An outbreak of Lactococcus garvieae Infection in Cage-cultured Red Lip Mullet Chelon haematocheilus with Green Liver Syndrome

  • Red lip mullet Chelon haematocheilus (body weight = $468{\pm}91g$) which became sick during an outbreak of disease at mariculture facilities at Cheonsu Bay, Korea, during July-August 2013, were examined to identify the cause of the disease. Diseased mullets displayed green liver syndrome, and Lactococcus garvieae were isolated from their internal organs. Argulus sp., Trichodina sp., and/or Vibrio spp. were also discovered in some infected fish. Histopathological examination revealed that fatty liver syndrome with hepatocyte degeneration, reflected in heterokaryons, inflammatory lesions, and melanomacrophage centers ($MMC_S$), had caused fibrosis around the kidney, spleen, and blood vessels. After the outbreak, visceral fat and green liver syndrome in the mullets were consistently observed throughout the year in the same mariculture facilities, indicating that the cultured mullets suffered a chronic metabolic disorder. Although Vibrio spp. were also isolated from some individuals, L. garvieae, which is known to be a causative agent of red lip mullet mortality, was isolated from all diseased individuals. This is the first report of L. garvieae infection in cultured red lip mullet.

  • Mucopolysaccharidoses (MPSs) are a group of rare inherited metabolic diseases caused by deficiency of lysosomal enzymes. MPSs are clinically heterogeneous and characterized by progressive deterioration in visceral, skeletal and neurological functions. The aim of this article is to review the treatment of MPSs, the unmet needs of current treatments and vision for the future including recent clinical trials. Until recently, supportive care was the only option available for the management of MPSs. Hematopoietic stem cell transplantation (HSCT), another potentially curative treatment, is not routinely advocated in clinical practice due to its high risk profile and lack of evidence for efficacy. From the early 2000s, enzyme replacement therapy (ERT) was approved and available for the treatment of MPS I, II and VI. ERT is effective for the treatment of many somatic symptoms, particularly walking ability and respiratory function, and remains the mainstay of MPS treatment. However, no benefit was found in the neurological symptoms because the enzymes do not readily cross the blood-brain barrier (BBB). In recent years, intrathecal (IT) ERT, substrate reduction therapy (SRT) and gene therapy have been rapidly gaining greater recognition as potential therapeutic avenues. Although still under investigation, IT ERT, SRT and gene therapy are promising MPS treatments that may prevent the neurodegeneration not improved by ERT.

The dynamics of proteinase activity at different stages of toxocariasis in mice (개회충에 감염된 생쥐에 있어서 시기별 단백분해효소 활성의 변동)

  • Toxocariasis is a parasitic zoonosis caused by infestation of humans with larvae of Toxocara canis, the common roundworm in dogs. Two syndromes have been identified : visceral larva migrans and ocular toxocariasis. In this study we were characterized proteinase activity in crude extracts from liver, lung, kidney and heart of mice infected with Toxocara canis and the dynamics of their changes in different stages of disease. The optimal pH was 5.5. In liver of mice infected with Toxocara canis, the maximun activity of proteinase was observed in 5 day post infection. In lung, the activity reached its maximun on 5th day in A group (infected with 100 embryonated eggs), and on 5th week in B group (infected with 50 embryonated eggs). In kidney, the maximum activity was shown at 6th week in A group, and in B group was shown at 10th day. In early infection, the activity reached its maximun in heart of mice infected with Toxocara canis. As we could see, the dynamics of the changes of proteinase activity in mice is similar in the case of the disease with other biochemical and immunological indices observed in toxocariasis.

Study on the individual characteristic factor by the body shape (신체(身體) 형태(形態)에서의 개체성(個體性) 관찰(觀察)에 대한 문헌(文獻) 연구(硏究))

  • Objectives For the excellent differentiation of syndromes, we study on the individual characteristic factor by the inspection of body shape. Methods To the subject of diagnosis special books and diagnostics textbook of korean medicine, we arrange the individual characteristic factor by the four methods of diagnosis. Results The individual characteristic factor analyzed by the inspection of the body shape in the "Huangdineijing 黃帝內經" and "Donguisusebowon 東醫壽世保元" was observed stereotyped. The inspection of the body shape means to diagnose the patient by examining the individual physical conditions of the patient. The body depends on visceral essence to nourish, while the functions of the viscera and the conditions of visceral essence may be reflected by the body. The inspection of individual physical strength and weakness may enable one to know the functions of the viscera and the conditions of ki(氣) and blood. And also the inspection of local regions is used to closely examine some regional areas to obtain necessary clinical data on the individual characteristic factor. Conclusion As the results, the individual characteristic factor by the inspection of body shape is very important item of the four methods of diagnosis(四診) and the differentiation of syndromes(辨證). And therefore the process on four methods of diagnosis and differentiation of syndromesis is necessary to divide the signs of individual characteristic factor and the signs of disease.

Successful Management with Vincristine after Failure of Prednisolone Therapy for Diffuse Neonatal Hemangiomatosis (Vincristine 투여로 호전된 미만성 신생아 혈관종증)

  • Hemangiomas are the most common benign tumors of infancy. Fifteen to 30% of these patients have multiple hemangiomas. Diffuse neonatal hemangiomatosis (DNH) is a disease that often has a fatal outcome if left untreated, and is characterized by multiple cutaneous and visceral hemangiomas. Corticosteroids are the commonly accepted first line treatment, but if no effect is seen, further treatment is required such as interferon, surgical excision, embolization and radiotherapy. Interferon is effective, but the neurologic sequela including spastic diplegia can be a complication. We experienced a case of DHN in a neonate. In this case, the baby presented with multiple cutaneous and visceral hemangiomas with Kasabach-Merritt syndrome (KMS) that included thrombocytopenia and consumptive coagulophthy. The baby was successfully treated with vincristine after the failure of steroid therapy.

The Effect of Enteral Nutrition Formula Supplement on Body Weight of Nonobese NIDDM Patients (비비만형 인슐린 비의존형 당뇨병 환자에서 영양음료를 사용한 체중 증가의 효과)

  • Nonobese NIDDM patients were studied were studied with respect to changes in visceral protein status, serum glucose and lipids and insulin secretion capacity before and after intake of enteral formula. Patients with renal or hepatic disease, gastrectomy, malabsorption, weight gain over past 6 months and poorly controlled blood glucose level were excluded. Eighteen patients served as case and administered, in addition of their usual diet, 400ml of enteral formula for 8 weeks. Another 18 patients participated in controls and had usual food intake for 8 weeks. In the begining, the levels of fasting and postprandial glucose, glycated hemoglobin, triglyceride, HDL, LDL, total cholesterol, albumin, total protein and transferrin and glucose response area on oral glucose tolerance test were not different between two groups. The response areas of insulin, C-peptide and free fatty acid and serum IGF-1 level were higher in the case than in the control group. Energy intake of patients given enteral formula exceeded their estimated energy requirements(108%) and they consumed a mean of 112g protein per day. Patients given enteral formula showed an increase in body weight(4.4%), serum transferrin(10%), IGF-1(13%) and triglyceride(34%) while controls showed no changes in those parameters at 8 weeks compared to initial values. There were no significant changes after 8 weeks in the levels of glucose, glycated hemoglobin, HDL, LDL, total cholesterol, total protein and albumin and response areas of glucose, insulin, C-peptide and free fatty acid in both groups compared to initial values. This study suggests that nutrition supplement with enteral formula can increase body weight and visceral protein status in nonobese NIDDM patients without changes in blood glucose. However, excessive calorie intake could temporarily increase serum triglyceride. In addition, this study indicates that serum transferrin and IGF-1 are more sensitive indicators to changes of protein intake than serum albumin and total protein.

Mass Mortality Caused by Nocardial Infection in Cultured Snakehead, Channa arga in Korea (Norcardia 감염증에 의한 양식 가물치의 대량 폐사)

  • A new disease causing mass mortality of farmed snakehead (Channa arga) has emerged in Korea over the summer of 2005. The affected fish showed no specific external signs with the exception of a distended abdomen and hemorrhaging around the anus. After opening the abdomen, the visceral organs, liver, spleen and kidney, present numerous white nodular structures. Histopathological examination revealed these nodules to be evidence of granulomas in the visceral organs. A Gram-positive, filamentous bacterium was isolated from all of the affected fish. Development of primers for a genus-specific peR assay for Nocardia, following analysis of the sequences of the complete 16S rRNA genes from Nocardia spp. and non-Nocardia bacterial genes, allowed identification of the causative organism as Nocardia. This is the first report of a nocardial infection of fish in Korea.

Concurrent treatment with ursolic acid and low-intensity treadmill exercise improves muscle atrophy and related outcomes in rats

  • The objective of this study was to analyze the concurrent treatment effects of ursolic acid (UA) and low-intensity treadmill exercise and to confirm the effectiveness of UA as an exercise mimetic to safely improve muscle atrophy-related diseases using Sprague-Dawley (SD) rats with skeletal muscle atrophy. Significant muscle atrophy was induced in male SD rats through hind limb immobilization using casting for 10 days. The muscle atrophy-induced SD rats were group into four: SED, sedentary; UA, daily intraperitoneal UA injection, 5 mg/kg; EX, low-intensity (10-12 m/min, $0^{\circ}$ grade) treadmill exercise; and UEX, daily intraperitoneal UA injection, 5 mg/kg, and low-intensity (10-12 m/min, $0^{\circ}$ grade) treadmill exercise. After 8 weeks of treatment, endurance capacity was analyzed using a treadmill, and tissues were extracted for analysis of visceral fat mass, body weight, muscle mass, expression of muscle atrophy- and hypertrophy-related genes, and endurance capacity. Although the effects of body weight gain control, muscle mass increase, and endurance capacity improvement were inadequate in the UA group, significant results were confirmed in the UEX group. The UEX group had significantly reduced body weight and visceral fat, significantly improved mass of tibialis anterior and gastrocnemius muscles, and significantly decreased atrophy-related gene expression of MuRF1 and atrogin-1, but did not have significant change in hypertrophy-related gene expression of Akt and mTOR. The endurance capacity was significantly improved in the EX and UEX groups. These data suggest that concurrent treatment with low-intensity exercise and UA is effective for atrophy-related physical dysfunctions.

Cytologic Features of Cancers Metastatic to the Lung and Diagnostic Usefulness of Immunohistochemistry : Distinction Between Primary and Secondary Lung Tumors (전이성 폐종양의 세포소견과 면역조직화학적 검사의 유용성)

  • The lungs are one of the most common visceral sites for metastatic disease. The identification of a metastasis from a second primary lung tumor is clinically important for patients with pulmonary metastases of an extrathoracic origin. Although the cytologic features of metastatic tumors involving the lung have been extensively described, making the cytologic diagnosis is usually not easy in the absence of clinical information. However, the immunohistochemical staining for many tumor markers and the different expressions of cytokeratin 7 and 20 are very useful in the diagnosis. This review presents the cytomorphological spectrum of metastatic tumors along with the immunohistochemical findings.

Intralobar Pulmonary Sequestration - A Report of Case - (내엽형 폐격리증 - 수술치험 1예-)

  • Intralobar pulmonary sequestration is rare congenital lung disease characterized by a cystic portion of the lung that derives its arterial blood supply though aberrant vessel directly of systemic circulation. Intralobar pulmonary sequestration is usually contained within the visceral pleura of a pulmonary lobe and its venous drainage to the pulmonary venous system. We experienced a case of pulmonary sequestration postoperatively confirmed. The patient was 48-year-old female whose complaints were cough and left chest pain. Chest film showed large homogenous opacity in left lower lung field. By operation, adult fist sized mass at the lower lobe were noted. An aberrant artery, measuring 1.0 cm. in diameter and 2.0 cm. in length, arose from the descending thoracic aorta just above the diaphragm. The anomalous systemic artery was ligatures and resection, and associated with left middle, lower bilobectomy was done. The postoperative course was uneventful, and 10 days later discharged.
