A new approach to the solubilization of waste activated sludge (WAS) using alginate-quaternary ammonium complex beads was investigated under controlled mild alkaline conditions. The complex beads were prepared by the reaction of sodium alginate (SA) with 3-(trimethoxysilyl)propyl-octadecyldimethylammonium chloride (TSA) in acid solution, followed by crosslinking with $CaCl_2$. Treatment of WAS with SA-TSA complex beads was effective for enhancing the efficacy of WAS solubilization. The highest value of soluble chemical oxygen demand (SCOD) concentration (3,900 mg/L) was achieved after 10 days of treatment with 30% (v/v) SA-TSA complex beads. The WAS solubilization efficacy of the complex beads was also evaluated by estimating the concentrations of volatile fatty acids (VFAs). The maximum value of VFAs was 2,961 mg/L, and the overall proportions of VFAs were more than 75% of SCOD. The main components of VFAs were acetic, propionic, iso-butyric, and butyric acids. These results suggest that SA-TSA complex beads might be useful for enhancing the solubilization of WAS. The potential use of VFAs as the external carbon substrate for the production of polyhydroxyalkanoate (PHA) by a mixed microbial culture (MMC) was also examined. The enrichment of PHA-accumulating MMC could be achieved by periodic feeding of VFAs generated from WAS in a sequencing batch reactor. The composition of PHA synthesized from VFAs mainly consisted of 3-hydroxybutyrate. The maximum PHA content accounted for 25.9% of dry cell weight. PHA production by this process is considered to be promising since it has a doubly beneficial effect on the environment by reducing the amount of WAS and concomitantly producing an eco-friendly biopolymer.
The 3rd Japan-Korea Workshop on Acupuncture and EBM was held at Kanazawa on June $16^{th}$. From Korea team, 4 papers were presented. Dr. Hahn introduced a new approach of data analysis on series of n-of-1 trials using the Bayesian statistics. It offered important information for the future n-of-1 trials. Dr. Park clearly demonstrated the significance of various sham devices proposed and stressed the importance of research questions when we choose the control intervention in RCT. Dr. Lee reported the results of survey in Korean Medical Doctors (KMD) for their point selection and techniques to the distal and local points. Dr. Kim presented the results of face to face survey on the KMD with 28 items for acupuncture treatment on the knee OA. Finally, a draft of protocol was introduced by Dr. Kim. The title was "multi-center, a randomized, single blinded, two arms, parallel-group study to compare the effectiveness and safety of 'individualized acupuncture' and 'standardized minimal acupuncture' in Korean and Japanese patients with knee osteoarthritis (Phase IV)". From Japan team, 7 speakers presented their comments and proposals on the protocol. Dr. Takahashi introduced several issues regarding n-of-1 trials and pointed out the importance of obtaining generalizability from n-of-1 trials. Dr. Shichidou pointed the importance of research design, selection of outcome measures and reduction of biases. Dr. Itoh presented the results of point selection for the knee OA based on the literature survey. Dr. Sumiya introduced several differences between KMD and Japanese acupuncturists based on the questionnaire used in KMD survey. Dr. Furuya demonstrated a result of press tack needle and its sham device on shoulder stiffness. Dr. Yamashita introduced the results of literature survey regarding adverse events occurred by acupuncture on knee OA. Dr.Tsukayama stressed the importance of responsibility of Institutional Review Board (IRB) for the conduction of clinical trials. After several issues were discussed, the need of continued meeting for final protocol development was agreed, then the workshop was closed.
The purpose of this study was to identify the effect on improvement of the Activity of Daily Living (ADL) and decrease the cognitive function and agitation behaviors by reminiscence with audio-visual stimulation for senile dementia. The quasi-experimental design was used in this study. Subjects were 26 with mild senile dementia who were cared for at a Day Care Center for Dementia in Seoul. The data were collected from March to July, 1999. Subjects were divided into three groups : Control Igroup with 10 subjects, reminiscence group(Control II group with 8 subjects), and reminiscence with audio-visual stimulation group(experimental group with 8 subjects). The Control I group got routine care as usual. Control II group participated in reminiscence sessions for one hour a day, five times a week , for a period of 4 weeks. The experimental group participated in reminiscence with audio-visual stimulation sessions for one hour a day, five times a week, for a period of 4 weeks. Instruments of this study were color photography with sound that was developed through an open questionnaire about events, objects, humans in action and animals that 100 Korean elderly over 60 would like to memorize. This was referred from the Sensory Stimuli Package by Namazi and Haynes(1994). The effects of treatment was evaluated through MMSE-K by Kwon & Park(1989). Also the Brief Cognitive Rating Scale(BCRS) by Reisberg et al(1983) for the cognitive function, through Agitation Inventory by Cohen- Mansfield and Colleague(1989) for behavioral response and through the Rapid Disability Rating Scale-2(RDRS-2) by Linn & Linn(1982) for the activity of daily living respectively. Data analysis was done using SPSS for $\chi$2- test, ANOVA, repeated measures ANOVA. The results were as follows : 1. Reminiscence with audio-visual stimulation did not improve cognitive function for senile dementia, but significantly improved verbal expression, the subscale of cognitive function. 2. Reminiscence with audio-visual stimulation reduced agitation behavior of experimental group significantly, but there was no significant difference between groups. 3. Reminiscence with audio-visual stimulation did not significantly effect the activity of daily living after treatment. In conclusion, it was shown that the reminiscence with audio-visual stimulation was an effective therapy to improve verbal expression and to reduce agitation behaviors of senile dementia. Further research with more indepth approach is needed, considering characteristic and level individualized for each senile dementia.

The Dentigerous cyst is reported to be one of the most common lesions of the jaw. It is derived from reduced enamel epithelium that surrounds the crown of an unerupted tooth. Dentigerous cysts usually remains asymptomatic and presents as well-circumscribed radiolucencies surrounding the crown of a tooth. Most dentigerous cysts are treated by surgical enucleation or marsupialization of the cyst to allow eruption of the permanent tooth. The nasopalatine duct cysts derived from the islands of epithelium remaining after closure of the embryonic nasopalatine duct. The cyst presents as a well-circumscribed oval or heart-shaped radiolucency located in the midline of the anterior maxilla between the roots of the central incisors. Treatment is by surgical enucleation, employing a palatal approach. This report present two cases. One case was diagnosed with dentigerous cyst and treated by enucleation. 12 Months later, affected second premolar was erupted normally. The other case was diagnosed with nasopalation duct cyst. Histological examination of the enucleated cyst confirmed a nasopalatine duct cyst. There was no recurrence during follow up period.

As the minimally invasive approach against white spot like early caries lesions in proximal surfaces of the teeth, therapeutic sealing has been introduced and studied for effective materials and methods to arrest the early caries lesion effectively, which is still going on. This study was performed for the purpose of evaluating its validity for the non-cavitated lesions according to the depth from surface using therapeutic sealing followed by artificial caries induction and evaluation with micro-CT, and we obtained the results as follows. 1. It was revealed that the deeper the caries lesions are, the lower radiation intensity at lesion body areas in pre-treatment specimen. 2. In the sealed groups, there were no differences in radiation intensity between pre- and post-treatment, whereas there were significant decreases in unsealed groups(p<0.05). 3. Even in the specimens with the lesions reaching deeply into DEJ, the effect of sealing was significant(p<0.05). Conclusively, it was thought therapeutic sealing can be an effective tool against the early caries lesions, regardless of their depth into tooth, unless cavitated.
Purpose: Perforated gastric cancer (PGC) is rare and occurs in $1\∼4\%$ of all gastric cancers. Possible dissemination of tumor cells at the time of perforation of the gastric carcinoma has been a matter of concern. The intraoperative determination of what kind of operation should be done and how extensive the lymphnode dissection should be still remains controversial. The purpose of this study is to evaluate the factors influencing the survival and to determine the optimal treatment for PGC. Materials and Methods: A total of 42 patients were operated on for a perforated gastric carcinoma at Soonchunhyang University Chunan Hospital from 1983 to 2000. the age and the sexes of the patients, the location of perforation, the diameter of perforation, the histologic type of the tumor, the depth of wall invasion, the absence or presence of lymph node metastasis / distant metastasis, the stage of disease, the type of operation, and the outcomes were examined. Statistically significant differences were analyzed by using Fisher's exact test. Results: The stage distributions according to the UICC classification were 1 case of stage I, 6 cases of stage II, 17 cases of stage III, and 11 cases of stage IV. An emergency gastrectomy was done in 26 patients ($61.9\%$), with a 5-yr survival rate of $44\%$. The survival of patients was significantly influenced by the depth of wall invasion, the lymphnode metastasis, distant metastasis, the stage of disease, and the type of operation. Conclusions: an emergency gastrectomy is the treatment of choice for most patients with resectable PGC. Choosing more a optimistic surgical approach for potentially curative cases of PGC should be one way to increase the patient's survival rate.
Objectives : The aim of this study was to assess the clinical effect of exercise program used for treatment of activity of daily living skill in patients with dementia. Methods : We searched foreign studies registered from 2007 to 2014 based on PubMed database, using the following search terms : dementia, alzheimer disease, cognitive impairment, exercise program, physical activity, activity of daily living, ADL. Total 5 studies included from 215 studies after analysis papers. Results : selected paper's average pedro score was 6, Katz index of ADLs, Bathel ADL index, IADL activity of daily living assessment tool were used. there was significant improves of activity of daily living skill in the use of exercise program groups and it prevent decreased of activity of daily living skill. Conclusion : This study provide evidence of exercise program applied in clinical use and demonstrate effective on activity of daily living ability. This results expected to help suggesting basic data for exercise program to improve activity of daily living ability and making effective treatment approach method in the future.
Background: Soft tissue sarcomas (STS) must be managed with a team involving pathologists, radiologists, surgeons, radiation therapists and medical oncologists. Treatment modalities and demographic charasteristics of Turkish STS were analysed in the current study. Material-Methods: Primary adult STS followed between 1999-2010 in Cukurova University Medical Faculty Department of Medical Oncology were analzied retrospectively Results: Of the total of 498 patients, 238 were male and 260 female. The most seen adult sarcomas were leomyosarcoma (23%). Localization of disease was upper extremity (8.8%), lower extremity (24.7%), head-neck 8.2%, thoracic 8%, retroperitoneal 5.6%, uterine 12.4%, abdominal 10%, pelvic region 3.6 and other regions 10%. Some 13.1% were early stage, 10.2% locally advanced, 8.2% metastatic and 12.2% recurrent disease. Patients were treated with neoadjuvant/adjuvant (12%) or palliative chemotherapy (7.2%) and 11.4% patients did not receive chemotherapy. Surgery was performed as radical or conservative. The most preferred regimen was MAID combination chemotherapy in the rate of 17.6%. The most common metastatic site was lung (18.1%). The overall survival was 45 months (95%CI 30-59), 36 months in men and 55 months in women, with no statistically significant difference (p=0.5). The survival rates were not different between the group of adjuvant and palliative chemotherapy (respectively 28 versus 18 months) (p=0.06), but radical surgery at 37 months was better than 22 months for conservative surgery (p=0.0001). No differences were evident for localization (p=0.152). Locally advanced group had higher overall survival rates (72 months) than other stages (p=0.0001). Conclusion: STS can be treated successfully with surgery, chemotherapy and radiotherapy. The survival rates of Turkish people were higher in locally advanced group; these results show the importance of multimodality treatment approach and radical surgery.
In cervical cancer, one of the most common malignant tumors in women worldwide, miR-126 has been reported to exhibit decreased expression. However, its role in cervical cancer cell proliferation and drug sensitivity has remained relatively unexplored. Here, we compared the expression of miR-126 in cervical cancer tissues (n = 20) with that in normal cervical tissue (n = 20) using quantitative RT-PCR. The viability of Siha cervical cancer cells was further measured by MTT assay after transfection with miR-126 mimic (Siha-miR-126 mimic) or microRNA mimic negative control (Siha-miR mimic NC) and after treatment with various concentrations of bleomycin (BLM). IC50s were calculated, and the survival rates (SRs) of Siha cells were calculated. miR-126 expression in cervical cancer tissue was significantly decreased compared with that in normal cervical tissue (P < 0.01). The relative SRs of Siha-miR-126 mimic cells were also significantly decreased compared with those of Siha-miR mimic NC cells at 24-96 h after transfection. The IC50 of BLM in Siha-miR-126 mimic cells ($50.3{\pm}2.02{\mu}g/mL$) was decreased compared with that in Siha-miR mimic NC cells ($70.5{\pm}4.33{\mu}g/mL$) at 48 h after transfection (P < 0.05). Finally, the SRs of Siha-miR-126 mimic cells were significantly lower than those of SihamiR mimic NC cells after cultured in medium containing 40 ${\mu}g/mL$ BLM for 24-96 h (P < 0.05). These results suggest that miR-126 is expressed at low levels in cervical cancer. Upregulation of miR-126 inhibited cervical cancer cell proliferation and enhanced the sensitivity to BLM. Thus, miR-126 may represent a novel approach to cervical cancer treatment.

This study reviewed the stuttering literature and reported the clinical experiment in stuttering intervention. There is still no single answer as to the cause of stuttering or to the most effective therapy for stutterers despite the vast amount of research. One certain thing is that we have come closer to a better understanding of the stuttering and to more effective therapy. There have been three main statements about the origins of stuttering ; biologic origins ; psychodynamic origins ; environmental-learning origins. There also have been various methods of the treatment of stuttering. Broadly, two major treatment approaches are attentive ; stuttering modification therapy and fluency shaping therapy. In this experiment, the researcher attempted to investigate complex elements that each child might have and to use an integrative approach rather than to keep the specific one. Individual subjects were evaluated by a multidisciplinary team. Initially, the subjects received individual therapy. They then were placed in group therapy. The purpose of the group therapy was to raise their fluencies to the higher communicative situation and to maintain improved fluency over time. All three subjects improved their fluencies in reading and in conversation and showed the better(SSI)scores in total stuttering behaviors. It was also discussed that it is necessary to have sensitive assessment tools to investigate each element of stuttering ; and to develop a therapy program reflecting current advanced stuttering theories.
