1989년 1월부터 1993년 12월까지 5년간 경북대학교병원 흉부외과학교실에서 심실중격결손으로 개심술을 받았던 환자 211례를 대상으로 하여, 정상군 191례와 잔류단락군 20례로 분류한 다음, 잔류단락 발생에 관여하는 인자들을 알아 보았다. 전체환자에서 연령분포는 7개월에서 39세까지로 평균년령은 5.6세였고, 남녀비는 1:0.61로 남자가 많았다. 수술시행년도에 따른 술 후 잔류단락의 발생률을 살펴보면, '89년도 21.4%, '90년도 6.2%, '91년도 8.5%, '92년도 6.5%, '93년도 8.9%로서 '90년도 이후에 비하여 '89년도에 유의하게 높은 발생률을 보였다. 심실중격결손의 크기를 비교하였을 때, 정상군에서는 중격결손의 평균직경 6.3$\pm$3.6mm,잔류단락쿤에서는 평균직경 10.6$\pm$5.7mm로써 양군간에 유의한 차이가 있었다. 그리고 폐-체혈압비와 평균 폐동맥압이 잔류단락군에서 유의하게 높았다. 잔류단락군에서 술후 정기적 일심초음파검 사에 의한 추적관찰을 실시하여 얻어진 성적을 보면, 잔류단락은 모두 혈류역학적인 의의가 없는 작은 크기 였고, 잔류단락에 따른 재수술례는 없었다. 그리고 추적관찰 기간중 잔류단락의 자연폐쇄가 9례에서 확인되었다.

Teflon patch closure of ventricular septal defect through transtricuspital procedure and division and suture closure of aorticopulmonary window performed under cardiopulmonary bypass with cardioplegia. The postoperative course was uneventful and discharged with good general condition.
Congenitally corrected transposition of the great arteries is a rare congenital heart anomaly, in isolation, has no hemodynamic consequences. It is usually associated with one or more of a variety of intracardiac lesions, ventricular septal defect, valvular or subvalvular pulmonary stenosis, and deformity of the systemic atrioventricular valve with insufficiency. This report describes a successful two stage operation for congenitally corrected transposition, [SLL] type, with ventricular septal defect, pulmonary atresia, persistent ductus arteriosus, and atrial septal defect. A 9 years old patient underwent modified Blalock-Taussig operation because of severe pulmonary hypoplasia. 2 years later a corrective operation, direct closure of ASD and PDA, VSD closure with Dacron patch, Enlargement of left pulmonary artery with pericardial patch and Relief of ROTO with Rastelli procedure could be successfully performed without complication.
There have been few reports documenting the outcome of arterial swich operations(ASO) in selected patients with transposition of great arteries(TGA) and with left ventricular outflow tract obstruction(LVOTO). In the case of TGA with LVOTO, if the atrial septal defect(ASD) is large and the ventricular septal defect(VSD) is restricive, this deprives the left ventricle(LV) of approporiate preload and could lead to underdevelopment of the ventircular mass and lead poor LV performance after the arterial switch operation, dspite a high pressure in the LV preoperatively. Because an increase in the systolic ventricular pressure is not necessarily paralleled by an increase in ventricular mass, which is also essential for optimal ventricular performance after the operation. We report here a case of rapid LV training after ASO in TGA with unprepared LV (because of large ASD and restrictive VSD) despite a high pressure in the LV(due to LVOTO) preoperatively.
Arterial switch operation for repair of nineteen cases of transposition of the great arteries associated with ventricular septal defect and three cases with double outlet right ventricle with subpulmonary ventricular septal defect[Taussig-Bing type DORV] was performed from November 1987 to September 1990 at the Seoul National University Children`s Hospital. Sixteen of them were under six months of age, and three were under one year of age with body weight ranged from three to fourteen kilograms. Preoperative cardiac catheterization was done in eighteen patients, in which the pressure of the left ventricle was greater than 70% of the right ventricle in all but one. Patent ductus was associated in thirteen cases[68.4%] of TGA+VSD, and atrial septal defect or patent oval foramen was in sixteen cases. Four atrial septostomy, one modified Blalock- Taussig shunt, one pulmonary artery banding, one coarctoplasty using subclavian arterial flap, were perfomed before arterial switch operation. There were five hospital deaths, all in the. patients with transposition of the great arteries with ventricular septal defect[overall mortality rate 22.7%]. Lecompte Maneuver was used in all patients, and in all patient the U-shaped flap of coronary arteries were transposed to V-shaped cleavage created in the neoaorta. Arterial defect in the neopulmonary artery was covered with 0.0625% Glutaraldehyde fixed autogenous pericardium There have been no late deaths, Postoperative cardiac catheterization and angiocardiogram in four patients has revealed no stenosis in the neopulmonary artery or neoaorta with reasonable P[RV/LV], Anatomic correction for transposition and double outlet right ventricle with subpulmonary ventricular septal defect would seem to be a good operative alternative to intraatrial switch procedures, with the advantage of incorporating the left ventricle to systemic circulation.

This report describes a 17 year old male patient who had congenitally corrected transposition of the great arteries associated with ventricular septal defect and patent ductus arteriosus, underwent patch closure of the ventricular septal defect and suture closure of patent ductus arteriosus, and was discharged on 9th day after surgery with good condition.
Total of 52 patients having ventricular septal defect were operated on using extracorporeal circulation in the 8 years period between 1969 and 1977. Of these, the very rare and interesting were the cases of ventricular septal defect seen in twin boys. Their defects were Type II and Type III in each which had been closed with Teflon felt patch in one and with primary suture in the other. Their postoperative courses were uneventful without complications of any.

A 2-month-old female Abyssinian cat was presented with a severe ascites, cyanosis, and exercise intolerance. Diagnostic studies revealed V/VI holosystolic murmur, sinus tachycardia, generalized cardiomegaly with marked left atrial enlargement and shunt flow between left and right ventricles. Doppler study showed bi-directional shunts in rest and right-to-left shunt after exercise. Despite initial improvement of clinical signs after initiation of medical treatment, the cat died of sudden cardiac arrest. Necropsy revealed a perimembranous ventricular septal defect.
Aortic valve insufficiency with ventricular septal defect is usually treatment by plication of prolased cusps. We have performed the aortic valvuloplasty by the Trusler`s method in 14 patients of ventricular septal defect associated with aortic insufficiency. The reoperations were performed in the 4 cases[29%] due to recurrent aortic insufficiency after aotic valvuloplasty. Two patients underwent aoritc valvular replacements for the tears of repaired aortic cusps after primary aortic valvuloplasty for aortic insufficiency. The other two patients had aortic valvuloplasties again. Death occured in one case of aortic valvular replacements. The cause of death was low cardiac output syndrome soon after aortic valvular replacement for recurrent aortic insufficiency .
