  • The masticatory function of tooth is maintained by the periodontal health, and periodontal health is also maintained by the masticatory function. Bite forces are withstanded by the PDL, and this thought to be through the viscoelastic theory. Mobility test and Bite force test are used to evaluate the viscoelastic theory of the PDL. In this study, the bite force test was used. In the same conditions of quantity of the supporting tissue, the maximum bite force according to the quality of the supporting tissue was evaluated. The study was conducted on 40 patients with moderate adult periodontitis, who were indications to the modified widman flap treatment. The maximum bite force in the premolar and molar regions were tested before treatment, 3weeks and 4 weeks after treatment. and the results were as follows. 1. In the premolar region, 3 weeks and 4 weeks after treatment showed higher maximum bite force than before treatment. And in the molar region the maximum bite force decreased 3 weeks after treatment, but increased after 4 weeks, compared to before treatment. 2. In the 1st premolar, there were only significant difference between before and 3 weeks after treatment, and between and 4 weeks after treatment. 3. In the 2nd premolar, there were only significant difference between before and 3 weeks after treatment, and between and 4 weeks after treatment. 4. In the 1st molar, there were no significant difference between before, 3 weeks after treatment, 4 weeks after treatment. 5. In the 2nd molar, there were no significant difference between before, 3 weeks after treatment, 4 weeks after treatment. From the results above, it shows that there were improvements in the maximum bite force through specific periodontal treatments, and thus it can be considered in clinical situations, that selection of the prosthodontic material, decisions of extraction, evaluation of the prognosis after periodontal treatment is a helpful method.

Methods We searched the clinical studies from the China National Knowledge Infrastructure (CNKI) that were published between January 2000 to February 2020 by key words '疝气', '直腸脫出', '直腸脫垂', '脫肛', '小腸疝', '腹股溝疝', '儿童', '小儿', '少儿', '幼年', '治療', '中医治療', '中藥', '中医藥', '顆粒', '膠囊', '自擬', '湯', '丸', '散', '方'. We analyzed the literatures in regards to the treatment methods and results. Results Among the 193 searched studies, 10 randomized controlled trials were selected and analyzed. In most of the studies, the effectiveness of the traditional Chinese medicine (TCM) treatment on inguinal hernia and rectal prolapse was significantly high. Conclusions Based on the results of the clinical studies from China, use of the TCM for the treatment of inguinal hernia and rectal prolapse has been shown to be effective in relieving symptoms. Also, based on the result of this study, it will be possible to widen the scope of the TCM treatment on inguinal hernia and rectal prolapse. Additional clinical studies and experimental studies are needed to be performed to solidify these findings. The TCM has been shown as an effective treatment for pediatrics as well. These research results can be utilized in other clinical studies and in treatment.

  • The purpose of this study is to investigate the effects of Ginseng saponin on the cadmium which is widely distributed in the environment, results in immune system alteration. For the experiments, 125 mice of ICR strain were used. The experimental groups were divided into 5 groups a control, a cadmium alone treatment group, three Cd and saponin (10, 50, 100 mg/kg) combined treatment groups. The mice were allocated 25 to each group and observed for 8 weeks. The results of experiment are as follows: 1. Body weight growth rates during 8 weeks were as this control group 36.47%, Cd alone group 32.48%, saponin combined treatment group (10, 50, 100 mg/kg) 32.49%, 39.17%, 24.27% respectively. 2. In all groups, the relative weights of liver and kidney were increased, compared with control group. In the case of spleen, saponin combined treatment group (50, 100 mg/kg) was high to the significant level compared with a control group (p<0.05). Thymus was not. 3. On blood lymphocyte count observation, Cd alone treament group has 25.6% less than control group, and saponin combined treatment group have increasing trends. but in thymus and spleen, there was no trends like blood. 4. On antibody titer, there was no difference among groups. 5. On total serum protein, saponin (100 mg/kg) combined treatment group was high to significant level compared with control group (p<0.05), and other treatment groups have increasing trends. 6. Cd accumulation in kidney was higher than in liver, and all treatment groups were high to the very significant level compared with the control group (p<0.05), but there was no difference among groups. From the results of this study, it can be concluded that the oral administration of Cd results in alteration of immune system and Ginseng saponin prevents this effect. But, Cd accumulation was not affected by saponin.

  • Objectives : To review RCTs on acupuncture treatment for shoulder pain in order to establish a standard acupuncture treatment model in treating shoulder pain. Methods : RCT articles on traditional acupuncture treatment for shoulder pain were searched through online database. Quality of studies were assessed using the FEAS and the modified Jadad score. Results : Eighteen trials of acupuncture for shoulder pain were analyzed. Based on the results of these reviews the following factors might contribute to optimal results from acupuncture treatment. 1) Usage of LI, SI, TE meridians, usage of $LI_{15}$, $TE_{14}$, $GB_{21}$, $LI_{11}$, $LI_4$, $SI_{14}$, $LI_{14}$, $TE_{15}$ acupuncture points. 2) More than four acupuncture points should be used. 3) More than 15 minutes of needle retention time. 4) Needle length-40mm and diameter-0.30mm. 5) More than 9 times treatment 6) More than 5 weeks treatment duration. Conclusions : There was no relation between quality of article and effectiveness of acupuncture. To improve the remedial value, it is necessary to mention De-qi, stimulation of acupuncture and correct variation in diagnosis with the above-mentioned. It is better that clinical trials of acupuncture treatment is designed that type of RCT and double blind. Also when it is set that sham nonpenetrating acupuncture, no treatment group as a control group, and participants don't distinguish wheather acupuncture treatment or not, it will be more meaningful.

  • Objectives: This study aimed to review clinical studies on thread embedding acupuncture (TEA) for the treatment of patients with post-stroke spasticity (PSS) Methods: Clinical studies related to TEA treatment for PSS were searched for in several electronic databases, including PubMed, Cochrane Library, MEDLINE, OASIS, and CNKI. After the selection/exclusion process, information on study design, disease, experimental/control group, intervention, outcome measurement, between-group differences, and adverse events was extracted. Results: A total of 13 randomized controlled trials were included in this review. The usual frequency of TEA treatment was once every 1-2 weeks, and the treatment most commonly included 4 sessions. In the dorsal area, EX-B2 and the acupoints in governing vessel meridian were commonly used. Acupoints in the meridian of the large intestine were most frequently used in the treatment of upper limb spasticity. Except for HT1, PC6, and PC8, all TEA points for treating PSS of the upper limb were on the Yang meridians. For the lower limb spasticity, the most frequently used acupoints were ST36, GB34, GB30, BL60, and BL57 on Yang meridians, and LR3, SP6, SP9, and SP10 on Yin meridians. TEA treatment showed better effects than conventional treatment for PSS in terms of spasticity, motor dysfunction, and activities of daily livings. Nevertheless, the absence of the follow-up observation, lack of sham TEA treatment, and low quality of the included studies necessitated caution in interpreting the results. Conclusions: The results of this review are expected to provide basic data on the modalities of TEA treatment for PSS and provide insights to facilitate well-designed studies in the future.

  • Objective: To systematically review the evaluation of the diagnostic accuracy of pre-treatment apparent diffusion coefficient (ADC) and change in ADC during the intra- or post-treatment period, for the prediction of locoregional failure in patients with head and neck squamous cell carcinoma (HNSCC). Materials and Methods: Ovid-MEDLINE and Embase databases were searched up to September 8, 2018, for studies on the use of diffusion-weighted magnetic resonance imaging for the prediction of locoregional treatment response in patients with HNSCC treated with chemoradiation or radiation therapy. Risk of bias was assessed by using the Quality Assessment Tool for Diagnostic Accuracy Studies-2. Results: Twelve studies were included in the systematic review, and diagnostic accuracy assessment was performed using seven studies. High pre-treatment ADC showed inconsistent results with the tendency for locoregional failure, whereas all studies evaluating changes in ADC showed consistent results of a lower rise in ADC in patients with locoregional failure compared to those with locoregional control. The sensitivities and specificities of pre-treatment ADC and change in ADC for predicting locoregional failure were relatively high (range: 50-100% and 79-96%, 75-100% and 69-95%, respectively). Meta-analytic pooling was not performed due to the apparent heterogeneity in these values. Conclusion: High pre-treatment ADC and low rise in early intra-treatment or post-treatment ADC with chemoradiation, could be indicators of locoregional failure in patients with HNSCC. However, as the studies are few, heterogeneous, and at high risk for bias, the sensitivity and specificity of these parameters for predicting the treatment response are yet to be determined.

  • It is known that the personality is the crucial factor in the treatment outcome of depression. The authors tried to identify the results of such studies and various components which determine the treatment outcome of depression. Nearly 60 papers published between the year 1990 and 2000 about the treatment of depression and personality were reviewed. Among them about 30 papers were selected to compare the research methods, results and discussions. The arguments and critics of the papers were discussed. In the many debates, the authors admitted the fact that premorbid personality trait influences the treatment outcome of depression negatively regardless of treatment method. Subtyping of depression is feasible along the presence of good or bad predictors of treatment outcome for depression. Differentiation of depression and personality seems to have no problem, however test of personality state before the development of psychiatric disorder such as depression is not amenable. For example, cluster A personality trait is often misunderstood as depression in clinical setting. In some cases cognitive behavioral therapy is effective in the treatment of depression accompanying personality disorder. The authors insist that the analysis of personality in the dimensional aspect rather than in the categorical aspect gives more information in the research of personality influencing the treatment outcome of depression. In addition, the reason why we understand the relationship between depression and personality were discussed.
