Dietary Practices, Addictive Behavior and Bowel Habits and Risk of Early Onset Colorectal Cancer: a Case Control Study

  • Background: The abrupt rise of colorectal cancer in developing countries is raising concern in healthcare settings. Studies on assessing relationships with modifiable and non-modifiable risk factors in the Pakistani population have been limited. The present investigation was designed to examine associations of dietary practices, addictive behavior and bowel habits in developing colorectal cancer (CRC) among patients in a low-resource setup. Materials and Methods: An age-gender matched case control study was conducted from October 2011 to July 2015 in Karachi, Pakistan. Cases were from the surgical oncology department of a public sector tertiary care hospital, while their two pair-matched controls were recruited from the general population. A structured questionnaire was used which included questions related to demographic characteristics, family history, dietary patterns, addictive behavior and bowel habits. Results: A family history of cancer was associated with a 2.2 fold higher chance of developing CRC. Weight loss reduced the likelihood 7.6 times. Refraining from a high fat diet and consuming more vegetables showed protective effects for CRC. The risk of CRC was more than twice among smokers and those who consumed Asian specific addictive products as compared to those who avoid using these addictions (ORsmoking: 2.12, 95% CI: 1.08 - 4.17, ORpan: 2.92, 95% CI: 1.6 - 5.33, ORgutka: 2.13, 95% CI: 1.14 - 3.97). Use of NSAID attenuated risk of CRC up to 86% (OR: 0.14, 95% CI: 0.07 - 0.31). Conclusions: Most of the findings showed concordance with the literature elucidating protective effects of consuming vegetables and low fat diet while documenting adverse associations with family history, weight loss, constipation and hematochezia. Moreover, this study highlighted Asian specific indigenous addictive products as important factors. Further studies are needed to validate the findings produced by this research.

학령기 말더듬 아동 치료에 있어 그룹지도의 효과 (EFFECTS OF GROUP THERAPY ON SPEECH FLUENCY IN ELEMENTARY SCHOOL STUTTERING CHILDREN)

  • 본 연구에서는 이제까지의 말더듬에 대한 여러 이론들을 살피고 말더듬 치료의 임상 경험을 보고하고자 하였다. 말더듬의 원인이나 치료법에 관한 수많은 연구가 있어왔으나 아직도 이에 대한 한마디의 명확한 답을 내리기는 어렵다 다만 말더듬에 대한 이해를 점차 넓혀가고 있고, 보다 효과적인 치료에 접근하고 있다고 본다. 말더듬의 원인을 기질적 요인으로 보는 주장, 심리적 요인으로 보는 주장, 학습적 요인으로 보는 주장, 환경적 요인으로 보는 주장들이 있어왔다. 이에 따라 치료법도 다양한데 크게는 말더듬 수정법과 단계적 유창성 완성법으로 나누어진다. 본 연구자는 말더듬 치료에 각 아동이 갖을 수 있는 복합적 요인을 살피고 치료에서도 한가지를 고수하기 보다는 통합적으로 접근해야 한다는 입장을 취하였다. 이에 따라 대상이된 아동들의 문제는 다각적으로 살펴졌고 각 아동은 개별 언어 치료를 받고 이어서 그룹치료를 받았다. 그룹치료의 목표는 좀더 어려운 언어 상황으로까지 유창성을 높이고 유창성의 일반화를 유지하는 것이었다. 대상이된 세 아동은 모두 ‘읽기’ 그리고 ‘대화’, 상황에서 유창성을 높였고 전체 말더듬 행동에도 호전을 보였는데 그룹치료에서는 특히 ‘대화’에서 유창성을 높일 수 있었다. 말더듬의 각 요인들을 살필 수 있는 민감한 진단 도구의 개발과 말더듬 이론이 반영된 보다 진보적인 치료 프로그램의 개발이 계속되어야 함이 토의되었다.

PEMOCVD of Ti(C,N) Thin Films on D2 Steel and Si(100) Substrates at Low Growth Temperatures

  • Titanium nitride (TiN) thin films have useful properties including high hardness, good electrical conductivity, high melting point, and chemical inertness. The applications have included wear-resistant hard coatings on machine tools and bearings, decorative coating making use of the golden color, thermal control coatings for widows, and erosion resistant coatings for spacecraft plasma probes. For all these applications as feature sizes shrink and aspect ratios grow, the issue of good step coverage becomes increasingly important. It is therefore essential to manufacture conformal coatings of TiN. The growth of TiN thin films by chemical vapor deposition (CVD) is of great interest for achieving conformal deposition. The most widely used precursor for TiN is TiCl4 and NH3. However, chlorine impurity in the as-grown films and relatively high deposition temperature (>$600^{\circ}C$) are considered major drawbacks from actual device fabrication. To overcome these problems, recently, MOCVD processes including plasma assisted have been suggested. In this study, therefore, we have doposited Ti(C, N) thin films on Si(100) and D2 steel substrates in the temperature range of 150-30$0^{\circ}C$ using tetrakis diethylamido titanium (TDEAT) and titanium isopropoxide (TIP) by pulsed DC plamsa enhanced metal-organic chemical vapor deposition (PEMOCVD) method. Polycrystalline Ti(C, N) thin films were successfully grown on either D2 steel or Si(100) surfaces at temperature as low as 15$0^{\circ}C$. Compositions of the as-grown films were determined with XPS and RBS. From XPS analysis, thin films of Ti(C, N) with low oxygen concentration were obtained. RBS data were also confirmed the changes of stoichiometry and microhardness of our films. Radical formation and ionization behaviors in plasma are analyzed by optical emission spectroscopy (OES) at various pulsed bias and gases conditions. H2 and He+H2 gases are used as carrier gases to compare plasma parameter and the effect of N2 and NH3 gases as reactive gas is also evaluated in reduction of C content of the films. In this study, we fond that He and H2 mixture gas is very effective in enhancing ionization of radicals, especially N resulting is high hardness. The higher hardness of film is obtained to be ca. 1700 HK 0.01 but it depends on gas species and bias voltage. The proper process is evident for H and N2 gas atmosphere and bias voltage of 600V. However, NH3 gas highly reduces formation of CN radical, thereby decreasing C content of Ti(C, N) thin films in a great deal. Compared to PVD TiN films, the Ti(C, N) film grown by PEMOCVD has very good conformability; the step coverage exceeds 85% with an aspect ratio of more than 3.

자폐범주성장애 아동의 나이에 따른 식행동 및 식품 기호도와 학부모의 영양교육 요구도 (Dietary Behavior and Food Preferences according to Age and the Parents' Nutrition Education Needs of Children with Autism Spectrum Disorder)

  • This study examined the eating habits and preferences of foods according to the age of children with autism spectrum disorders and identified the nutritional needs of parents to support their healthy eating habits and provide basic data for the development of educational programs. A survey of 100 parents of children with autism spectrum disorders was conducted on their children's snack intake, food preferences, and parents' nutrition education needs. As a result, there were four times more boys than girls, and the body mass index differed according to age (p<0.001). Most children ate snacks more than once a day, and parents found that they wanted to develop nutritious snacks that could replace high-protein, low-calorie meals with the minimal use of additives. For the food preference, there was a significant difference in grains and starches, vegetables/seafood and fruits, milk and dairy products (p<0.05), fats and sugars, and Korean fermented pastes (p<0.01) between the two age groups. Regarding whether nutrition education was required, many parents had a high demand for nutrition education (4.24±0.70), and the intention to participate was 3.62±1.09. The most important content to be dealt with in parent education was 'Inducing a change in bad eating habits and behaviors', which was the highest (63%), and mostly face-to-face education was preferred. In conclusion, young children require diet education, and older children need to focus on weight control and obesity.

여성 베이비부머들의 식생활 태도와 미래 식생활 요구도 조사 (A Study on the Dietary Behaviors of Female Baby Boomers and the Needs for Future Perspectives of Dietary Life)

  • The purpose of this study is to examine female baby boomers' dietary habits and their attitudes together with their needs for future perspectives of dietary life. Our aim is to use these findings as a basic data when forecasting for food-related industries or policy making. A survey is being carried out for a total of 358 female baby boomers and analyzed by SPSS 12.0. The following is a summary of this study. The average age is 52.6 years old, most of them graduated from highschool (63.1%) and had a nuclear type of family (76.1%). Only 39.0% is composed of housewives, others had either full-time or part-time jobs. Self-assessment of stress is not so high and only 8.1% are dissatisfied with their lives. 38.2% are either overweight or obese in terms of BMI, and most of them are non-smokers (97.2%) or non-drinkers (63.0%). Their mean dietary habit scores are $70.6{\pm}11.8$, and the scores show significant relations with their education levels (p<0.01), monthly income (p<0.01), life satisfaction rates (p<0.001), stress levels (p<0.001), smoking habits (p<0.05), drinking habits (p<0.05), regular exercises (p<0.001) and regular health check-ups (p<0.05). The rate of skipping breakfast, lunch and dinner are 18.2%, 1.1%, 5.2% respectively. The main reason for skipping breakfast is the 'lack of time'. With regards to the frequency of grocery shopping, almost half of the subjects (55.7%) said '1~2 times per week' and bought mainly raw food sources such as vegetables, fruits, and meats. The majority of the subjects (91.3%) report that they cooked meals at homes, and took about 1 hour of time. The subjects also point out that cooking was a bothering task, and only 46.4% would prepare meals at home, while others would rather eat out or eat convenience foods. The main reasons for not wanting meal services in the elderly welfare facility are because they didn't want to live such places (48.4%) and the meals are tasteless (31.3%). As for delivery meal services, 60.1% are aware of it, and 39.9% would consider using it in the future. Factors to be considered when using the delivery meal service are sanitation (43.7%), nutrition (28.7%), taste (18.4%), price (6.3%), and brand name (2.9%). This study is expected to be used as useful information when developing food-related strategies for baby boomers in the future.

전주지역 중, 노년층의 생활습관과 건강상태 조사(II) (A Study of Health-related Habits, Dietary Behaviors and the Health Status of the Middle-aged and the Elderly Living in the Chonju Area (II))

  • This study investigated the anthropometric and biochemical indices, and the health and nutritional factors influencing the two indices among 194 middle-aged and elderly subjects (108 middle-aged and 86 elderly) residing in a medium sized city for more than 10 years. In the examination of their dietary habits, 8.3% of the middle-aged subjects and 14.0% of the elderly subjects had two meals a day, and more female subjects had two meals per day. Of the subjects who ate meals at regular times, 75.0% were middle-aged and 79.1% were elderly, and the degree of irregularity of meals was greater for female subjects. The study of the dietary behavior of the subjects indicated that 71.3% and 66.3% of the middle-aged and elderly, respectively responded that the amount of food in each meal was sufficient. The subjects ate alone comprised 19.7% of the middle-aged females and 31.5% of the elderly females. The prevalence of smoking among the subjects was 28.1% for the middle-aged, 18.8% for the elderly male and 7.4% for the elderly females. The percentage of the subjects who drank alcohol was 34.4% of the middle-aged males and 13.2% of the middle-aged females. Slightly less than half of the subjects exercised more than once a week, with the male subjects showing a higher rate than the female subjects. The average body mass indices (BMI) were 24.5 and 24.6 for the middle-aged male and female, respectively, and 22.6 and 24.0 for the elderly male and female, respectively. BMI assessment showed that underweight subjects (BMI < 20) comprised 3.7% of the middle-aged, 14.0% of the elderly, and that 40.7% of the middle-aged and 24.4% of the elderly were overweight (25 < BMI < 30) , and 0.9% of the middle-aged and 1.2% of the elderly were classified as obese (BMI $\geq$ 30) . A waist/hip ratio (WHR) greater than 0.8 was found in 89.5% of the middle-aged females and 90.7% of the elderly females, showing high abdominal fat deposition in the majority of females. The average systolic blood pressure of females was 121.1 $\pm$ 17.1 mmHg for the middle-aged and 129.6 $\pm$ 21.3 mmHg for the elderly subjects. The systolic blood pressures showed a significantly difference between the two age groups. Those defined as anemic subjects based on hemoglobin values comprised 13.0% of the middled-aged group and 16.3% of the elderly group. There was a tendency for higher fasting glucose levels among the elderly subjects. An increase in total plasma cholesterol levels with age was shown. The female subjects had higher cholesterol levels than the males'The study of the correlation between the daily habits and health status showed that the amount of food eaten at each meal, the frequency of eating out, and the use of dietary supplements appeared to influence BMI, WHR, the plasma triglyceride and plasma cholesterol levels; omitting one meal had a positive correlation with the systolic blood pressure and plasma cholesterol. These results suggest that desirable dietary habits and concerns for health are contributing factors for maintaining good health, as indicated by normal blood lipid levels.

  • 자연휴양림 이용자들의 자연휴양림 시설의 이용, 자연휴양림 이용에 대한 기대 및 성취도 등을 분석하여 차후 자연휴양림의 조성은 물론 기존의 자연휴양림에 대하여 이용자들이 원하는 바를 더욱 보완하고 보다 질 좋은 자연휴양림을 조성 관리하기 위한 기초자료를 제공하고자 자연휴양림 이용자들의 시설물 이용행태와 자연휴양림 이용의 만족도를 조사분석한 바 다음과 같은 결과를 얻었다. 자연휴양림 이용자들이 휴양림내 시설물 중 화장실, 야영장, 산림욕장 등을 가장 많이 이용하였고, 가장 필요로 하는 시설물은 화장실, 매점, 급수대 등이었다. 자연휴양림의 이용 전에는 아름다운 자연풍경 감상과 피로를 풀고 휴식을 취하고자 하는 기대도가 가장 높았으며, 이용후의 성취도는 기대도와 마찬가지로 아름다운 자연풍경 감상과 피로를 풀고 휴식을 취하는데 가장 높은 성취도를 나타냈다. 자연휴양림 이용자의 만족도를 결정짓는 주요 설명변수로서, 자연휴양림 시설물의 배치 및 조화 측면에서는 청결 및 보수상태, 시설물의 유지관리, 시설물 주변 이용자들의 소음정도, 시설물 이용의 편리도, 주변의 자연경관 등이 작용하였고, 자연휴양림의 식생상태에 대한 측면에서는 주변식생의 다양도, 녹음의 양, 자연림 보호관리, 식생과 주변과의 조화 등이 만족도 결정의 주요 변수로 작용하였다. 또한 자연휴양림 시설상태에 대한 측면에서는 휴식시설, 화장실의 이용편리도 및 청결상태, 쓰레기통의 배치상태, 취사장의 이용편리도 등이 작용하였고, 자연휴양림의 운영관리실태에 대한 측면에서는 산림의 보호와 육성, 관리인의 이용자 행락지도, 휴양림에 대한 안내와 홍보수준이 만족도 결정의 주요 변수로 작용하였다.

초등학생의 유튜브 이용과 심리적 적응 : 부모양육태도, 자아존중감, 학교생활만족도의 매개효과 (Children's YouTube Watching Behaviors and Their Adjustments: Mediation Effects of Parental Attitude, Self Esteem, and School Satisfaction)

  • 본 연구는 초등학생의 유튜브 이용실태와 동기를 확인하고, 유튜브 이용과 부모양육태도, 자아존중감, 학교생활만족도와의 관계를 규명하고자 하였다. 이를 위해 초등학교 4, 5, 6학년 재학생 238명을 대상으로 유튜브 사용량과 동기, 부모 양육태도, 자아존중감, 학교생활만족도를 측정하였다. 연구 결과, 조사 대상 초등학생의 과반수는 하루에 1시간 이상 유튜브를 이용하고 있으며, 전체의 15.6%는 하루에 3시간 이상 유튜브를 이용한다고 응답해, 초등학생들의 상당 수가 매우 적극적으로 유튜브를 사용하고 있음이 드러났다. 유튜브 이용빈도가 높을수록 부모의 애정, 자아존중감, 학교생활 만족도는 낮은 편이었다. 또 부모의 애정적 양육태도는 초등학생의 유튜브 이용빈도를 부분매개하여 적응에 영향을 미치는 것으로 드러났다. 반면 거부적 양육태도 및 과보호적 양육태도의 경우 유튜브 사용량과 큰 관련성이 없었으며, 매개효과도 발견되지 않았다. 본 연구의 결과는 초등학생의 과도한 유튜브 이용의 구체적인 심리적 악영향을 밝힌 국내 최초의 경험적 연구로, 초등학생의 유튜브 이용에 대한 적절한 가이드라인이 필요함을 시사하였다. 또한 아동의 유튜브 이용에 있어서도 부모의 양육태도가 관여할 수 있음을 보여주었다.

Bayesian 기법을 이용한 혼합 Gumbel 분포 매개변수 추정 및 강우빈도해석 기법 개발 (A Bayesian Approach to Gumbel Mixture Distribution for the Estimation of Parameter and its use to the Rainfall Frequency Analysis)

  • 우리나라의 기후 지형적 특성에 따라 연강수량의 50% 이상이 여름철에 내린다. 이러한 짧은 기간에 집중적으로 내리는 강수량 조건하에 수공구조물을 설계할 경우 대부분 극치빈도분석을 활용한다. 특히 우리나라의 경우 Gumbel 분포를 활용한 극치빈도분석을 많이 이용한다. 하지만, 최근 이상기후로 인하여 전세계적으로 강수량의 특징이 급격히 변하고 있으며, 우리나라 연강수량 특징도 바뀌고 있다. 즉, 기존의 단일 분포형으로 재현이 가능했던 수문기상 자료들이 혼합분포형의 특징을 가지게 되었으며 이러한 변화를 고려할 수 있는 극치빈도분석 개발이 요구되고 있는 실정이다. 본 연구에서는 두 개 이상의 첨두를 가지는 형태의 극치강수량 자료에 대해서 기존의 단일 Gumbel 분포형 기반 극치빈도분석과 혼합 Gumbel 분포형 기반의 극치빈도분석 결과를 비교하였다. 확률분포의 매개변수 산정시 우도함수를 Bayesian 기법을 통해 산정하여 각 분포형의 Bayesian information criterion (BIC) 값을 비교하였다. 분석한 결과, 앞서 제안된 혼합 Gumbel 분포형은 하나의 첨두를 가지는 단일 Gumbel 분포형에서 반영되지 못한 꼬리(tail)부분의 이중첨두 부분의 거동을 효과적으로 모의하는 것을 확인할 수 있었다. 결과적으로 설계강수량을 추정할 때 보다 신뢰성있는 접근이 가능하였다. 이러한 점에서 우리나라 극치강우자료 분석시 기존 단일분포기반의 빈도해석기법에 대안으로 적용이 가능할 것으로 판단된다.

삼척지역 성인 및 노인 남녀의 연령별 식행동과 식사의 질 평가 (The Evaluation Study on Eating Behavior and Dietary Quality of Elderly People Residing in Samcheok According to Age Group)

  • The purpose of this study was to evaluate the eating behaviors and dietary quality of various aged adults. This study conducted through questionnaire and dietary survey by 24-hr recall to 850 adults residing in Samcheok. Subjects were divided into four groups according to the age in male and female, which included 20~29 yrs group (male n = 94, female n = 66), 30~49 yrs group (male n = 201, female n = 216), 50~64 yrs group (male n = 88, female n = 96), and $\geq65$ yrs group (n = 46, female n = 43). In male, 20~29 yrs group had a larger proportion of subjects who skipped breakfast, lunch and dinner compared to other groups (p < 0.001, p < 0.05, p < 0.01). The daily food and energy intakes in $\geq50$ yrs group were significantly lower than those of < 50 yrs. Protein and fat intakes of 20~29 yrs group were significantly higher than those of the other groups. But ND (Nutrient Density) of minerals as like calcium, iron and plant iron in 20~29 yrs group were significantly lower than those of other groups. The KDDS (Korean's Dietary Diversity Score) of 20~29 yrs group, 30~49 yrs group, 50~64 yrs group and $\geq65$ yrs group in male were 4.1, 3.8, 3.7 and 3.5 respectively and there was significant difference (p < 0.001). In female, skipping breakfast and dinner were significantly higher in 20~29 yrs group than in other groups (p < 0.001, p < 0.001). And the use of nutrient supplementation was higher in 30~49 yrs group than that of other groups (p < 0.05). The daily energy intakes was higher in 20~49 yrs group than that of $\geq65$ yrs group. The ND (Nutrient Density) of animal protein, vitamins A and $B_1$ in 20~29 yrs group were significantly lower than those of other groups. The KDDS of 20~29 yrs group, 30~49 yrs group, 50~64 yrs group and $\geq65$ yrs group in female were 4.0, 3.9, 3.9 and 3.6 respectively and $\geq65$ yrs group was significant lower than other groups. According to these results, $\geq65$ yrs and 20~29 yrs group had more dietary habit problems and poor nutrition status than 30~64 yrs group. The results of this study revealed that nutritional management and education for adequate meals should be emphasized in adults and elderly.
