Diagnosis of Recurrent Uterine Cervical Cancer: Computed Tomography versus Positron Emission Tomography

  • Objective: To determine the accuracy of CT and positron emission tomography (PET) in the diagnosis of recurrent uterine cervical cancer. Materials and Methods: Imaging findings of CT and PET in 36 patients (mean age, 53 years) in whom recurrent uterine cervical cancer was suspected were analyzed retrospectively. Between October 1997 and May 1998, they had undergone surgery and/or radiation therapy. Tumor recurrence was confirmed by pathologic examination or follow-up studies. Results: In detecting recurrent uterine cervical cancer, the sensitivity, specificity, and accuracy of CT were 77.8%, 83.3%, and 80.5%, respectively, while for PET, the corresponding figures were 100%, 94.4%, and 97.2%. The Chi-square test revealed no significant difference in specificity (p = .2888), but significant differences in sensitivity (p = .0339) and accuracy (p = .0244). Conclusion: PET proved to be a reliable screening method for detecting recurrent uterine cervical cancer, but to determine the anatomical localization of recurrent tumors, and thus decide an adequate treatment plan, CT was eventually needed.

Uterine Arterial Embolization for the Treatment of Leiomyomas Accompanying with Adenomyosis (자궁선종을 동반한 자궁근종에서 자궁동맥 색전술을 이용한 치료의 효과)

  • Objective: The purpose of this study was to make a guideline of uterine artery embolization for the treatment of uterine leiomyomas accompanying with adenomyosis in Korea. Materials and Methods : We performed the retrospective study for 37 women who had uterine leiomyomas accompanying with adenomyosis. Bilateral uterine artery embolization was performed in 37 patients (age range 25-65) during 17 months with pain, hypermenorrhea, urinary frequency etc due to leiomyomas. Ultrasound imaging was performed before the procedure and at mean 6.9 months after the procedure. Results: All procedures were technically successful. Mean clinical follow-up was 12.8 months. Minor complication occurred in 82% patients after the procedure. After imaging follow-up (mean, 6.9 months postprocedure), median uterine volume decreased 34.4%, and dominant myoma volume decreased 86%. There was no statistical difference in uterine volume reduction and dominant myoma size reduction whether occluding agents was polyvinyl alcohol, polyvinyl alcohol plus gelfoam, and gelfoam, and whether ultrasound measured Resistance Index value before the procedure was low or high. Conclusion: Primary candidates for uterine artery embolization include those with symptomatic uterine leiomyomas who no longer des ire fertility but wish to avoid surgery or are poor surgical risks. To our study, uterine volume reduction and dominant myoma size reduction in patients who had adenomyosis were similar to previous other studies in patients who had not adenomyosis. Therefore adenomyosis should not be considered as a contraindication for uterine artery embolization. Because there is little data about subsequent reproductive potential after this procedure, it should not be routinely advocated for infertile women. Further investigation is warranted for occluding agents and Resistance Index.

  • It is well known that the uterine contractility is affected by sexual hormone. In this experiment, the authors attempted to study the influences of testosterone and estrogen or the uterine contractility to oxytocics. The contractile sensitivity of the excised uterine muscle of non-castrated and castrated rabbits with testosterone and estrogen 24 hours before experiment is observed respectively. And the cholinesterase activity and electrolytes (Na, K, Ca and Mg) in the uterine muscle are measured in order to study the relationship with contractile sensitivity and those changes. The results obtained were summarized as follows: 1. The contractile effect of spareng on the excised uterine muscle of non-castrated rabbits pretreated with estrogen was markedly increased in small dose, but that of rabbits pretreated with testosterone was significantly increased in large dose, comparing with that of the control group. In castrated rabbits, the contractile sensitivity of the uterine muscle to spareng was significantly increased by pretreatment with estrogen in large dose but it was markedly decreased by pretreatment with testosterone in small dose. 2. The contractile effect of quinine on the excised uterine muscle of non-castrated rabbits pretreated with estrogen was significantly decreased but that of castrated rabbits pretreated with both estrogen and testosterone were markedly increased comparing with that of the control group. 3. The cholinesterase activity in the uterine muscle of non-castrated rabbits was significantly increased by pretreatment with small dose of estrogen or large dose of testosterone, but that of castrated rabbits was markedly decreased by pretreatment with large dose of estrogen. 4. Na and K contents in the uterine muscle of non-castrated rabbits were markedly increased by pretreatment with both estrogen and testosterone, but that of castrated rabbits was significantly increased by pretreatment with small dose of estrogen. 5. Ca content in uterine muscle of non-castrated rabbits was significantly decreased by pretreatment with both large dose of estrogen and testosterone but increased by pretreatment of testosterone. In castrated rabbits, Ca content was significantly decreased by pretreatment with both estrogen and testosterone. 6. Mg content in the uterine muscle of non-castrated rabbits was markedly increased by pretreatment with estrogen and small dose of testosterone, but that of castrated rabbits was significantly decreased by pretreatment with both large dose of estrogen and testosterone.

Alterations in uterine hemodynamics caused by uterine fibroids and their impact on in vitro fertilization outcomes

  • Objective: To investigate the impact of fibroids on the blood flow of the uterine and subendometrial arteries and in vitro fertilization (IVF) outcomes. Methods: In this study, we analyzed 86 IVF/intracytoplasmic sperm injection (ICSI) cycles in which a gonadotropin-releasing hormone antagonist protocol was used for controlled ovarian stimulation between January 2008 and March 2009. The subjects comprised 86 infertile women with (fibroid group, n=43) or without (control group, n=43) uterine fibroids. Results: Patient characteristics were similar between the fibroid and control groups. The IVF/ICSI outcomes in patients with fibroids were similar to those of patients in the control group. The resistance index (RI) and pulsatile index (PI) of the uterine and subendometrial arteries on the day of embryo transfer were also comparable between the two groups. IVF outcomes and uterine hemodynamics in patients with multiple (${\geq}2$) fibroids were similar to those of patients with a single fibroid. However, clinical pregnancy and implantation rates were significantly lower in patients with fibroids who experienced uterine cavity distortion than in patients with fibroids who had a normal uterine cavity (both p<0.05). The RI and PI of the subendometrial artery were significantly higher on the day of embryo transfer in patients with fibroids who experienced uterine cavity distortion than in patients with fibroids who had a normal uterine cavity (both p<0.05). Conclusion: Fibroids which distorting the uterine cavity might impair the subendometrial artery blood flow clinical pregnancy rate and embryo implantation rate in infertile patients undergoing IVF. Otherwise, IVF outcomes were not influenced by the presence of uterine fibroids.

  • Fertilization is the beginning of a new life that occurs in the female uterine. The female reproductive tract is composed ovary, oviduct, uterine, vagina and cervix, their physiological features are regulated by estrous cycle. Of these, uterine is a main point to establish embryo development and implantation, and intercommunication between embryo and uterine environment is necessary for suitable pregnancy. Endometrium is part of the uterine, its morphology is repetitively changed by hormones, and characteristic of uterine fluid from endometrium is also changed. Recently, massive proteins of endometrium and uterine fluid can be detected according to develop proteomics and bioinformatics and have been accelerated the understanding of the reproductive biology fields. Moreover, the massive protein information is actively studying with deeply studied theory such as sex hormone signal pathway and angiogenesis in mammals. In this paper, we review understanding of endometrium remodeling, uterine gland and fluid during estrous cycle, additionally studies on endometrium and uterine fluid based on proteomics techniques. Lastly, we introduced methods of the protein-protein correlation using bioinformatics tool that interaction with hormone receptors, representative angiogenetic factors and detected proteins using proteomics in endometrium and uterine fluid. This review will be useful to understanding the study on search of new cell mechanism in endometrium and uterine fluid.

Serial Ultrasonographic Appearance of Postpartum Uterine Invoution in Shih-tzu Bitches (Shih-tzu 견에서 분만 후 자궁수복의 연속적 초음파상)

  • This study was undertaken to determine the normal serial ultrasonographic appearance of the postpartum uterine involution. Postpartum changes in uterine shape, architecture, echogenicity and diameter were monitored by ultrasonography in 10 Shih-tzu bitches. Serial ultrasonographic examination was done daily during the first week, 3 days interval from 8 to 30 days, and weekly from 31 to 100 days postpartum. All 10 postpartum bitches had normal involution by gross finding, vaginal discharges, and by ultrasonographic findings, uterine shape and echogenicity. The uterine diameter in the placental sites was decreased from $23.56{\pm}1.45$ mm at 1 day to $14.08{\pm}1.55$ mm at 7 day, and in the interplacental sites was decreased from $14.64{\pm}1.28$mm at 1day to $9.61{\pm}1.46$mm at 7 day postpartum. At 65 days postpartum the uterine diameter was 5-6 mm both placental and interplacental sites, and the uterine horns were uniform hypoechic, tubular structures without enlargement. Therefore, complete involution of the uterus occured 65 days. It was concluded that normal postpartum uterine involution in Shih-tzu bitches appeared to be completed 65 days postpartum by gross findings such as vaginal discharges, and ultrasonographic findings. And ultrasonographic characteristics of the postpartum uterine involution were described. Therefore, these result suggest that ultrasonographic assessment is a reliable method for diagnosing the subinvolution of placental sites and uterine dysfunction, such as pyometra in the bitch.

Risk Awareness on Uterine Cancer among Australian Women

  • Uterine cancer is the most common invasive gynaecological cancer in Australia. Early detection is a key predictive factor achieved by increasing public awareness and participation in screening. This observational study measures awareness of gynaecological malignancies, particularly uterine, among women in two rural areas of New South Wales, Australia. Patients presenting to gynaecology clinics in January to March 2014 were invited to complete a structured questionnaire. Women with a history of cancer and incomplete questionnaires were excluded. Of the 382 patients invited to participate, 329 (86%) responded with complete feedback. Most respondents were younger than than 50 years (66%) and married with at least 2 children (74%). The majority (94%) of participants had no awareness of uterine cancer and many (46%) were unable to identify common risk factors including obesity, diabetes and hypertension. The ability to identify risk factors was correlated to age, marital status and obesity. The study identifies poor awareness on uterine malignancies in two typical areas of rural Australia. Although external validity is limited by sociological factors, poor awareness of uterine cancer among rural patients in this study represents a valid public health concern. It is imperative to improve awareness of uterine cancer and available screening programs to facilitate early detection and cure.

A Study on Skin Resistance Variability (SRV) of Women with Uterine Myoma (자궁근종환자의 피부저항 변이도에 관한 연구)

  • Objectives : Uterine myoma is the most common pelvic benign tumor and may cause abnormal menstrual pattern. The growth of uterine myoma depends on hormones. The 7-zone-diagnostic system (CP-6000A) is a measuring system using skin resistance variability (SRV). The machine shows organic energy and was reported by the gynecologic department to detect functional abnormality of hypothalamus-hypophysis-ovarian axis (H-P-O). Our study was to investigate the SRV of women with uterine myoma by the 7-zone diagnostic system. Methods: Two groups were selected from those who took the CP-6000A test in the College of Korean Medicine Hospital of Sangji University from March 2003 to September 2006. They were divided into normal control (n=40) and uterine myoma groups (n=40). Electrodermal activity of the two groups was compared. Results : The mean value of electrodermal activity of the uterine myoma group was lower than that of thenormal group in all areas on first and second measurement. Especially, the mean value of the patient group was significantly lower than that of the normal group and fell below normal range in the 1, 2, and 3 areas on first and second measurement. Conclusions : The results suggest that women with uterine myoma may have organic energy deficiency and functional abnormality of the H-P-O axis. Further studies to determine the feasibility of this 7 -zone diagnostic system as a reliable diagnostic tool are needed.

Descriptive Epidemiology of Common Female Cancers in the North East India - a Hospital Based Study

  • Background: Cancers of the breast, uterine cervix and ovary are common cancers amongst females of North East India. Not much is known about the descriptive epidemiology of these cancers in our population. The present retrospective analysis was therefore performed. Materials and Methods: The data set available at the hospital based cancer registry of a regional cancer center of North-East India, containing information on patients registered during the period of January 2010 to December 2012, was applied. A total of 2,925 cases of breast, uterine cervix and ovarian cancer were identified. Results: Of the total, 1,295 (44.3%) were breast cancers, 1,214 (41.5%) were uterine cervix and 416 (14.2%) ovarian cancer, median age (range) for breast, uterine cervix and ovary were 45 (17-85), 48 (20-91) and 45 years (7-80), respectively. Some 43.5% of cases with uterine cervix patients were illiterate, 5.4% and 5.7% stage I in breast and cervix respectively and 96.4% of ovarian cancers in advanced stage. Conclusions: Improvement of female education can contribute to increase the proportion of early stage diagnosis of breast and uterine cervix in our population. Any population-based intervention for the detection of cancers of breast, uterine cervix and ovarian cancer should be started early in our population.

Responsiveness of Pregnant Rat Uterus to Oxytocin (임신쥐에 대한 옥시토신의 반응)

  • In vitro contractile response of the uterus in the pregnant rat to oxytocin increases with advancing gestation. This increase coincides with an increase in uterine oxytocin receptor number. However, in vitro the change in uterine contractile response of the pregnant uterus to oxytocin with advancing gestation is not clear. The purpose of the present study was to find out that the uterine response in vitro to oxytocin changes as delivery a, pp.oaches. Secondly, to determine if the incubation of uterine tissue in vitro altered the uterine oxytocin receptor number and affinity and thus explain the ambiguity between the in vitro and in vitro results. The studies were performed on rats at days 15, 20 and 21 or pregnancy. In vitro the uterine contractile response to oxytocin was sgreater (P<0.05) at day 21 compared to days 15 and 20 (Emax : 724.8 88.9, 130.0 81.5 and 133.4 53.4, respectively). This correlated with a significant increase (P<0.05) in uterine oxytocin receptor number at day 21 (days 21, 15 and 20 : 540 89, 53 24, 89 35 fmoles/mg protein, respectively). However, the kids at days 15, 20 and 21 did not differ (P>0.05). Finally no difference (P>0.05) in oxytocin receptor number or affinity was detected between incubated and non-incubated tissue. The results of these studies suggest that either pre-oxytocin or post-oxytocin receptor factors are important in determining the uterine myometrial responsiveness to oxytocin.
