Clinical Study on a Case of Idiopathic Oculomotor Nerve Palsy Patient (특발성 동안신경마비로 인한 안검하수와 안구운동장애 환자의 치험1례)

  • Oculomotor nerve palsy is a disorder which causes eyeball movement trouble, dipleopia, dizziness and eyelid ptosis. It is caused by aneurysm, mass, truma, intracranial hypertension, diabetes mellitus, infection and so on. But sometimes we can't establish the cause. We report a case of oculomotor nerve palsy associated with spleen energy(脾氣). We treated a patient with herb medicine and acupuncture. This study suggests that oriental medicine theraphy has a good effect on the oculomotor nerve palsy.

Beloved: Identity Recovery through Rememory (『빌러비드』: 재기억을 통한 정체성 회복)

  • The purpose of this study is to research how the writer describes characters of the text who overcome traumatic experiences and restore their identity through rememory in Toni Morrison's Beloved. The writer, Morrison gives the female characters their voices to recover their ethnic identities. By breaking silence, they establish their identities and become Americans from "unspeakable thoughts" to "speaking subjects." The ex-slave Sethe and her daughter, Denver have experienced trauma which works from traces of memory and history after slavery was abolished. Sethe and Denver are isolated from the community at the 124 Bluestone Road. When Beloved, ghost who was killed by Sethe, appears, Sethe and Denver are wondering who she is. Rememorying in Beloved is the important form of narrative that Morrison uses to recover their trauma. Morrison emphasizes the need to reconcile with the community and the aid of community for Sethe and Denver to heal their truma. Thanks to Beloved who leads Sethe and Denver to the community, they can be finally one of the community members in America.

Disaster Victims' Post-trauma Risk, Posttraumatic Growth and Subjective Well-being - Social Support as a Mediator and Income as a Moderator - (재난피해자의 외상 후 위기와 외상 후 성장, 주관적 안녕감 간의 관계 - 사회적 지지의 매개효과, 소득의 조절효과 -)

  • The current study examined the relationships between disaster victims' post-trauma risk and posttraumatic growth, and the mediational role of social support for this relationships. In addition, we tested the hypothesis that income status would moderate the relationship between post-trauma risk and subjective well-being. Two hundred disaster victims completed Post-trauma Risk Checklist (PRC), Posttraumatic growth scale, Social support scale and Concise Measure of Subjective Well-Being (COMOSWB) as well as questions about their demographic characteristics. Results showed that those with high post-trauma risk demonstrated significantly higher levels of posttraumatic growth as compared to those with low post-trauma risk and social support partially mediated this relationship. Also, high levels of post-trauma risk predicted low levels of recent subjective well-being. Participants with high income obtained higher subjective well-being than did those with low income among high post-trauma risk groups. Implications for post-trauma risk and posttraumatic growth are discussed.

Effects of Dietary Protein Level on Catecholamine Concentration of Laparotomized Rats (식이단백질 함량이 개복수술을 받은 흰쥐의 Catecholamine 농도에 미치는 영향)

  • This study was performed to investigate the effects of dietary protein content on recovery of laparotomized rats in terms of urinary epinephrine and norepinephrine plasma epinephrine and norepinephrine, . Fortyeigh male Sprague Dawley rats average-weighing 160g were divided into two diet groups ; LPD(10% low protein diet) group HPD(25% high protein diet) group After 3 days of adaptation period rats were given experimental diet for 14 days. Experimental period consists of 7-days pre-trauma and 7-days post-trauma period. Rats were laparotomized by 4cm abdominal incision under sodium pentobarbital anesthesia. During 7-days before and after the surgery urine and plasma were collected for the analysis of epinephrine and norepine-phrine. The results are as follows: 1) After laparotomy urinary epinephrine level was not increased in two diet groups. Urinary epinephrine concentration of rats in LPD group was considerably increased in post-trauma day 3 and thereafter gradually reduced. By post-trauma day 4 however urinary epinephrine concentration of rats in HPD groups was recovered to pre-trauma level 2) Urinary norepinephrine concentration of rats in LPD group was significantly increased after trauma(p<0.01) and the difference between LPD and HPD in post-trauma average norepinephrine concentration was not significant. 3) Post-traum average plasma epinephrine concentration was higher but not significant than that of pre-truma average in both groups [LPD(19.88ng/ml vs 20.93ng/ml) HPD(17.20ng/ml vs 19.37ng/ml)] 4) Plasma norepinephrine concentration of rats in LPD group was significantly increased in post-trauma period(p<0.01) In HPD group however post-trauma average plasma norepi-nephrine concentration was significantly lower than pre-trauma average. Thus the results suggest that norepinehrine concentration was affected by trauma and rats in HPD group excreted less amount of norepinephrine than rats in LPD group.

FUNCTIONAL RECONSTRUCTION OF MANDIBULAR DEFECTS WITH FREE BONE GRAFT (유리골 이식을 통한 하악골 결손부의 기능적 재건술)

  • Mandibular discontinuity defect due to benign tumor, malignant tumor, infection, or truma results in major esthetic and biologic compromise. The primary goal of reconstruction is full restitution of function, which secondarily lead to normalization of the cosmetic deformity. The authors make a clinical study of 61 consecutive bone graft cases for mandibular reconstruction of discontinuity defect which were studied retrospectively using clinical data and radiographic findings. The cases were reviewed to evaluate the clinical success in the period from 1981 to 1990 in the Dept. of Oral & maxillofacial Surgery, Seoul National University Hospital. The criteria of the success in bone graft, are no residual infection, graft in with maintain its integrity, and remain over a half of its original size of graft in the radiographic features. The purpose of this clinical survey is to study of the mandibular discontinuity defects and success rate of free bone graft in mandibular defects. To summarize the clinical study of free bone graft, the main type of autogenous bone graft is iliac bone and corticocancellous type. Overall success rate is 80.3% in 61 followup cases over 6 months. Wire fixation and Extraoral approach has realtively better prognosis than other methods. It showed relatively poor prognosis in symphysis defects than other recipient site.

Biographical Context of Participation in Social Movement : A biography-reconstructive analysis on experiences in Miryang struggle against 765kV transmission tower (구술생애사 기록을 통해 본 사회운동참여의 맥락 밀양765kV송전탑건설반대운동에 참여한 여성주민들의 구술생애사 분석을 중심으로)

  • This article analyzes biographies of women participants in Miryang struggle against 765㎸ transmission tower and finds that the experience of state violence under the ideological conflicts works the biographical context of participation in environment movement. The cases analyzed in this article disclose the family member's experience of state violence and their trauma through the participation in social movement. And they legitimize their family member's life, heal their trauma, and live a new life by the social movement participation. But the power of healing trauma comes from solidarity and support in social movement. Findings of this article imply that experiences of state violence have influence on the formation and development of various Korean social movements and we need more solidarity and democracy for healing the trauma occurred by state violence and concealed in individual memory. Findings of this article also suggest the importance of biography documents. Biography documents can contribute to widen and to deepen understandings on the social interaction and social history, because they are the document about people's experience who are excluded from the official history and character culture. Moreover biography documents can compensate the official documents, because they can offer the context to the social actions in the official documents. More accumulations and analyses on biographies can serve to widen and to deepen understanding and explanation of Korean society having multilayered exclusion in the process of the compressed modernization and the history of national division.

Traumatic Injuries of the Diaphragm (외상성 횡격막 손상)

  • From January 1980 to July 1995, 32 patients with blunt diaphragmatic trauma and 12 wish penetrating diaphragmatic trauma were treated at Kyungpook University Hospital. The mean age of patients with blunt trauma was 37.6 years, and with penetrating trauma 29.2 years. The mean age was older in patients with blunt trauma by 8.4 years (p< 0.05). When simple chest X-ray diagnosis was performed, 24cases (75%) of the diaphragmatic injuries were found in blunt trauma and 4 case(33%) in penetrating trauma. When opei.ations were done due to injuries of thoracic or abdominal organ, 7 diaphragmatic injuries (58%) were found in penetrating trauma. Herniation occurred in 24 patients (75%) in blunt trauma and 5 patients (42%) in penetrating truma. The size of injured diaphragms in 29 cases in which hernia was discovered was 10.9 $\pm$ 4.3cm. On the other hand, the size in 15 cases in which hernia did not occur was 3.5 $\pm$ 2.9cm. There was a remarkable difference in their size depending on the presence or absence of hernia (p< 0.051. That is, a close relation between the sire of injured diaphragms and hernia was shown. All diaphragmatic injuries were repaired primarily. In blunt trauma approach of repair were as follows : 20 (63%) thoracic, 9 abdominal, 2 thoracoabdominal, and 1 thoracic and in penetrating trauma 6 (50%) abdominal, 4 thoracic, and 2 thoracic and abdominal. The complication rate was 19% in blunt trauma and 25% in penetrating trauma. Two patients with blunt trauma died with a mortality rate of 6.3% . All patients with penetrating trauma recovered. This study suggests that diaphragmatic injury should be suspected in all patients w th blunt as well as penetrating injury of the chest and abdomen. The size of injured diaphragms was larger in blunt trauma than in penetrating trauma. For herniation, it appeared to be more common in blunt trauma. The morbidity and mortality were related primary to the severity of associated injuries.
