PNF 매트 운동이 뇌졸중 환자의 균형수행력 향상에 미치는 효과 (The Effect of Mat Activities in PNF on Improvement of Balance Performance in Stroke Patients)

  • Purpose : The purpose of this study was to investigate the effect of mat activities in PNF on improvement of balance performance in stroke patients. Methods : Included 12 persons with stroke who were living in the community. They participated in exercise class for 60minutes duration three times a week for 6 weeks. Mat activities in PNF program was modified PNF patterns and techniques on various posture. Balance indexes were measured at pre-treatment, post-treatment and follow-up by K.A.T.3000. Results : Balance index scores had larger improvements after 6 weekends of treatment(p<.05) than pre-treatment, also follow-up test(p<.05). Conclusion : The result of this study showed that PNF mat activities intervention can improve balance performance in disabled persons after stroke.

상악 전치부 다발성 수평 치근 파절의 치료: 증례보고 (Treatment of Multiple Horizontal Root Fractures of the Maxillary Anterior: A case report)

  • Objectives: Management of a horizontal root fracture of an anterior teeth is challenging and often requires multiple approaches for improving the functional and esthetic outcomes. This case report describes the treatment and 2-yr follow up of 3 maxillary incisors with horizontal root fracture. Two maxillary central incisors were treated with Mineral Trioxide Aggregate (ProRoot MTA, Dentsply, Tulsa, OK, USA). Left maxillary lateral incisors were treated with endodontic treatment and submerged. During 2-yr of follow-up evaluation, the root-fractured teeth of the present patients were well retained in the arch, showing periodontal healing even after endodontic treatment.

전폐절제술 후 생긴 기관지 흉막루의 치료 (Treatment of Postpneumonetomy Bronchopleural Fistula)

  • A bronchopleural ~stula[BPF] is a major therapeutic challange to the thoracic surgeons due to difficulties in its management. We have experienced 9 cases of postpneumonectomy BPF, and all of them occured after the right side pneumonectomy. Seven patients underwent pneumonectomy due to lung cancer, and one patient each received pneumonectomy due to bronchiectasis and pulmonary tuberculosis. The most frequently used method of treatment was Eloesser operation which was done in 7 patients, and one had open thoracostomy and one had betadine irrigation until he died suddenly due to pulmonary vein rupture. The result of treatment was poor with five mortalities including one operative death. Two patients are in good condition in 28 months and 4 years of follow-up, but two are lost in follow-up. Tissue sealant such as Tisseel was used in three patients but seems to have no effect at all in the treatment of postoperative BPF.

  • Purpose: Axillary osmidrosis is caused by excessive apocrine gland secretion, which causes an unpleasant odor. Axillary osmidrosis causes difficulty in social activities and personal handicap. We studied the long term follow up results of our surgical procedures and sought to find a best surgical treatment methods. Methods: From January 1991 to December 2006, a total of 1864 patients(587 men, 1277 women) had been treated with this procedure for axillary osmidrosis and hyperhidrosis. Follow up periods varied from 10 to 15 years. We used the subdermal excision technique, using two small incisions over the axilary folds and using a Kawata dissector(instrumental shaving) and additional manual subdermal excision(manual shaving). Results: Among the total patients, 782 patients were followed up. Except a pure hyperhidrosis patients, 759 osmidrosis patients was included. 588 patients(77.4%) had a good results, 148 patients(19.5%) had a moderate results and 23 patients(3.1%) had a recurrence. Complication developed in 189 patients(10.1%): hematoma or seroma in 25 patients, wound dehiscence in 86 patients, partial skin necrosis in 45 patients, and infection in 28 patients. Conclusion: We conclude that our method has several advantages such as 1) short operation time, 2) minimal scaring, 3) lower complication rates, 4) high satisfactory rates.

Management of Precancerous Cervical Lesions in Iran: A Cost Minimizing Study

  • Background: Cervical cancer is a common, preventable and manageable disease in women worldwide. Objectives: This study was conducted to determine the cost of follow-up for suspicious precancerous cervical lesions within a screening program using Pap smear or HPV DNA test through the decision tree. Materials and Methods: Patient follow-up processes were determined using standard guidelines and consultation with specialists to design a decision tree model. Costs of treatment in both public and private sectors were identified according to the national tariffs in 2010 and determined based on decision tree and provided services (visits to specialists, colposcopy, and conization) with two modalities: Pap smear and HPV DNA test. The number of patients and the mean cost of treatment in each sector were calculated. The prevalence of lesions and HPV were obtained from literature to estimate the cost of treatment for each woman in the population. Results: Follow-up costs were determined using seven processes for Pap smear and 11 processes for HPV DNA test. The total cost of using Pap smear and HPV DNA process for each woman in the population was 36.1$ and 174$ respectively. Conclusions: The follow-up process for patients with suspicious cervical lesions needs to be included in the existing screening program. HPV DNA test is currently more expensive than Pap smear, it is suggested that we manage precancerous cervical lesions with this latter test.

Treatment of Proximal Humeral Fracture Using Polarus Nail and Philos Plate

  • Background: To compare the treatment of the proximal humerus fracture using a Polarus nail or Philos plate, we aimed to analyze the functional recovery and the factors affecting the selection between the two types of surgery. Methods: The study included 107 patients with proximal humerus fracture who underwent surgery at our institution. Of these patients, 67 underwent surgery with Polarus nails (G1) and 40 with Philos plates (G2). In G1, the cases of two- and three-part fractures were 60 and 7 cases, in G2, the cases of two-, three-, and four-part fractures were 28, 10, and 2 cases, respectively. The average age was 61 years old, and the average follow-up period was 32.5 months. We compared radiological results, the functional recovery retrospectively. Results: The radiological union time was 6.8 weeks and 8.7 weeks on average in G1 and G2 (p < 0.05). At the one-year follow-up period, these were visual analogue scale (VAS) 1.355, forward flexion (FF) 130.968, external rotation (ER) 50.161, internal rotation (IR) L2 in G1, and VAS 0.781, FF 135.806 ER 51.25, IR L1 in G2, respectively, showing no significant differences between the two groups (p > 0.05). Similar observations were made at the final follow-up. In terms of functional recovery, no significant differences were seen at the one-year or at the final follow-up period (p > 0.05). Conclusions: For the surgical treatment of proximal humeral fracture, the selection of the type of surgery is affected by the fracture pattern. However, both methods give satisfactory outcomes and do not show significant differences in the functional outcome after the surgery.

Treatment of Acute Acromioclavicular Joint Injuries Using AO Hook Locking Plate

  • Background: To evaluate clinical and radiological outcome using AO hook locking plate in acute acromioclavicular joint injuries. Methods: This study was based on patients with Rockwood type 3 or 5 acromioclavicular joint injuries who received surgery with AO hook locking plate from June 2008 until June 2009. Among the 22 patients, 19 of them were male and 3 were female, the mean age was $44.4{\pm}15.57$ years (20-72 years) and follow-up period was $15.5{\pm}3.90$ months (12-23 months). Preoperatively, postoperatively, and at the final follow-up after the plate removal, both coracoclavicular distances were measured from the anteroposterior radiograph. Also, the Shoulder Rating Scale of the University of California at Los Angeles scores (UCLA scores), the American Shoulder and Elbow Surgeons scores (ASES scores), Constant scores, and the Korean Shoulder Society scores (KSS scores) were measured at the final followup to evaluate the function of the shoulder joint. Results: At the time of injury, the mean coracoclavicular distance of the injured side was $17.69{\pm}4.23mm$ (9.57-27.82 mm) and the unaffected side was $7.55{\pm}2.20mm$ (3.24-13.05 mm). The mean coracoclavicular distance measured postoperatively and at the final follow-up was $6.87{\pm}2.34mm$ (4.07-14.13 mm) and $8.47{\pm}2.96mm$ (4.37-17.48 mm), respectively. The mean UCLA, ASES, Constant, and KSS scores measured in the final follow-up were $33.5{\pm}1.30$ (31-35), $90.8{\pm}8.36$ (72-100), $78.6{\pm}8.80$ (62-100), and $94.4{\pm}5.08$ (84-100) each. Conclusions: From this short-term research, the surgical treatment using AO hook locking plates in acute acromioclavicular joint injuries is clinically and radiographically satisfying and considered as a useful treatment method.

초기 족관절 골관절염 환자에서의 히알루론산 관절강 내 주사 요법 (Efficacy of Intra-articular Hyaluronic Acid Injection in Early Stage Ankle Osteoarthritis)

  • Purpose: This study evaluated the efficacy of an intra-articular injection of sodium hyaluronate in the treatment of early stage osteoarthritis of the ankle. Materials and Methods: Early stage ankle osteoarthritis (Takakura stage I and II) patients who received hyaluronic acid injection therapy were retrospectively reviewed. Patients underwent intra-articular injections of 2 mL of sodium hyaluronate for 3 consecutive weeks. Clinical evaluation and Visual Analogue Scale (VAS) scoring were performed at every three months after treatment. Results: Total 27 patients were involved and mean age was 55 year old (range 33 to 77 years). There were 13 male and 14 female patients. Mean follow up duration was 13 month and. Pre-intra-articular VAS score was $8.9{\pm}0.7$ and three month follow up score was $3.8{\pm}2.8$. VAS score of last follow up was $3.2{\pm}3.4$. The effect of hyaluronic acid continued about one year when analyzed the VAS score change of the patients followed for more than one year. Patients' satisfaction was "very satisfied" in nine, "satisfied" in 12, "fair" in one, and "not satisfied" in five patients. Overall satisfaction rate was 82%. There were no ankle osteoarthritis stage changes in serial follow up radiograph. Conclusion: Symptomatic relief of signs and symptoms of osteoarthritis of the ankle was achieved by injection of an intra-articular hyaluronate injection. Efficacy of Hyaluronate acid injection persisted more than 1 year in our study. Intra-articular hyaluronate injection to ankle osteoarthritis is safe and effective as knee joint and should be considered as a valid conservative treatment for ankle osteoarthritis.

폐결핵의 절제요법 640례에 대한 수술 및 원격성적 (Resection in Pulmonary Tuberculosis: Results and Follow-up of 640 Cases)

  • Pulmonary resectlon has evolved as the treatment of choice in certain pulmonary tuberculosis cases. This study represents an analysis of 640 cases of pulmonary resections between March, 1953 and August, 1966 and 370 cases of 13 years follow-up results at the 36th Army Hospital. 1] The ages ranged from 20 to 45 years and all cases were males. 2] The extent of disease revealed 116 Far advanced, 472 Moderate advanced and 52 Minimal Cases. 3] The extent of resections were 373 lobectomies, 130 segmental resections, 58 pneumonectomies,25 multlple lobectomies, 25 lobectomies with thoracoplasties, 22 lobectomies with segmental resections, and 7 wedge resections. 4] The postoperative complications occurred in 71 cases[11.09 percent]. Of these complications,bronchopleural fistula occurred in 13 cases [2.03 percent]. 5] The early operative mortality within 24 hours was 2.0 percent and late mortality within 6 months was 1.4 percent, a total mortality from all causes of 3.4 percent: 15.5 percent following pneumonectomy, 2.1 percent following lobectomy, 1.5 percent following segmental resectlon. 6] All 370 patients were followed for periods ranging from 6 months to 5 years and 5 years to 13 years. Of these former group of 241 patients, 73.3 percent of the cases had returned to full active llfe and remained well, while 15.4 percent were still under treatment. Of these latter group of 129 patients, 76.0 percent of the cases were well and 4.7 percent were still under treatment. 7] In the follow-up results according to extent of disease, the cure rate was greatest in cases of minimal group and lowest in cases of far advanced group. 8] In the results by extent of resection, the cure rate was greatest in cases of lobectomy group. 9] Through the all follow-up periods, 11 patents [3.0 percent] were died. Of these, 3 were suicide and 8 were unknown causes.

Endoscopic Ultrasonography in Pediatric Patients with Pancreatobiliary Disease: Single-Center Trial

  • Purpose: The use of Endoscopic ultrasonography (EUS) in pediatric patients is not as common as in adults. The aim of this study is to evaluate the role of EUS in the diagnosis of pancreatobiliary disease in childhood. Methods: Between December 2016 and January 2018, the findings of patients who underwent EUS were evaluated retrospectively. Results: Of the 41 patients included in the study 25 were girls (61.0%), mean age was 12.2±4.2 years. EUS was performed for biliary colic in 21 (51.2%), for recurrent pancreatitis in 12 (29.2%), for cholecystitis/cholangitis in 5 (12.2%), and for acute pancreatitis in 3 (7.4%) patients. EUS had a significant clinical effect in the decision of treatment and follow-up of 6/21 biliary colic cases, in diagnosis and follow-up of 6/12 recurrent pancreatitis cases, in decision-making and monitoring of invasive procedures (ERCP/surgery) of 3/5 acute cholecystitis/cholangitis and 2/3 of acute pancreatitis cases as well as in follow-up of the other cases. The effectiveness of EUS in determining direct treatment and invasive intervention was 43.9%. None of the patients had complications related to the EUS procedure. Conclusion: Although current guidelines show that EUS can be used in pediatric patients, this is limited to a few published studies. In this study, it is shown that EUS is a safe method for the diagnosis, follow-up and treatment of common pancreatobiliary pathologies in childhood.
