A Lingual Sound Analysis based on Oriental Medicine Auscultation for Heart Diseases Diagnosis (심장(心臟) 질환(疾患) 진단(診斷)을 위한 한의학적 청진(聽診) 기반의 설음(舌音) 분석)

  • Oriental medicine lacks diagnosis data in fixed quantity possible to express visually to patients by depending on clinician's intuition than Western medicine that continues to development by various diagnosis devices. For that, this paper intends to examine relation between heart and voice signal regarded as center organ and source of life and mind in order to implement objectification through the visualization of oriental diagnosis method above all. According to because the heart is related to the tongue among five organs, by thinking with sounds, we would design the way of identifying existence of heart diseases focused on the fact that lingual sound pronunciation of heart patient is inexact. For this, we achieved a comparison, analysis of statistical bandwidth and morphological modeling of the second formants frequency about a lingual sound for their voice constituted subject group of heart diseases and normal people. Finally, we analyzed interrelationship to the result of experiment by designed method.

A Study on the Sasang Constitutional Diagnosis by Perceptual Voice Analysis (청각적(聽覺的) 성음분석(聲音分析)을 통한 사상체질진단(四象體質診斷)에 관한 연구(硏究))

  • 1. Objectives This study was performed by means of perceptual evaluation of the voices of Sasang Constitution. 2. Methods 73 female subjects were classified by means of 3 kinds of Questionnaire papers(QSCCII, QSCCI, Sasang Pattern Identification Questionnaire). So they were categorized into 3 groups, 23 Soyangin, 28 Taeumin and 22 Soeumin. 73 voice samples were presented three times to a group of 5 judges. The time interval between ratings was 14 days. The four goals of this study were to evaluate the intraobserver reliability between each rating, to evaluate the interobserver reliability, to evaluate the reliability between the each rating and Questionnaire result and to make the notion of the consensus of Sasang Constitution's Voice. 3. Results & Conclusions The intraobserver reliability between the first and second rating showed significance statistically among all observers. And the intraobserver reliability between the second and third rating showed significance except one observer. The interobserver reliability among the three ratings showed significance statistically except one to two observers in the first rating and other one to another one in the second rating. In the reliability between the each rating and Questionnaire result, one in the first rating, other one in the second rating and another two in the third rating showed significance. To make the notion of the consensus of Sasang Constitution's Voice, classification into 4 categories was made: clear/hoarse, high/low, fast/slow, powerful/powerless. The voice of Soyangin group was classified as powerful and fast, and that of Taeumin group as powerful, hoarse and low and that of Soeumin group as powerless and slow.

Analysing of the recent trends on the constitution diagnosis and treatment device patents (체질진단 및 치료기기 최신 특허동향 분석)

  • In this paper, We tried to analyze the patent trend on constitution diagnosis and treatment related technologies. For this, constitution diagnosis and treatment related technologies divided into 16 sub-technology by the advisory committee. And we analyzed patents applied in Korea, Japan, U.S.A., China, and Europe. The 16 sub-technology consist of pulse analyzer, skin diagnosis, tongue analyzer, face and body detector, face analyzer, voice analyzer, intelligence ontology, meridian diagnosis, infrared thermography, electric stimulation, laser, high and low frequency, physical, magnetic, and ultrasound therapy. As a result we found that patents of constitution diagnosis and treatment in Korea has been growing steadily in both quality and quantity since 1980s. The number of patent the pulse analyzer and low frequency therapy are larger than others. But applied relevant international patents, marketability of the patent and Cites per Patent (CPP) index are shown to be very low. In conclusion, to occupy key original technologies of the Oriental medical device, we need to apply more patent of the whole related technology and international patents.

Differences in Patient Characteristics between Spasmodic Dysphonia and Vocal Tremor (연축성 발성장애와 음성 진전 환자의 감별)

  • Spasmodic dysphonia, essential tremor, and vocal tremor related with Parkinson's disease are different disorders showing fairly similar symptoms such as difficulty in the speech onset, and tremble in the voice. However, the cause and the resulting treatment of these diseases are different. Spasmodic dysphonia is a vocal disorder characterized by spasms of the laryngeal muscles during a speech, invoking broken, tense, forced, and strangled voice patterns. Such difficult-to-treat dysphonia disease is classified as central-origin-focal dystonia, of a yet unknown etiology. Its symptoms arise because of intermittent and involuntary muscle contractions during speech. Essential tremor, on the other hand, is characterized by a rhythmic laryngeal movement, resulting in alterations of rhythmic pitch and loudness during speech or even at rest. Severe cases of tremor may cause speech breaks like those of adductor spasmodic dysphonia. In the case of hyper-functional tension of vocal folds and accompanying tremors, it is necessary to distinguish these disorders from muscular dysfunction. A diversified assessment through the performance of specific speech tasks and a thorough understanding for the identification of the disorder is necessary for accurate diagnosis and effective treatment of patients with vocal tremors.

  • Sasang medicine is our country's unique traditional medicine based on the classification of individual physical constitution. In the Sasang medicine, what is considered the most important task is to categorize Sasang constitution exactly. Therefore, security of objective elements and diagnosis index is a problem awaiting solution in Sasang constitution classification. To this the paper abstracted result value from objectification, visualization, a fixed quantity of Sasang constitution to analyze face image signals and voice signals. And, comparing the differences constitution would like to develop system classification of Sasang constitution. Specially, image and voice signals are different because of gender, age, region so it composed Sasang constitution group to 40-50 years women in Seoul. To extract of these image and voice signals wanted to perform comparison, analysis in constitution. Finally, it would like to prove a significance of research result through experiment.

Auditory-Perceptual and Acoustic Evaluation in Measuring Dysphonia Severity of Vocal Cord Paralysis (성대마비의 음성장애 측정을 위한 청지각적 및 음향학적 평가)

  • Background and Objectives : The purpose of this study was to investigate the criterion-related concurrent validity of two standardized auditory-perceptual assessments and the Acoustic Voice Quality Index (AVQI) for measuring dysphonia severity in patients with vocal cord paralysis (VCP). Materials and Methods : Total 210 patients with VCP and 236 normal voice subjects were asked to sustain the vowel [a:] and to read aloud the Korean text "Walk". A 2 second mid-vowel portion of the sustained vowel and two sentences (with 26 syllables) were recorded. And then voice samples were edited, concatenated, and analyzed according to Praat script. Two standardized auditory-perceptual assessment (GRBAS and CAPE-V) were performed by three raters. Results : The VCP group showed higher AVQI, Grade (G) and Overall Severity (OS) values than normal voice group. And the correlation among AVQI, G, and OS ranged from 0.904 to 0.926. In ROC curve analysis, cutoff values of AVQI, G, and OS were <3.79, <0.00, and <30.00, respectively, and the AUC of each analysis was over .89. Conclusion : AVQI and auditory evaluation can improve the early screening ability of VCP voice and help to establish effective diagnosis and treatment plan for VCP-related dysphonia.

Usefulness of Cepstral Peak Prominence (CPP) in Unilateral Vocal Fold Paralysis Dysphonia Evaluation (일측성 성대마비 환자 평가에서 Cepstral Peak Prominence의 유용성)

  • Background and Objectives : The purpose of this study was to compare the usefulness of Cepstral peak prominence (CPP) with parameter of Multiple Dimensional Voice Program (MDVP) in evaluating unilateral vocal fold paraylsis patients with subjective voice impairment. Materials and Methods : From July 2014 to August 2016, 37 patients with unilateral vocal fold paralysis who had been diagnosed with unilateral vocal fold paralysis and had received two or more voice tests before and after the diagnosis were evaluated for maximum phonation time (MPT), MDVP and CPP. Respectively. Voice tests were performed with short vowel /a/ and paragraph reading. Results : The CPP-a (CPP with vowel /a/) and CPP-s (CPP with paragraph reading) of the Cepstrum were statistically negatively correlated with G, R, B, and A before the voice therapy. Jitter, Shimmer, and NHR of MDVP were positively correlated with G, R, B. Jitter, Shimmer, and NHR of the MDVP were significantly correlated with the Cepstrum index. G, B, A and CPP-a and CPP-s showed a statistically significant negative correlation and a somewhat higher correlation coefficient between 0.5 and 0.78. On the other hand, in MDVP index, there was a positive correlation with G and B only with Jitter of 0.4. Conclusion : CPP can be an important evaluation tool in the evaluation of speech in the unilateral vocal cord paralysis when speech energy changes or the cycle is not constant during speech.

  • Now that we are entering an aging society, the user interface for new devices and IoT technology is very inconvenient for senior generation. To improve this, we propose an AI technology-based intelligent senior assistant voice recognition system. This system implements Cloud platform based API to accumulate data for machine learning processing, provides content for diagnosis and prevention of dementia, and provide chat-bot content for senior generation. We hope that senior generations will increase the accessibility and convenience of IoT devices and new technology devices with our system.

Evaluation of Larynx Cancer via Chemometrics Assisted Raman Spectroscopy

  • Larynx cancer is a potentially terminal and severe type of neck and head cancer in which malignant cells start to grow and spread upwards in the larynx, or voice box. Smoking tobacco, drinking hot beverages and drinking alcohol are the main risk factors for these tumors. In this study, we aimed to develop a precise, accurate and rapid chemometrics assisted Raman spectroscopy method for diagnosis of larynx cancer in deparaffinized tissue samples. In the proposed method, samples were deparaffinized and 20 microns of each tissue were located on a coverslip. Both healthy (n = 13) and cancerous tissues (n = 13) were exposed to a Raman laser (785 nm) and excitations were recorded between wavenumbers of $50{\sim}1500cm^{-1}$. An Orthogonal Partial Least Square algorithm was applied to evaluate the Raman spectrum obtained. Sensitivity and specificity of the proposed method is high enough with the aid of Principal Component Analysis (PCA) to test the whole model. Healthy and cancerous tissues were accurately and precisely clustered. A rapid, easy and precise diagnosis algorithm was developed for larynx cancer. By this method, some useful data about differences in biomolecules of each group (phospholipids, amides, tyrosine, phenylalanine collagen etc.) was also obtained from the spectra. It is claimed that the optimized method has a great potential for clustering and separating tumor tissues from healthy ones. This novel, rapid, precise and objective diagnosis method may be an alternative for the conventional methods in literature for diagnosis of larynx cancer.

Clinical Evaluation of 3 patients with Paradoxical Vocal Cord Movement (역설적 성대운동을 보이는 3명의 환자에 대한 임상분석)

  • Background and Objectives : Paradoxical vocal cord movement is a series of paroxysmal adduction of the anterior two-thirds of the vocal cords during respiration or during phonation. The choking, stridor, and wheezing in this condition occur primarily on inhalation, rather than on exhalation. The two pathognomonic diagnostic criterias that need to be assessed during an acute presentation are laryngoscopy with direct visualization of paradoxical adduction of the vocal cords and pulmonary function testing. Materials and Methods : A retrospective review of 3 patients who were referred to otolaryngologist from pulmonology department, and were confirmed by typical laryngoscopic findings with paradoxical adduction of the vocal cords was conducted. Results The patients were misdiagnosed as exercised-induced asthma, and unresponsive to corticosteroid and bronchodilators. Improvement was achieved only by diagnosis with paradoxial vocal cord movement. Biofeed back therapy, voice therapy, treatment for reflux laryngitis improved symptoms. Conclusion The etiology of paradoxical vocal cord movement is unknown. It may be functional or emotional. The functional factors that were proposed are neurologic deficit and gastroesophageal reflux. Management methods of this condition consist of psychological counselling, voice therapy, and antireflux medication.
