내·외측 Wedge와 넙다리네갈래근 각의 차이가 안쪽빗넓은근/가쪽넓은근 비에 미치는 영향 (Effects of Medial, Lateral Wedge and Difference of Quadriceps Angle on Vastus Medialis Oblique/Vastus Lateralis Muscle Activity Ratios)

  • Patellofemoral pain syndrome (PFPS) is often attributed to malalignment and maltracking of patella within the patellofemoral joint. Most exercise for PFPS has focused on selectively strengthening the vastus medialis oblique muscle (VMO). This study was designed to identify the effect of medial, lateral wedge and difference of Quadriceps angle (Q-angle) on vastus medialis oblique/vastus lateralis muscle (VL) activity ratios. The subjects were twenty young adult males who had not experienced any knee injury. They were asked to perform isometric contraction exercises in three postures using medial and lateral wedge. The EMG activity of the VL and VMO were recorded in three postures by surface electrodes and normalized by %MVC values derived from seated, isometric knee extensions. The normalized EMG activity levels (%MVC) of the VL and VMO for the three postures of the lower extremities were compared using 2-way repeated measures ANOVA with 1 between-subject factor (group), and 1 within-subject factor (wedge). Results of repeated measures of ANOVA's revealed that the medial wedge isometric contraction exercise produced significantly greater EMG activity of VMO/VL ratios in Group I (Q-angle $18^{\circ}$ or less) (p<.05). But, the medial wedge isometric contraction exercise was no significant difference of VMO/VL ratios in Group II (Q-angle $19^{\circ}$ or more) (p>.05). These results have important implications for selective VMO muscle strengthening exercises in PFPS patients.

엉덩관절 굽힘 각도에 따른 쪼그려 앉기 운동 시 하지의 근활성도 비교 (Comparison of Low Limb Muscle Activity during Squat Exercise according to Hip Joint Flexion Angle)

  • Background: The purpose of this study was to determine if there is a difference in activation of the rectus femoris, vastus medialis oblique, vastus lateralis, semitendinosus, and biceps femoris when performing normal free squat with standinding position and free squat with $30^{\circ}$ flexed hip joint. Methods: Electromyograph surface electrodes were placed on the rectus femoris, vastus medialis oblique, vastus lateralis, semitendinosus, and biceps femoris of 19 healthy college students. The participants performed standing bilateral squats and standing bilateral squats with $30^{\circ}$ flexed hip joint with EMG measures taken upon initiation of muscle activity as confirmed by an electronic goniometer. Participants completed one trial with the EMG time measurements on each type. Results: There was a significant difference between normal squats(standing squats) and normal squats with $30^{\circ}$ flexed hip joint. The normal squat exercise was statistically higer than normal squat exercise with $30^{\circ}$ flexed trunk except for semitendinosus and biceps femoris that shown slightly high. Conclusions: As a result of this study, there were increases of muscle activity in both ways. In particular, it may be more beneficial for knee joint stabilization to perform normal squat exercise with standing position relatively.

  • Background: The purpose of this study is to identify effects of neuro-muscular control training on vastus medialis oblique (VMO) after menisectomy of the knee. Methods: The subjects of this study are women aged 42 and 39 each who did menisectomy. Case 1 was applied quadriceps setting exercise and neuro-muscular contrlol training and case 2 was applied quadriecps setting. Intervention was done 5 times a week for 2 weeks. Measurement of muscle activity on VMO and vastus lateralis (VL) was standardized signals of each muscle to %RVC using surface EMG. Results: On comparison of exercise before and after on VMO and VL, VL activation of case 2 was increased more than case 1. Conclusion: Quadriecps-setting exercise and selective neuro-muscular control training of VMO is effective intervention on VMO activity and muscle activity ratio of VMO to VL.

구강악안면재건을 위한 전외측대퇴피판의 해부학적 고찰 (Anatomical Review of Anterolateral Thigh Flap for the Oral and Maxillofacial Reconstruction)

  • The anterolateral thigh flap (ALTF) is a versatile fasciocutaneous or myocutaneous flap, which can be harvested incorporating several skin islands and muscle components. The perforator of the ALTF is usually derived from the descending or transverse branch of the lateral circumflex femoral artery, and these vessels are based mainly on musculocutaneous perforators traversing the vastus lateralis muscle, and also based on the septocutaneous vessels running in between the rectus femoris and vastus lateralis muscle. Despite its usefulness for the oral cavity reconstruction, anatomic variations of these nutrient vessels, such as three main branches of ALTF and its relations with sartorius, vastus lateralis, tensor fasciae latae and rectus femoris muscle, have been reconstructive surgeons to be hesitated for the selection of ALTF. For the better understanding of ALTF as a routine reconstructive procedure in oral and maxillofacial surgery, various anatomical findings must be learned and memorized by young doctors in the course of the special curriculum periods for the Korean national board of oral and maxillofacial surgery. This review article will discuss the vascular anatomy and relavant anatomical variations of ALTF with Korean language.

스쿼시 백핸드 드라이브 동작 시 운동학적 분석과 근활성도에 관한 연구 (The Kinematic Analysis and the Study of Muscle Activities during Backhand Drive in Squash)

  • The purpose of this study was to examine the differences of kinematics and muscle activities depending on the changes of angle approaching balls during backhand drive in squash. The results are as follows. Stride time took the longest at AD2 and step lengths were the biggest at AD1 of left foot contact and right foot contact and AD2 of impact and follow-through. The center of gravity and the speed of racket head were the highest at AD3 and at AD2. Angle of shoulder joint were the biggest at AD1 of left foot contact, right foot contact and impact and AD3 of follow-through. Angle of elbow joint were the biggest at AD3 of left foot contact, right foot contact and follow-through and AD2 of impact. Angle of pelvis joint were the biggest at AD2 of left foot contact, AD1 of right foot contact and AD3 of impact and follow-through. Angle of knee joint were the biggest at AD2 of left foot contact, AD1 of right foot contact and AD3 of impact and follow-through. Angle of ankle joint were the biggest at AD1 of left foot contact and AD3 of right foot contact, impact and follow-through. According to the analysis results of triceps brachii, latissimus dorsi, brachioradialis muscle and flexor carpi ulnaris muscle activities were high at AD1 of all phases. Analysis results of vastus lateralis, vastus medialis, tibialis anterior and gastrocnemius medial muscle activities were high at AD2 of phase1 and phase3. Those of vastus lateralis, vastus medialis and tibialis anterior, gastrocnemius medial were high at AD3 of Phase 2 and AD1 of phase2.

무릎넙다리통증증후군 환자의 계단보행 시 발뒤꿈치뼈 교정 테이핑이 슬개대퇴각과 하지근육에 미치는 영향 (Effects of Calcaneus Fixation Taping on Quadriceps Angle and Lower Limb Muscles During Stairway Gait of a Patient with Patellofemoral Pain)

  • Purpose: This study was conducted to determine the structural change in knees caused by quadriceps angle and the muscle activity in lower limbs. Indirect intervention was provided by using taping for stability in the ankle joints, which affected patellofemoral pain. Methods: The subjects in this study were 20 patients with patellofemoral pain who visited ${\bigcirc}{\bigcirc}{\bigcirc} $ Hospital in Busan. The visual analogue scale measured the dynamic quadriceps angle and the degree of pain felt by the patients while walking down stairs, which was a known factor of patellofemoral pain. In addition, muscle activities in the rectus femoris, vastus lateralis, vastus medialis, tibialis anterior, peroneus longus, and biceps femoris, which affect the knees and ankles, were measured using surface electromyography. The muscle activities were converted into %RVC for this study. The data obtained in this study were analyzed with the Wilcoxon signed-rank test using the SPSS Ver. 25.0 statistical program. The significance level ${\alpha}$ was 0.05. Results: The study results showed that the pain and dynamic quadriceps angle were significantly reduced statistically when applying the calcaneus fixation taping (p<0.05). Muscle activity in the lower limbs was significantly decreased in the vastus medialis, vastus lateralis, and tibialis anterior (p<0.05). Conclusion: The summary of the study results verified that the calcaneus fixation taping reduced the pain and dynamic quadriceps angle by providing stability in the ankle joints. It also produced efficient movement due to the difference in lower-limb muscle activity.

발과 무릎관절 위치가 편마비 환자의 안쪽넓은근과 가쪽넓은근 근활성도에 미치는 영향 (The Effects of Foot and Knee Position on Electromyographic Activity of the Vastus Medialis and Vastus Lateralis for Hemiplegic Patients)

  • Purpose: The purpose of this study was to evaluate the electromyographic (EMG) activity of vastus medialis oblique (VMO) and vastus lateralis (VL) muscles on foot position and knee angle for hemiplegia patients. Methods: Ten stroke subjects (10 males) participated in the study. Subjects were all right-hemiplegic patients. All subjects did $0^{\circ},\;20^{\circ}$ and $40^{\circ}$ knee flexion while maintaining the foot in a neutral position, or at $30^{\circ}$ adduction or at $30^{\circ}$ abduction. Surface EMG data were collected for VMO and VL muscles on the non-hemiplegic side and hemiplegic side. Collected data were analyzed using two-way ANOVA. Results: VMO and VL activities for the non-hemiplegic and the hemiplegic sides were highest for $40^{\circ}$ knee flexion while maintaining the three foot positions. There were no significant differences in EMG activity of the VMO and VL muscles with different foot positions. There were significant differences between VMO and VL activity for knee flexion angle while maintaining the foot in neutral (p<0.05), at $30^{\circ}$ adduction (p<0.05), or at $30^{\circ}$ abduction (p<0.05). Conclusion: Foot position does not influence VMO and VL activities. But, knee flexion exercise in a closed chain can increase VMO and VL muscle activity for hemiplegic patients. In particular, VMO and VL activities for both the non-hemi side and the hemi side were highest for $40^{\circ}$ knee flexion.

Comparison of Beef Palatability Characteristics between Longissimus Thoracis and Vastus Lateralis Muscles from Different Grades during Postmortem Aging

  • The objectives of this study was to compare palatability changes of the longissimus thoracis (LT) and vastus lateralis (VL) muscles of Hanwoo steers from different beef quality grades (1+ and 1) during 28 d of wet-aging in order to improve the utilization of the VL muscle as a steak. The VL muscle showed a higher collagen content and a lower intramuscular fat content than the LT muscle (p<0.05). As expected, the Warner-Bratzler shear force value was greater in the LT 1 grade (LT-1) muscle than the LT-1+ muscle (p<0.05); whereas no difference was observed between the grades in the VL muscle at 24 h postmortem. Compared to 0 d of aging, tenderness scores significantly increased after 14 and 21 d of aging in the LT and VL muscles, respectively (p<0.05). Additionally, there was no difference in tenderness score between the VL-1+ aged for 21 d and the LT-1 at 24 h postmortem, although tenderness score was greater in the LT than the VL at each period (p<0.05). Moreover, the VL-1+ steak exhibited a higher tenderness score than the VL-1 steak at 21 and 28 d of aging (p<0.05). On the other hand, the effect of aging time on juiciness and flavor in the VL muscle was somewhat limited unlike the LT muscle. Taken together, the VL muscle requires a longer aging time than the LT muscle to improve consumer preference. Considering the tenderness, using a higher quality grade for aging is more useful in the VL muscle.

Effects of Agility Training with Kinesio Taping on Muscle Tone, Muscle Strength Lower Extremity and Dynamic Stability in Women Softball Players

  • Objective: The aim of this study was to investigate the effect of agility training with kinesio taping (ATKT) on muscle tone, muscle strength lower extremity and dynamic stability of women softball players. Design: Two groups pre-post randomized controlled design Methods: 34 softball players were recruited randomly assigned into the ATKT group and agility training with sham kinesio taping (control group). All subjects performed agility training for 30 min/day, 5 times/week for 4 weeks. The subjects in the ATKT group underwent agility trainingwith kinesio taping on vastus madialis, vastus medialis, rectus femoris, and vastus lateralis where the subects in the control group underwent agility trainingwith sham kinseio taping. Muscle tone was measured using myotonPRO. A Hand dynamometer was used to evaluate muscle strengthening of lower -extremity. Dynamic stability was measured using the side hop test. Results: Muscle tone of lower-extremity was significantly more increased in the ATKT group (mean change 0.39 ± 0.31) than in the control group (mean change 0.19 ± 0.31) (p<0.05). Muscle strength of lower-extremity was significantly more increased in the ATKT group than in the control group (p<0.05). Dynamic stability was significantly more increased in the ATKT group (mean change -1.39 ±0.45) than in the control group (mean change -0.60 ± 0.46) (p<0.05). Conclusions: We confirmed that the benefits that ATKT is effective for increasing in softball players and suggested that knee joint kinesio taping. Also, it was observed improvement of muscle tone, muscle strength and Dynamic stability.

축구선수의 킥 동작시 키네시오 테이핑 적용에 따른 하지근활성화 및 동작분석 (Analysis of Low-leg Activation and Movement of Soccer Players during Kicking Action by Applying Kinesiotaping)

  • In this study, we are concluded like this : 5 men who are soccer player of P university in B city measure the Electromyography with an angle of motion according to kinesiotaping's application when practicing in-step kick. When exercising in-step kick, the change of realization of muscle load of lower limbs enhence Gastrocnemius Lateralis and Vastus Medialis in the section of backswing, and improve Tibialis Anterior, Vastus Medialis, Rectus Femoris prior to impact after back-swing. Before impact, it mainly impoved Tibialis Anterior, Vastus Medialis, Rectus Femoris. After impact, it generally improved Gastrocnemius Lateralis, Vastus Medialis. Average integral electromyography value, it was such a small difference(; the difference of the value in Tibialis Anterior, Rectus Femoris, Vastus Medialis) that we can't compare case of after taping than before. In Electromyography, in case of after taping was considerably decreased at Gastrocnemius Lateralis, there was statistically significant difference between before and after. It was a little increased, after taping than before at Knee angle. And degree was a little decreased at Ankle angle. But, It's so delicate difference, there was not statistically significant difference between before and after.
