Analysis of factors that affect drainage volume after expander-based breast reconstruction

  • Background Closed-suction drains are widely used in expander-based breast reconstruction. These drains are typically removed using a volume-based criterion. The drainage volume affects the hospital stay length and the recovery time. However, few studies have analyzed the factors that influence drainage volume after expander-based breast reconstruction. Methods We retrospectively analyzed data regarding daily drainage from patients who underwent expander-based breast reconstruction between April 2014 and January 2018 (159 patients, 176 expanders). Patient and operative factors were analyzed regarding their influence on total drainage volume and drain placement duration using univariate and multivariate analyses and analysis of variance. Results The mean total drainage volume was 1,210.77±611.44 mL. Univariate analysis showed correlations between total drainage volume and age (B=19.825, P<0.001), body weight (B=17.758, P<0.001), body mass index (B=51.817, P<0.001), and specimen weight (B=1.590, P<0.001). Diabetes history (P<0.001), expander type (P<0.001), and the surgical instrument used (P<0.001) also strongly influenced total drainage. The acellular dermal matrix type used did not affect total drainage (P=0.626). In the multivariate analysis, age (B=11.907, P=0.004), specimen weight (B=0.927, P<0.001), and expander type (B=593.728, P<0.001) were significant predictors of total drainage. Conclusions Our findings suggest that the total drainage and the duration of drain placement needed after expander-based breast reconstruction can be predicted using preoperative and intraoperative data. Patient age, specimen weight, and expander type are important predictors of drainage volume. Older patients, heavier specimens, and use of the Mentor rather than the Allergan expander corresponded to a greater total drainage volume and a longer duration of drain placement.

전자기기의 전원 노이즈 저감을 위한 EMI 필터 경험적 설계에 관한 연구 (A study on the Empirical Design of EMI Filters for Power Supply Noise Reduction in Electronic Devices)

  • 본 논문은 전자기기에 사용되는 전원의 노이즈 저감을 위한 전도성(Conducted) EMI 필터의 경험적 설계를 통하여 제안하였다. 제안된 구조는 A형, B형, C형, D형의 구조를 설계하였으며, 다양한 X-커패스터와, Y-커패스터, Air-인덕터로 LC 네트워크 사용으로 전도성 노이즈 저감을 확인하였다. L1, L2는 10[μH]를 사용하였으며, C1, C2는 4.7[nF]를 사용하였다. 공통모드(Common Mode)용 L3는 13[μH]를 사용하였으며, C5, C6, C7은 10[nF]을 사용하여 설계하였다. 설계된 EMI 필터의 측정된 삽입손실 값은 3.2MHz에서 -74.4[dB], 4MHz에서 -75.4[dB], 13.56MHz에서 -75.3[dB]로 나타났다. 따라서 제안한 EMI 필터는 다양한 전자기기에 사용되는 전원 노이즈 저감에 이용될 것이다.

IV형의 골질로 재생된 골내에 식립된 원통형 임플란트의 유한요소법적 연구 (FINITE ELEMENT ANALYSIS OF CYLINDER TYPE IMPLANT PLACED INTO REGENERATED BONE WITH TYPE IV BONE QUALITY)

  • Stress transfer to the surrounding tissues is one of the factors involved in the design of dental implants. Unfortunately, insufficient data are available for stress transfer within the regenerated bone surrounding dental implants. The purpose of this study was to investigate the concentration of stresses within the regenerated bone surrounding the implant using three-dimensional finite element stress analysis method. Stress magnitude and contours within the regenerated bone were calculated. The $3.75{\times}10-mm$ implant (3i, USA) was used for this study and was assumed to be 100% osseointegrated, and was placed in mandibular bone and restored with a cast gold crown. Using ANSYS software revision 6.0, a program was written to generate a model simulating a cylindrical block section of the mandible 20 mm in height and 10 mm in diameter. The present study used a fine grid model incorporating elements between 165,148 and 253,604 and nodal points between 31,616 and 48,877. This study was simulated loads of 200N at the central fossa (A), at the outside point of the central fossa with resin filling into screw hole (B), and at the buccal cusp (C), in a vertical and $30^{\circ}$ lateral loading, respectively. The results were as follows; 1. In case the regenerated bone (bone quality type IV) was surrounded by bone quality type I and II, stresses were increased from loading point A to C in vertical loading. And stresses according to the depth of regenerated bone were distributed along the implant evenly in loading point A, concentrated on the top of the cylindrical collar loading point B and C in vertical loading. And, In case the regenerated bone (bone quality type IV) was surrounded by bone quality type III, stresses were increase from loading point A to C in vertical loading. And stresses according to the depth of regenerated bone were distributed along the implant evenly in loading point A, B and C in vertical loading. 2. In case the regenerated bone (bone quality type IV) was surrounded by bone quality type I and II, stresses were decreased from loading point A to C in lateral loading. Stresses according to the depth of regenerated bone were concentrated on the top of the cylindrical collar in loading point A and B, distributed along the implant evenly in loading point C in lateral loading. And, In case the regenerated bone (bone quality type IV) was surrounded by bone quality type III, stresses were decreased from loading point A to C in lateral loading. And stresses according to the depth of regenerated bone were distributed along the implant evenly in loading point A, B and C in lateral loading. In summary, these data indicate that both bone quality surrounding the regenerated bone adjacent to implant fixture and load direction applied on the prosthesis could influence concentration of stress within the regenerated bone surrounding the cylindrical type implant fixture.

EPS 샌드위치 패널 심재의 열방출율에 관한 연구 (A Study on the Heat Release Rate of EPS Sandwich Panel Core)

  • 본 연구에서는 EPS 샌드위치 패널 심재에 대한 일정한 외부 복사열에 의한 질량감소속도와 열방출특성을 분석하였다. 일정한 외부 복사열원에 노출된 EPS 샌드위치 패널 심재의 질량감소속도와 열방출특성을 분석하기 위해 3가지 Type의 시료를 사용하였으며, 연소열을 측정하기 위해 Oxygen bomb calorimeter를 질량감소속도와 열방출특성을 분석하기 위해 Mass loss calorimeter를 사용하였다. 질량감소속도와 열방출 특성을 분석하기 위해 $100mm{\times}100mm{\times}50mm$ 크기의 시료를 사용하였다. 연구결과 50 kW/$m^2$의 외부복사열원에서 평균질량감소속도는 Type A와 B의 경우 각각 2.7 g/$m^2s$, 2.8 g/$m^2s$로 비슷한 경향을 나타낸 반면, Type C는 2.3 g/$m^2s$로 상대적으로 낮게 나타났으며, 평균열방출속도는 Type B와 C의 경우 각각 47.19 kW/$m^2$, 50.06 kW/$m^2$으로 큰 차이가 없었으나, Type A는 58.23 kW/$m^2$으로 상대적으로 높게 나타났다. 열방출특성의 결과를 캐나다 분류체계에 적용할 경우 Type A와 C의 경우 C-3등급, Type B의 경우 C-2등급으로 분류되었다. 향후 콘칼로리미터법을 이용한 샌드위치 패널 심재에 대한 열방출율 특성과의 비교연구가 필요할 것으로 판단된다.

스키니 진 착용이 인체생리반응과 주관적 압박감에 미치는 영향 (The Effect of the Skinny Pants on the Physiological Responses and Subjective Pressure)

  • The purpose of this study was to analyze and determine the relationship between physiological responses including blood flow rate and garment pressure, and the feeling of restrictive tightness associated with the wearing skinny pants as a popular clothing style. Evaluation was based on material type, posture and activity type, and body part location. Five female college students took part in this research. Five kinds of experimental clothes with waist measurements of 66cm were chosen. An analysis of the selected skinny pants demonstrated the degree of the whole looseness was higher in this order: clothing type A>B>D>C, and E with E being knitted cloth. Garment pressure was the highest in the front knee portions and was lowest in the outside thigh region. Garment pressure was highest in this sequence : clothing type C>=D>A>=B>E. In terms of posture and activity types, garment pressure was the highest when research participants were crouching, and was the lowest when standing. The blood flow rate was highest in this order: clothing type E>D>B>A>C. Type C skinny pants impeded blood flow and demonstrated the tightest and most restrictive relationship. Blood flow rate varied depending on the type of movement and was highest in this order: getting up, rowing, kicking, jumping and O-shaped leg posture. The results of subjective pressure evaluation demonstrated that pressure was highest in this order: E>=C>B>A>D. These results suggests the need to improve on the patterns and the material design in the area of the front knees. The degree of the looseness when wearing skinny pants did not always correspond to garment pressure or subjective sensation.

도농통합형 도시에 있어서 생물서식처 공간특성에 관한 연구(I) - 천안시 성환읍 매주리지역을 중심으로 - (A Study on the Biotope's Characters of the Mixed Rural City(I) - In the case of Maejuri area in Sunghwan eup, Chonan city -)

  • This study was aimed to establish the method and process of the Biotope Mapping System.(B.M.S.) The main aspects of research for biotope were divided by 4 schemes such as biosphere, geosphere, antroposphere and evaluation informations. The cartographic function of GIS(geographic information system) was also used to mapping them. The B.M.S. had been progressed with 5 steps : (1) making research goals, (2) constructing the graphic and attribute databases, (3) classifying 3 types of biotope such as tree/shrub biotop, grass biotop and wetland biotop. (4) cross-analyzing biotope types against land use, habitat characters, relief characters and danger/disturbance elements, (5) evaluating the biotope's potentialities. Only 3 steps of them were done in this study. The results of applicating the B.M.S on the research site as followings : 1. The interdisciplinary researches were needed to develop the B.M.S in Korea since ecological habitat conditions of Germany, England are differed from them of Korea. 2. The distributions of the land uses in Maejuri, Seunghwan eup in Chonan city were mainly recorded by forest(29.82%), orchard(14.11%) and landscaping around building site(9.05%). 3. The percentages of the Tree/Shrub Biotope type were ranged by the orchard type(23.8%), natural and artificial deciduous type with natural coniferous(23.6%) and etc. 4. The Grass Biotope type was mainly recorded by the wild grassland type(27.99%), garden type(14.21%) and peddy field with wild grassland(13.24%). 5. The distributions of the wetland were pointed high at the wetland type with reed marsh and edge vegetation around wetland(54.79%) and etc.

한국 남성의 얼굴 피부색 분류와 유형에 어울리는 색채 연구 (Clustering of Facial Color Types and Their Favorable Colors on Korean Adult Males)

  • The colors of apparel are getting more important to give the differentiated character on fiber and fabrics. This study was to extract the favorable colors that become to facial color types. Research was carried out to classify the facial colors into several similar facial color groups. With JX-777, 2 points of face: forehead and cheek, were measured and classified into 3 facial color types. Sample size was 418 Korean adult males and other 15 of new males subjects. New chosen 3 subjects who had the classified facial color types, wore silver gown and black hat on his head to minimize the interaction of the clothe color an hair. The 40 standardized color samples were used to extract the favorable colors. 187 respondents answered the degree of becomingness of color samples on 3 facial color types. Data were analyzed by K-means cluster analysis, ANOVA and Duncan multiple range test using SPSS Win. 12. Findings were as follows: 1. 418 subjects who had YR colors were classified into 3 kinds of facial color groups. Type 1 was 4.59YR 5.89/5.12, Type 2 was 5.61 YR 5.41/4.79 and Type 3 was 4.38YR 6.49/4.89 respectively. 2. Favorable colors for Type 1 were 2 colors that belonged to ' a ' group from among colors that were divided into a, b, c group and 18 colors that belonged to ' a ' group from among colors that were divided into a, b group by Duncan post hoc test. 3. Type 2 showed that this type had many unfavorable colors. Unfavorable colors were 16 colors that belonged to ' c ' by Duncan test. 5. Favorable colors for Type 3 were 14 colors that belonged to ' a ' from among colors that were divided into a, b, c and 16 colors that belonged to ' a ' from among colors that were divided into a, b by Duncan test.

단일 단순수직통로의 연돌효과와 중성대에 관한 수치해석 연구 (A Numerical Study on the Stack Effect and the Neutral Plane of a Single Simplified Shaft)

  • 초고층건물의 수직통로에서 발생하는 연돌효과의 기체역학적 특성과 관련된 차압과 중성대의 위치 계산 및 연돌효과 발생의 가시화를 위하여 3가지 형태의 개구부를 갖는 단일 단순수직통로에 대하여 CFD 모델을 사용하여 수치해석을 하였다. 연돌효과의 차압은 이론식 계산 결과와 비교 시 수직통로의 높이가 높아질수록 차이가 커진다. 중성대 위치는 연돌효과 초기(t = 10s)에 Type A의 경우 수직통로의 높이가 30m 미만인, Type B의 경우 30m 이상인 모델에서 이론식 계산 결과와의 편차가 5% 미만으로 잘 일치한다. 따라서 수직통로 높이가 30m 이상인 다중개구부 수직통로(Type B)의 경우 CFD 모델을 사용하여 중성대 위치를 계산하는 것이 가능하다. 연돌효과의 가시화를 통하여 연돌효과의 공기 유동 현상을 쉽게 이해할 수 있다.

Roles of $Ca^{2+}-Activated\;K^+$ Conductances on Spontaneous Firing Patterns of Isolated Rat Medial Vestibular Nucleus Neurons

  • To investigate the contributions of intrinsic membrane properties to the spontaneous activity of medial vestibular nucleus (MVN) neurons, we assessed the effects of blocking large and small calcium-activated potassium channels by means of patch clamp recordings. Almost all the MVN neurons recorded in neonatal $(P13{\sim}P17)$ rat were shown to have either a single deep after-hyperpolarization (AHP; type A cells), or an early fast and a delayed slow AHP (type B cells). Among the recorded MVN cells, immature action potential shapes were found. Immature type A cell showed single uniform AHP and immature B cell showed a lack of the early fast AHP, and the delayed AHP was separated from the repolarization phase of the spike by a period of isopotentiality. Application of apamin and charybdotoxin (CTX), which selectively block the small and large calcium-activated potassium channels, respectively, resulted in significant changes in spontaneous firings. In both type A and type B cells, CTX (20 nM) resulted in a significant increase in spike frequency but did not induce bursting activity. By contrast, apamin (300 nM) selectively abolished the delayed slow AHP and induced bursting activity in type B cells. Apamin had no effect on the spike frequency of type A cells. These data suggest that there are differential roles of apamin and CTX sensitive potassium conductances in spontaneous firing patterns of MVN neurons, and these conductances are important in regulating the intrinsic rhythmicity and excitability.

유도 맞잡기 타입에 따른 허벅다리걸기의 Kinematic 분석[I] (A Kinematic Analysis of Uchi-mata(inner thigh reaping throw) by Kumi-kata types in Judo)

  • The purpose of this study was to analyze the kinematic variables when Uchi-mata(inner thigh reaping throw) performing by Kumi-kata(engagement position, basic hold) types A, B(A: grasping part-behind neck lapel, B: chest lapel) in Judo with three dimensional analysis technique DLT method by videography. The subjects were four male judokas who have been training in Yong-In University(YIU), on Korean Representative level and Uchi-mata is their tokui-nage(favorite technique), the throwing form was filmed on two S-VHS 16mm video camera( 30frame/sec. Panasonic). Kinematic variables were temporal, posture, and COG. The data collection was performing by Uchi-mata. Six good trials were collected for each condition (type A, B) among over 10 trials. The mean values and the standard deviation for each variable were obtained and used as basic factors for examining characteristics of Uchi-mata by Kumi-kata types. The results of this analysis were as follows : 1) Temporal variables The total time elapsed(TE) by Uchi-mata of types A, B were 1.45, 1.56 sec. respectively. Types A shorter than B. 2) Posture variables In performing of Uchi-mata, the range of flexion in type A, left elbow was $45^{\circ}$ and B was $89^{\circ}$ from Event 2(E2) to Event 6(E6). Type A and B were quite different in right elbow angle in Event1(E1). Left shoulder angle of type A was extended and type B was flexed in E4. Both types right shoulder angles were showed similar pattern. Also both hip angles(right/left) were showed similar pattern. When type A performed Uchi-mata the knee-angle of supporting foot showed $142^{\circ}$in the 1st stage of kake phase[KP], and extended to $147^{\circ}$in the 2nd stage of KP. And the foot-ankle angle of supporting foot showed $83^{\circ}$in the 1st stage of KP, and extended to $86^{\circ}$in the 2nd stage of KP. moreover, The knee angle of attacking foot showed $126^{\circ}$in the 1st stage of KP, and extended to $132^{\circ}$in the 2nd stage of KP, and the foot-ankle angle of attacking foot showed $106^{\circ}$in the 1st stage of KP, and extended to $121^{\circ}$in the 2nd stage of KP. When type B performed Uchi-mata the knee-angle of supporting foot showed $144^{\circ}$in the 1st stage of KP, and extended to $154^{\circ}$in the 2nd stage of KP. And the foot-ankle angle of supporting foot showed $83^{\circ}$in the 1st stage of KP, and extended to $92^{\circ}$in the 2nd stage of KP. moreover, The knee angle of attacking foot showed $132^{\circ}$in the 1st stage of KP, and extended to $140^{\circ}$in the 2nd stage of KP, and the foot-ankle angle of attacking foot showed $103^{\circ}$in the 1st stage of KP, and extended to $115^{\circ}$in the 2nd stage of KP. During Uchi-mata performing, type A showed pulling pattern and type B showed lift-pulling pattern. As Kumi-kata types, it were different to upper body(elbow, shoulder angle), but mostly similar to lower body(hip, knee, ankle angle) on both types. 3) C. O. G. variables When the subjects performed Uchi-mata, COG of type A, B up and down in vertical aspect was 71cm, 73.8cm in height from the foot in the 2nd stage of KP. As Kumi-kata types, it were different on medial-lateral direction aspect but weren't different in Kuzushi phase on vertical direction aspect.
