  • Electron contamination due to the interaction between radiation beam and material was analyzed for the factors such as source-skin distance (SSD), field size, tray characteristics and position of filter, which can affect the surface dose in Cobalt teletherapy. Surface dose in open beam was more influenced by SSD with increasing field size. Relative surface charge (RSC) increased with the use of tray (solid, circular hole, slotted), compared with open beam, which is thought to be due to increased electron contamination of the tray. To reduce the surface dose, 0.4mm thick Lipowitz metal filter was used. Compared with open beam, RSC decreased by 8.8%, 11.3%, 13.3%, 16.6%, 19.3% and 21.7% for the field size of $5{\times}5$, $10{\times}10$, $15{\times}15$, $20{\times}20$, $25{\times}25$ and $30{\times}30cm^2$, respectively. On the contrary, use of Lipowitz metal filter increased RSC at 60cm or less SSD. Surface dose was effectively reduced with Lpowitz metal filter placed right below solid tray in Cobalt teletherapy.

  • Dose calculation algorithms play an important role in radiation therapy and are even the basis for optimizing treatment plans, an important feature in the development of complex treatment technologies such as intensity-modulated radiation therapy. We reviewed the past and current status of dose calculation algorithms used in the treatment planning system for radiation therapy. The radiation-calculating dose calculation algorithm can be broadly classified into three main groups based on the mechanisms used: (1) factor-based, (2) model-based, and (3) principle-based. Factor-based algorithms are a type of empirical dose calculation that interpolates or extrapolates the dose in some basic measurements. Model-based algorithms, represented by the pencil beam convolution, analytical anisotropic, and collapse cone convolution algorithms, use a simplified physical process by using a convolution equation that convolutes the primary photon energy fluence with a kernel. Model-based algorithms allowing side scattering when beams are transmitted to the heterogeneous media provide more precise dose calculation results than correction-based algorithms. Principle-based algorithms, represented by Monte Carlo dose calculations, simulate all real physical processes involving beam particles during transportation; therefore, dose calculations are accurate but time consuming. For approximately 70 years, through the development of dose calculation algorithms and computing technology, the accuracy of dose calculation seems close to our clinical needs. Next-generation dose calculation algorithms are expected to include biologically equivalent doses or biologically effective doses, and doctors expect to be able to use them to improve the quality of treatment in the near future.

  • This paper provides the design of system software for the management of radiation dose that is generated by using computerized tomography(CT). Recently, the radiation leakage incident of Japanese nuclear power plant was in the news internationally and there is a growing interest not only in nuclear power plant but in medical radiation exposure. In spite of the fact that currently safety management of radiation is under control only the workers of the radiation involved, now the exposure management of patients have been required. As surgery and inspections using the radiation have increased, this medical radiation exposure is increasing too. But it is a real situation that medical institutions don't know the level of radiation exposure applied to the patient. Therefore, a system for managing the radiation exposure of a patient from the medical institution is required. This paper proposes a design of a software program that manages the radiation exposure of CT which is a typical imaging tool to use the radiation in the medical institution. By check the amount of radiation dose and set the limit of dose, we would be of help to optimize the medical exposure of the patient.

  • The radionuclide therapy is a protocol for tumor control by administering radionuclides as the cytotoxic agents. Radionuclides concentrated at the site of cancerous lesion are expected to kill the cancerous cells with minimal injury to the normal tissue. The efficacy of every radionuclide treatment can be evaluated by examining the toxicity to the lesion differentiated from that to the normal tissue. Radiation dosimetry is the procedure of quantitating the energy absorbed by target volumes of interest. Dosimetric information plays an indicator of the expected radiation damage and thus the therapeutic efficacy. This paper summarizes the dosimetric aspects in radionuclide therapy in terms of radionuclides of use, radiation dosimetry methodology and considerations for each treatment in practical use.

  • Considering the variety of application and other factors in performing the optimization of radiological protection, it is necessary to use well-structured optimization procedure. In this study, an optimization procedure for design stage of nuclear power plant was proposed, and applied in the selection of alternatives for reducing occupational radiation exposure. As an aiding technique for decision-making, multi-criteria technique was used. As a result, priorities of alternatives are determined, and a problem of applying this result to real situation is reviewed. The developed procedure for optimization can be extended to other areas such as radioactive waste management, medical use of radiation, etc.

  • The primary treatment modality of malignant tumors of the nasopharynx is radiation therapy owing to its inaccessibility to surgical intervention. Over the last two decades there were many changes in techniques of delivery, which include the use of higher doses of radiotherapy, the use of wide radiation field, including the elective radiation of the whole neck, the combined use of brachy- and teletherapy, and the use of split-course therapy. In spite of these advances local and regional recurrences remain the major cause of death. As a boost therapy after external irradiation, high-dose-rate intracavitary irradiation using remote control afterloading system(RALS) was used in two patients. Our results were satisfactory, however, this procedure should only be performed by those who have developed enough expertise in the use of intracavitary techniques for the treatment of nasopharyngeal cancer and have a supportive team including a physicist, dosimetrist, nurse, and trained technologist.

  • This study investigated the effectiveness of radiation safety rules in animal hospital and the awareness and behavior of veterinary radiation workers. With the questionnaires, the data was collected from randomly selected veterinarians in animal hospitals and animal medical imaging centers. Collected data were about radiation device, shielding device, regulations, safety management, education, knowledge, behavior and awareness. Frequency, correlation and multiple regression analysis were performed. The medical devices related with radiation in animal hospital were X-ray (59%), CT (15%), fluoroscopy (12%), mobile X-ray (12%) and others (2%). The number of people using radiation shielding device is high. The answers were low on knowing radiation related regulation and receiving radiation protection education. The group with higher knowledge and awareness shows positive correlation with safety behavior. The increase of use of the radiation related medical devices in veterinary hospital causes the increase of radiation exposure risk. This study suggests that radiation safety management system and policies need to be developed to protect radiation workers and give them correct information and consciousness.

  • Image-guided radiation therapy (IGRT) is a process of incorporating imaging techniques such as computed tomography (CT), magnetic resonance imaging (MRI), Positron emission tomography (PET), and ultrasound (US) during radiation therapy (RT) to improve treatment accuracy. It allows real-time or near real-time visualization of anatomical information to ensure that the target is in its position as planned. In addition, changes in tumor volume and location due to organ motion during treatment can be also compensated. IGRT has been gaining popularity and acceptance rapidly in RT over the past 10 years, and many published data have been reported on prostate, bladder, head and neck, and gastrointestinal cancers. However, the role of IGRT in lymphoma management is not well defined as there are only very limited published data currently available. The scope of this paper is to review the current use of IGRT in the management of lymphoma. The technical and clinical aspects of IGRT, lymphoma imaging studies, the current role of IGRT in lymphoma management and future directions will be discussed.
