Quality of Life of Adolescents with Inflammatory Bowel Disease (염증성 장 질환을 가진 청소년의 삶의 질)

  • Purpose: This study was conducted to identify the quality of life (QOL) of adolescents who have inflammatory bowel disease. Methods: This was a descriptive comparative study, and participants were 58 adolescents with inflammatory bowel disease and 55 healthy adolescents. The Korean version of $PedsQL^{TM}$ 4.0 scale was used. Ten adolescents with inflammatory bowel disease were also interviewed. Data were analyzed with SPSS Win. Results: Average score on QOL for these adolescents was statistically lower (p<.001) than healthy adolescents, especially for physical and school functioning. QOL of the adolescents who had the disease for more than 4 years, and were taking 3 or more medications was lower QOL than other adolescents with this disease. In interviews, adolescents complained of physical malfunctioning. In emotional functioning, they worried about the reoccurrence of bloody diarrhea, and mood changes. As for social functioning, dietary restrictions, gastrointestinal symptoms, and poor peer relationships were primary concerns. Lastly, in school functioning, frequent absences and lower academic performance were problems. Conclusion: Considering these results, adolescents with inflammatory bowel disease felt anxious because of the uncertain origin of their disease and unpredictable course of the illness. Interventions should include diaries on food, along with education and counseling as nursing interventions.

Relationship between Anomalous Pancreaticobiliary Ductal Union and Pathologic Inflammation of Bile Duct in Choledochal Cyst

  • Purpose: Choledochal cyst is a cystic dilatation of common bile duct. Although the etiology is presently uncertain, anomalous pancreaticobiliary ductal union (APBDU) is thought to be a major etiology of choledochal cyst. In this study, we analyzed the clinical and anatomical characteristics and pathologies of patients diagnosed with choledochal cyst in a single institute for 25 years. Methods: A total of 113 patients, diagnosed with choledochal cyst and who received an operation in Severance Children's Hospital from January 1988 to May 2013, were included. Medical records were reviewed, including clinical and demographic data, surgical procedures. Abdominal ultrasonography, magnetic resonance cholangiopancreatography, and intraoperative cholangiography were used as diagnostic tools for evaluation and classification of choledochal cyst and the presence of anomalous pancreaticobiliary ductal union. Todani's classification, and relationship between APBDU and surgical pathology. Results: Among 113 patients, 77 patients (68.1%) presented symptoms such as hepatitis, pancreatitis and/or cholecystitis. Eighty three patients (73.5%) had APBDU, and 94 patients (83.2%) showed inflammatory pathologic changes. APBDU, pathologic inflammation, and serological abnormalities such as hepatitis or pancreatitis showed a statistically significant correlation to one another. Conclusion: APBDU is thought to be one of the etiologic factors of choledochal cyst. It is related to the inflammatory changes in bile duct that can lead to the cystic dilatation.

A Study on the Genetic Inheritance of Ankyloglossia Based on Pedigree Analysis

  • Background Ankyloglossia or tongue-tie is a congenital anomaly characterized by an abnormally short lingual frenum. Its prevalence in the newborn population is approximately 4%. Its mode of inheritance has been studied in some articles, but no conclusion has been established. Also, no relevant report has been published in Korea. This study was conducted to elucidate the genetic inheritance of ankyloglossia via pedigree analysis. Methods In this study, 149 patients with no other congenital anomaly who underwent frenuloplasty between March 2001 and March 2010 were studied. Pedigrees were made via pre- or post-operative history taking, and patients with uncertain histories were excluded. In the patient group that showed a hereditary nature, the male-to-female ratio, inheritance rate, and pattern of inheritance were investigated. Results One hundred (67.11%) of the patients were male and 49 (32.89%) were female (male-female ratio=2.04:1). Ninety-one (61.07%) patients reported no other relative with ankyloglossia, and 58 (38.93%) patients had a relative with this disease. The inheritance rate was 20.69% in the 58 cases with a hereditary nature. In the group with no family history of ankyloglossia, the male-female ratio was 3.79:1, which significantly differed from that of the group with a family history of ankyloglossia. X-chromosome mediated inheritance and variation in the gene expression was revealed in the pedigree drawn for the groups with hereditary ankyloglossia. Conclusions Ankyloglossia has a significant hereditary nature. Our data suggest X-linked inheritance. This study with 149 patients, the first in Korea, showed X-linked inheritance in patients with a sole anomaly.

2012 Newborn Infants' Stage Protective Injection Analysis (2012년 출생아의 시기별 예방접종력 분석)

  • This study is the analysis about materials (from 2012 to 2014) which was stored in the data made from the 397 infants subjects who were born in 2012, with the basis of the preventive injection management, public health care information system by using SPSS win 18.0 program. After we analyzed the present non-injection state and the non-injection reasons with the basis of each birth month, each vaccine, each injection period, we found out that 89 infants(22.4%) were the ones who were not computerized. Except the 20 infants-the ones having uncertain phone numbers and addresses, and computerization errors, 69 infants (17.4%) were the ones that didn't get injection. We guided protective injection to them by sending SMS text message and we could inject 39 infants (9.8%) of them. We could achieve the high 92.4% injection rate. we want this research to be used to try wipe out the infectious diseases in this district and be utilized as the basic materials of protective injection cost support and health care policies, etc.

  • The construction environment has the trend of oversizing and professionalizing, the increase of the construction period, and the risk factors and the uncertain factors which are important in the construction bid. The misunderstanding and the lack of knowledge of the subcontractor result in the decrease of the profit or the deficit. In conclusion, these are supposed to give the financial burden to the subcontractor. So, it is very important to predict the construction preparation cost calculation by the risk factors in the bidding stage and to calculate the bid unit cost considering the target profit of the subcontractor in the process of the mutual agreement and performance of the contract. In these points of view, this study analyzes how the importance of the risks influencing the bidding cost calculation affect the construction cost, and this analysis can be used as the basic data to establish the suitable bid strategy of the subcontractor.

  • The number of car accident is Recently on the increase in Korea because of the explosive increase of cars, the poor road condition, the lack of safety facility, and others. The insurant with a accident has to decide whether receiving a insurance or not. In this paper, we represent a reasonable decision support material by calculating the approximate insurance fee based on the discount rate and premium additive rate, which is changed by the accident type and the accident expenditure. Practically, there is difference in the standard insurance rate and premium additive rate according to the accident type and the accident expenditure in Korea. The premium additive rate is assessed considering the number of accident, the pattern of accident, and the reason of accident for 3 years. In this paper, we represent a decision making method considering not only the first-time car accident but also the future car accident. For considering the repeated accident, we analyzed the real data accumulated until the year of 1996 from S Insurance Company, and estimated the probability density function between the first and the second-time accident, and executed the goodness of fit test using ARENA and STATISTICA software. Using this conditional PDF, we can calculate the insurance fee next 3 years and compare the insurance fee with the equivalent present value of cash flows. The program performing this analysis is represented, and written in VISUAL BASIC Language. We tried to suggest an accurate guideline for the insurant to decide the insurance coverage rationally, and tried to correct a wrong idea of dependence on the car insurance only by the amount of the accident expenditure. And we expect this study can generally be applied to many different accident types under the uncertain circumstances in our daily life.

Helicobacter pylori and Gastric Carcinogenesis (Helicobacter pylori와 위암발생)

  • Ever since the World Health Organization classified Helicobacter pylori as a class I carcinogen, a variety of discussions over the actual role of H. pylori infection in gastric carcinogenesis has existed. Although a majority of researches support the positive correlation between H. pylori infection and the development of gastric cancer, many aspects of this association are yet uncertain, and some data even suggest that there may be no correlation between H. pylori infection and gastric carcinogenesis. However, there are proofs indicating these reports underestimated the prevalence of H. pylori infection and therefore, the association of the infection and gastric adenocarcinoma. In this report, I reviewed the epidemiology of H. pylori and gastric cancer, evidence supporting and against the positive correlation of the infection and the disease, and the possible pathological role H. pylori infection may have in gastric carcinogenesis referring particular to published literature. As a conclusion, despite a few reports of a possible negative or no relationship between gastric cancer and H. pylori infection, I was able to find that H. pylori infection did have a pathological role in the development of gastric cancer.

Preventing Extracellular Diffusion of Trigeminal Nitric Oxide Enhances Formalin-induced Orofacial Pain

  • Nitric oxide (NO), a diffusible gas, is produced in the central nervous system, including the spinal cord dorsal horn and the trigeminal nucleus, the first central areas processing nociceptive information from periphery. In the spinal cord, it has been demonstrated that NO acts as pronociceptive or antinociceptive mediators, apparently in a concentration-dependent manner. However, the central role of NO in the trigeminal nucleus remains uncertain in support of processing the orofacial nociception. Thus, we here investigated the central role of NO in formalin (3%)-induced orofacial pain in rats by administering membrane-permeable or -impermeable inhibitors, relating to the NO signaling pathways, into intracisternal space. The intracisternal pretreatments with the NO synthase inhibitor L-NAME, the NO-sensitive guanylate cyclase inhibitor ODQ, and the protein kinase C inhibitor GF109203X, all of which are permeable to the cell membrane, significantly reduced the formalin-induced pain, whereas the membrane-impermeable NO scavenger PTIO significantly enhanced it, compared to vehicle controls. These data suggest that an overall effect of NO production in the trigeminal nucleus is pronociceptive, but NO extracellularly diffused out of its producing neurons would have an antinociceptive action.

Optimal Design of Process-Inventory Network under Cycle Time and Batch Quantity Uncertainties (이중 불확실성하의 공정-저장조 망구조 최적설계)

  • The aim of this study is to find an analytic solution to the problem of determining the optimal capacity of a batch-storage network to meet demand for finished products in a system undergoing joint random variations of operating time and batch material loss. The superstructure of the plant considered here consists of a network of serially and/or parallel interlinked batch processes and storage units. The production processes transform a set of feedstock materials into another set of products with constant conversion factors. The final product demand flow is susceptible to joint random variations in the cycle time and batch size. The production processes have also joint random variations in cycle time and product quantity. The spoiled materials are treated through regeneration or waste disposal processes. The objective function of the optimization is minimizing the total cost, which is composed of setup and inventory holding costs as well as the capital costs of constructing processes and storage units. A novel production and inventory analysis the PSW (Periodic Square Wave) model, provides a judicious graphical method to find the upper and lower bounds of random flows. The advantage of this model is that it provides a set of simple analytic solutions while also maintaining a realistic description of the random material flows between processes and storage units; as a consequence of these analytic solutions, the computation burden is significantly reduced. The proposed method has the potential to rapidly provide very useful data on which to base investment decisions during the early plant design stage. It should be of particular use when these decisions must be made in a highly uncertain business environment.

Use of Neural Networks on Concrete Mix Design (콘크리트의 배합설계에 있어서 신경망의 이용)

  • In concrete mix design we need the informations of the codes, the specifications, and the experiences of experts. However we can't consider all factors regarding concrete mix design. The final acceptance depends on concrete quality control test results. In this process we meet the uncertainties of materials. temperature, site environmental situations, personal skillfulness. and errors in calculations and testing process. Then the mix design adjustments must be made. Concrete mix design and adjustments arc somewhat complicated, time-consuming. and uncertain tasks. In this paper, as a tool to minimize the uncertainties and errors the neural network is applied to the concrete mix design. Input data to train and test the neural network are obtained numerically from the results of design following the concrete standard specifications of Korea. The 28-days compressive strengths which are variate according to the uncertainties and errors are considered. The results show that neural networks have a strong potential as a tool for concrete mix design.
