Use of ultrasound Doppler to determine tooth vitality in a discolored tooth after traumatic injury: its prospects and limitations

  • When a tooth shows discoloration and does not respond to the cold test or electric pulp test (EPT) after a traumatic injury, its diagnosis can be even more difficult due to the lack of proper diagnostic methods to evaluate its vitality. In these case reports, we hope to demonstrate that ultrasound Doppler might be successfully used to evaluate the vitality of the tooth after trauma, and help reduce unnecessary endodontic treatments. In all three of the present cases, the teeth were discolored after traumatic injuries and showed negative responses to the cold test and EPT. However, they showed distinctive vital reactions in the ultrasound Doppler test during the whole observation period. In the first case, the tooth color returned to normal, and the tooth showed a positive response to the cold test and EPT at 10 wk after the injury. In the second case, the tooth color had returned to its normal shade at 10 wk after the traumatic injury but remained insensitive to the cold test and EPT. In the third case, the discoloration was successfully treated with vital tooth bleaching.

변색된 치아의 심미성 회복에 관한 임상증례 (ESTHETIC IMPROVEMENT OF DISCOLORED TEETH : A CASE REPORT)

  • There were two cases of esthetic improvement of discolored teeth. First case was 12-year-old girl whose upper right central incisor was discolored after traumatic injury. She was treated by walking bleaching technique. Endodontic retreatment was done, and then the mixture of Superoxol and sodium perborate was applied and changed every week. After 3 weeks, esthetic result was obtained and fracture site was rcovered with light-curing composite resin. Second cases was 11-year-old girl whose teeth were discolored by fluorosis. She was treated by vital bleaching technique. A resin splint was made and then 30% and 15% carbamide peroxide was applied using this splint. After 4 weeks, a noticeable improvement in the color and the brightness of the teeth was achieved.

유치열과 혼합치열기 어린이의 상악 절치부 외상 (TRAUMATIC INJURIES TO THE UPPER INCISORS IN PRIMARY AND MIXED DENTITION)

  • 현대인의 생활양식과 환경이 도시화 및 인공화되고 취미활동이 다양화됨에 따라 외상을 받을 수 있는 요인들이 증가하고 특히 어린이 활동량이 많아져 외상의 기회가 잦아지고 있다. 어린이에서 외상이 발생하는 경우, 어린이의 일상적인 활동 뿐 아니라, 사회성 및 정서 발달에도 영향을 미칠 수 있으며 그 보호자에게도 부정적인 영향을 미칠 수 있기에 외상의 발생 빈도를 줄이고 예방하는 것이 중요하다. 본 연구는 유치열과 혼합치열기에 발생하는 치아 외상을 예방하기 위한 기초 자료를 얻기 위하여, 특히 구강악안면부의 외상시 치아 외상을 가장 많이 받는 상악 유절치와 영구절치를 연구하기 위하여, 2002년 6월부터 2003년 5월까지 연세대학교 치과대학병원 소아치과에 내원한 유치열과 혼합치열기의 신환 8,799명중 상악 절치부 외상을 주소로 내원한 어린이 487명(남자 316명, 여자 171명 )의 상악 절치 955개(유절치 543개, 영구절치 412개)를 조사하여 다음과 같은 결과를 얻었다. 1. 상악 절치부 외상이 호발하는 나이는 유절치에서 $1{\sim}3$세, 영구절치에서 7세였으며 남자대 여자의 비율은 1.85:1이었다. 2. 상악 절치부 외상은 유절치와 영구절치 모두 오후 시간에 많이 발생하였으며, 유절치는 외상 장소로 집이 가장 많았고(44.1%), 영구절치는 길에서 다치는 경우가 가장 많았다(17.5%). 3. 외상의 원인으로 유절치와 영구절치 모두 넘어져 다치는 경우가 가장 많았으며(39.8%, 12.9%), 다음으로 부딪히는 경우가 많았다(22.2%, 6.0%). 유절치에서는 다음으로 떨어져 다치는 경우 교통사고, 폭력과 운동에 의한 외상 순이었으며, 영구절치에서는 폭력, 운동, 교통사고, 떨어져 다치는 순이었다. 4. 조사대상 어린이 1명당 유절치는 1.6개, 영구절치는 2.8개에 외상을 입었다. 5. 유절치와 영구절치 모두에서, 치관 파절 양상은 치수미노출 치관 파절이 많았고, 치주조직 외상 양상은 아탈구가 가장 많았다. 최근 어린이의 단체 생활이나 운동과 취미 활동에 참여하는 나이가 예전에 비하여 점차 낮아지고 있어, 이번 연구 결과와 같이, 외상이 발생하는 나이 역시 지속적으로 낮아질 수 있다. 나이에 따른 어린이의 외상을 받는 장소는 특별한 곳이 아닌, 집이나 학교와 같은 일상의 생활공간이기 때문에 단순한 주의만으로도 외상의 발생을 예방할 수 있을 것으로 사료된다.

예후 불량한 치아를 대체하는 제3대구치를 이용한 자가치아이식술 (Autogenous Transplantation of Third Molar to Replace Hopeless Tooth)

  • 임플란트 술식은 그 동안 많은 발전을 하였고, 자연치를 대체할 훌륭한 치료법임에는 부인할 수 없지만 치주인대가 존재하지 않는 구조적 한계점, 발치 직후 시작되는 bundle bone의 흡수는 자연치와 같은 적절한 emergence profile을 만들어내기 어려울 수 있고 이는 심미적인 결과로 이어질 수 있다. 만약 해당 환자의 구강 내에 건전한 제3대구치가 있고, 결손부위의 염증상태가 심각하지 않으면 건전한 제3대구치를 결손된 부위로 이식하여 치아를 재건할 수 있다. 성공적인 자가치아이이식술은 저작기능을 회복하고, 자가치아를 이용하기에 생체 친화적이며, 치조골의 흡수를 예방하는 훌륭한 치료이기 때문에 가능하기만 하다면 첫번째 치료로 충분히 고려될 수 있을 것이다.

Autotransplantation: A biological treatment alternative for a patient after traumatic dental injury

  • Traumatic dental injury is considered a public dental health problem because of a high childhood incidence, high treatment costs, and prolonged treatment time. Although management guidelines for traumatized teeth have been outlined, tooth loss following trauma is occasionally unavoidable. Here, we describe the successful interdisciplinary management of a traumatized central incisor in an 11-year old boy that was extracted because of a poor prognosis and restored by the autotransplantation of an immature donor tooth into the site. The patient underwent orthodontic treatment in order to close the donor site space and bring the autotransplanted tooth to an ideal position. Postorthodontic treatment radiographs and photographs revealed an esthetic and functional natural tooth replacing the lost tooth. The findings from this case suggest that autotransplantation offers unique advantages as a treatment modality for the restoration of missing teeth, particularly in growing children.

Apexogenesis and revascularization treatment procedures for two traumatized immature permanent maxillary incisors: a case report

  • Traumatic injuries to an immature permanent tooth may result in cessation of dentin deposition and root maturation. Endodontic treatment is often complicated in premature tooth with an uncertain prognosis. This article describes successful treatment of two traumatized maxillary central incisors with complicated crown fracture three months after trauma. The radiographic examination showed immature roots in maxillary central incisors of a 9-year-old boy with a radiolucent lesion adjacent to the right central incisor. Apexogenesis was performed for the left central incisor and revascularization treatment was considered for the right one. In 18-month clinical and radiographic follow-up both teeth were asymptomatic, roots continued to develop, and periapical radiolucency of the right central incisor healed. Considering the root development of these contralateral teeth it can be concluded that revascularization is an appropriate treatment method in immature necrotic teeth.

Effect of calcium hydroxide on inflammatory root resorption and ankylosis in replanted teeth compared with other intracanal materials: a review

  • Calcium hydroxide (CH) is the gold-standard intracanal dressing for teeth subjected to traumatic avulsion. A common complication after the replantation of avulsed teeth is root resorption (RR). The current review was conducted to compare the effect of CH with that of other intracanal medications and filling materials on inflammatory RR and replacement RR (ankylosis) in replanted teeth. The PubMed and Scopus databases were searched through June 2018 using specific keywords related to the title of the present article. The materials that were compared to CH were in 2 categories: 1) mineral trioxide aggregate (MTA) and endodontic sealers as permanent filling materials for single-visit treatment, and 2) Ledermix, bisphosphonates, acetazolamide, indomethacin, gallium nitrate, and enamel matrix-derived protein (Emdogain) as intracanal medicaments for multiple-visit management of avulsed teeth prior to the final obturation. MTA can be used as a single-visit root filling material; however, there are limited data on its efficacy due to a lack of clinical trials. Ledermix and acetazolamide were comparable to CH in reducing RR. Emdogain seems to be an interesting material, but the data supporting its use as an intracanal medication remain very limited. The conclusions drawn in this study were limited by the insufficiency of clinical trials.

외상과 다수의 치아 파절을 동반한 골격성 II 급 부정교합 환자의 치험례 (Post-traumatic reconstruction of skeletal Class II malocclusion with multiple teeth fracture)

  • We report the rehabilitation of Class II malocclusion with multiple teeth fracture due to trauma. A multidisciplinary team approach was necessary to treat patient's problems such as bone fracture, malocclusion, and multiple teeth fracture. Emergency conservative treatment, orthodontic treatment and prosthetic restoration successfully restored the occlusion. However, special considerations were needed along the orthodontic treatment process due to the unexpected complications such as ankylosis, root resorption and detection of additional teeth fractures.

외상성 골낭에 관한 연구 (A STUDY OF THE TRAUMATIC BONE CYST)

  • Traumatic bone cyst is a pathologic cavity that is not lined with epithelium. It is, therefore, not a true cyst. It may be a normal variant rather than a disease process. The etiology of the condition is unknown. This condition is occured widely ranging ages(2 to 75years), however, most are ,found during the second decade of life. Radiographically, this condition is radiolucent lesion with well-defined outline, scalloping of superior margins, Cyst enucleation and curettage is the treatment of choice. The authors compared and analyzed the clinicoradiologic features of the five cases of traumatic bone cyst, diagnosed at the Dental college hospital in Chosun University, Kwangju, Korea. The five cases were shown the followed results; 1. 3cases occured in second decade of life & no significant sex differences (M:F, 2:3) All cases occurred in mandible. 2. Two patients complained symptoms, but three cases had no symptom with encountering during routine examination. 3. In 3 of 5cases, teeth vitality existed except one tooth and no checking of teeth vitality in two cases. 4. All cases didn't have any accurate trauma history, but one case was in orthodontic treatment, another case was postextraction site area. 5. Radiologically, 'scalloping appearance' were evident in all cases; in 3 cases, multilocular tendency & only one case seen intact mandibular canal image. 6. Histologically, all section showed bone trabeculae with blastic activity, 2 cases showed no epithelial lining, and other 2 cases were seen innflammatory cell infiltration in edematous tissue. 7. Surgical intervention (curettage) was that treatment of choice.

  • 낙상에 의해 치아 및 주변 조직이 손상되어 구강 내 경조직 및 연조직의 상실이 발생한 경우 이를 재건하기 위한 방법으로 외과적 접근법과 보철적 접근법이 제시될 수 있다. 치은부가 포함된 보철물을 이용하는 보철적 접근법은 외과적 수술을 생략함으로써 치료 과정을 단순화 하고 치료 시간과 비용을 줄일 수 있다는 장점이 있다. 특히, 상악 전치부 치아 및 치조골의 상실을 보철적으로 재건하는 경우 치은부가 포함된 보철물에 의해 확실한 안면부 지지를 제공하게 되어 안모의 심미적 결손을 보상할 수 있고 상 하악의 관계를 개선시킬 수 있다. 본 증례는 낙상으로 인해 상악 전치부에 광범위한 손상을 입은 노인 환자에서 수술을 배제하고, 치은 색상의 도재가 포함된 고정성 보철물을 이용하여 수복함으로써 예지성 있는 심미적 결과를 얻었기에 이를 보고하고자 한다.
