The Electrophysiological Studies of the Trapezius Muscle in Patients with Amyotrophic Lateral Sclerosis (근위축성 측상경화증 환자에서 등세모근의 신경생리학적 검사)

  • Background: Needle electromyography (EMG) and motor evoked potential (MEP) of the genioglossus (tongue) are difficult to perform in evaluations of the craniobulbar region in amyotrophic lateral sclerosis (ALS). Therefore, we investigated the yields of needle EMG and MEP recorded from the upper trapezius, since it receives innervation from the lower medulla and upper cervical cord. Methods: Needle EMG and MEP of the upper trapezius were obtained in 17 consecutive ALS patients. The needle EMG parameters recorded included abnormal spontaneous activity and motor unit action potential (MUAP) morphology. An upper motor neuron (UMN) lesion was presumed when either response to cortical stimulation was absent, or the central conduction time was delayed (>mean+2SD). Results: Of the five patients with bulbar-onset ALS, four had abnormalities in the upper trapezius and four in the tongue by needle EMG. In contrast, of the 12 patients with limb-onset ALS, 11 had abnormalities in the upper trapezius, and only five in the tongue. When MEP was performed, it was found that three of the five patients with bulbar symptoms and three of the six patients with isolated limb involvement had abnormal MEP findings. Conclusions: Electrophysiological studies of the upper trapezius are more sensitive those of the tongue in patients without bulbar symptoms. Thus, needle EMG and MEP of the upper trapezius are alternative tools for assessing bulbar and rostral neuraxial involvement in the diagnosis of ALS.

Effects of Leaf and Pod Removal on Dry Matter Accumulation of Soybean Plants (적엽 및 제협처리가 콩의 건물축적에 미치는 영향)

  • Effects of leaf and pod removal on changes in leaf and seed number, and leaf, seed, pod and stem dry weight of soybean [Glycine max (L,) Merr.] cultivar ‘Hwangkeumkong’ were measured at the research farm of Korea University in 1992. The upper 40% and lower 60% of leaves and pods were subjected to treatments at the growth stage of beginning pod(R3). Leaf number and dry weight of lower part were increased by upper leaf-lower pod removal, but seed number and dry weight were decreased. Upper leaf-lower pod removal increased lower stem dry weight and decreased upper pod dry weight. Leaf-seed ratio of the upper leaf-lower pod removal was the highest with 3.54 and harvest index was the lowest with 27% among the treatments. Lower leaf-upper pod removal showed that more assimilates from upper leaves were translocated to lower seeds than from lower leaves to upper seeds in upper leaf-lower pod removal treatment.

Surgical Outcomes Associated with Operable Gastric Cancer in a Tertiary Care Indian Hospital

  • Purpose: Data on operable gastric cancer from India is sparse. The purpose of this study was to investigate the clinical details, histopathological demographics, and 5-year overall survival (OS) and disease free survival (DFS) associated with operable, non-metastatic gastric cancer in a dedicated upper gastrointestinal (GI) surgical unit in India. Materials and Methods: Data for patients diagnosed with operable gastric cancer between January 2006 and December 2014 were retrospectively analyzed. Data were collected from electronic hospital records in addition to mail and telephonic interviews when possible. Results: A total of 427 patients were included. The tumor was located in the pyloro-antral region in 263 patients (61.7%). Subtotal gastrectomy was performed in 291 patients and total gastrectomy in 136 patients. Tumor stage classification revealed 43 patients (10.0%) with stage I, 40 patients (9.4%) with stage IIA, 59 patients (13.9%) with stage IIB, 76 patients (17.8%) with stage IIIA, 96 patients (22.5%) with stage IIIB, and 113 patients (26.4%) with stage IIIC disease. Follow-up data were available for 71.6% of the patients with a mean duration of 32.4 months. Five-year DFS and OS were 39% and 59%, respectively. Conclusions: Despite presenting at an advanced stage, the 5-year DFS and OS of patients with operable gastric cancer treated at a dedicated upper GI unit of a tertiary care center in India was good.

The Effects of Virtual Reality Therapy With Compensation Inhibition and Feedback on Upper Extremity Function in Hemiplegic Patients With Chronic Stroke (보상작용 억제와 피드백을 제공한 가상현실 치료가 만성 뇌졸중 편마비 환자의 상지기능에 미치는 영향)

  • The purpose of this study was to investigate the effects of virtual reality (VR) therapy with compensation inhibition and feedback (CIF) on upper extremity function in chronic stroke patients. Seven chronic stroke patients participated in this study, which was a randomized controlled trial with a crossover design. Self upper extremity exercise, conservative VR therapy, and VR therapy with CIF were performed for one hour per session, 5 times per week, over a 3 week period. The main outcome measures involved range of motion (ROM) including shoulder, elbow, and wrist joints, a Manual Function Test (MFT), and a Motor Activity Log (MAL). Data were calculated as posttest and pretest changes in every session and were analyzed using Friedman and Wilcoxon signed-rank tests at p<.05. The results were as follows: 1) Statistically significant increase in ROM measurements of shoulder and elbow joints were seen with VR therapy with CIF compared to VR therapy and self upper extremity exercise (p<.05), whereas no significant increasing was noted for the wrist joint (p>.05). 2) Statistically significant increase in the MFT was seen with VR therapy with CIF compared with VR therapy and self upper extremity exercise (p<.05). 3) VR therapy with CIF also resulted in statistically significant increase in both activity of use (AOU) (p<.05) and quality of movement (QOM) (p<.05) on the MAL test when compared with VR therapy and self upper extremity exercise, respectively. In conclusion, VR therapy with CIF was more effective than conservative VR therapy and self upper extremity exercise in improving the upper extremity function in hemiplegic patients with chronic stroke.

A Study on the Academic Discussion on the Presence of Upper and Lower Gates of Urinary Bladder (방광(膀胱)의 상하구(上下口) 유무(有無) 논쟁에 대한 고찰(考察))

  • Objectives : The paper's objective is to study into the books dealing in the debate surrounding the existence, or nonexistence, of urinary bladder's upper gate and lower gate, a debate that ensured since the publication of Huangdineijing. Methods : Sikuquanshu and Zhongyishijia databases were searched to collect related materials, and these materials were reviewed to get an understanding of the historical development of the debate. Results : The upper gates of urinary bladder was first mentioned in Nanjing. Since then, Wanglu's Yijingsuhuiji asserted that the upper gate exists while the lower didn't, but many argued after the dawning of Ming Dynasty that the upper doesn't exist while the lower does. Additionally, some urged in relation to the assertion of the nonexistence of upper gate that water liquid pervades into the urinary bladder through sebaceous membrane or oil net. Conclusions : Behind the debate between the existence or nonexistence of the upper and lower gates in urinary bladder is the theory of qi transformation. Even the anatomical knowledge of urinary bladder was submitted as an evidence. In general, the debate developed depending on how the differences between Huangdineijing's osmotic opinion and Nanjing's existence of the upper gate were perceived.

Somatotype of Women's Upper Body in their thirties through a Development Figure of the Surface of the Body (체표면 전개도에 의한 30대 여자 상반신의 유형분석)

  • The purpose of this study is to classify the upper body of women into several kinds of somatotypes, using the method of Surgical Tape and making their shells. The subjects are 53 females 30 to 39 years-old. Fifty-three anthropometric data are measured per shell of bodysurface; six somatotype factors are obtained through principal component analysis and orthogonal rotation by the method of Varimax, Somatotype of women's upper body is achieved by cluster analysis, using the standardized factor score as an independent variable and the FASTCLUS of SAS by Kmeans. The results are as follows: 1. The number of the factors which explain the somatotype is six and those factors comprise 76.12 percent of total variance. Factor 1: related to the size of shape in the front of upper body Factor S: related to the size of shape in the back of upper body Factor 3: related to the type of the upper chest over the chest circumference line Factor 4: related to the length of·the upper body Factor 5: related to the part of the neck Factor 6: related to the type of the lower chest under the chest circumference line 2. Cluster analysis results in classification of upper body into five clusters. Cluster L: the length is the largest and the circumference is small. The part of waist is the largest and widest among surface areas. Cluster 2: Slender body line from chest to waist is characteristic. The length is longer. The part of upper and lower chest is larger among surface areas. Cluster S: the circumference is the smallest and armhole is small. The length and surface area are small. Cluster 4: the circumference and armhole is the largest. The length is the smallest. Cluster 5: the circumference is average and the length is a little long. The body line(silhouette) from chest to waist is curved slightly.

The Type Analysis of Middle Aged Woman's Upper Body (중년여성 상반신의 유형분석)

  • This study conducted an analysis of the middle aged women whose ages are from 35 to 59 years old in order to give those ladies much external satisfaction and covered body lines. I have come to present a few characters of body types as silhouettes which resulted in the factors of the upper bodies of middle aged women. I have adopted SAS(statistical analysis system) as a data process method of this survey. The followings are the results analysis. 1. The item of height have decreased, but increased in the items of thickness among the late middle aged women. 2. The body componant factor of middle aged women are extracted as 10 items, and the rate of their cumulative contribution is 78.9%. The first is the thickness of upper body, the second is the thickness of side upper body, the third is the height of upper body, the fourth is the length of upper body the fifth is width of upper body, the sixth is the projected rate of back, the seventh is the projected rate of breast, the eighth is the squared rate of shoulder, the ninth is the crooked rate of neck, and the tenth is the upper from of breast. 3. The body types of middle aged women can be classified into 4 types, and the followings are their characters. Type 1 is a standard type as the most common body character of the middle aged women, showing the appearing rate of 42.4%. Type 2 is a semi-curved body type like the curved figure of side body, showing the appearing rate of 34.5%. Type 3 is a reverse body type with the projected breast feature of front body, showing the appearing rate of 20.9%. Type 4 is a crooked body type with a tall height and crooked neck, showing the appearing rate of 2.2%.

Relationship between occupational stress and musculoskeletal symptoms of upper extremities among dental hygienists (치과위생사의 직무스트레스와 상지 근골격계 증상의 관련성)

  • Objectives : This study was purposed to estimate prevalence of upper extremity musculoskeletal symptoms, level of occupational stress and their relationship among dental hygienists. Methods : This questionnaire study was performed during May to October 2010 with 317 dental hygienists worked in Busan and Gyeongsangnam-do recruited as study population. The occupational stress and upper extremity musculoskeletal symptoms were accessed using questionnaire of the Korean Occupational Stress Scale and musculoskeletal symptoms from the Korea Occupational Safety Health Agency. Data analysis was performed with the descriptive analysis, chi-square test, and multiple logistic regression analysis using SPSS(ver 19.0K)program. Results : The prevalence of upper extremity musculoskeletal symptoms and the level of occupational stress were 79.2% and $52.67{\pm}4.86$ in dental hygienists. By multiple logistic regression analysis, job insecurity(OR=2.86, 95% CI: 1.38-5.94 in stress high risk group; reference-stress low risk group) and organizational system(OR=3.32, 95% CI: 1.55-7.08 in stress high risk group; reference-stress low risk group) were associated with upper extremity musculoskeletal symptoms in dental hygienists. Conclusions : The overall prevalence of upper extremity musculoskeletal symptoms and level of occupational stress in dental hygienists were relatively high and some sub-scales of occupational stress were related with upper extremity musculoskeletal symptoms. Not only previously reported risk factors of musculoskeletal symptoms but also occupational stress should consider to prevent and improve upper extremity musculoskeletal symptoms in dental hygienists.

Systematic Review on Effect of Mirror Therapy on Upper Extremity Function for Stroke Patients (뇌졸중 환자의 상지기능 향상을 위한 거울치료 효과에 관한 체계적 고찰)

  • Recent years, mirror therapy or mirror neuron therapy has been used to improve neural damage of upper extremities among stroke patients. This systematic review was conducted to assess the clinical effect of mirror therapy used for the treatment of upper extremities for patients with stroke. After literature search, researchers selected for 9 randomized controlled clinical trial studies registered up to October 2013 based on PubMed database, using the following search terms: mirror therapy, mirror neuron, stroke, paresis, hemiplegia, upper extremity. There were significant improves of distal upper extremity function in the use of mirror therapy groups but the research using range of motion(ROM) as outcome measure. In conclusion, mirror therapy was more effective than conventional therapies or sham therapies for upper recovery of distal upper extremity function among stroke patients.

A Comparative Study on the Upper Garment in the Ancient East and West (고대(古代) 동서양(東西洋) 상의(上衣) 비교연구(比較硏究))

  • The purpose of this thesis is to find out how the upper garment styles in the Ancient East and est had been influenced with each other. Analytical studies conclude the fellowing findings: 1) Upper garment styles in the feat Asia and the Egypt already highly developed in 28th century B.C. and show us the original style of the wrap-over to the left and that of the round neckline(曲領). Upper garment of the open in the center front shown in Babylonia in 18th century B.C. had been inherited to the caftan of the Hebrew and later succeeded to the Persia. 2) The tunic styles of the round neckline, the wrap-over to the left and the open in the cotter front, which were the basic styles of the upper garment, had teen widely accepted to the central Asia and the East Asia, as well as the Northern Europe, from the West Asia. 3) The styles of the wrap-over to the right originated from China since it had begun to show in the Shang Dynasty(商代, 殷代). 4) The East and the West costumes had been very much intermixed in 4th century B.C. Alexander the Great of Macedoria in 4th century B.C. expanded his territory to the central Asia and built up the Bacteria, when the most western civilization had been greatly transmitted to the Orient. Meanwhile the tunic being clad in the West and Central Asia began to be worn by soldiers in the period of the Warring States in China (326-299 B.C.) and afterwards worn even by civil officials since the age of the T'ang Dynasty of China. 5) The Upper garments of the open in the center front, the wrap-over to the right, the wrap-over to the left and the round neckline were found in Korea, which mean that the upper garment styles in the Ancient Korea were intermixed of the factors from the West Asia, the central Asia and the East Asia. 6) The styles of costume in the East Asia were influenced by the West Asia through the central Asia. The upper garment styles Europe were also influenced by the West Asia. Thus the upper garment styles in the Ancient East and West had been mutually affected with each other.
