손목형 생체신호수집 장치에 대한 연구 (A Study on Wrist Band Type Vital Sign Acquisition Device)

  • In this study, we proposed a new method that can be measure ECG (Electrocardiography) and PPG (Photoplethysmography) in realtime on the site of the wrist for check the state of health in daily life. For convenience measurement of ECG the lead I method was used on the wrist, and omit the reference junction ECG I was measured in the right hand and the left hand of the potential difference. Then the measured electrocardiogram was amplified by the differential amplifier and the signals were passed HPF, LPF, and BPF filters. For removing the PPG's noise from the Motion artifact and temperature, we apply the reflective photoelectric volume pulse wave measurement method using green LED as a light source. The circuits was designed to be able to check the waveform using higher active amplification method at weak signals. For the validation of our device, the measured signals were compared with E2-KIT on same time. The results shows that the error does not exceed the maximum one, most of the data is confirmed to be issued Peak inspection of the same number.

과체를 이용한 태음인 긴장성 두통 치험례 3례 (3 Case Reports of Tension-type Headache Taeeumin patients using Melonis Pedicus)

  • Objectives The aim of this study was to report a improvement of tension type headache severity after using the Melonis Pedicus. Methods 3 Taeeumin patients, suffering from headache, treated with Melonis Pedicus, herb medicine, and acupuncture. After the treatment, Severity of headache was evaluated by Numerical Rating Scale(NRS). Also we checked the reponse of the Melonis Pedicus, and vital sign. Results & Conclusions After using the Melonis Pedicus, the severity of headache decreased from NRS 8 to NRS 1(case1), from NRS 10 to NRS 7(case 2), and from NRS 8 to NRS 6(case 3). And there was not any adverse effect. In conclusion, this study shows that using Meloni Pedicus can be effective treatment for Taeeumin tension type headache patient.

병원 전 응급간호사의 응급 처치 수행 능력과 영향 요인 (Factors Influencing on Prehospital Emergency Nurses' Activities and Procedures in the Field)

  • Purpose: This study shows the prehospital emergency nursing practices, and analyzes them associated with their individual characteristics and job conditions. Method: Based on a survey of the National Emergency Medical Center in Korea(2008), principal components were extracted from 7 prehospital emergency nursing practices by factor analysis, and some regression analyses of principal components(CPR-AED and V/S-I.V.) were executed on individual characteristics and job conditions. Results: The PENs gave themselves higher order ratings for vital sign check, airway management for loss of consciousness patients, CPR for suspicious cardiac arrest, keeping vein open for shock patients, AED for abnormal pulse rate, AED for suspicious cardiac arrest, and AED for loss of consciousness. Age and duty periods were statistically significant influential factors on the CPR-AED component. Conclusion: The results indicate that the PENs were competent in overall prehospital emergency activities and procedures even some weak self-evaluations, and that the standard curriculum and practice standard for prehospital nursing should be developed in order to increase nursing leadership in prehospital emergency settings.

자가 섬유아세포-히알루론산 복합체를 이용한 당뇨발의 치료 (Treatment of Diabetic Ulcer Using Autologous Fibroblast-Hyaluronic Acid Complex)

  • Purpose: To evaluate clinical efficacy and safety of hyaluronic acid based autologous dermal fibroblasts (Hyalograft 3D) in the treatment of diabetic foot ulcers. Methods: A total of 28 patients with diabetic ulcers were randomized to either the control group with nonadherent foam dressings(n=14) or the treatment group with autologous tissue - engineered grafts(n=14). Weekly assessment contained vital sign checks, ulcer size measurements, and wound photos. At 12th week, percentages of complete wound healing and mean healing times were compared. Safety was also monitored by adverse events. Results: Complete wound healing was achieved in 84.6% of the treatment group and 23.1% of the control group (p<0.005). The mean times of closures for the treatment versus control groups were 6.1 weeks and 10.9 weeks, respectively. No adverse events related to the study treatment occurred. Conclusion: The use of hyaluronic acid based autologous fibroblast grafts was found to be a safe and effective treatment for diabetic foot ulcers.

Application of Percutaneous Cardiopulmonary Support for Cardiac Tamponade Following Blunt Chest Trauma: Two Case Reports

  • Since the advent of percutaneous cardiopulmonary support (PCPS), its application has been extended to massively injured patient. Cardiac injury following blunt chest trauma brings out high mortality and morbidity. In our cases, patients had high injury severity score by blunt trauma and presented sudden hemodynamic collapse in emergency room. We quickly detected cardiac tamponade by focused assessment with sonography for trauma and implemented PCPS. As PCPS established, their vital sign restored and then, they were transferred to the operation room (OR) securely. After all injured lesion repaired, PCPS weaned successfully in OR. They were discharged without complication on day 26 and 55, retrospectively.

응급구조(학)과 학생의 구급차 동승실습 술기시행 관련 요인 (Related factors involved in the direct practice of practical procedures during the ambulance ride practice of paramedic students)

  • Purpose: The purpose of this study was to provide an appropriate direction for an effective field practice program by analyzing the factors related to practical procedures in the ambulance ride practice of paramedic students in Korea. Methods: Questionnaires were answered by 202 paramedic students from March 28 to April 15, 2014. The collected data were analyzed with SPSS WIN 18.0 program. Results: The general level of procedural exposure during ambulance ride practice was 59.0% on average. Considering the practice of specific areas, vital sign checking, patient assessment, and oxygen supply showed relatively high percentages of 92.3%, 77.4% and 72.6%, respectively, whereas the areas of fluid and drug, airway, and cardiopulmonary resuscitation recorded relatively low percentage of 25.8%, 24.1% and 50.2%, respectively. The level of direct practice was significantly high in the group that included the largest number of paramedics. Conclusion: For an effective field practice, the place of practice needs to have various cases of patients to ensure a wide experience of procedures during the practice, and ambulance workers with a high level of expertise should be selected to ride with students.

정주진정 하에 매복치 발거 시 발생된 Pulsus Paradoxus (Severe Airway Obstruction) -증례보고- (Pulsus Paradoxus During Extraction of Impacted Tooth under Intravenous Sedation -A Case Report-)

  • Pulsus paradoxus has been defined as a decrease in systolic blood pressure (SBP) of 10 mmHg or more during inspiration. This report describes pulsus paradoxus detected by pulse oximetry during dental procedure. Case: A 10 years old boy who had impacted mandibular premolar with malformation scheduled for extraction under intravenous sedation with Fentanyl and Propofol. The patient showed upper airway obstruction with stridor and pulsus paradoxus. Though pulsus paradoxus is generally critical condition, in this case, respiration and other vital sign was maintained comparatively well with care in administering oxygen and considerate monitoring of pulse oximetry and capnography. Discussion: Noninvasive continuous monitoring of pulse oximetry allows recognition of pulsus paradoxus which can lead to serious problems. Clinicians should know very well about it and be able to manage of this kind of situation.

Long-run Equilibrium Relationship Between Financial Intermediation and Economic Growth: Empirical Evidence from Philippines

  • The financial sector is one of the most important building blocks of the economy. When this sector efficiently implemented a well-crafted program on banking and financial system to translate financial activities to income-generating activity, economic growth will be realized. Hence, this study analyzed the effect of financial intermediation on economic growth and the existence of cointegrating relationship using time-series data from 1986 to 2015. The influence of financial intermediation in terms of bank credit to bank deposit ratio, private credit, and stock market capitalization and time trend to economic growth was estimated using ordinary least squares (OLS) multiple regression. The results showed that all the financial intermediation indicators and time trend exert significant effect on Gross Domestic Product (GDP) per capita. The positive sign of the time trend indicates that there is an upward trend in GDP per capita averaging approximately 0.06 percent annually. Furthermore, the cointegration test using the Johansen procedure revealed that there is a presence of long-term equilibrium relationship between financial intermediation and time trend and economic growth, and rules out spurious regression results. This study established the idea that financial intermediation in the Philippines has a significant and vital role in stimulating growth in the economy.

종기(宗氣)의 의미에 대한 고찰(考察) - 기존 논의에 대한 재검토를 중심으로 - (A Study on the Meaning of Zong Qi - A Reconsideration of Existing Discussion -)

  • Objectives : To determine and analyze problems in existing discussions on Zong Qi and to establish a new concept of Zong Qi based on the initial idea that gave birth to this concept through examination of early texts. Methods : Descriptions of Zong Qi in contemporary theoretical texts published in both Korea and China were thoroughly examined. Also, theories of major doctors that dealt with this concept were selectively examined, after which problems of existing discussions on Zong Qi were analyzed. Next, verses that mention contents related to Zong Qi in the "Huangdineijing" were studied for better understanding of the concept. Results & Conclusions : The findings determined that existing discussions could not explain the meaning of Zong Qi properly. Based on new discussions, Zong Qi can be understood as a concept of maintaining the constancy of life dynamic in the Later Heaven(後天) that is lead by Qi from food and drinks. Clinical observation of aforementioned life phenomenon maintained by Zong Qi and additional discussion on its concept is necessary.

Subcapsular Splenic Hemorrhage in Vivax Malaria

  • In malaria, splenic rupture is a serious complication potentially leading to death. Subcapsular hemorrhage of spleen is thought to be an impending sign of splenic rupture; however, the characteristics of subcapsular hemorrhage are not well known. We report 3 cases of subcapsular hemorrhage of the spleen in vivax malaria, with varying degrees of severity. Case 1 showed subcapsular hemorrhage without splenic rupture, was treated by antimalarial drug without any procedure. The healing process of the patient's spleen was monitored through 6 computed tomography follow-up examinations, over 118 days. Case 2 presented subcapsular hemorrhage with splenic rupture, treated only with an antimalarial drug. Case 3 showed subcapsular hemorrhage with splenic rupture and hypotension, treated using splenic artery embolization. They all recovered from subcapsular hemorrhage without any other complications. These 3 cases reveal the process of subcapsular hemorrhage leading to rupture and a potentially fatal outcome. The treatment plan of subcapsular hemorrhage should be determined carefully considering the vital signs, changes in hemoglobin, and bleeding tendency.
