Object : Patients with facial asymmetry accompanying mandibular prognathism have various causes and clinical features. So, it is difficult to find a satisfactory treatment method functionally and esthetically. Every traditional classification and interpretation to find etiopathogenesis and/or to establish ideal surgical modality has many limitations because it can't be applied simply to various conditions of patients with facial asymmetry accompanying mandibular prognathism. Therefore, we employ a new classification to interpret more details of the morphologic change of mandible and the spatial change of mandible and maxilla. Materials and Methods : Using panoramic X-ray films, PA cephalograms and submentovertex films of 126 patients diagnosed with facial asymmetry accompanying mandibular prognathism as resources, the following results were gathered after analyzing each characteristics through distributing the patterns according to the morphological mandibular asymmetry and mandibular and maxillary spatial asymmetry. Results : Almost frequency of morphological mandibular asymmetry was shown. In case of condyle-ramus elongation and body elongation group, it's frequency was the highest. Higher frequency of compensating vertical growth was shown on the side of over growing maxilla in case of vertical length difference between left and right condyle-ramus. On the other hand, higher frequency of no compensating vertical growth difference between left and right side was shown in case of no vertical length difference in condyle-ramus. Spatial mandibular asymmetry generally occurred when there was no morphological mandibular asymmetry. Correlation between condyle length difference and condyle-ramus length difference between left and right side was very high, but correlation between condyle length difference and body length difference, and correlation between condyle length difference and body vertical length difference was low. Conclusion : In case of patients with facial asymmetry accompanying mandibular prognathism, it is suggested that various pattern of facial asymmetry is occurred by the independent growth of each unit rather than dependent growth of other unit by major growth unit abnormality. Due to the untypical pattern and the various asymmetry occurring according to the changes of each mandibular growth unit, it is considered that an appropriate surgical method should be searched based on the accurate recognition of the each pattern for patients with facial asymmetry accompanying mandibular prognathism.
The purpose of this study was to find the characteristics of the frontal natural head position(NHP) of patients with facial asymmetry, and to contribute to the diagnosis of facial asymmetry in the clinical examination of orthodontic patients. Twenty adult patients who had apparent facial asymmetry and no severe sagittal skeletal discrepancy were selected as the asymmetry group, and 21 young adults who had symmetric faces were selected as the symmetry group. Frontal cephalograms were obtained in the state of NHP using a pivot-mounted fluid level device. The degree of the menton deviation was defined as the angle between the line drawn through crista galli and anterior nasal spine and the line drawn through crista galli and menton. The following angles were measured and each of them was compared with the degree of the menton deviation one is the angle between the true vertical line and the supra-orbital line which is a tangent line to the extreme cranial point on the supra-orbital margin, and the other is the angle between the true vertical line and the cervical line drawn through the midpoint of atlas and the 4th cervical vertebra. Through the statistical analysis, following results were obtained. 1. The angle between the supra-orbital line and the true vertical line was much mote deviated from the right angle in the asymmetry group than in the symmetry group. 2. The angle between the cervical line and the true vertical line in the asymmetry group showed greater tendency than in the symmetry group, but the difference was not statistically significant. 3. In the asymmetry group, the degree of the menton deviation was positively correlated with the angle between the supraorbital line and the true vertical line. The above results suggest that racial asymmetry patients show the tendency to have the tilted NHP to compensate the deviation of menton position.
The purpose of this study was to examine the difference of condylar and ramal vertical asymmetric indices on paonramic radiographs between temporomandibular disorder patients and normal group and according to classification of temporomandibular disorders. One hundred and twenty temporomandibular disorder patients and thirty normal individuals were selected. After tracing the panoramic radiographs, measurements for asymmetric indices were identified. The means and standard deviations of the measurement of each group were calculated and compared between two groups. Through the comparison of the measurement between two groups, following results were obtained 1. As a result of comparing condylar and ramal vertical asymmetry indices of the temporomandibular disorder patients with that of normal group, there was significant difference in condylar vertical asymmetry index between two groups and no significant difference in ramal vertical asymmetry index between two groups. 2. Condylar and ramal vertical asymmetry indices of temporomandibular joint disorder and muscle disorder patients showed greater value than normal group, but there was no significant difference between temporomandibular joint disorder patients and muscle disorder patients. 3. Among temporomandibular joint disorder patients, muscle disorder patients, and normal group, there was only significant difference in condylar vertical asymmetry index between temporomandibular joint disorder patients and normal group. 4. Among disc displacement, disc dislocation with reduction, and disc dislocation without reduction, there were no significant difference in condylar and ramal vertical asymmetry indices. 5. There were no correlation between aging and condylar and ramal vertical asymmetry indices. The results of the present study showed that condylar vertical asymmetry index of temporomandibular disorder patients showed greater values than normal group, and this difference could be considered as the features of temporomandibular disorders. Thus measuring the condylar vertical asymmetry index on panoramic radiographs is considered as the important factor for diagnosis and treatment of temporomandibular disorders.
Purpose : To assess the relationship between soft tissue asymmetry and bone tissue asymmetry using the standardized photographs and the posteroanterior (PA) cephalometric radiographs in mandibular asymmetric patients. And to clarify that the lack of morphologic balance among different skeletal components can often be masked by compensatory soft tissue contributions. Methods: Experimental group consisted of 58 patients whose chief complaints were facial asymmetry, they were taken with standardized facial photographs and PA cephalometric radiographs. Control group consisted of 30 persons in the normal occlusion. The reproducibility of the facial photograph was confirmed by model test. The differences of fractional vertical heightand horizontal width from standardized facial photographs and PA cephalometric radiographs were compared and analyzed. Results: The difference of fractional vertical bone height was 0.63 and fractional vertical soft height was 0.58 in control group, 3.10 and 2.01 in asymmetric group, respectively. The difference of fractional horizontal bone width was 0.52 and fractional horizontal soft width was 0.70 in control group, 2.51 and 1.70 in asymmetric group, respectively. Both soft and bone tissue showed significant difference between control and asymmetric group (p<0.05). The difference of bone tissue was greater than that of soft tissue (p<0.05) in the experimental group but, not in control group. Conclusions: Soft tissue components may compensate for underlying skeletal imbalances.

Objective: The purpose of this study was to examine the symmetry and parallelism of the skeletal and soft-tissue poria by three-dimensional (3D) computed tomographic (CT) imaging. Methods: The locations of the bilateral skeletal and soft-tissue poria in 29 patients with facial asymmetry (asymmetric group) and 29 patients without facial asymmetry (symmetric group) were measured in 3D reconstructed models of CT images by using a 3D coordinate system. The mean intergroup differences in the anteroposterior and vertical angular deviations of the poria and their anteroposterior and vertical parallelism were statistically analyzed. Results: The symmetric and asymmetric groups showed significant anteroposterior angular differences in both the skeletal and the soft-tissue poria (p = 0.007 and 0.037, respectively; Mann-Whitney U-test). No significant differences in the anteroposterior and vertical parallelism of the poria were noted ($p{\leq}0.05$; Wilcoxon signed-rank test). Conclusions: In general, the skeletal poria are parallel to the soft-tissue poria. However, patients with facial asymmetry tend to have asymmetric poria.

In surgery for facial asymmetry, mandibles can be classified into two types, rotational and translational, according to the required mandibular movements for surgery. During surgery for rotational mandibular asymmetry, a bilateral sagittal split ramus osteotomy (BSSRO) may cause a large bone gap between the proximal and distal segments as well as condylar displacement, resulting in a relapse of the temporomandibular joint disorder, especially in severe cases. The intraoral vertical ramus osteotomy has an advantage, in this respect, because it causes less rotational displacement of the proximal segment on the deviated side and even displaced or rotated condylar segments may return to their original physiologic position. Unilateral intraoral vertical ramus osteotomy (UIVRO) on the short side combined with contralateral SSRO was devised as an alternative technique to resolve the spatial problems caused by conventional SSRO in cases of severe rotational asymmetry. A series of three cases were treated with the previously suggested protocol and the follow-up period was analyzed. In serial cases, UIVRO combined with contralateral SSRO may avoid mediolateral flaring of the bone segments and condylar dislocation, and result in improved condition of the temporomandibular joint. UIVRO combined with contralateral SSRO is expected to be a useful technique for the treatment of rotational mandibular asymmetry.
Objective: The purpose of this study was to determine the asymmetry of vertical ground reaction force (GRF) components between dominant and non-dominant legs in rested and fatigued states in prolonged running. Method: Twenty healthy men, heel strikers, were included (age: $24.00{\pm}5.0years$; height: $176.1{\pm}6.0cm$; body mass: $69.0{\pm}6.0kg$) in this study. Subjects ran on an instrumented treadmill for 130 minutes. During treadmill running, GRF data (1,000 Hz) were collected for 20 strides at five minutes (rested) and 125 minutes (fatigued) running while they were unaware of collecting data. Asymmetry indexes (ASI) were calculated to quantify the asymmetry magnitude in rested and fatigued states. Paired t-test was used to verify the differences between dominant and non-dominant legs in rested and fatigued states. In addition, one-way repeated measure analysis of variance was applied for comparison of ASI of both states. The level of significance was set at p < .05. Results: Passive force peak magnitude, loading rate, and impulse affecting the development of running injury were found significantly greater in dominant leg than in non-dominant leg at rested state (p < .05). However, passive force peak time and active force peak magnitude were found significantly different between legs in fatigued state (p < .05). To determine changes in percentage of asymmetry between legs in both states, ASI was used. ASI for all variables increased in fatigued state; however, no significant differences were found between both states. Conclusion: This study found that fatigue did not affect differences in vertical GRF between dominant and non-dominant legs and asymmetry changes.

Objectives: The aim of the study was to quantify and compare craniofacial asymmetry in subjects with and without symptoms of temporomandibular joint disorders (TMDs). Materials and Methods: A total of 126 adult subjects were categorized into two groups (63 with a TMDs and 63 without a TMDs), based on detection of symptoms using the Temporomandibular Joint Disorder-Diagnostic Index (TMD-DI) questionnaire. Posteroanterior cephalograms of each subject were traced manually and 17 linear and angular measurements were analyzed. Craniofacial asymmetry was quantified by calculating the asymmetry index (AI) of bilateral parameters for both groups. Results: Intra- and intergroup comparisons were analyzed using independent t-test and Mann-Whitney U test, respectively, with a P<0.05 considered statistically significant. An AI for each linear and angular bilateral parameter was calculated; higher asymmetry was found in TMD-positive patients compared with TMD-negative patients. An intergroup comparison of AIs found highly significant differences for the parameters of antegonial notch to horizontal plane distance, jugular point to horizontal plane distance, antegonial notch to menton distance, antegonial notch to vertical plane distance, condylion to vertical plane distance, and angle formed by vertical plane, O point and antegonial notch. Significant deviation of the menton distance from the facial midline was also evident. Conclusion: Greater facial asymmetry was seen in the TMD-positive group compared with the TMD-negative group. The mandibular region was characterized by asymmetries of greater magnitude compared with the maxilla. Patients with facial asymmetry often require management of temporomandibular joint (TMJ) pathology to achieve a stable, functional, and esthetic result. Ignoring the TMJ during treatment or failing to provide proper management of the TMJ and performing only orthognathic surgery may result in worsening of TMJ-associated symptoms (jaw dysfunction and pain) and re-occurrence of asymmetry and malocclusion. Assessments of facial asymmetry should take into account TMJ disorders to improve diagnostic accuracy and treatment outcomes.

Maxillary canting and vertical shortening of the unilateral mandibular ramus height is common in cases of severe facial asymmetry. Normally, mandibular distraction osteogenesis (DO) with horizontal osteotomy at the ascending ramus is used for vertical lengthening of the mandibular ramus to correct facial asymmetry with an absolute shortened ascending ramus. In this case report, vertical lengthening of the ascending ramus was performed successfully with unilateral DO and sagittal split ramus osteotomy (SSRO), where the posterior part of the distal segment can be distracted simultaneously in an inferior direction with maxillary DO, resulting in a lengthening of the medial pterygoid muscle. This case describes the acquired unilateral mandibular hypoplasia caused by a condylar fracture at an early age, which resulted in abnormal mandibular development that ultimately caused severe facial trismus. The treatment of this case included two-stage surgery consisting of bimaxillary distraction osteogenesis for gradual lengthening of the unilateral facial height followed by secondary orthognathic surgery to correct the transverse asymmetry. At the one year follow-up after SSRO, the vertical length was maintained without complications.

Preoperative surgical simulation in orthognathic surgery has progressed in recent years; the movement of the mandible can be anticipated through three-dimensional (3D) simulation surgery before the actual procedure. In this case report, the mandible was moved to the intended postoperative occlusion through preoperative surgical 3D simulation. Right-side condylar movement change was very slight in the surgical simulation, suggesting the possibility of mandibular surgery that included only left-side ramal osteotomy. This case report describes a patient with a mild asymmetric facial profile in which the mandibular menton had been deviated to the right and the lips canted down to the left. Before surgery, three-dimensional surgical simulation was used to evaluate and confirm a position for the condyle as well as the symmetrical postoperative state of the face. Facial asymmetry was resolved with minimal surgical treatment through unilateral intraoral vertical ramus osteotomy on the left side of the mandible. It would be a valuable complement for the reduction of the surgical treatment if one could decide with good predictability when an isolated intraoral vertical ramus osteotomy can be done without a compensatory osteotomy on the contralateral side.
