Factors Influencing the Development of Pressure Ulcers in Surgical Patients (수술환자의 욕창발생 예측요인에 관한 연구)

  • Purpose: The purpose of this study was to identify the influencing factors on the development of pressure ulcers in patients undergoing surgery which lasted more than two hours. Method: One hundred nineteen surgical adult patients were included in the study. Data was measured on each participant from December 2003 to February 2004. It was collected using a structured researcher-administered sheet and analyzed by descriptive statistics, t-test, chi-square test and logistic regression analysis. Result: The prevalence of a perioperative pressure ulcer was 26.1%. The level of moisture, friction and shear, length of surgery, and perioperative irrigation were significantly higher in the pressure ulcer group than those in the non-pressure ulcer group. The level of activity and level of consciousness were significantly lower in the pressure ulcer group than those in the non-pressure ulcer group. Significant influencing factors on the development of pressure ulcer were 'moisture' and 'irrigation' and those variables explained 23.1% of varience in the development of a pressure ulcer during surgery. Conclusion: It is necessary to develop a strategy to prevent pressure ulcer by taking 'moisture' and 'irrigation' into account during the preoperative, perioperative and postoperative period.

  • Cytomegalovirus (CMV)-associated esophageal ulcer is rare in immunocompetent infants. The presence of inclusion bodies and immunohistochemical staining for CMV in biopsy specimens obtained during esophagogastroduodenoscopy (EGD) indicate that such ulcers occur because of CMV infection. A 7-week-old female infant who experienced frequent vomiting and feeding intolerance was diagnosed with a massive CMV-associated ulcer in the distal esophagus. The ulcer improved after conservative treatment using proton-pump inhibitors; however, ganciclovir was not administered. In a follow-up EGD biopsy specimen, no CMV inclusion bodies were present, and immunohistochemical staining results for this virus were negative. The presence of CMV inclusion bodies indicates active viral replication. If persistent inclusion bodies or positive immunohistochemical staining for CMV is observed in follow-up biopsy specimens, ganciclovir may be used to treat CMV-associated esophageal ulcers.

Prevention effect of Sunkiwhajung-tang, a prescription, on the gastric ulcer induced by indomethacin in rats (순기화중탕이 Indomethacin으로 유발된 위궤양에 미치는 영향)

  • In order to evaluate the prevention effect of Sunkiwhajung-tang (SWT) which has been used as a traditional prescription for the treatment of digestive disease in Korea on the gastric ulcer induced by indomethacin in rats, the changes of number and size of ulcerative lesions, parietal, chief, Grimelius and Serotonin-positive cells in the peri-ulcerative tissues were detected with histological examinations of ulcerative and peri-ulcerative lesions after oral injections of SWT extracts (125, 250 and 500 mg/kg, respectively). SWT prevented to a great extent the expected indomethacin-induced elevation in hemorrhagic ulcerative lesions, the number and size of ulcerative lesions, and the number of parietal cell, chief cell, Grimelius-positive cells and Serotonin-positive cells in the peri-ulcerative lesions in a dose dependent manner. These results provide a story evidence that SWT produced an protective effect on gastric ulcer induced by indomethacin. Determination of the specific mechanisms involved in the protective effect of SWT on the gastric ulcer will require additional study.

An experimental study of Daegunchungtang on the gastric ulcer and the function of gastrointestinal tract in rats and mice (대건중탕(大建中湯)의 항궤양(抗潰瘍) 및 위장관(胃腸管)에 미치는 효과(效果)에 관(關)한 실험적(實驗的) 연구(硏究))

  • This study was done to investigate the clinical effects of Daegunchungtang on the peptic ulcer and the function of gastointestinal tract. The liquid extracts of Daegunchungtang was administerd orally to the rats and mice. Then the effects of isolated ileum, pylorus-ligated and indomethacine-induced ulcer, gastric juice secretion, and transport abilities of the small and large intestine, were studied. The following results have been obtained : 1. Daegunchungtang showed to have an inhibitory effects on the smooth muscle contraction of the isolated ileum by acetylcholine chloride and barium chloride. 2. Daegunchungtang showed to have an inhibitory effect on the contraction of the gastric fundus-strip by acetylcholine chloride and barium chloride. 3. The preventive effects were recognized on the pylorus-ligated ulcer, by administration of Daegunchungtang. 4. The preventive effects were recognized on the indomethacine-induced ulcer, by administration of Daegunchungtang. 5. The Inhibitory effects on the secretion gastric juice, free & total acidity, and pepsin-output were recognized, by administration of Daegunchungtang. 6. The transport abilities in the small intestine were increased, by administration of Daegunchungtang. 7. The transport abilities in the large intestine were not recognized, by administration of Daegunchungtang. According to the above results, it is considered that the experimental effects of Daegunchungtang has correspond to the oriental medical literatures.

Two Cases of Grade Four Pressure Ulcer Patients Treated with Pharmacopuncture Soyeom and Herbal Medicine Taklisodok-eum (소염약침과 탁리소독음의 복합 치료를 이용한 4도 욕창 환자 치험 2례)

  • Objectives : This study reports two cases of grade four pressure ulcer patients mainly treated by pharmacopuncture Soyeom and Taklisodok-eum. Methods : Two patients with grade four pressure ulcers at the oriental medicine hospital of Woosuk University were treated by pharmacopuncture Soyeom and herbal medicine Taklisodok-eum. Soyeom was treated once a day and Taklisodok-eum was given three times a day. We then observed the patients' appearances of pressure ulcer and changes of blood test results. Results and Conclusions : After ten days or three months each, wound sizes were reduced, tissue regeneration was accelerated and blood tests were improved. In conclusion, it is our belief that pharmacopuncture and herb medicine are effective to treat grade four pressure ulcers, and future studies will be required to ascertain this method on grade four pressure ulcers.

  • Objectives: This case study was designed to evalute the effect of Korean medical treatment on genital ulcer. Methods: This patient was treated with herbal medicine of Palmul-tang (八物湯), Yongdamsagan-tang (龍膽瀉肝湯), acupuncture and herbal applications. This efficacy of treatment was evaluated with inspection and patient's report. Results: After treatments, the patient's symptoms such as genital ulcer, erythema and swelling were improved. Conclusions: This case report shows that the Korean medical treatment is effective in the treatment of genital ulceration.

  • Objectives: This study reports on the efficacy of using Korean medical treatment for suspected gastric ulcer. Methods: We treated a patient with suspected gastric ulcer using Korean medical treatments (herbal medicines, acupuncture, and herbal acupuncture therapy). We evaluated the results using a numerical rating scale (NRS) to measure abdominal pain. The degree of gastrointestinal symptoms was also evaluated using the Korean Gastrointestinal Symptom Rating Scale (KGSRS). The generic health status was evaluated using the European Quality of Life-5 Dimensions (EQ-5D) scale. Results: The NRS and EQ-5D scale scores were improved after the treatments. The change in the KGSRS scores showed that the patient's abdominal pain was relieved. Conclusions: The results of this case study suggest that Korean medicine therapies may be effective in the treatment of suspected gastric ulcer.

  • Object : This study was carried out to examine the effects of Shogunjungtang-ga-younggol morea on gastric ulcers. Methods : In order to study the effects of Shogunjungtang-ga-younggol morea on gastric ulcers. Gastric ulcers were induced in HCI-aspirin in rats. The experiments were done by oral administration and measured by anatohistological features of ulcer lesions, and the changes of the number of parietal cells, chief cells, gastrin and somatostatin- immunoreactive cells. Results : In the Shogunjungtang-ga-younggol morea administrated groups, no gross lesions of ulcer and anatohistologically, just minor injury of gastric mucosa were detected. The number of parietal cells were significantly decreased, and the number of chief cells were significantly increased, in administrated groups. The number of gastrin-immunoreactive cells and somatostatin-immunoreactive cells was significantly increased in administrated groups. Conclusion : According to the results, it is considered that the administration of Shogunjungtang-ga-younggol morea seems to be applicable to the treatment of gastric ulcers.

Review of the Properties of the Laser and the Spectrum of Laser Instruments for Diabetic Ulcer (당뇨병성 궤양에 사용되는 레이저의 특성에 대한 연구)

  • Objectives : One of major complications of diabetes, diabetic ulcer is also one of the main reasons for amputation, and the prevalence rate is 4-10%. Laser therapy is widely used for leg ulcer and diabetic ulcer, and it is known to improve wound epithelialization, cellular content, and collagen deposition. The purpose of this study is to investigate the properties of the laser and the spectrum of laser instruments for diabetic ulcer. Methods : We performed literature search using the PubMed, Cochrane, CINAHL and Web of science for the data in English. In addition, other databases were checked for different languages such as OASIS and NDSL for the literature in Korean, CNKI in Chinese, and CiNii and J-STAGE written in Japanese. We excluded all review article and experimental studies, and only clinical studies using laser or light emitting diode (LED) for diabetic ulcer were selected. Results : A total twenty papers were selected. Different light sources were used as follows: LED, HeNe, InGaAlP, GaAlAs, GaAs, CO2, and KTP. The number of LED studies was 9, and HeNe laser was 7, and InGaAlP and GaAlAs laser was 2, GaAs, CO2, and KTP laser was 1 for each. Various energy density of the clinical study were reported. Conclusions : It is suggested that to select appropriate laser type and give the adequate output power to treat diabetic ulcer. Further evaluation and research for the condition of laser therapy to treat diabetic ulcers are warranted.

  • Objectives: The purpose of this study is to report the improvement of vulvodynia and genital pruritus caused by unidentified genital ulcer after Korean medicine treatment. Methods: The patient who complained vulvodynia and genital pruritus caused by unidentified genital ulcers was treated by acupuncture, moxibustion, fumigation, and herbal medicine as Eunhwasagan-tang, Guibi-tang-gami-bang for 5 days. The effect of treatment was evaluated by Numeral Rating Scale (NRS), The 5 level of EuroQol 5 Dimension scale (EQ-5D-5L), EuroQol-Visual Analog Scale (EQ-VAS). Results: After the treatment, the chief complaint and general conditions were improved. NRS was decreased from 8 to 3 and scores of EQ-5D-5L and EQ-VAS were increased from 0.416, 10 to 0.904, 80 each. Conclusion: This study shows that the skin lesion, vulvodynia, and pruitus caused by genital ulcer was improved after the Korean traditional treatment and it can be effective medical alternatives or options for genital ulcer patients.
