The main objective of this study is to evaluate the cost-effectiveness of tafluprost compared with latanoprost in primry open Angle Glaucoma (POAG) or ocular hypertension OH patients in Korea. A decision analytic model was developed from a societal perspective to estimate clinical outcome, drug cost and glaucoma related cost. The model assumes branch like following: successful treatment, switching to other drug, adding other drug, laser or surgery. Treatment success rate is defined as the percentage of patients with elevated IOP achieving <20% reduction, and discontinuation rate is the percentage of patients who were withdrawn due to severe adverse events. A model that is comprised of 1 month cycle length has 1 year. Treatment success rate and discontinuation rate were obtained from published literatures searched in database. Resource utilizations and costs were calculated with national health insurance data and clinical expert opinions. Sensitivity analyses were performed on crucial parameters. Tafluprost is less costly than latanoprost, $609.0 vs $651.2 expected cost. Thus tafluprost was shown to be dominant compared with latanoprost. The results of sensitivity analysis revealed stable across most of the included parameters. According to this study, tafluprost shows more clinical outcome for 1 year than latanoprost. In addition, first-line treatment of tafluprost is a more cost-minimizing strategy associated with POAG or OH compared with latanoprost.

This experiment was performed to investigate the effects of Hyeonsamdansameum(HDE) on hypertension. For the study of HDE, we had divided Sprague-Dawley rats to three groups-normal, control, HDE. The control group was injected subcutaneous with monocrotaline(50 mg/kg). The treatment group was injected subcutaneous with monocrotaline(50 mg/kg) and orally administered with HDE extract for 4 weeks(once a day, 208 mg/kg). Then we measured blood pressure, heart rate, on the plasma aldosterone, catecholamine, electrolyte, uric acid, BUN, creatinine, and observed the lung, cardiac muscle. liver. etc. The results of these were as follows: 2,2-diphenyl-1-picrylhydrazyl (DPPH) scavenging activity and superoxide dismutase(SOD) - like activity were increased. reactive oxygen species (ROS) was decreased. Angiotensin converting enzyme(ACE) inhibitory activity was increased in a concentration-dependent by HDE. HDE significantly increased body weight in monocrotaline hypertensive rat, so supported nearly normal group. HDE significantly decreased blood pressure and heart rate in monocrotaline hypertensive rat. HOE significantly decreased aldosterone in adrenocortical hormones. HDE significantly decreased dopamine. norepinephrine, epinephrine. Na+. Cl- were intended to decrease. K+ was decreased significantly by HDE. Uric acid. BUN were significantly decreased and creatinine was intended to decrease by HDE. HDE inhibited lung, liver and heart injury connected with hypertension. These results suggest that HDE is usefully applied in treatment and prevention of hypertension.
Purpose: This study was a descriptive study to investigate correlations between self-care behaviors and quality of hypertension management by hypertensive patients being cared by community health practitioners. Method: Participants were 583 people who were prescribed antihypertensive medications at a health clinic located in South Chungcheong Province. The survey was done from June 1 to July 30, 2018. A self-report questionnaire was administered, and data analysis was performed using descriptive statistics and Pearson correlation coefficients with the SPSS 24.0 program. Results: The rate of awareness, treatment, and control of hypertension were 97.9%, 99.1% and 92.8%, respectively. The number of hypertensive self-care behaviors was 1.82 (${\pm}0.36$) out of 3 points. The quality of hypertension management was 3.22 (${\pm}0.46$) out of 4 points. There was a moderate correlation between hypertensive self-care behaviors and quality of hypertension management (r=.340, p<.001). Conclusions: Results of this study confirm that the quality of hypertension management by community health practitioners is related to self-care behaviors of hypertensive patients. Therefore, it is necessary to improve the quality of hypertension management by health care specialists for self-management of hypertension patients. In addition, a systematic program to improve the quality of hypertension management by community health practitioner is needed.
The study aimed to investigate the effects of differences between personal income and social deprivation on the association between cardio-cerebrovascular disease (hypertension, stroke) and periodontal disease. This study used 12 years of cohort data from the Korea National Health Insurance Service covering the years 2002 to 2013. Among the patients aged more than 40 years who had received treatment for periodontal disease 224,067 and 284,730 who had not received treatment for hypertension and stroke, respectively, were included in the analysis. The Kaplan-Meier analysis revealed differences in the rate of treatment for cardio-cerebrovascular disease (hypertension, stroke) according to regional differences, the rate of treatment increased as the composite deprivation index value increased. The difference in treatment rates for cardio-cerebrovascular disease (hypertension, stroke) according to income was found to be higher in the treatment group with low income. This study empirically proved that the association between systemic disease and periodontal disease varies depending on personal income and the regional socioeconomic deprivation level. This shows that the clinical influence of periodontal illness on systemic disease differs according to the personal socio-demographic characteristic and residential area and that an individual's characteristic (income and the regional) needs to be considered along with the patient's clinical intervention in the disease treatment process.
Objectives: The prevalence of hypertension in Korean rural elderly was significantly higher than that of the general population. Determining the potential risk factors of hypertension would be useful for managing and improving the treatment and prevention of hypertension in rural areas. Methods: We studied 336 elderly individuals 110 males, 226 females) aged between 65 years and 95 years residing in the rural area, S-gun Jeonbuk. Health-related habits, frequency of intake of food groups, nutrient intakes, anthropometric and biochemical measurements were assessed. Subjects were defined as hypertensive if SBP was ${\geq}140mmHg$ or if DBP was ${\geq}90mmHg$ or take an antihypertensive drug. Results: The rate of prevalence of hypertension in the study group was 51.8% (male 40.0%, female 57.5%). The risk of occurrence of hypertension was higher among females (OR, 1.98), 75 years old or older (OR, 1.62), BMI ${\geq}25kg/m^2$ (OR, 2.84), acceptable range (upper end) of body fat (OR, 2.29) and unhealthy (too high) range of body fat (OR, 3.28), hypertriglyceridemia (OR, 2.17) and hypercholesterolemia (OR, 5.42), low protein intakes (OR, 1.78). However, health related habits, frequencies of intake of food groups and most nutrient intakes except for protein did not show any significant relationship with the occurrence of hypertension. Conclusions: To reduce the risk of occurrence of hypertension among elderly individuals in rural areas, it is needed to avoid increase of body fat, 25 or higher BMI ($kg/m^2$) and hyperlipidemia and low intake of proteins.

Lead (Pb) is a metal that is generally considered to be toxic to the cardiovascular system. Pb-exposed animals display the evidence of increased oxidative stress and hypertension. The current study was designed to examine whether Korean red ginseng (KRG) has protective effects against Pb-induced hypertension and oxidative stress in Pb-exposed rats. Male Sprague-Dawley rats were randomly assigned to Pb exposure or control groups. KRG was administered in drinking water at a concentration of 100 mg/kg/day; the control group received plain drinking water. Animals in the Pb-exposed groups were provided with drinking water containing 100 ppm Pb acetate for 12 weeks. Blood pressure, plasma glutathione, blood Pb concentration, and hematologic data, such as red blood cell quantity, were determined. Pb poisoning was assessed by measuring the blood Pb concentration. Pb exposure (100 ppm) for 12 weeks resulted in a marked rise in systolic blood pressure and blood Pb concentration, as well as a significant reduction in plasma glutathione levels and red blood cell quantity. Other measurements, such as heart rate, body weight, and white blood cell quantity, were unchanged. Treatment with KRG significantly lowered blood pressure, raised plasma glutathione and increased red blood cell numbers in Pb-exposed animals; it also had no effect on heart rate, body weight, or white blood cell quantity. However, the elevated blood Pb concentration was not reduced by treatment with KRG (100 mg/kg). Taken together, these data indicate that treatment with KRG in Pb-exposed animals can reduce oxidative stress and lower blood pressure, suggesting that KRG might be protective against Pb-exposed hypertension and oxidative stress.
The safety and efficacy of fimasartan have been evaluated through post-marketing surveillance in real world clinical practice. The multi-center, prospective, open-label and non-interventional study. A total of 3,945 patients (3,729 patients for safety assessment and 3,473 patients for efficacy assessment) were screened in patients with essential hypertension in 89 study centers from 9 September 2010 through 8 September 2016. Among the total patients, 2,893 patients (77.6%) were administered fimasartan for 24 weeks or longer and were classified as 'patients with long-term follow-up', and the additional safety and efficacy analysis were performed. The improvement was defined as systolic blood pressure (SBP) controlled to ${\leq}140mmHg$ or decreased SBP differences ${\geq}20mmHg$ after treatment or diastolic blood pressure (DBP) controlled to ${\leq}90mmHg$ or decreased DBP differences ${\geq}10mmHg$ after treatment. Adverse drug reactions (ADRs) were reported in 3.8% patients; dizziness, and hypotension were the most frequently reported ADRs in total patients. The results of patients with long-term follow-up were comparable with total patients. The overall improvement rate in all efficacy assessment at the last visit was 87.1% (3,025/3,473 patients). The overall improvement rate of the patients with long-term follow-up was 88.9%. Fimasartan was well tolerated, with no new safety concerns identified and an effective treatment in the real world clinical practice for Korean patients with hypertension.
Objectives : To identify potential correlation between patients' expectations and clinical outcome in a randomized control study of acupuncture. Methods : In a clinical trial of acupuncture for hypertension, 60 participants with pre and mild hypertension were randomized into an acupuncture group and a control group. After randomization, all participants were asked to rate their expectation for the intervention on a scale of 0 to 10. To analyze the effect of expectation on clinical outcome, change of blood pressure was compared between high and low expectation groups. Results : There was no significant difference of baseline blood pressure between low expectation group and high expectation group. Proportion of acupuncture group and control group was also not different between low and high expectation groups (p = 0.638). The change of systolic blood pressure was -1.55 mmHg in low expectation group and -3.07 mmHg in high expectation group, and it was not significantly different (p = 0.54). There was no significant difference in the change of diastolic blood pressure between two groups (p = 0.58), with -3.24 mmHg in low expectation group and -2.34 mmHg in high expectation group. Conclusion : In this study, the expectation of intervention (including acupuncture treatment) was not associated with the effect of intervention.
The present study examined the effect of a methanol extract of Evodiae Fructus on the cardiovascular function in N-nitro-L-arginine methyl ester (NAME)-induced hypertensive Wistar rats after treatment over 6 weeks. In rats treated with NAME, blood pressure, weight of heart, aorta media thickness and media/lumen ratio significantly (p<0.05) increased, whereas coronary flow and heart rate of isolated heart significantly (p<0.05) decreased compared with control group at 6 weeks. In rats treated with NAME and Evodiae Fructus, blood pressure, aorta media/lumen ratio significantly(p<0.05) decreased compared with NAME treated group at 6 weeks. These results suggest that Evodiae Fructus is applicable to the treatment of hypertension and vascular hypertrophy.
Objectives : Aim of this study was to investigate the antihypertensive effect of Korean Hwa-Acupuncture in hypertensive patients. Methods: We measured the blood pressure of 14 patients who were admitted in the Oriental Medical Clinic of Balance & Harmony Korean Acupuncture Association from 25th March 2006 to 21th July 2006. Eligible participants had systolic blood pressure ${\geq}120mmHg$ diastolic blood $pressure{\geq}80mmHg$. Blood pressure and pulse rate measurements were after the patient had been bed rest for at least 30 min. 10 times of Hwa-Acupuncture treatment over 3 weeks period were performed in the patiants. To evaluate the effect of the Hwa-Acupuncture, the blood pressure, pulse rate were measured 2 times before and after each acupuncture treatment for total of 10 times. Results : After 3 weeks, there were significant decreased in the systolic blood pressure(P<0.0297) and significant decreased in diastolic blood pressure(P<0.0223) treated by Hwa-Acupuncture 10 times but pulse rate was failed to decreased. The effects of Hwa-Acupuncture by measuring time on blood pressure were as follows : systolic blood pressure were deceased significantly from 1st to 10th visit (P<0.0207) but diastolic blood pressure and pulse rate was not decreased significantly. Conclusion : The results suggest that Hwa-Acupuncture is effective in decreasing the systolic blood pressure.
