Effect of tranexamic acid on blood loss reduction in patients undergoing orthognathic surgery under hypotensive anesthesia: a single-center, retrospective, observational study

  • Objectives: Orthognathic surgery is a surgical procedure performed by intraoral approach with established and safe techniques; however, excessive blood loss has been reported in rare cases. In response, investigative efforts to identify methods to reduce the amount of blood loss have been made. Among such methods, the administration of tranexamic acid was reported to reduce the amount of intraoperative blood loss. However, few studies to date have reported the effect of tranexamic acid in orthognathic surgery under hypotensive anesthesia. The present study aimed to investigate the effect of the administration of tranexamic acid on intraoperative blood loss in patients undergoing bimaxillary (maxillary and mandibular) orthognathic surgery under hypotensive anesthesia. Patients and Methods: A total of 156 patients (mean age, 27.0±10.8 years) who underwent bimaxillary orthognathic surgery under hypotensive anesthesia performed by the same surgeon between June 2013 and February 2022 were included in this study. The following data were collected from the medical records of each patient: background factors (age, sex, and body mass index), use of tranexamic acid, surgical procedures, previous medical history, duration of surgery, American Society of Anesthesiology physical status findings before surgery, intraoperative blood loss as a primary outcome, in-out balance, and blood test results. Descriptive statistics were calculated for statistical analysis, and a t-test and the chi-squared test were used for between-group comparisons. Group comparisons were performed after 1:1 propensity score matching to adjust for confounding factors. Statistical significance was set at P<0.05. Results: Comparison between the groups based on the use of tranexamic acid revealed a significant difference in operation time. Propensity score matching analysis revealed that intraoperative blood loss was significantly lower in the tranexamic acid group. Conclusion: The administration of tranexamic acid was effective in reducing intraoperative blood loss in patients undergoing bimaxillary orthognathic surgery under hypotensive anesthesia.

일차 슬관절 전치환술 중 트라넥삼산의 정맥 내 투여와 국소 사용의 효과에 관한 비교 연구 (Efficacy of Tranexamic Acid during Primary Total Knee Arthroplasty: Comparative Study between Intravenous Use and Topical Use)

  • 목적: 일차 슬관절 전치환술을 받은 환자들에게서 트라넥삼산의 효능을 확인하고, 트라넥삼산의 정맥 내 사용 방법과 관절 내 투여 군을 비교하여 어떤 방법이 혈액 손실 감소 효과가 더 있는지 알아보고자 하였다. 대상 및 방법: 이 연구는 광주보훈병원에서 2017년 3월부터 2019년 2월까지 일차 슬관절 전치환술을 받은 환자들에 대한 후향적 연구이다. 단측 일차 슬관절 전치환술을 시행 받은 총 210명의 환자가 연구에 포함되었다. 트라넥삼산 사용 방법에 따라 각각 70명의 환자들에 대해 3개의 그룹으로 나누었다(Group I: 정맥 내 투여 군, Group II: 관절 내 투여 군, Group III: 트라넥삼산을 사용하지 않은 군). 그룹 간 수술 후 총 실혈량, 헤모글로빈 감소치, 수혈 단위를 비교하였다. 결과: 총 실혈량은 관절 내 투여 군(1,136±339 ml) 및 트라넥삼산을 사용하지 않은 군(1,366±866 ml)보다 정맥 내 사용 군(987±449 ml)에서 더 낮았다(p=0.004). 수술 후 헤모글로빈의 손실 또한 정맥 내 사용 군(1.8 g/dl)에서 관절 내 투여 군(2.9 g/dl)이나 투여하지 않은 군(3.5 g/dl)보다 현저히 낮았다(p<0.01). 수혈 빈도는 트라넥삼산을 사용하지 않은 그룹(5.7%)에서 더 높았으며 정맥 내 사용 시 1.4%를 보이고, 관절 내 투여한 군에서 수혈을 받은 환자는 없었다. 혈전색전증의 발생 빈도는 그룹 간 통계적으로 유의하지 않았다. 결론: 일차 슬관절 전치환술 시행 시 트라넥삼산을 사용한 환자 군에서 사용한지 않은 군에 비해 총 실혈량을 유의하게 감소시켰고, 관절 내 투여보다는 정맥 내로 투여하는 것이 더 효과적이었다. 정맥 내 투여로 인한 합병증은 관절 내 투여 및 트라넥삼산을 사용하지 않은 군과 비교해 유의한 차이는 없었다.

Effectiveness of hemocoagulase, tranexamic acid, and their combination for reducing blood loss in bimaxillary orthognathic surgery: a retrospective study

  • Objectives: Orthognathic surgery is a corrective intervention for maxillofacial deformities. Bleeding is a major concern for oral and maxillofacial surgeons. Various agents, such as hemocoagulase, tranexamic acid, and aprotinin have been developed to reduce intraoperative bleeding and transfusion requirements. Therefore, in this study we aimed to investigate the effects of hemocoagulase and tranexamic acid, as well as their simultaneous use, to reduce bleeding during orthognathic surgery. Patients and Methods: This retrospective study included patients who had undergone simultaneous orthognathic surgery of the maxilla and mandible between January 2013 and September 2022 and were classified into three groups based on drugs administered: hemocoagulase (Botropase), tranexamic acid, and a combination of both drugs. We recorded patient age, sex, weight, blood loss, and duration of surgery. Red blood cell (RBC), hemoglobin, hematocrit, and platelet levels were measured before, immediately after, and one day after surgery. Results: No statistically significant differences were found in blood loss, RBC, hemoglobin, hematocrit, or platelet levels between any of the groups. There were no differences in the drug effects between Le Fort I and bilateral mandibular sagittal split osteotomies, with or without double genioplasty. However, there were significant reductions in RBC, hemoglobin, hematocrit, and platelet levels during genioplasty. Conclusion: Tranexamic acid, hemocoagulase, and their combination had similar efficacy in patients who underwent Le Fort I and bilateral mandibular sagittal split osteotomies with and without genioplasty.

The antiplaque and bleeding control effects of a cetylpyridinium chloride and tranexamic acid mouth rinse in patients with gingivitis

  • Purpose: This study aimed to evaluate the effects of a cetylpyridinium chloride (CPC) and tranexamic acid (TXA) mouth rinse on patients with gingivitis. Methods: This randomized, placebo-controlled, double-blind, parallel-group, clinical trial included 45 healthy adults with gingivitis, who were randomized into 2 groups. The experimental group used a 0.05% CPC and 0.05% TXA mouth rinse, and the control group used a placebo mouth rinse. The following clinical indices were assessed at baseline, at 3 weeks, and at 6 weeks: the Turesky-Quigley-Hein plaque index (QHI), the $L{\ddot{o}}e-Silness$ gingival index (GI), and bleeding on marginal probing (BOMP). The subjects used the mouth rinse during the experimental period for 20 seconds, 4-5 times daily (10 mL each time). Results: There were no significant differences in the clinical indices between the groups at baseline. In the experimental group (CPC+TXA), a statistically significant improvement was evident in the QHI, GI, and BOMP at 3 and 6 weeks. These results were similar to those observed in the control group at 3 and 6 weeks, although the change in BOMP was not statistically significant in that group. At 6 weeks, the experimental group had a significantly lower mean score for the QHI than the control group. Conclusions: This study demonstrated that a CPC and TXA mouth rinse exhibited significant antiplaque and anti-gingivitis efficacy, and had a positive effect on bleeding control when used daily for 6 weeks.

Effectiveness and Safety of Tranexamic Acid in Spinal Deformity Surgery

  • Objective : Spinal deformity surgery has the potential risk of massive blood loss. To reduce surgical bleeding, the use of tranexamic acid (TXA) became popular in spinal surgery, recently. The purpose of this study was to determine the effectiveness of intra-operative TXA use to reduce surgical bleeding and transfusion requirements in spinal deformity surgery. Methods : A total of 132 consecutive patients undergoing multi-level posterior spinal segmental instrumented fusion (${\geq}5$ levels) were analyzed retrospectively. Primary outcome measures included intraoperative estimated blood loss (EBL), transfusion amount and rate of transfusion. Secondary outcome measures included postoperative transfusion amount, rate of transfusion, and complications associated with TXA or allogeneic blood transfusions. Results : The number of patients was 89 in TXA group and 43 in non-TXA group. There were no significant differences in demographic or surgical traits between the groups except hypertension. The EBL was significantly lower in TXA group than non-TXA group (841 vs. 1336 mL, p=0.002). TXA group also showed less intra-operative and postoperative transfusion requirements (544 vs. 812 mL, p=0.012; 193 vs. 359 mL, p=0.034). Based on multiple regression analysis, TXA use could reduce surgical bleeding by 371 mL (37 % of mean EBL). Complication rate was not different between the groups. Conclusion : TXA use can effectively reduce the amount of intra-operative bleeding and transfusion requirements in spinal deformity surgery. Future randomized controlled study could confirm the routine use of TXA in major spinal surgery.

Is Tranexamic Acid an Effective Prevention in the Formation of Epidural Fibrosis? Histological Evaluation in the Rats

  • Objective : The present study aimed to determine the topical and systemic efficacy of tranexamic acid (TXA) on epidural fibrosis in a rat laminectomy model. Methods : Thirty-two 12-month-old adult Sprague-Dawley rats were used in this study. Each rat underwent bilateral laminectomy at the L1 and L2 vertebral levels. Rats were divided into four groups : in group I (control group, n=8), a laminectomy was performed and saline solution was applied into the surgical space. In group II (topical group, n=8), laminectomy was performed and 30 mg/kg TXA was applied to the surgical site before skin closure. In group III (systemic group, n=8), 30 mg/kg TXA was administered intravenously via the tail vein in the same session as the surgical procedure. In group IV (topical and systemic group, n=8), TXA was administered 30 mg/kg both topical and intravenous. The rats were sacrificed at 4 weeks postoperatively. Masson's trichrome and hematoxylin and eosin were used to assess acute inflammatory cells, chronic inflammatory cells, vascular proliferation, and epidural fibrosis. Results : Epidural fibrosis, acute inflammation, chronic inflammation, and sum histologic score value were significantly lower in the systemic TXA group, systemic and topical TXA groups than in the control group (p<0.05). In addion, the sum histologic score was significantly lower in the topical TXA group than in the control group (p<0.05). Conclusion : In this study, epidural fibrosis formation was prevented more by systemic application, but the topical application was found to be effective when compared to the control group. As a result, we recommend the systemic and topical use of TXA to prevent epidural fibrosis during spinal surgery.

퇴행성 관절염 환자에게 동시에 시행하는 양측 인공 슬관절 전치환술에서 관절강 내 Tranexamic Acid 주입의 장점 (Advantages of Intra-Articular Tranexamic Acid Injection Following Simultaneous Bilateral Primary Total Knee Arthroplasty)

  • 목적: 양측 슬관절 퇴행성 관절염으로 동시에 시행하는 양측 인공 슬관절 전치환술(simultaneous bilateral total knee arthroplasty, SBTKA)의 정맥 투여 금기에 해당하는 환자를 포함하여 관절강 내 트라넥삼산(tranexamic acid, TXA) 투여 후 배액량, 실혈량, 수혈률, 수혈량 및 합병증 발생의 차이를 비교해 보고자 하였다. 대상 및 방법: 2016년 4월부터 2018년 12월까지 퇴행성 관절염으로 SBTKA를 시행하였던 환자 중 관절낭 봉합 후 배액관을 통해 관절강 내로 한쪽당 3 g의 TXA를 주입한 139명을 실험군(group T), 2007년 10월부터 2010년 8월까지 TXA를 주입하지 않고 수술하였던 SBTKA 환자 57명(group A)을 대조군으로 설정했다. 양 군은 나이, 성별에 따른 유의한 차이는 없었다(p=0.572, 0.474). TXA는 정맥 내 투여의 금기에 해당하는 환자들에게도 관계 없이 주입하였고 염증성 관절염으로 수술을 시행한 환자는 본 연구에서 제외시켰다. 각각 평균 배액량, 실혈량, 수혈률, 수혈량과 일 평균 혈색소의 변화 추이, 일 평균 수혈량을 비교하였다. 합병증으로 심부 정맥 혈전증, 폐색전증, 감염, 뇌경색, 심근경색 발생 여부를 조사하였다. 결과: 평균 실혈량은 group A와 group T에서 각각 2195.32±1175.63 ml, 1145.09±382.95 ml, 평균 배액량은 1178.30±484.59 ml, 774.19±310.06 ml로 group T에서 각각 유의하게 적었다(p=0.002, <0.001). 수혈률은 각각 77.2% (44/57), 0.7% (1/139), 전체 평균 수혈량은 각각 735.44±550.83 ml, 4.60±54.28 ml로 group T에서 각각 유의하게 적었다(p<0.001, <0.001). 혈색소는 group A, group T에서 각각 술 후 3, 4일째 증가하였다(p<0.001, 0.001). 합병증으로 심부 정맥 혈전증이 group T에서 2예(1.4%), 폐색전증이 3예(2.2%) 발생하였으나 group A에서는 발생하지 않았다. 감염, 뇌경색, 심근경색은 모두에서 발생하지 않았다. 결론: 동시에 시행하는 SBTKA에서 TXA의 관절강 내 주입은 TXA의 평균 배액량, 실혈량, 수혈률, 수혈량을 유의하게 감소시키며 정맥 내 투여 금기에 해당하는 환자들을 포함하여도 합병증 발생률이 높아지지 않는다.

  • To identify inhibitors of melanogenesis, we compared the effects of 5 compounds on mushroom tyrosinase, human melanocytic tyrosinase activity and melanin content. The cytotoxicyty of the components were also tested on cultured human melanoctes. Kojic acid showed marked inhibitory effect both on mushroom and human tyrosinase activity. This action of kijic acid is stronger than that of ascorbic acid. Arbutin inhibited human tyrosinase activity of cultured melanocytes although it had slightly inhibitory effect on mushroom tyrosinase activity. Azelaic acid had no effect on human tyrosinase activity. Melanin production was inhibited significantly by kojic acid and tranexamic acid. MTT assay showed that all of the compounds were non-cytotoxic to melanocytes at the concentrations tested. These results suggest that the effect of kojic acid on cultured meanocytes involve inhibition of tyrosinase activity and melanogenesis without affection the cell number.

Role of Adjunctive Tranexamic Acid in Facilitating Resolution of Chronic Subdural Hematoma after Surgery

  • Objective : Chronic subdural hematoma (CSDH) is a common neurosurgical disease and generally treated with burr-hole surgery alone. Tranexamic acid (TXA) is an antifibrinolytic agent that potentially reduces recurrence rates and the residual hematoma volume. However, the role of postoperative TXA medication remains unclear to date. This study aimed to verify the effectiveness of adjunctive TXA in the view of early hematoma resolution. Methods : Between January 2018 and September 2021, patients with CSDH who underwent burr-hole trephination in a single tertiary institute were reviewed. The study population was divided into three groups, TXA, non-TXA, and antithrombotics (AT) groups, according to the medical history of cardio-cerebrovascular disease and TXA administration. The primary endpoint was CSDH recurrence, defined as re-appearance or re-accumulation of CSDH requiring neurosurgical interventions. The secondary outcome was CSDH resolution, defined as complete or near-complete resorption of the CSDH. The CSDH resolution time and serial changes of hematoma thickness were also investigated. Results : A total of 240 patients was included in the analysis consisting of 185 male and 55 female, with a median age of 74 years. During the median imaging follow-up period of 75 days, 222 patients were reached to the primary or secondary endpoint. TXA was administered as an adjunctive therapy in 41 patients (TXA group, 16.9%) while 114 patients were included in the non-TXA group (47.9%) and 85 were in the AT group. The recurrence rate was the lowest in the TXA group (2.4%), followed by non-TXA (7.0%) and AT (8.2%) groups. However, there was no statistical significance due to the small number of patients with recurrence. CSDH resolution was achieved in 206 patients, and the median estimated time to resolution was significantly faster in the TXA group (p<0.001). Adjunctive TXA administration was a significant positive factor for achieving CSDH resolution (p<0.001). The hematoma thickness was comparable among the three groups at the initial time and after surgery. However, CSDH thickness in the TXA group decreased abruptly in a month and showed a significant difference from that in the other groups (p<0.001). There was no TXA-related adverse event. Conclusion : The adjunctive use of TXA after CSDH surgery significantly facilitated the resorption of residual CSDH and resulted in the early CSDH resolution. Adjunctive TXA may be an effective treatment option to reduce recurrence by enhancing CSDH resolution in the selective patients.
