국내 축산 환경 중의 항생제 내성균 모니터링에 관한 연구 (Monitoring of Antimicrobial Resistant Bacteria from Animal Farm Environments in Korea)

  • 국내 지역을 경기도, 강원도, 충청도, 전라도, 경상도로 크게 5그룹으로 구분한 후 각각을 2곳의 군단위로 나누어 총 10곳의 지역으로 구분한 뒤, 국내 축산의 주요 축종인 소, 돼지, 닭 축사를 중심으로 분변, 퇴비, 축산인의 손, 지하수, 토양, 하천에서 E. coli 197균주, S. aureus 42균주, C. jejuni/coli 13균주, E. faecium/faecalis 223균주를 분리하여 항생제 감수성 시험을 실시하였다. 모니터링 결과 축산환경에서 분리된 균 중 한 가지 이상의 항생제에 대한 내성율은 E. coli 69.4%(137균주), S. aureus 78.6%(33균주), C. jejuni/coli 100%(13균주), E. faecium/faecalis 82.1%(183균주)를 나타내었다. 또한 4계열 이상의 항생제 내성이 있는 다제 내성율은 각각 19.2%(38균주), 11.9%(5균주), 15.4%(2균주), 6.2%(14균주)였다. 그리고 환경 중에서 VREF 2개 균주를 분리하였다. 축종별 항생제 사용추이를 살펴보면, 소, 돼지, 닭 및 수산용 등 주요 사육가축에 한정하여 판매량을 조사한 결과 소의 경우 01년부터 연도별로 6%, 9%, 8% 판매, 돼지 경우 01년부터 연도별로 57%, 56%, 55% 판매, 닭의 경우 01년부터 연도별로 23%, 23%, 26% 판매, 수산용의 경우 01년부터 연도별로 14%, 12%, 11%판매된 것으로 조사되었다[18]. 이는 소축사 환경에서 분리한 균이 돼지나 닭 축사 환경에서 분리한 균보다 내성율이 낮게 나타나 항생제 사용량과 관계가 있는 것을 추측할 수 있었다. 전체적으로 분리된 균의 내성율이 임상에서 분리되는 균의 항생제 내성율에 비해서는 낮게 검출되었다. 그러나 하천, 토양, 축산인의 손, 지하수 등에서 분리된 균의 항생제 내성회득 결과는 동물의 분변 등 환경관리에 대한 좀 더 많은 관심과 정책이 필요할 것으로 사료되었다 또한 축산 환경에서 분리된 내성균이 많지 않은 국내 현실을 감안할 때 본 연구에서 분리된 균주들은 향후 항생제 내성균 전파 경로를 추적하는 좋은 연구 자료가 될 것으로 사료되었다.

구강악안면 영역의 치성 감염 환자에 대한 세균학적 연구 (A STUDY OF MICROORGANISMS IN ORAL & MAXILLOFACIAL INFECTED PATIENTS)

  • Oral & maxillofacial infections are most commonly odontogenic in origin. Although such infections are usually self-limiting, they may occasionally spread deeply into fascial spaces or planes far from the initial site of involvement. If early diagnosis and appropriate therapy is delayed, complications such as mediastinal extension, retropharyngeal spread and airway obstruction could happen to the patients. For the study of the microbiology, we have retrospectively analysed the oral & maxillofacial infected patients in the Dept. of Oral & Maxillofacial Surgery. In-Ha University Hospital from 1997 September to 2000 April. The results were as follows 1. The male patients were more common than female, with male 61.9% and female 38.1%. 2. Dental originated infections were most common cause with the incidence of 62%. 3. Most common fascial space involved was buccal space 42cases(37.2%) followed by submandibular space 13cases(11.5%), infraorbital space 13cases(11.5%), masseteric space 11cases(9.7%), periapical abscess 11cases(9.7%). 4. The causative organisms isolated from the pus culture were Gram Positive Bacterial species, which were 46cases(31.9%) of Streptococcus viridans, 16cases(8.6%) of ${\alpha}$ and ${\beta}-hemolytic$ streptococcus, 4cases(3.1%) of Strep.-group D non enterococci, 7cases(5.1%) of Staphylococcus Coa. neg., 5cases(3.9%) of Staphylococcus aureus, 3cases(2.3%) of Enterococcus faecalis, 1case(0.8%) of Bacillus species, 1case(0.8%) of Peptostreptococcus, 1case(0.8%) of Clostridium and Gram negative bacterial species, which were 4cases(3.1%) of Acinetobacter baumannii, 2cases(1.6%) of Pseudomonas aeruginosa, 2cases(1.6%) of Burkholderia cepacia, 1case(0.8%) of Neisseria species, 1case(0.8%) of Klebsiella pneumoniae, 1case(0.8%) of Klebsiella oxytoca, 1case(0.8%) of Escherichia coli. 5. In drug sensitivity test, high resistant tendency was found in Penicillin system(Penicillin G 83.3%, Ampicillin 60%) and Aminoglycosides (Gentamycin 50%, Tobramycin 45.5%), but tertiary Cephalosporin system(Cefoperazone 9.1%, Ceftazidime 18.2%), and glycopeptides system (Teicoplanin 0%, Vancomycin 0%) showed lower resistancy.

감염된 당뇨병성 족부 병변의 균주 조사 및 항생제 선택 (Microbiology and Antibiotic Selection for Diabetic Foot Infections)

  • Purpose: To help the empirical antibiotics selection in diabetic foot infection patients, we investigated prevalence of microorganisms and their antibiotics sensitivity results. Materials and Methods: Patients who came to our clinics to treat diabetic foot infections with deep ulceration and were followed up more than 6 months until complete recovery were adopted. From March 2006 to June 2009, there were 140 patients who corresponded with such a inclusion criteria. Wound cultures were done by deep tissue or bone debris at first visit to our clinics. Microorganisms which was documented by wound culture and most susceptible antibiotics by minimum inhibitory concentrations were surveyed retrospectively. Results: Microorganisms were confirmed in 113 cases (80.7%). In the other 27 cases (19.3%), there were no cultured microorganisms. In bacterial growth group, there were 72 cases (63.7%) of gram-positive bacteria and 41 cases (36.3%) of gram-negative bacteria. All of them were aerobic microorganisms and there were no anaerobic microorganisms. Methicillin-sensitive staphylococcus aureus was the most common pathogen and accounted for 35 cases (31.0%). As other common pathogens, there were Enterobacter cloacae (11 cases, 9.7%), pseudomonas aeruginosa (10 cases, 8.8%), Methicillin-resistant staphylococcus aureus (10 cases, 8.8%) and enterococcus faecalis (6 cases, 5.3%), and so on. Common susceptible antibiotics in gram positive microorganism were vancomycin (60 cases, 83.3%), teicoplanin (60 cases, 83.3%), nitrofurantoin (60 cases, 83.3%) and ciprofloxacin (53 cases, 73.6%). In gram negative ones, common susceptible antibiotics were imipenem (35 cases, 85.3%), piperacillin/tazobactam (33 cases, 80.5%) and gentamicin (31 cases, 75.6%). Conclusion: Methicillin-sensitive Staphylococcus aureus in gram positive and enterobacter cloacae in gram negative was the most common pathogen in each group. Ciprofloxacin and gentamicin might be adaptable as a first-line empirical antibiotics in infected diabetic foot patients.
