As the dental pulp is encased with a rigid, noncompliant shell, changes in pulpal blood flow or vascular tissue pressure can have serious implication for the health of pulp. Numerous studies have demonstrated that orthodontic force application may influence both blood flow and cellular metabolism, leading degenerative and/or inflammatory responses in the dental pulp. The aim of this case report is to present a case about tooth with chronic periapical abscess which showed normal vital responses. Excessive orthodontic force is thought to be the prime cause of partial pulp necrosis. Owing to remaining vital tissue, wrong dianosis can be made, and tooth falsely diagnosed as vital may be left untreated, causing the necrotic tissue to destroy the supporting tissuses. Clinician should be able to utilize various diagnostic tools for the precise diagnosis, and be aware of the endodontic-orthodontic inter-relationship.
Various concentration of formaldehyde solution (1%, 5%, 10%) were applied to cut pulp surface, for 3 minutes and the remaining pulp were carefully examined histo-pathologically. The result were as follows,
1. One week survival group which were treated by 1% formaldehyde solution showed seperation of odontoblast layer randomly and the weil's zone was undistinguishable. Deeper portion of this area was necrotic or coagulated.
2. Two week survival group which were treated 1% formaldehyde solution showed necrotic appearance spread from odontoblastic layer to weil's zone.
3. One and two week survival group which were treated 5% formaldehyde solution showed the odontoblastic layer was highly necrotic, under this zone empty net-work was seen.
4. One and two week survival group treated 10% formaldehyde solution showed complete destruction of pulp.
The purpose of this study was to determine the close relation between clinical symptom and histopathologic finding in pulp. 20 samples of pulp tissue were collected from badly decayed teeth with considerable pulp vitality. Hemogram was also made with the first drop of bleedieg after total pulp extirpation (10cases) and vital pulpotomy(10cases). Histologic specimen was made routinely with extirpated pulp and stained by H&E and P.A.S. Stains. The results under microsopic examination were as follows; 1. Hemogram obtained from dental pulp with acute inflammation revealed increase of polymorphonuclear leukocyte in number, more lymphocytes and monocytes. 2. Hemogram from pulp with chronic inflammation shown the number of lymphocyte and monocyte was remarkably increased. 3. Histologi specimen obtained from teeth with severe clinical symptome showed polymorphonuclear leukocyte infiltration and the hemogram also showed polymorphonuclear leukocytes. 4. Specimen from teeth with moderate symptom showed lymphocyte infiltration and vessel dilatation underneath decayed area. Erythrocytes were massively accumulated inside of the vessel wall.
The present report presents a case of dens invaginatus (DI) in a patient with 4 maxillary incisors. A 24-year-old female complained of swelling of the maxillary left anterior region and discoloration of the maxillary left anterior tooth. The maxillary left lateral incisor (tooth #22) showed pulp necrosis and a chronic apical abscess, and a periapical X-ray demonstrated DI on bilateral maxillary central and lateral incisors. All teeth responded to a vitality test, except tooth #22. The anatomic form of tooth #22 was similar to that of tooth #12, and both teeth had lingual pits. In addition, panoramic and periapical X-rays demonstrated root canal calcification, such as pulp stones, in the maxillary canines, first and second premolars, and the mandibular incisors, canines, and first premolars bilaterally. The patient underwent root canal treatment of tooth #22 and non-vital tooth bleaching. After a temporary filling material was removed, the invaginated mass was removed using ultrasonic tips under an operating microscope. The working length was established, and the root canal was enlarged up to #50 apical size and obturated with gutta-percha and AH 26 sealer using the continuous wave of condensation technique. Finally, non-vital bleaching was performed, and the access cavity was filled with composite resin.
Objectives: The purpose of this study was to assess the ability of two new calcium silicate-based pulp-capping materials (Biodentine and BioAggregate) to induce healing in a rat pulp injury model and to compare them with mineral trioxide aggregate (MTA). Materials and Methods: Eighteen rats were anesthetized, cavities were prepared and the pulp was capped with either of ProRoot MTA, Biodentine, or BioAggregate. The specimens were scanned using a high-resolution micro-computed tomography (micro-CT) system and were prepared and evaluated histologically and immunohistochemically using dentin sialoprotein (DSP). Results: On micro-CT analysis, the ProRoot MTA and Biodentine groups showed significantly thicker hard tissue formation (p < 0.05). On H&E staining, ProRoot MTA showed complete dentin bridge formation with normal pulpal histology. In the Biodentine and BioAggregate groups, a thick, homogeneous hard tissue barrier was observed. The ProRoot MTA specimens showed strong immunopositive reaction for DSP. Conclusions: Our results suggest that calcium silicate-based pulp-capping materials induce favorable effects on reparative processes during vital pulp therapy and that both Biodentine and BioAggregate could be considered as alternatives to ProRoot MTA.
This study is concerned with the effect on Penetration of varing the length of time among the applications of stains, preparations of medicine and radio-active isotope of siver in 120 carious or non carious human teeth with a vital pulp. The study revealed the following conclusions :
1) The stains and preparations penetrated through the dentinal tube and it's seem to be ceased at zone of secondary dentinal area.
2) The stains and preparations did not penetrated in enamel tissues.

In case of luxation injuries, loss of tooth vitality is common. And in case of trauma in the immature permanent teeth, precise diagnosis of pulp necrosis is very difficult. That is because limitation in distinguishing between normal dental papilla in immature permanent teeth, transient apical breakdown(TAB), which is part of normal healing process, and apical radiolucency in pulp necrosis. Especially in non-vital immature permanent tooth, the treatment is complex and requires long time. This clinical case report shows that severely infected immature teeth with periradicular periodontitis can undergo healing and apexogenesis or maturogenesis with no definative treatment or after conservative treatment. In the cases reported, we emphasize the considerable power of regeneration of the tooth, probably due to its large number of undifferentiated mesenchymal cells in the dental papilla, pulp tissue, periodontal ligament tissues. Thus, when endodontic treatment in immature permanent teeth, over instrumentation is not recommend for preserve the apical vital stem cells.

Objectives: This study aimed to evaluate vital pulp tissue removal from different endodontic instrumentation systems from root canal apical third in vivo. Materials and Methods: Thirty mandibular molars were selected and randomly divided into 2 test groups and one control group. Inclusion criteria were a positive response to cold sensibility test, curvature angle between 10 and 20 degrees, and curvature radius lower than 10 mm. Root canals prepared with Hero 642 system (size 45/0.02) (n = 10) and Reciproc R40 (size 40/0.06) (n = 10) and control (n = 10) without instrumentation. Canals were irrigated only with saline solution during root canal preparation. The apical third was evaluated considering the touched/untouched perimeter and area to evaluate the efficacy of root canal wall debridement. Statistical analysis used t-test for comparisons. Results: Untouched root canal at cross-section perimeter, the Hero 642 system showed 41.44% ± 5.62% and Reciproc R40 58.67% ± 12.39% without contact with instruments. Regarding the untouched area, Hero 642 system showed 22.78% ± 6.42% and Reciproc R40 34.35% ± 8.52%. Neither instrument achieved complete cross-sectional root canal debridement. Hero 642 system rotary taper 0.02 instruments achieved significant greater wall contact perimeter and area compared to reciprocate the Reciproc R40 taper 0.06 instrument. Conclusions: Hero 642 achieved higher wall contact perimeter and area but, regardless of instrument size and taper, vital pulp during in vivo instrumentation is not entirely removed.

Carbamide peroxide is usually used for vital teeth bleaching at home. Complications such as tooth hypersensitivity and/or gingival irritation are frequently reported. Therefore, this study was performed to evaluate any possible histological changes in pulp and periodontal tissue by carbamide peroxide bleaching gel in rats. 10% and 15% carbamide peroxide containing nightguard for upper molar were worn for 4 hours a day. The rats were sacrificed after 1 day, 2 days, 3 days, 4 days and 6 days application of carbamide peroxide respectively. The results were as follows : Mild infiltration of inflammatory changes below the junctional epithelium and hyperplasia of epithelium were observed in both 10% and 15% carbamide peroxide treated groups. In all experimental groups, odontoblasts were changed from columnar to cuboidal shape and/or obliterated and the focal loss of predentin was observed in pulp horn. With increasing time of application, these changes were more remarkable, but limited in pulp horn. Inflammatory reactions, vacuolar changes and hyaline degenerations of the pup tissue were also observed in some cases. These results suggested that carbamide peroxide gel used in home bleaching could cause reversible pulpal irritation.
