The ethanol extract of Sida acuta Burm. (ALSA) whole plant was studied for its anti-ulcer activity against aspirin plus pylorous ligation induced gastric ulcer, HCl-ethanol induced ulcer, and water immersion stress induced ulcer (WISIU) in rats. We found that ALSA at a dose of 300 mg/kg, (orally) markedly decrease the incidence of ulcers in the first two models. ALSA showed reduction in gastric volume, free acidity, and ulcer index (53.69%). It has not reduced the total acidity significantly and no significant change in pH. It also showed 55.14% gastro protective activity, wheres standard drug sucralfate showed 94.85%. WISIU showed protection index 24.4%, whereas standard drug omeprazole (OMEZ) showed protection index 100%.
This study examined the effect of dietary salt levels on the incidence and cure of gastric ulcer in rats. Two sets of experiment were conducted . In the first experiment, the rats were divided into 3 groups. The 3 groups were fed 0%, 4%, and 8% NaCl diets respectively for 20days. The rats were given water -immersion restraint stress at the end of the dietary period , and sacrificed. The ulcer index by histological test was higher in rats fed the 8% NaCl diet than those in the other groups. The hexosamine and glutathione levels were significantly lower in the rats fed the 8% NaCl diet. Hematocrit and total iron binding capacity(TIBC) showed lower values caused by bleeding of gastric mucosa. The second experiment was designed to determine the effect of soldium concentration on the cure of gastric ulcer . As the gastric ulcer was recovered, ulcer length was gradually deceased in the control group but not changed in the 8% NaCl diet group. The gastric hexosamine and hepatic glutathione were increased in the control group but decreased in the 8% NaCl diet group. The hematologic indices of stressed rats showed the same tendency. As a result, dietary salt per se did not cause gastric ulcer . Once an ulcer is formed by stress or any other factor, higher levels of dietary salt may be detrimental to gastric mucosa, thereby delaying the healing of the ulcer. It is recommended that dietary salt intake be reduced in stress-prone people.
랫드에서 스트레스에 의한 위궤양 모델 개발과 이에 영향을 미치는 몇가지 요인을 알아보기 위하여 본 실험을 실시하였다. 랫드를 적당한 크기의 속박틀에 넣어 속박만 시킨 것, 속박과 함께 침수시킨 것, 수온을 달리한 것 ($20^{\circ}C와\;30^{\circ}C$), 주령을 달리한 것 (5주령과 10주령), 성(sex)을 달리한 것, 절식시킨 것과 절식시키지 않은 것, Strain(Wistar 랫드와 Sprague-Dawley 랫드)을 달리한 것 등의 조건을 두어 8시간 있게 하였고, 발생한 궤양의 길이를 합하여 궤양지수 (ulcer index)를 구하였다. 본 연구의 결과는 속박과 함께 물에 침수시킨 것이 속박만 시킨 것에 비해 궤양지수가 유의성있게 (p<0.001) 높았으며, 궤양은 선위(glandular stomach) \에 국한되어서 발생했다. 또한 수온이 낮은 것이 높은 것보다 궤양지수가 높았고(p<0.001), 절식시킨 것이 절식시키지 않은 것보다 궤양지수가 높았으며(p<0.001), 연령이 낮은것(5주령)이 높은 것 (10주령)보다 궤양지수가 높았다.(p<0.001) 그러나, 성별(sex)에 의한 감수성 차이나 품종간(strain)에 의한 감수성의 차이는 없었다.
Purpose: Diabetic foot ulcer is one of the most important diabetic complications because it increases the risk of amputations. Moreover, it lowers the quality of patients' life and increases the social medical expenses. Authors analyzed risk factors of intractable diabetic foot ulcer using retrospective study. Materials and Methods: From January 2007 to December 2010, 40 patients who could not achieve complete healing despite more than 12 weeks of proper management among who had been diagnosed and treated as diabetic foot ulcer at our hospital were included and evaluated retrospectively. We compared the risk factors between two groups who were finally treated by amputation and non-amputation. Results: The sample was composed of 31 male patients (77.5%) and 9 female patients (22.5%). Comorbidity including hypertension and hyperlipidemia were 77.5% and 80% each. By Wagner classification, 30 patients (80%) had ulcerative lesion over the grade 3. From bacteriology results, 29 patients (72.5%) had polybacteria infection. 35 patients (87.5%) had neuropathy and 26 patients (65%) had vascular stenosis at least one level. The mean initial ankle-brachial index and toe-brachial index were 0.982 and 0.439. In comparison between amputation group and non-amputation group, ulcer severity, number of stenotic vessel and initial ankle-brachial index/toe-brachial index had statistical significance. Conclusion: The most commonly risk factor of intractable diabetic foot ulcer was peripheral neuropathy reaching 87.5% of cases. In comparison with non-amputation group, ulcer severity according to Wagner classification, number of stenotic vessel and initial ankle-brachial index/toe-brachial index were demonstrated as a risk factor of amputation in intractable diabetic foot ulcer.

The effects of ethanol extract of Ruta graveolens on ulceration induced by ethanol and wound healing properties were investigated in mice. Oral administration of the extract reduced the incidence of ulcer, ulcer index and ulcer length produced by ethanol. The gastroprotective effects of R. graveolens were observed in a dose-dependent manner. However, this activity was statistically less potent than the anti-ulcer drug, omeprazole. When the extract applied on the wound, it promoted wound healing in mice. The wound contraction was faster in these mice when compared to untreated wounds. Based on these present findings, R. graveolens possesses anti-ulcer activity and promotes incision wound healing in mice.

Background: A diabetic foot is the most common cause of non-traumatic lower extremity amputations (LEA). The study seeks to assess the risk factors of amputation in patients with diabetic foot ulcers (DFU). Methods: The study was conducted on 351 patients with DFUs from January 2010 to December 2018. Their demographic characteristics, disease history, laboratory data, ankle-brachial index, Wagner classification, osteomyelitis, sarcopenia index, and ulcer sizes were considered as variables to predict outcome. A chi-square test and multivariate logistic regression analysis were performed to test the relationship of the data gathered. Additionally, the subjects were divided into two groups based on their amputation surgery. Results: Out of the 351 subjects, 170 required LEA. The mean age of the subjects was 61 years and the mean duration of diabetes was 15 years; there was no significant difference between the two groups in terms of these averages. Osteomyelitis (hazard ratio [HR], 6.164; 95% confidence interval [CI], 3.561-10.671), lesion on percutaneous transluminal angioplasty (HR, 2.494; 95% CI, 1.087-5.721), estimated glomerular filtration rate (eGFR; HR, 0.99; 95% CI, 0.981-0.999), ulcer size (HR, 1.247; 95% CI, 1.107-1.405), and forefoot ulcer location (HR, 2.475; 95% CI, 0.224-0.73) were associated with risk of amputation. Conclusion: Osteomyelitis, peripheral artery disease, chronic kidney disease, ulcer size, and forefoot ulcer location were risk factors for amputation in diabetic foot patients. Further investigation would contribute to the establishment of a diabetic foot risk stratification system for Koreans, allowing for optimal individualized treatment.
This was performed to investigate the antiulcer effects of Aloe vera on the stomach ulcer induced by restraint and water-immersion stress in rats. For this experiment, 60 male Sprague-Dawley strain were used. The experimental groups were divided into five: a control(CA) and 4 aloe treatment groups. Each dose of aloe was 50 mg/kg BW(AA), 100mg/kg BW(AB), 200 mg/kg BW(AC), and 400 mg/kg BW(AD). The rats were allocated to each group by 12 and observed for 4 weeks. The results are as following. 1. The stomach surface pH in each group showed no significant difference, byt the values of aloe treatment groups were higher than the value of the control group. 2. The gastric wall mucus was significantly increased in all aloe treatment groups(p<0.05) compared with the control group. Especially in AC and AD group the differences were higher(p<0.01). 3. At shear rate rate 11.25, 45.0, 90.0, 225.0 sec-1, whole blood viscosity and plasma viscosity were measured. Most of the values of aloe treatment groups were significantly low compared with those of the control group(P<0.05). 4. The ulcer index of aloe treatment groups were significantly low campared with control group(p<0.05). Especially in AC and AD group the differences were more significant(p<0.01). 5. Less severe ulcers were observed in AA and AB group than in the control group. Tissues of AC and AD group had only slight ulcers and necrosis of tissue was not observed in these groups. Especially in AD group, there was more mucus than other groups and it seemed that alove vera stimulated the epithelial regeneration. From the results of this study, it can be concluded that the oral administration of Alove vera results in protection of stomach ulcer by stimulating the secretion of gastric mucus and the circulation of blood.
This study was designed to determine the effects of protein sources on the curation of gastric ulceration, protein metabolism, and nitrogen balance in rats with gastric ulcer induce by restraint and water immersion stress. After the rats were fed 10% casein diet for 3 weeks, four groups of the rats were forced in 5$\times$5$\times$15cm plexiglass cage. The restraint and water immersion stress was carried at 20$\pm$2$^{\circ}C$ for 8-hour. The other one group(control group) was not exposed to stress. After stress 4 kinds of different diets containing 20% protein were given for 5 days. The protein sources were casein, whey protein, soy protein, gluten. The control group was fed to 10% casein diet. The results were as follows ; the weights of rats were not different among the diet groups During the experiment period follows ; the weights of rats were not different among the diet groups during the experiment period (for 5 days). The ulcer index of rats fed 10% gluten and soy protein diet was significantly higher than those of casein and whey protein diet groups(p<0.05). The level of serum albumin was not significantly different among diet groups. But hematocrit and the level of $\alpha$-amino-N, BU and UUN of plant protein diet groups were higher than animal diet groups, the urinary hydroxyproline of soy protein group was the highest and the whey protein was the lowest. The digestibility and BV of nitrogen of gluten diet group were significantly higher than those of casein and whey protein diet groups(p<0.05). The animal proteins had more curative effects of ulcer than plant animals. The results of this study provide useful information concerning diet therapy for the patients with gastrointestinal diseases and the field of enteral diet materials.
In an attempt to establish anti-ulcerogenic activity of potato tubers, inhibitory activity against ethanol- and indomethacin-induced gastric ulcer models in rats was evaluated for the first time. From several varieties of potato tubers including Solanum tuberosum L. cv. Superior (white skin and fresh potato) and two new varieties of (Bora valley and Gogu valley), raw potato juice was prepared and the starch was obtained from each juice by filtration and drying. Upon oral administration to rats, raw potato juice showed more or less inhibitory activity. The starch showed higher and dose-dependent inhibitory activity, suggesting that the active ingredient in raw potato juice may be the starch. Particularly, the starch obtained from the tubers of new potato variety, "Bora valley," with purple color, showed the highest inhibitory activity (62.4% and 37.1% inhibition of ulcer index at 500 mg/kg), while omeprazole (proton pump inhibitor) used as a reference drug showed 74.4% and 75.7% inhibition at 20 mg/kg against ethanol- and indomethacin-induced ulcer formation, respectively. The present study provides a first evidence of anti-ulcerogenicity of raw potato juice and the starch. Especially, the starch from "Bora valley" strongly inhibited ulcer formation in rats. Considering that these are food components, they may be safely used for anti-ulcerogenic nutraceuticals.
Purpose: The purpose of this study was to develop an algorithm for preventing and managing of pressure ulcer and to verify the its appropriateness. Methods: The first step was development of a pre-algorithm through a literature review and expert opinion. The second step was to establish content validity by submitting the algorithm questionnaires about the content to 12 experts. The third step was the revision of the algorithm. The fourth and last step was to establish the clinical validity of the algorithm with 25 experienced nurses. Results: For the ease of the practitioner the algorithm for prevention and the management of pressure ulcers was confined to one page depicting the main algorithm pathway and seven stepwise guidelines. The guidelines included skin care of pressure ulcer prevention, mechanical loading care, support surface care, reposition care of pressure ulcer, and Stages II, III and IV explanations along with debridement/wound irrigation and infection control. Most of all algorithm courses chosen more than 80% of agreement by expert index of content validity. The usefulness, appropriateness, and convenience of the algorithm were demonstrated through clinical validity with intensive care unit and ward nurses. Conclusion: The algorithm will improve the quality of pressure ulcer nursing care as it provides a model for decision making for clinical nurses as well as providing consistent and integrated nursing care for patients with pressure ulcer throughout an institution.
