Objective : Several surgical procedures have been reported for the treatment of chronic subdural hematoma (CSDH). We compared the results of treatments for CSDH obtained from one burr-hole craniostomy with closed system drainage with or without irrigation, two burr-hole craniostomy with closed system drainage with irrigation, and small craniotomy with irrigation and closed-system drainage. Methods : Eighty-seven patients with CSDH underwent surgery at our institution from January 2004 to December 2008. Our patients were classified into three groups according to the operative procedure; group I, one burr-hole craniostomy with closed system drainage with or without irrigation (n=25), group II, two burr-hole craniostomy with closed system drainage with irrigation (n=32), and group III, small craniotomy with irrigation and closed-system drainage (n=30). Results : Age distribution, male and female ratio, Markwalder's grade on admission and at the time of discharge, size of hematoma before and after surgery, duration of operation, Hounsfield unit of hematoma before and after surgery, duration of hospital treatment, complication rate, and revision rate were categories that we compared between groups. Duration of operation and hospitalization were only two categories which were different. But, when comparing burr hole craniostomy group (group I and group II) with small craniotomy group (group III), duration of post-operative hospital treatment, complication and recurrence rate were statistically lower in small craniotomy group, even though operation time was longer. Conclusion : Such results indicate that small craniotomy with irrigation and closed-system drainage can be considered as one of the treatment options in patients with CSDH.
Purpose: Oral candidiasis is the most common fungal infection in the oral cavity which is usually diagnosed from clinical findings. A retrospective study was conducted to identify risk factors for oral candidiasis and to characterize the demographic and clinical features of affected patients. Methods: From January 1, 2019 to December 31, 2019, it consisted of 90 oral candidiasis patients diagnosed based on clinical finding and treated with antifungal drugs. As a retrospective study of those people, surveys were conducted on sex, age, systemic disease, a use of dentures, complaints of dry mouth, smoking and alcohol consumption, culture on potato dextrose agar (PDA) medium, culture on chromogenic agar (CA) medium and a duration of antifungal treatment. Results: Among 90 selected patients, the male and female ratio was 41:49. Overall, female had a higher infection rate than male in all age groups. In this study, oral candidiasis was not clearly susceptible to dry mouth, smoking or drinking, wearing dentures and association with systemic disease. Among 90 patients with oral candidiasis, 83 had colonies formed on PDA medium and 53 had colonies formed on CA medium. The duration of antifungal treatment was highest between 5 and 8 weeks. In addition, there was statistical significance between the culture results in CA medium and the duration of antifungal treatment. Conclusions: Generally, old age or infants, dry mouth, smoking, a use of dentures and endocrine abnormalities are risk factors to increase oral candidiasis; however, in this study, it was mainly found in the elderly aged 60 or older regardless of sex and the incidence of oral candidiasis was not obviously related with patients with dry mouth, smoking or drinking, denture wearers and endocrine abnormalities. Interestingly, when the fungi were cultured in CA medium, the duration of antifungal treatment was increased.
Objective: The purpose of this study is to report the clinical treatment of a nighttime sensation of heat on the palms and soles of the feet of a 71-year-old female patient using Sammulhwanggeum-tang. Methods: Sammulhwanggeum-tang was administered for 19 days to reduce the patient's symptoms, which were assessed using a visual analog scale (VAS) and by duration. Results: The heat sensation began to improve after four days and continued to do so for the subsequent 15 days. From May 12, X to May 30, X, the degree of subjective symptoms decreased from a VAS score of 10 before treatment to 0 after treatment, and their daily duration decreased from 10 to 0 hours. In addition, considering that the duration of the patient's symptoms was about 30 days, the duration of the symptoms decreased to 17 days, including the period before the start of treatment. Conclusion: This clinical study suggests that Sammulhwanggeum-tang might be effective in resolving the sensation of heat on the palms and soles of the feet at night.
Delayed onset muscle soreness (DOMS) is a painful condition that arises from exercise-induced muscle damage after unaccustomed physical activities. Various therapeutic interventions have been applied to reduce the intensity and duration of DOMS-related symptoms. Recently, pulsed electromagnetic field (PEMF) intervention has been introduced as an alternative noninvasive treatment for DOMS. This randomized, double-blind, placebo-controlled experiment was conducted to examine the effects of PEMF therapy on DOMS in elbow flexors at 24, 48, and 72 hours after the experimental DOMS induction. Thirty healthy volunteers ($23{\pm}2.4$ yrs, $175{\pm}5.7$ cm, and $74{\pm}7.8$ kg) participated in this study. Each was randomly assigned to a PEMF or placebo group. On the first day, DOMS was induced in the elbow flexors by repeated isokinetic motions at low ($60^{\circ}/s$) and fast ($120^{\circ}/s$) speeds in all subjects. Thereafter, the PEMF group received 15-min daily treatment with a PEMF device. The placebo group received sham treatment of the same duration. Overall, PEMF application was more effective than the sham treatment in reducing the physiological symptoms associated with the DOMS including perceived soreness, median frequency, and electromechanical delay of the surface electromyography. In addition, median frequency and isokinetic peak torque of the PEMF group recovered to the pre-DOMS induction level earlier than the placebo group. In conclusion, this study suggests that PEMF can be applied as a new recovery strategy in reducing DOMS symptoms. Further experiments are required to examine the effect of the PEMF treatment on different types of exercise conditions and to determine the optimal treatment dosage and duration in a real clinical setting.
Purpose: Cholestasis resulting from cytomegalovirus (CMV)-induced hepatitis manifests in 40% of patients with a CMV infection. Ganciclovir treatment in children with CMV infections has proven to be highly effective. Until now, there are very few studies have identified predictive factors for liver biochemistry improvement after ganciclovir therapy. This study aimed to identify the predictors of liver biochemistry improvement in patients with CMV cholestasis after ganciclovir treatment. Methods: A retrospective cohort study was conducted using medical records from Dr. Sardjito General Hospital Yogyakarta, Indonesia from 2013 to 2018. CMV cholestasis was confirmed based on serum CMV IgG and IgM positivity and/or blood and urine CMV antigenemia positivity. Incomplete medical records and other etiologies for cholestasis, such as biliary atresia, choledochal cyst, metabolic diseases, and Alagille syndrome, were excluded. Patient age at cholestasis diagnosis and ganciclovir treatment, duration of CMV cholestasis, history of prematurity, central nervous system involvement, and nutritional status were analyzed and presented as an odds ratio (OR) with a 95% confidence interval (95% CI). Results: CMV cholestasis with ganciclovir therapy was found in 41 of 54 patients. Multivariate analysis showed that a shorter duration of CMV cholestasis (OR: 4.6, 95% CI: 1.00-21.07, p=0.04) was statistically significant for liver biochemistry improvement after 1 month of ganciclovir treatment. The remaining factors that were analyzed were not significant predictors of liver biochemistry improvement in patients with CMV cholestasis after ganciclovir treatment. Conclusion: A shorter duration of CMV cholestasis is the predictor of liver biochemistry improvement after 1 month gancyclovir treatment.
Objectives : The purpose of this study is to report the effectiveness of Korean medical treatment on 222 patients with alopecia areata. Methods : The subjects of this study were 222 patients who had received Korean medical treatment among the patients who visited the Balmer's Korean Medicine Clinic from January 1, 2016 to December 31, 2017. The extent of the alopecia areata was measured every month to determine the improvement. We analyzed the relationship between the types of alopecia areata, the extent of hair loss, duration of illness, age at first visit, and the treatment of alopecia areata. Results : 1. There were 100 males(45.0%) and 122 females(55.0%). The mean age of patients was 35.3 years old. 2. Of the 222 patients, 142(64%) were "cured". 31(14%) showed "good", 21(9.5%) showed "fair", 28(12.6%) showed "poor" improvement. 3. According to clinical records, of the 64 patients with alopecia areata(AA), 48(75.0%) were cured. Of the 100 patients with alopecia areata multiplex(AAM), 68 patients (68.0%), 12(50.0%) out of 24 patients with ophiasis, 5(55.6%) out of 9 with alopecia semi-totalis(AST), 3(50.0%) out of 6 patients with alopecia totalis(AT), 6(31.6%) out of 19 patients with alopecia universalis(AU) were cured. 4. Patients with the wider hair loss extent and with more severe type of alopecia areata required longer treatment period to cure, and the cure rate was lowered(p<0.05). 5. The shorter duration of the disease was related to the higher cure rate(p<0.05). If the treatment was started after six months of the duration, the cure rate was lowered. Therefore, beginning treatment within six months of the onset is recommended. 6. The period until terminal hair showed was 1.5 months for AA, 1.7 months for AAM, 1.6 months for ophiasis, 1.9 months for AST, 2.3 months for AT, 3.2 months for AU. 7. The treatment period to cure was 4.9 months for AA, 7.3 months for AAM, 7.7 months for ophiasis, 6.8 months for AST, 7.7 months for AT, 16.3 months for AU. 8. The major prognostic factors were extent and type of alopecia, duration of the disease and relapse. Conclusions : We analyzed the effectiveness of Korean medical treatment through cure rate according to the prognostic factors. This study will provide useful data for the treatment of alopecia areata.
Objective : This study is designed to find out the effects of indirect moxibustion at Geogol (LI16) on the shoulder pain. Methods : The patients were hospitalized Donghwa oriental medical clinic from 19th Sep. 2006 to 4th Oct. 2006, and treated with indirect moxibution at Geogol (LI16) on the shoulder pain. Results : 1. The effects of treatment by duration was follow : Below 1 month duration was improved highly. 2. The effects of treatment by condition of patients was follow : After treatment, the percentage of above "Good" level was 81%. 3. The effects of treatment by pain region of Meridian part. was follow : pain region of LI(手陽明大腸經) Meridian part was improved highly. Conclusion : According to the results, indirect moxibustion at Geogol (LI16) may be good effects on the shoulder pain.

The response of silkworm, Bombyx mori L. to phytoecdysteroid (PE) when administered at different ages of $5^{th}$ instar was studied in the popular bivoltine ($CSR2{\times}CSR4$) and multi${\times}$bivoltine ($PM{\times}CSR2$) silkworm hybrids, reared on the Victory-1 variety of mulberry leaves. PE was administered to $5^{th}$ instar silkworm per os at a rate of $250{\mu}g$ per 100 larvae to different batches of silkworm at 0, 24, 48, 72, 96, 120, 144 hrs and at the onset of cocoon spinning when a few larvae were ripe. The larval and mounting duration, cocoon yield and cocoon characters were influenced by PE. The intensity of influence was dependent on the time of application. The larvae treated at the beginning of the instar, improved the economic traits significantly with a marginal increase in larval duration. In the larvae treated at the middle of the instar, larval duration was shortened remarkably but the economic traits were adversely affected. This particular treatment can become a good management strategy in the case of mulberry leaf shortage or disease incidence. In the larvae treated at the onset of cocoon spinning, the mounting duration was substantially reduced without much effect on the cocoon traits which would be a big benefit in commercial sericulture. The physiological significance of varied response of silkworm to PE administration is discussed.
Objective : The study was performed to evaluate the effect of treatment for thoracolumbar compression fracture by using Bee Venom Acupuncture that is well known for anti-inflammatory and analgesic effect. Methods : We investigated 39 cases of patient with thoracolumbar compression fracture. The patients with thoracolumbar compression fracture were treated at the department of acupuncture & moxibustion of Kyung-Won University Oriental Hospital from 1st January 2001 to 30th August 2002. We selected for two groups. One group was treated by Bee Venom Acupuncture therapy(Bee Venom Acupuncture Group : BAG), the other group was treated by Filiform Acupuncture therapy(None Bee Venom Acupuncture Group : NBAG). Results : 1. The distribution showed female predominance in sex and 70's predominance in age. The largest group was "accidental fall" and the next was "lifting heavy objects" in causes, and within 2 weeks in the duration of disease 2. In the duration of admission, the largest group was within 4 weeks. In regard to level of injury, T12 was found to be most predominant, followed in turn by L1, L2, L5. In regard to grade of clinical symptoms, Grade III was most predominant, followed by Grade II, Grade IV. 3. In the results of treatment, 87.5% were above "Good" in BAG and 47.8% in NBAG. 4. In the result of treatment due to the grade of clinical symptoms, BAG was more excellent than NBAG on the whols grade. 5. In the result of treatment due to duration of admission, the duration of admission was not in proportion to the results of treatment. 6. In the change of lumber flexion, the improvement above $80^{\circ}$ was 50% in BAG and 21.7% in NBAG 7. In the stability of treatment, the BAG and NBAG did't show any detrimental change in GOT, GPT, r-GTP, BUN, creatinine. Conclusions : In the treatment of thoracolumbar compression fracture, the Bee Venom Acupuncture can be regarded as more effective treatment than Filiform Acupuncture in the clinical practice. This is expected to be available for clinical use.
This study was conducted to investigate the possible installation of small hydro-power plant at the discharge point of Gumi sewage treatment plant (STP) using treated wastewater. Sufficient amount of water to transfer to electric power and the selection of proper location are two essential elements for the construction of small hydro-power facility. Preliminary analysis based on site visit and existing data in Gumi STP were made. Capacity of the small hydro-power plants and exact location were determined by geomorphological condition and flow duration characteristics. Flow duration characteristics and its duration curve were identified using monthly rainfall data in Gumi STP. Relevant facts of small hydro-power system in other STP were referred to adopt to Gumi STP situation. Flowrate of treated effluents and effective head between flow chamber and the location of hydraulic turbine in Gumi STP are found to be $3.70m^3$/sec and 3.5m respectively. Electric generation rate based on this feasibility study was estimated to be 86.3kW/h. Yearly electric generation rate was expected to be 932.4MMh. Proposed small hydro-power plant construction in Gumi STP is to be reasonable.
