A Study of depression symptom in patients with voice disorders (음성장애환자에게서의 우울감 연구)

  • The objectives of this study are to research the frequency of depression symptom in patients with voice disorders and to investigate parameters associated with depression from voice evaluation. A hundred ninety six patients(106 males and 90 females) who had been diagnosed with voice disorders first in their lifetime were selected. All the patients were examined by laryngeal stroboscopy. For depression and voice study, personal interview, acoustic and aerodynamic analysis, voice handicap index(VHI), reflux symptom index(RSI), and beck depression index(BDI) were done respectively. Mild to severe BDI were seen in 26.2%(52 patients) of the whole patients. A BDI mean score of female patients was $8.8{\pm}7.5$ which was higher than that of male patients($5.6{\pm}6.6$), the difference observed being statistically significant(p<0.001). In the acoustic analysis, the score of sent_duration parameter was increasing in the patients with depression, which was significantly higher than the score of the patients without depression(p<0.05). In the addition, the scores of VHI and RSI were higher in the patients with depression(p<0.001). Our findings suggest that the prevalence of depression in patients with voice disorders is related to female, speaking velocity, and self-questionnaire. This result can be used for psychologically based approach to therapy.

Acoustic and Stroboscopic Characteristics in Clergies (목사들의 음성발성에 대한 음향분석학적 특징)

  • Objectives : To compare the objective differences in voice quality and voice problems between clergies and normal male control group. Materials and Methods : The sustained vowel sound of 46 clergies and 40 normal persons were analyzed, using a videostroboscopy and acoustic analyzer. Together with these analyses, a questionnaire associated with current and past voice problems was handed over to the patients. Results : The most common symptom in subjective group was the voice fatigue. Stroboscopic findings in subjective group were as following 23 cases(50%) of pachydermia, 17 cases(37%) of phase difference, 12 cases(25%) of anterior-posterior contracture, 6 cases(13%) of vocal polyp and 3 cases(7%) of vocal nodule. The mean maximal phonation time in clergies was 17.8 seconds and in control group was 19 seconds. litter, pitch perturbation quotient and shimmer were significantly increased in subjective group than in control group(p<0.05), but there were no significant differences between two groups in fundamental frequency, vFo, amplitude perturbation quotient and noise to harmonic ratio. Conclusion : In the clergies using loud and forceful voice, vocal polyp and functional voice disorder findings were frequently noted in stroboscopic examination. litter and shimmer, reflecting the roughness of voice, were increased in acoustic analysis. Therefore, clergies, classified into untrained professional voice users, need professional career guidance and counseling.

A Study of Korean Literature Review Related to Speech Characteristics and Speech Therapy in Patients with Parkinson Disease (파킨슨병 환자의 말 특성과 언어치료 관련 국내문헌연구)

  • The purpose of this study was to investigate the speech characteristics and speech therapy of Parkinson disease (PD). This study selected 28 papers published in Korea from 1998 to 2018 after searching the terms 'Parkinson voice' and 'Parkinson speech therapy.' Literature review had been conducted in the two aspects of speech characteristics and speech therapy. The speech characteristics were divided into respiration, phonation, articulation, prosody, vowel production, and voice questionnaire. Speech therapy was divided into Lee Sliverman voice treatment (LSVT) and other voice therapy. PD patients did not differ in respiration function compared to normal elderly people, but their speech and articulation function were poorer. There was also a difference in the speech rate, frequency of pause, and accuracy of vowel production compared with normal elderly people. PD had a lower VHI score and their voice related quality of life was a little poorer. The LSVT was typically used in speech therapy for PD. The methods of speech therapy for PD have been shown to improve respiration and phonation. It is necessary to establish voice norms in PD patients and develop effective speech therapy in the following study.

  • Background and Objectives : A burn injury to the glottis differs from a burn injury to the trachea, bronchi, and lung parenchyma, in that thermal injury does not occur to any significant degree below the level of the larynx, due to the effective cooling of air by the upper airway and to reflex closure of the vocal cords from a blast of hot air. Therefore, the laryngeal inhalation injury give rise to airway problem and voice change. The objectives of this study is to assess management of laryngeal inhalation injury and voice change after management. Materials and Methods : Voice choses and laryngeal injuries of eight laryngeal inhalation patients were analyzed through questionnaire, voice dynamic laboratory, and laryngeal stroboscopy. Operative management was performed to five patients for airway patiency and vocal cord movement on laryngeal pathology ind voice therapy was performed to all patients. One-year after, voice changes and laryngeal injuries were reanalyzed with same methods. Results : Vocal breathiness, decreased voice intensity, reduced voice range, and easy fatigability were major complaints of laryngeal inhalation patients. Glottic stenosis were developed to five of eight patients, and vocal cord atrophy, bowing were developed to others. Vocal cord mucosal waves were significantly decreased in all patients. Jitter(%), Shimmer(dB) were increased and Maximal phonation time(MPT) was decreased. One-year after, subjective voice changes and objective voice parameters were improved. And vocal cord mucosal waves were recovered in all patients. Conclusions : Subjective voice quality and objective voice parameters were improved after operative management for laryngeal pathology and voice therapy. And we observed recovery of vocal fold mucosal waves by laryngeal stroboscopy. We think that early preventable tracheotomy is necessary to reduce the laryngeal contact injury in laryngeal inhalation patients.

Pediatric Vocal Fold Nodules : Long-term Follow up with Voice Handicap Index after Voice Therapy (소아 성대결절 : 음성치료 후 음성장애지수를 이용한 장기 추적관찰 결과)

  • Objectives : The purpose of this paper is to know the efficacy of voice therapy for vocal fold nodules in children. Materials and Methods : Sixty two patients with vocal nodules(46 boys and 16 girls) were retrospectively reviewed. Age ranged from 4 to 15 years with mean age of 8 years. Questionnaire survey was carried out with voice handicap index, at pretherapy and post-therapy(3 months and 5 years after voice therapy). Results : Sixty two percent of the patients showed improvement by voice therapy. Twenty five patients continued education at home after voice therapy, but only eight childrens were included this group in pre-school aged children. Voice handicap index was improved in 3 months and 5 years after voice therapy. In contrast, pre-school children group showed slight aggravation in 5 years, especially in emotional subdomain. Conclusion : Voice therapy is effective primary treatment in children with vocal fold nodules. Different result between preschool aged children and school aged group shows needs for age specific approach of voice therapy.

The Study on the Relationship between Service Provider's Authenticity and Customer Voice in Service Failure Situation (서비스실패상황에서 서비스제공자의 진정성과 고객발언의 관계에 관한 연구)

  • This study examines the relationship between service provider's authenticity, customer voice, and customer behavior in failure situation. This study employed questionnaire survey and collected 216 data. The structural equation modeling (SEM) method is used to test the hypotheses of the study. As a result, the service provider's authenticity has a significant positive effect on the constructive voice of the customer and a negative effect on the destructive voice in the failure situation. In addition, constructive voice had a significant positive effect on relationship retention intentions and destructive voice had no significant effect on relationship retention intentions. This study suggests the importance of customer voice as well as the factors to increase customer positive voice in the failure situation and provides an opportunity to be interested in customer voice.

Study on Correlation between Acoustic Profiles and Fatigue (노권상과 음성 지표간의 상관성에 관한 연구)

  • Objectives : The purpose of this study is to find out the correlation between the Vocal indicators and the 'Buzhongyiqi-Tang questionnaire' and the 'Chalder fatigue scale.' Methods : This study examined the mean value of each factor in the 'Buzhongyiqi-Tang Questionnaire,' 'Chalder fatigue scale' and the different voice indicators conducted on 81 healthy adult participants in relation to the results of the /a/ /e/ /i/ /o/ /u/ pronunciation test. Results : There was significant correlation between the pronunciation of /a/ /e/ /i/ /o/ /u/ vowels' F0 indexes and 'the Deficiency symptoms of Buzhongyiqi-Tang'. The results of the regression analysis showed the following significant findings for each pronouncing vowels: /i/ as a factor for 'the Deficiency symptoms of Buzhongyiqi-Tang'.; /a/ for 'the Consumptive fever of Buzhongyiqi-Tang'.; /i/ for 'the Vocal inflammation of Buzhongyiqi-Tang.; and /e/ as a factor of 'the Chadler physical fatigue'. Conclusions : The study showed a negative correlation between the Fundamental Frequency and the mean value of the questionnaire, which could be understood as higher the fatigue level, increased level of vocal vibration and higher pitch tone compared to the less fatigued group. We expect future studies to conduct research on methods to diagnose other illnesses using the vocal indicators based on the correlation between the vocal index and illnesses prescribed under traditional oriental medicine.

Effect of the leader's behavioral integrity on the trust in leaders and voice behavior of the Members -moderating effect of phychological safety- (상사의 언행일치가 상사신뢰와 구성원의 발언행동에 미치는 영향 -심리적 안전감의 조절효과-)

  • This study confirmed the effects of leaders' behavioral integrity on organizational members' voice behavior. It verified whether trust in leaders mediates behavioral integrity and voice behavior and ascertained the moderating effect of members' psychological safety on trust in leaders and members' voice behavior through leaders' behavioral integrity. It targeted organizational members in the service industry, including insurance, securities, banks, consulting, and credit card companies, with a questionnaire survey carried out with 424 response copies. The study results confirmed a significant and positive effect of leaders' behavioral integrity on trust in leaders and voice behaviors. Second, trust in leaders was found to mediate between leaders' behavioral integrity and members' voice behavior. Third, members' psychological safety had a moderating effect on trust in leaders and members' voice behavior. When psychological safety was higher than average, there was a moderated mediating effect of psychological safety in the behavioral integrity on voice behavior with trust mediation in leaders. Therefore, this study has significance in that it was determined that trust in leaders and the psychological safety of the members are essential for leaders' behavioral integrity to enhance members' voice behavior.

A Validity Study on Measurement of Mental Fatigue Using Speech Technology (음성기술을 이용한 정신피로 측정에 관한 타당성 연구)

  • This study proposes a method to measure mental fatigue using speech technology, which has not been used in previous research and is easier than existing complex and difficult methods. It aims at establishing a relationship between the human voice and mental fatigue based on experiments to measure the influence of mental fatigue on the human voice. Two monotonous tasks of simple calculation such as finding the sum of three one digit numbers were used to measure the feeling of monotony and two sets of subjective questionnaires were used to measure mental fatigue. While thirty subjects perform the experiment, responses to the questionnaire and speech data were collected. Speech features related to speech source and the vocal tract filter were extracted from the speech data. According to the results, speech parameters deeply related to mental fatigue are a mean and standard deviation of fundamental frequency, jitter, and shimmer. This study shows that speech technology is a useful method for measuring mental fatigue.
