The Impact of Childhood Cancer on The Korean Family (암 환아 발생이 가족에게 미치는 영향에 관한 연구)

  • This study identified the impact of childhood cancer on the Korean family. The purpose was to contribute knowledge for family nursing and pediatric hospice care practice with sick children and their families. This descriptive study was conducted during a 6 month period with children who were being treated for cancer at six university hospitals in Seoul. The data were gathered from members of 68 families ; 24(Group A), with a child newly diagnosed with cancer : 27(Group B), with a child under treatment and without complications, and 17 (Group C), with a child in relapse. Medical records, structured questionnaires and interviews were used for data collection. The questionnaires and interview schedules had been used previously in Martinson's research in the USA and China. The findings, conclusions, and suggestions are as follows. 1. The impact of childhood cancer on the family. Members of the family experienced fear, helplessness, guilty feelings, and anger at the time of the initial diagnosis and at relapse. Mothers complained of headache, anorexia and poor appetite, weight loss, sleep disturbance, and bad dreams. Many of the fathers either lost or changed jobs, and all working mothers stopped working. Half the parents reported changes in their marital relationships such as frequent quarrels but also stronger unity. Family members perceived cancer as the most frightening disease. Change in their world view was expressed as living on faith understanding suffering, determining to live a better life, wanting to live an upright life and valuing health as the most important. Religious activities are found most helpful through this difficult experience. Financial debt due to the treatment and care of the sick child, burdened 22 families. The above mentioned impact was most evidant in Group B(those presently undergoing treatment) and Group C(those in relapse). Findings indicate that nursing care should embrace the family of a child who is being treated for cancer. 2. Characteristics of the child with cancer The majority of the children in this sample had a diagnosis of leukemia. Their mean age was 6.8 and the ratio of boys to girls was 1.12 ; 1. The mean hospitalization frequency was 13.5 times and the mean duration of illness was 16.8 months. Most of 1.he children perceived cancer as the most frightening disease ; 32.7% of the children described their sickness as serious. Children in Group C were hospitalized more frequently, stayed in hospital for longer periods, and expressed their sickness as quite serious more often than the other two groups. These findings indicate how much comprehensive pediatric hospice nursing care services are needed along with relevant research and nursing education. 3. Characteristics of the families. The mean age of the father was 39.5 and the mother, 36,6 ; they are in their most productive life period. Mothers especially expressed feelings of financial uneasiness and powerlessness about giving up their jobs, and guilty feelings for not providing enough care and concern to other children due to taking care of the sick one. The burden of caring for the sick child can bring negative changes in family dynamics which they think provoke potential health problems in members of the family These findings suggest a need for nursing support and counselling resources. Findings also suggest the need for ethical inquiry about such questions as who should give information to the child in regard to diagnosis and prognosis, when, and how. Other suggestions included : 1) Quality health care for childhood cancer such as home care and pediatric hospice programs should be established. 2) Special and practical consideration for long-term patients should be made in the present insurance coverage. The reimbursement period for long-term patients should be lengthened. 3) Further in-depth qualitative studies are needed. 4) Education programs including guided practice experience for pediatric hospice care practitioners are needed.

Analysis of occlusal contact and guidance pattern during maximal intercuspal position and protrusive movement (최대교두감합위 및 하악 전방운동 시의 교합접촉 및 교합유도 양상에 관한 분석)

  • Purpose: The importance of occlusal contacts of the natural dentition for durability of teeth, mandibular stabilization, and restorative dentistry is well known. The purpose of this study is to analyze the occlusal contact and guidance pattern of Koreans by evaluating the static occlusion on maximal intercuspal position and measuring dynamic occlusion during straight protrusion. Materials and methods: The occlusal contacts at maximal interincisal position and the occlusal guidance pattern during straight protrusion of 29 subjects were recorded with shimstock foil (Whaledent, Langenau, Germany), T-Scan III (Tekscan Inc., Boston, MA, USA), polyvinylsiloxane registration material (Genie Bite, Sultan Healthcare, Hackensack, NJ, USA) and compared. Occlusal registration procedures were repeated 3 times. The position was fixed to an upright position and the head position was fixed with the Frankfurt horizontal plane paralleling the horizontal plane. Fisher's Exact Test (R-General Public License, ver. 2.14.1) and Pearson's Test were used to assess the significance level of the differences between the experimental groups (${\alpha}=.05$). Results: When using shimstock foil, T-Scan III system, and polyvinylsiloxane registration material, most of the patients showed contact on anterior, premolar, and molar teeth during maximal intercuspal position. Approximately 51% of maximal intercuspal position showed anterior contact using shimstock foil. When examining the protrusive movement using shimstock foil and T-Scan III system, guidance pattern with the central incisor was the most common. Conclusion: During maximal intercuspal position, there were cases in which not all of the teeth showed occlusal contact. During mandibular protrusive movements, one or more maxillary central incisors frequently joined in straight protrusion and the posterior teeth were disoccluded. Therefore, the anterior teeth protect the posterior teeth, and vice versa. Thus, mutually protected occlusion should be applied when reconstructing occlusion.

Effects of Dense Planting on the Growth and Producivity in Hot Pepper(Capsicum annum L.) (재식밀도(裁植密度)가 고추의 생육(生育) 및 생산성(生産性)에 미치는 영향(影響))

  • This experiment was carried out to investigate effects of dense planting on the growth and productivity in varieties of hot pepper at open field. Three varieties (Joseng jinheung, Weonkyo No. 304 and Bulam house Putgochu) were sowed on Jan. 30th and transplanted on May 12th. The planting densities used were 3,300, 5,500 and 8,300 plants per 10a. The results obtained are as follows: 1) With higher plant density, the plant shape was changed from open-type to upright-type and the yield was increased, presumably due to the increase of leaf area index. 2) LAI and SLA per unit area were increased by denser planting. Wonkyo No. 304 presented the highest LAI and the lowest SLA. 3) Dry matter weight per unit area was also increased, however the LW/SW ratio was tended to decrease on denser planting. 4) Through crop growth analysis, it was determined that CGR and NAR were increased during the early growth stages and became decreased from last June. 5) Light intensity under the canopy formed by denser planting was decreased by 40% measured at 60 ㎝ above the soil surface. The vertical distribution of fruits became concentrated the upper part of plant. Fruit yield per unit area in denser planting was increased as compared to the conventional planting. The number of fruit setting decreased. 6) Generally, no differences in yield were detected in comparison between denser planting and conventional planting harvested on Aug. 30th and Oct. 15th, respectively. But Weonkyo No. 304 increased yield by 18% in denser planting.

A Study on the Efficiency of Hand-Knee Position in GB Stone Ultrasonography (담낭결석 초음파검사에서 Hand-Knee position의 효율성에 관한 연구)

  • The evaluation of GB stones with ultrasound has proved to be useful procedure in patient with symptoms of cholelithiasis. GB is evaluated for size, wall thickness, presence of internal reflections within the lumen and posterior acoustic shadowing or enhancement in Ultrsonography. The patient position should be shifted during procedure to demonstrate further the presence of stone within the GB. Patient scanned at the Rt. subcostal region in supine, right lateral, Lt. down decubitus, and upright sitting position. So GB stone should shift to dependent area of GB. Often, GB is not markedly distended in the presence of cholethiasis, and so the diagnosis becomes more difficult. One of the more difficult areas for detection of a GB stones are embeded in the cystic duct region. And since the GB is adjacent to the duodenum and hepatic flexure, its may be difficult to visualizing a GB stone. When patient study position changes frome supine to other position, stones displaced the site. But if its are polyps, not changes the site whatever patient positions. It is very important to what make different GB stones or polyps. We have studied about mobility of GB stones according to the patients position(supine, Lt. down decubitus, $30^{\circ} LAO. sitting and hand-knee). So we have a result, stones wherever localized within the GB, changed 100% its position in the hand-knee position and the others appeared at least 90%. In this study, when a large stones are located through fundus-body and body-neck, does not changing the stones position in spite of varied patient's positions. But hand-knee positions can identified GB stones, because its make changed the position of stons from posterior wall to anterior wall within the GB. We recommend the hand-knee position for differentiation GB stones from polyps.

Anatomical Characteristics of Major Plantation Species Growing in Indonesia II (인도네시아산 주요 조림수종의 해부학적 특성 II)

  • The anatomical characteristics of eight major wood species planted in Indonesia were investigated to provide valuable information for their effective utilization. The growth-ring boundaries of Damar and Sumatran pine were indistinct. Resin canal was found in Sumatran pine but it was not observed in Damar. Cupressoid pit and taxodioid pit were found in Damar and window-like pit and pinoid pit were observed in Sumatran pine. Tracheid length of Damar and Sumatran pine was shorter than $3,000{\mu}m$. There were uniseriate rays in Damar and Sumatran pine and fusiform ray in Sumatran pine. All the hardwood species observed in this study were diffuse-porous. They had different vessel groups, i.e., solitary pore in Afrika and Simpur Batu, pore cluster in Angsana and mostly 2-4 rows of radial pore multiple in Mahoni. Mindi and Trembesi had mostly 2-3 rows of radial pore multiple with paratracheal parenchyma as aliform and confluent types. Afrika, Mahoni and Simpur Batu showed heterocellular rays which composed of procumbent cells in the body and mostly 1-2 rows of upright and/or square cells in the margin. All ray cells procumbent was observed in Angsana, Mindi and Trembesi. The large rays commonly exceeding 1 mm in height and ray width of 3~6 cells were observed in Simpur Batu. The other five hardwood species showed ray width of 1~3 cells. Vessel number per $mm^2$ of Angsana and Simpur Batu was higher than those of the other hardwood species. The length of wood fiber and tracheid showed a tendency to increase from pith to bark. By IAWA list, fiber length of hardwoods was classified into long in Simpur Batu and short in Angsana and Trembesi.

Yield Response of Rice Affected by Adverse Weather Conditions Occurred in 1999 (1999년에 발생한 기상재해 유형별 벼 수량반응조사 연구)

  • The objectives of this study were to investigate weather conditions which induced discolored grains and viviparous germination, and to evaluate yield responses following viviparous germination during mid- and late- ripening stage, the submergence during reproductive growth stage, and lodging in the yellow ripe stage. Weather conditions which caused glume discoloration at heading stage were 21.3-26.4$^{\circ}C$ in average temperature, 75.2-98.4% in relative humidity, 19.3 in transpiration coefficient and 10.8-13.8 m/sec. in wind speed. Yield reduction was 26-27% and 10~17%, respectively, when the glume discoloration rates were 63.2-65.7% and 38.3-45.2%, obviously due to the decrease in percent of fertile grain and ripening ratio. Weather conditions during continuous rain for 7 days were 96% in relative humidity, 18.9$^{\circ}C$ in average temperature, 21.9$^{\circ}C$ in maximum temperature, and 16.8$^{\circ}C$ in minimum temperature, causing the most viviparous germination in Juanbyeo(45.5%), followed by Jinbubyeo(14.5%), Bongkwangbyeo(14.2%), and Obongbyeo(12.6%). Lateral tillers started to occur when the submergence at the depth of 1.5-2 m lasted one day during the reproductive growth stage. The submergence for 2-3 days at 3-4 m of water depth induced 269-571 lateral tillers/m$^2$, supporting 32-52% of the total yield. The rice yield in the paddy fields which were left under the lodging conditions until harvesting was not different compared to that of the paddy fields which were kept upright by tieing them together after lodging, but perfect grain ratio decreased about 9.1% in the transplanting culture and 12.5% in the direct seeding culture on dry paddy field because of the increase in immature grains.

Deformation History of Precambrian Metamorphic Rocks in the Yeongyang-Uljin Area, Korea (영양-울진 지역 선캠브리아기 변성암류의 변형작용사)

  • Precambrian metamorphic rocks of Yeongyang-Uljin area, which is located in the eastern part of Sobaegsan Massif, Korea, are composed of Pyeonghae, Giseong, Wonnam Formations and Hada leuco granite gneisses. These show a zonal distribution of WNW-ESE trend, and are intruded by Mesozoic igneous rocks and are unconformably overlain by Mesozoic sedimentary rocks. This study clarifies the deformation history of Precambrian metamorphic rocks after the formation of gneissosity or schistosity on the basis of the geometric and kinematic features and the forming sequence of multi-deformed rock structures, and suggests that the geological structures of this area experienced at least four phases of deformation i.e. ductile shear deformation, one deformation before that, at least two deformations after that. (1) The first phase of deformation formed regional foliations and WNW-trending isoclinal folds with subhorizontal axes and steep axial planes dipping to the north. (2) The second phase of deformation occurred by dextral ductile shear deformation of top-to-the east movement, forming stretching lineations of E-W trend, S-C mylonitic structure foliations, and Z-shaped asymmetric folds. (3) The third phase deformation formed I-W trending open- or kink-type recumbent folds with subhorizontal axes and gently dipping axial planes. (4) The fourth phase deformation took place under compression of NNW-SSE direction, forming ENE-WSW trending symmetric open upright folds and asymmetric conjugate kink folds with subhorizontal axes, and conjugate faults thrusting to the both NNW and SSE with drag folds related to it. These four phases of deformation are closely connected with the orientation of regional foliation in the Yeongyang-Uljin area. 1st deformation produced regional foliation striking WNW and steeply dipping to the north, 2nd deformation locally change the strike of regional foliation into N-S direction, and 3rd and 4th deformations locally change dip-angle and dip-direction of regional foliation.

  • The purpose of this study was to evaluate the stress distributions at the periodontal ligament (PDL) and displacements of the maxillary first molar when mesially directed force was applied under various molar angulations and rotations. A three dimensional finite element model of the maxiilary first molar and its periodontal ligament was made Upright position, mesially angulated position by $20^{\circ}$ and distally angulated position of the same degree were simulated to investigate the effect of molar angulation. An anteriorly directed force of 200g countertipping moment of 1,800gm-mm (9:1 moment/force ratio) and counterrotation moment of 1,000gm-mm (5:1 moment/force ratio) were applied in each situation. To evaluate the effect of molar rotation on the stress distribution, mesial-in rotation by $20^{\circ}$ and the same amount of distal-in rotation were simulated. The same force and moments were applied in each situation. The results were as follows: In all situations, there was no significant difference in mesially directed tooth displacement Also, any differences in stress distributions could not be found, in other words. there were no different mesial movements. Stress distributions and tooth displacement of the $20^{\circ}$ mesially angulated situation were very similar with those of the $20^{\circ}$ distal-in rotated situation. The same phenomenon was obserned between the $20^{\circ}$ distally angulated situation and $20^{\circ}$ mesial-in rotated situation. When the tooth was mesially angulated, or distal-in rotated, mesially directed force made the tooth rotate in the coronal plane. with its roots moving buccally, and its crown moving lingually. When the tooth was distally angulated, or mesial-in rotated, mesially directed force made the tooth rotate in the coronal plane, with its roots moving lingually and its crown moving buccally. When force is applied to au angulated or rotated molar, the orthodontist should understand that additional torque control is needed to prevent unwanted tooth rotation in the coronal plane.

The effect of Postural Changes on Pleural Fluid Constituents (흉수 구성 성분의 체위에 따른 차이)

  • Background : Measurement of pleural fluid constituents are of value in the diagnosis of pleural effusions and in the seperation of exudates from transudates. The position of the patient(sitting or lying) prior to thoracentesis may result in difference in the measurement of these constituents. The purpose of this study is to determine whether postural differences in pleural fluid constituents exist, and if so, whether they are of any clinical significance. Method : 41 patients with pleural effusions on chest roentgenography were prospectively studied. The fluid cell counts, partial gas tension, and concentrations of chemical constituents were compared in the supine and upright positions. Results : 1) A total of 10 patients were found to have an transudative effusion. In the transudates there was no significant difference in pleural fluid constituents according to posture change. 2) A total of 31 patients were found to have an exudative effusion. Statistically significant postural changes were noted in pH, WBC counts, protein, and LDH concentrations in the exudates. It may be due to postural sedimentary effect in the pleural space. 3) The PCO2 measurements and glucose concentration were not affected by changes in position in exudates or transudates. Conclusion : Postural sedimentary effect occurs in the pleural space with reference to the measurement of certain pleural fluid constituents when an inflammatory process is present. Therefore it is recommended that thoracentesis after 30 minutes in the sitting position should be performed.

ORTHODONTIC TRACTION OF HORIZONTALLY ERUPTED LOWER LATERAL INCISOR ON THE LINGUAL SIDE (설측으로 수평 맹출한 하악 측절치의 교정적 견인)

  • Tooth eruption is the movement of the tooth from the developing place in the alveolar bone to the functional position in the oral cavity. The permanent incisors originate from the dental lamina on the lingual side of preceding deciduous tooth and erupt to the level of the occlusion through the well developed gubernacular cord. Ectopic eruption is a developmental disturbance in the eruption pattern of the permanent dentition. Most of the ectopically erupted lower incisor has been found in lingual side. The ectopically erupted tooth could be repositioned by orthodontic force in the early mixed dentition, which could help preventing the problems of loss of space and the lingual tilting of the lower anterior teeth. An eight-year-old girl visited the department of pediatric dentistry, Yonsei Dental University Hospital, for the evaluation and the treatment of the lower right lateral incisor, which was horizontally erupted in the lingual side, parallel to the mouth floor. Her tongue was placed on the labial side of that tooth. There was no previous dental history of dental caries or trauma on the pre-occupied primary incisor. Clinical and radiographic examinations including the computed tomography(CT), showed no evidence of dilacerations on root. Therefore, we decided to start active orthodontic traction of the lower right lateral incisor. We designed the fixed type of buccal arch wire and the lip bumper with hook for the traction. Button was attached to the lingual side of the ectopically positioned tooth. Elastic was used between the appliance and the button on that tooth. After the tooth become upright over the tongue level, appliance was change to the removable type and periodic check-up with occlusal guidance was followed to monitor the position of the tooth. In this case using the fixed appliance with modified form of lip bumper and hook embedded in acrylic part instead of extraction was very efficient up-righting the ectopically erupted tooth toward the occlusal plane.
