A Clinical Case Study of a Patient with Complex Regional Pain Syndrome (CRPS) Type I Treated by 8 Constitution Acupuncture (팔체질침(八體質鍼)으로 치료한 복합부위통증증후군 1형 환자 1례에 대한 증례보고)

  • Objectives : The purpose of this study is to report the clinical effects of 8 constitution acupuncture on a patient with complex regional pain syndrome (CRPS) type I. Methods : In this study, a patient with CRPS was treated by 8 constitution acupuncture therapy. He was diagnosed in the Cholecystonia of 8 constitution diagnosis. The progress of symptoms were evaluated by visual analog scale (VAS) and sleeping time. VAS and sleeping time were evaluated at 7:00 am daily. Results : VAS decreased from 10 to 4, while sleeping time increased from 30 minutes to 3 hours. Conclusion : The results suggest that 8 constitution acupuncture is effective for treating symptoms of CRPS type I.

Clinical Effects of ByeolGab Pharmacopuncture for Primary Headache in Three Case Reports (원발성 두통에 대한 별갑약침의 임상적 효과 3례 보고)

  • Objectives: To report the clinical application and effects of ByeolGab (Trionycis Carapax) pharmacopuncture in patients with primary headache. Method: Three patients who suffered from primary headache were treated with ByeolGab pharmacopuncture and acupuncture for 2 weeks. We used the Henry Ford Headache Disability Inventory (HDI), a six point scale, and a visual analogue scale (VAS) three times and analyzed the results. Result: After treatment with ByeolGab pharmacopuncture, the VAS, HDI, and six point scale scores were improved in all three patients. The symptoms of headache and other symptoms were also improved. Conclusion: ByeolGab pharmacopuncture is an effective treatment for patients complaining of primary headache.

  • Objectives: This report describes the case of a patient suffering tremor of the hands and feet, which was improved by Gamigyejak-tang-gamibang. Methods: For relieving symptoms, Korean medicine treatments, including herbal medicine, acupuncture, and moxibustion, were performed. To evaluate the effects of treatments, effective rate, visual analog scale, and patient's condition scale were used. Results: After treatment for 3 months, the clinical symptoms of the patient were improved. Effective rate increased from 10% to 95%. Both visual analog scale and patient's condition scale scores decreased from 8 to 1. Conclusions: This report suggests that Gamigyejak-tang-gamibang is effective for treating essential tremor.

  • Objectives : The purpose of this study is to report the effectiveness of Sihochunggan-tang gagambang and Gagamchengyoung-tang in the seven patients with seborrheic dermatitis. Methods : This case study was conducted for seven seborrheic dermatitis patients, who have visited Korean Medicine Clinic from 2013.10.01 to 2017.09.30. All seven patients were treated with Sihochunggan-tang gagambang and Gagamchengyoung-tang and acupucture. During taking medicine, we let them avoid bad food like fatty food and alcohol. We evaluated the symptom change through photographs and used Visual Analogue Scale(VAS) as the assessment method. Results : VAS such as skin severity of symptoms, sleep disorder, pruritus was decreased as a result of Sihochunggan-tang gagambang and Gagamchengyoung-tang and acupucture treatment in seven seborrheic dermatitis patients. Conclusions : After the treatment, the grade of VAS was decreased and most symptoms of seven patients were improved. As a result of examining VAS scores and photographs, Sihochunggan-tang gagambang and Gagamchengyoung-tang can be effective on the seborrheic dermatitis in seven cases in our study.

Corneal Abrasion Occurred during Transconjunctival Blepharoplasty in a Patient with History of LASIK Operation (라식수술을 시행한 환자에서 결막절개를 통한 하안검성형술 시 발생한 각막손상)

  • Purpose: LASIK, one of the currently popular ophthalmic procedures, can sometimes result in dry eye symptoms. In the patient who had underwent LASIK operation, the periorbital surgery may be more likely to lead to such a complication as dry eye symptom. We would like to report a case of corneal abrasion occurred after transconjunctival blepharoplasty in post-LASIK state and suggest the method about preventing the complication. Methods: A 30-year-old female patient underwent transconjunctival blepharoplasty and microfat graft into the face. She had history of LASIK operation 2 years ago. Corneal protector was applied to both eye during transconjunctival operation. After the surgery, she complained of visual blurring at left side. Ophthalmic examination revealed corneal abrasion of left eye. Results: Cornea protecting lens was applied to left eye for 2 weeks. Steroid and antibiotic eye drops were applied for relieving the symptoms and the prevention from progressing of complication. After 1 week, visual power and acuity was recovered to preoperative state. No other specific complications happened. Conclusion: Post-LASIK patient may have possibility of decreased corneal sensation and tear production. Preoperative ophthalmic examination is recommended for the prevention of complication. When corneal protector is applied, we suggest Optagel$^{(R)}$ as a useful lubricant.

A Case Report of Sjögren's Syndrome with Influenza A Virus Infection Treated with Korean Medicine (A형 독감을 동반한 쇼그렌 증후군 환자에 대한 한의치료 1례)

  • Objectives: The purpose of this study was to report the treatment of a patient suffering from $Sj{\ddot{o}}gren^{\prime}s$ Syndrome with influenza A virus infection with Korean medicine. Methods: We used herbal medicine, acupuncture, and moxibustion to treat a patient during hospitalization. We observed the changes in symptoms using the European League against Rheumatism $Sj{\ddot{o}}gren^{\prime}s$ Syndrome Patient Reports Index (ESSPRI) and a visual analogue scale (VAS). Results: After treatment for 17 days, the patient's symptoms showed improvement in joint pain, dry eye, and dry mouth. The ESSPRI score was decreased from 10 to 5.3. The VAS for dry mouth and dry eye were decreased from 10 to 6 and from 10 to 5, respectively. Conclusion: This clinical case study suggests that Korean medicine treatment that includes Insamyangyoung-tang-gami could be effective in the treatment of $Sj{\ddot{o}}gren^{\prime}s$ Syndrome.

  • Objectives This study was designed to evaluate the effects of Korean medicine therapy(Cheongsimyeonja-tang) on Taeeumin patients diagnosed with Sj$\ddot{o}$gren's syndrome with arthritis. Methods The Sj$\ddot{o}$gren's syndrome with arthritis patient was treated with Cheongsimyeonja-tang. The progress was evaluated with the visual analogue scale and erythrocyte sedimentation rate on hematologic examination. Results There was improvement in the patient's Sj$\ddot{o}$gren's syndrome symptoms. The patient's symptoms of the alimentary system, dried fever and arthralgia evaluated with the visual analogue scale improved. Erythrocyte sedimentation rate was decreased on hematologic examination. Conclusions This study suggests that Korean medicine(Cheongsimyeonja-tang) is effective in the treatment of Sj$\ddot{o}$gren's syndrome on Taeumin patients.

Measuring Utility Weights with VAS, TTO and EQ-5D for Severe Stomach Cancer (VAS(Visual Analogue Scale), TTO(Time Trade-Off), EQ-5D(EuroQol-5 Dimension) 방법을 이용한 중증위암의 효용가중치 측정)

  • Stomach cancer shows the highest incidence among cancer patients in Korea and it deteriorates the patients' health-related quality of life considerably. This study measured utility weights of severe stomach cancer symptoms using VAS (Visual Analogue Scale), TTO (Time-trade Off) and EQ-5D (EuroQol-5 Dimension) methods. A survey questionnaire was developed to describe the symptoms of severe stomach cancer comprehensively and concisely using VAS, TTO and EQ-5D. A face-to-face interview was conducted for 21 cancer inpatients in a hospital and 25 non-patients. The mean utility weight is 0.498 for VAS, 0.375 for EQ-5D and 0.300 for TTO. Using VAS or TTO methods, there were no statistically significant differences between patients and non-patients while there was significant difference in EQ-5D. The utility weight with VAS and EQ-5D has shown similar trend - higher score in patient group, male and aged while it has opposite trend with TTO.

A Case Report of Essential Tremors Treated with Yeokhan-san (억간산으로 호전 보인 본태성 떨림 환자 1례)

  • Objective: This report describes the case of a patient with essential tremors of the mouth and hands, which were improved by Yeokhan-san treatment. Methods: To relieve the symptoms, Korean medicine treatments, including Yeokhan-san, Ohaeng-Hwa acupuncture, and electronic moxibustion, were performed. To evaluate the effects of the treatments, the visual analog scale and Fahn-Tolosa-Marin clinical rating scale for tremor were used. Results: After the treatment, the patient's clinical symptoms were improved. The visual analog scale score decreased from 10 to 2 for the hand tremor and from 10 to 1 for the mouth tremor. The Fahn-Tolosa-Marin tremor scores decreased from 3 to 1 for both the hand and mouth tremors. Conclusions: The present case suggests that Yeokhan-san and Ohaeng-Hwa acupuncture are effective for the treatment of essential tremor.

Change of Symptoms after Total Hip Arthroplasty in Patients with Hip-Spine Syndrome

  • Purpose: Elderly patients with degenerative diseases undergo treatment for the hip and spine; these patients present with various symptoms. This study focused on patients with residual symptoms, predominantly pain, even after receiving treatment for their spinal lesions. Materials and Methods: Patients who underwent total hip arthroplasty (THA) between 2016 and 2022 at a single tertiary hospital were included in the study. Of the 417 patients who underwent primary THA, a retrospective review of 40 patients with previous lesions of the spine was conducted. Patients were stratified to two cohorts: Patients with symptoms related to the spine (Group A), and those with hip-related symptoms (Group B). Preand postoperative comparisons of groups A and B were performed. Results: Improvements in patients' symptoms were observed in groups A and B after THA. In Group A, the mean preoperative visual analog scale (VAS) score was 5.10±0.876, which showed a postoperative decrease to 2.70±1.767. In Group B, the mean preoperative VAS score was 5.10±1.539, which showed a postoperative decrease to 2.67±1.493. Conclusion: According to the findings, promising results were achieved with THA in treatment of debilitating diseases of the hip for both the prognosis of the disease, as well as the patients' symptoms. In addition, in some cases elderly patients with dual pathologies underwent treatment for spinal lesions without performance of any evaluation related to the hip. Thus, evaluation of a patient's hip must be performed and performance of THA in patients with symptoms even after treatment of spinal lesions is recommended.
