A Comparative Study of BMD between Osteoporosis Patients Taking Fosamax and Taking Fosamax and Samgieumgamibang (골다공증 환자에 대한 Fosamax 단독복용군과 삼기음가미방 병용투여군의 유효성 비교분석)

  • Purpose: The purpose of this study is to evaluate the synergic efficacy of oriental herb medicine for osteoporosis patients by examining whether there is any change in BMD. Osteoporosis patients are classified into a group having taken Fosamax and the other group having taken Fasamax and Samgieumgamibang in this study. Methods: Mean BMD and T-score are measured in all patients before and 3 months after treatment. Results: 1. The group having taken Fosamax showed no statistically significant mean BMD change before and after treatment. 2. The group having taken Fosamax showed no statistically significant T-score change before and after treatment. 3. The group having taken both Fosamax and Samgieumgamibang showed increase in mean BMD, which does not bear statistically significant change. 4. The group having taken both Fosamax and Samgieumgamibang showed statistically significant T-score change before and after treatment(p<0.05). Conclusions: After treatment, BMD in patients having taken Fosamx did not change significantly, but in patients having taken both Fosamax and Samgieumgamibang increased, which proves the efficacy of oriental herb medicine for osteoporosis patients.

  • Osteoporosis is a common disease characterized by bone mass reduction, leading to an increased risk of bone fracture, and it is caused by an imbalance of osteoblastic bone formation and osteoclastic bone resorption. Current osteoporosis drugs aim to reduce the risk of bone fracture, either by increasing osteoblastic bone formation or decreasing osteoclastic bone resorption. However, osteoblasts and osteoclasts are closely coupled, such that any reagent altering the differentiation or activity of one eventually affects the other. This tight coupling between osteoblasts and osteoclasts not only limits the therapeutic efficacy but also threatens the safety of osteoporosis drugs. This review will discuss the biological mechanisms of action of currently approved medications for osteoporosis treatment, focusing on the osteoblast-osteoclast coupling.

  • There is tremendous interest of research which surrounds the concept of 'osteoporosis,' as shown by the intense and growing research activity in the field. The urgency to advance knowledge in this area is motivated by the need to understand not only the causes, diagnosis and treatment but also need for early identification or detection of this silent disease. Despite the various researches work is going on, important issues remain unresolved. In this paper, Osteoporosis has also been discussed with respect to biological, engineering, biochemical and physical aspects. The diagnostic and therapeutic techniques have been described for osteoporosis, for better health care. The novelty of the review paper lies in clarifying several myths, explaining the disease in details with biomedical engineering aspects and focuses on the several detection techniques, providing a new direction for early diagnosis of this deadly disease and gives new directions for the POCT device for Osteoporosis.

Incidence and severity of medication-related osteonecrosis of the jaw in patients with osteoporosis using data from a Korean nationwide sample cohort in 2002 to 2019: a retrospective study

  • Background: Medication-related osteonecrosis of the jaw (MRONJ) is a significant concern, particularly among patients taking bisphosphonates (BPs), denosumab, and selective estrogen receptor modulators (SERMs) for osteoporosis. Despite the known risks, large-scale cohort studies examining the incidence and severity of MRONJ are lacking. We aimed to ascertain the incidence and risk of MRONJ among these patients, whom we stratified by age groups, medication types, and duration of use. Methods: We utilized data from the National Health Insurance Service's sample cohort database, focusing on patients aged 40 years and above diagnosed with osteoporosis. The patients were divided into three groups: those prescribed BPs only, those prescribed SERMs only, and those prescribed both. Results: The overall incidence rate of MRONJ was 0.17%. A significantly higher incidence rate was observed among those taking osteoporosis medications, particularly among females with a relative risk of 4.99 (95% confidence interval, 3.21-7.74). The SERM group also had an incidence rate comparable to that of the BP group. Severity was assessed based on the invasiveness of the treatment methods, with 71.3% undergoing invasive treatment in the medication group. Conclusion: This study provides valuable insights into the incidence and severity of MRONJ among a large cohort of patients with osteoporosis. It underscores the need for comprehensive guidance on MRONJ risks across different medication groups and sets the stage for future research focusing on specific populations and treatment outcomes.

  • The average life expectancy has been increased, so the proportion of elderly patients that visit to the dental clinic for prosthetic restoration has increased. Elderly patients have various chronic diseases. Recent trends show an increase of osteoporosis in elderly patients, and thus, the number of osteoporosis patients is expected to escalate. Currently, the most widely used drug for osteoporosis is bisphosphonate. However, osteonecrosis of the jaw has been reported as a side effect derived from longterm oral administration or injection treatment of the drug. Surgical dental treatment was the main cause of medication related osteonecrosis of the jaw (MRONJ). As MRONJ is very difficult to cure, it is important to take preventive measures. Surgical operation may be needed for the mouth preparation before prosthetic restoration. For successful treatment, the dentist should have a full understanding of osteoporosis and show a continued interest toward this disease for careful management.

The Effects of Aquatic Exercise on the Osteoporosis of Rats Caused by Ovariectomy (난소적출로 유발된 골다공증 백서에 대한 수중운동의 효과)

  • Osteoporosis is the most prevalent metabolic bone disease and is characterized by diminished bone strength predisposing to an increased risk of fracture. Thus, this study was performed (1) to examine the effect of aquatic exercise on osteoporosis caused by an ovariectomy in the rats, and (2) to elucidate involvement of histopathologic findings in mediating these actions. The results of this study were as follows: 1. Body weight of the subjects increased after the experiment. There was a statistically significant difference (p<.05) between Group I (sham operated) and Group II (control group). 2. In respect to the length, circumference and weight of the femur, there was no statistically significant difference between the control group and the other groups (p>.05). 3. With the osteocalcin level used as one of the bone formation indexes the bone mineral density increased in every group in comparison with the control group, and in particular, there was a statistically significant difference between Group III and Group IV. The above results suggest that aquatic exercise treatment is effective in the prevention and treatment of osteoporosis.

A Efficacy of Trigonellae Semen On Treatment of Osteporosis In Ovariectomized Rats (호로파(胡蘆巴)가 골다공증(骨多孔症) 흰쥐의 골밀도와 체중에 미치는 영향)

  • Objectives : The present study had been undertaken to investigate the effects of Trigonellae Semen on treatment of osteoporosis in ovariectomized rats. Methods : In this experiment, the rats were ovariectomized. Rats were administered by water extracts of Trigonellae Semen. The levels of bone mineral density and body weight were measured. Results : The levels of spinal bone mineral density was significantly increased in comparison with OVX group at 4 and 8 weeks after medication in Trigonellae Semen group. The levels of femoral and fibula-tibial bone mineral density were significantly increased in comparison with OVX group at 4 weeks after medication in Trigonellae Semen group. The levels of body weight was significantly increased in comparison with OVX group at 4 weeks after medication in Trigonellae Semen group. Conclusion : Reviewing these experimetal results, it appears that Trigonellae Semen have efficacy on treatment of osteeoporosis. And further study should be conducted to illustrate in depth the curing and prevention of osteoporosis.

  • As the population grows older and medical treatments are advancing, the number of spine surgeries in elderly patients has been increasing. To obtain a successful outcomes of spine surgery in elderly patients, surgeons should prepare meticulously because elderly patients can have osteoporosis and surgery can be more extensive than in younger patients. Therefore, this study reviewed the perioperative medical treatment, particularly for osteoporosis, to improve the surgical outcomes in elderly patients.

Network Analysis of Herbs that are Frequently Prescribed for Osteoporosis with a Focus on Oasis Platform Research (골다공증 다빈도 처방과 구성 약물의 네트워크 분석 - 오아시스 검색을 중심으로)

  • Objectives: This study analyzed, through network analysis and data mining analysis, the relationship between herbs used in osteoporosis prescriptions, diversified the analysis of osteoporosis-related prescriptions, and analyzed the combination of herbs used in osteoporosis-related prescriptions. Methods: The prescriptions used in osteoporosis treatment and experiments were established by conducting a full survey of the papers published by the OASIS site. A database for osteoporosis-related prescriptions was established, herbs were extracted, and the frequency of frequent herbs and prescriptions were investigated using Excel (MS offices ver. 2013). Using the freeware R version 4.0.3 (2020-10-10), igraph, and arules package, network analysis was performed in the first second of prescription composition. Results: Among the osteoporosis-related prescriptions, the most studied prescriptions are as follows.: Yukmijihwang-tang (六味地黃湯) and Samul-tang (四物湯). In the osteoporosis prescription network, herbs with connection centrality, proximity centrality, mediation centrality, and eigenvector centrality appeared in the order of Rehmanniae Radix Preparata, Angelicae Gigantis Radix, Poria Sclerotium, Paeoniae Radix, and Glycyrrhizae Radix et Rhizoma. After extracting the herbal combination network, including the corresponding herbs, and clustering it, it can be divided into drugs of the Yukmijihwang-tang (六味地黃湯) series and the Samul-tang (四物湯). Conclusions: This study could assist researchers in diversifyingy formula analysis in future studies. Moreover, the herbal combination used in osteoporosis prescriptions could be used to search for osteoporosis prescriptions in other databases or to create a new prescription.

  • Osteoporosis is a major underlying cause of bone fracture in postmenopausal women and older person in general, and so it is a major public health problem. It is a condition in which bone mass decreases, causing bones to be more susceptible to fracture. A trivial trauma can easily cause one or more bones to break in a person with severe osteoporosis. Physicians and patients alike are concerned with the optimum approach to the treatment and prevention of osteoporosis. The diagnostic method and proper use of agents, such as calcium, vitamin D, estrogen, and fluorides, as well as the role of exercise are issues that have generated major research efforts. So these are to be mentioned in this paper.
