the food waste production but also to enhance the reuse and recycle. The performance of elutriated acid fermentation to evaluate the effects of elutriating ratios was investigated. The fermenters were operated with elutriating water to food waste volumetric ratio of 2, 1, 0.5 and 0.25. Initial pH of elutriating water was set for 9 based on the pH effects study. The cumulative amounts of SCOD production rate were $0.34gSCOD/gVS_i$, $0.45gSCOD/gVS_i$, $0.26gSCOD/gVS_i$ and $0.28gSCOD/gVS_i$ with the ratios of 2, 1, 0.5 and 0.25, respectively. The cumulative productions of VFAs were 0.12 gVFAs as $COD/gVS_i$, 0.28 gVFAs as $COD/gVS_i$, 0.21 gVFAs as $COD/gVS_i$ and 0.14 gVFAs as $COD/gVS_i$ with the ratios of 2, 1, 0.5 and 0.25, respectively. The volume reduction were 58%, 52%, 45% and 47% with the ratios of 2, 1, 0.5 and 0.25, respectively.

Effects of Chemical Admixture on the Quality Characteristics of Grout for Prestressed Concrete (화학 혼화제가 PSC용 그라우트 품질 특성에 미치는 영향)

  • The study investigates the effects of the content and using method of chemical admixtures such as superplasticizer and viscosity modifying admixture on the fluidity, bleeding ratio, volumetric change and compressive strength of the grout in order to provide basic data for the development of high-quality grout for prestressed concrete. It appeared that the combination of superplasticizer and viscosity modifying admixture decreased the fluidity of grout with small content of superplasticizer. On the contrary, Grout used more than 0.1% of superplasticizer appeared to have significant effect on the improvement of the fluidity. On the other hand, bleeding of grout reduced according to increasing the content of viscosity modifying admixture. Superplasticizer with less than 0.05% had practically no effect on the reduction of bleeding, whereas superplasticizer with more than 0.1% appear to have significant effect on the reduction of bleeding. Also the combination of superplasticizer with 0.15% and viscosity modifying admixture with 0.15% resulted in satisfactory fluidity accompanied with fair reduction of bleeding and shrinkage of the grout.

Peritumoral Brain Edema after Stereotactic Radiosurgery for Asymptomatic Intracranial Meningiomas : Risks and Pattern of Evolution

  • Objective : To investigate the risks and pattern of evolution of peritumoral brain edema (PTE) after stereotactic radiosurgery (SRS) for asymptomatic intracranial meningiomas. Methods : A retrospective study was conducted on 320 patients (median age 56 years, range 24-87 years) who underwent primary Gamma Knife radiosurgery for asymptomatic meningiomas between 1998 and 2012. The median tumor volume was 2.7 cc (range 0.2-10.5 cc) and the median follow-up was 48 months (range 24-168 months). Volumetric data sets for tumors and PTE on serial MRIs were analyzed. The edema index (EI) was defined as the ratio of the volume of PTE including tumor to the tumor volume, and the relative edema indices (rEIs) were calculated from serial EIs normalized against the baseline EI. Risk factors for PTE were analyzed using logistic regression. Results : Newly developed or increased PTE was noted in 49 patients (15.3%), among whom it was symptomatic in 28 patients (8.8%). Tumor volume larger than 4.2 cc (p<0.001), hemispheric tumor location (p=0.005), and pre-treatment PTE (p<0.001) were associated with an increased risk of PTE. rEI reached its maximum value at 11 months after SRS and decreased thereafter, and symptoms resolved within 24 months in most patients (85.7%). Conclusion : Caution should be exercised in decision-making on SRS for asymptomatic meningiomas of large volume (>4.2 cc), of hemispheric location, or with pre-treatment PTE. PTE usually develops within months, reaches its maximum degree until a year, and resolves within 2 years after SRS.
