Factors associated with Utilization and Satisfaction of Traditional Korean Medicine in Korea: A National survey

  • Objective : To assess the utilization patternsand satisfaction factors of Traditional Korean Medicine(TKM) in Korea. Method : The study design of this paper is from a nationally representative random e-mails and interview surveys. In this survey, we asked 2000 Korean adults between the ages of 20 and 69, whether they have used TKM in the past three years and if so, what were their experiences and how satisfied they were with the care they have received. Results : Sixty percent of respondents(n=1239) answered that they had used TKM in the past three years. The odds of using TKM were higher in female(OR 1.48; 95% CI 1.24-1.78) and academic backgrounds of university graduate or beyond (OR 1.27; 95% CI 1.01-1.60), and lower in unmarried person (OR 0.68; 95% CI 0.55-0.84) in the multivariable analysis. The most frequent purpose for seeking TKM and receiving treatment was to 'treat illness'.In addition, the use herbal medicine among types of TKM is statistically significant in sexual groupings.748(60.4%) among TKM users responded that they had a satisfying experience during TKM treatment. In multivariable logistic model, the odds of satisfaction from using TKM were higher in employed groups(OR 1.34; 95% CI 1.02-1.77), the reason for using of personalized medicine into TKM(OR 1.50; 95% CI 1.00-2.26). Conclusion : Korean traditional medicine doctors need to develop and provide the personalized medicine for the group of high utilization rate and satisfaction, and make strategies to disseminate TKM utilization for the group of lower utilization.

한의학 교육과정 변화의 조건 탐색 - 통합교육과정을 중심으로 - (Exploring the Conditions that will Allow for Change in Traditional Korean Medicine Curricula - Focusing on the Integrated Curriculum -)

  • Objectives : To suggest conditions that will allow for change in Traditional Korean Medicine (TKM) curricula through analysis of the failure of widespread acceptance of the integrated curriculum into all Traditional Medicine Colleges. Methods : First, the process of change between TKM education and medical education was compared. Next, the conditions that would stimulate change in TKM curricula were analyzed in three levels: structure, policy, and actor. Results : TKM colleges have not experienced a structural change from the six-year to four-year structure. The accreditation standards of TKM education (KAS2021) should be more supportive instead of forcing change, and the goals should be reset to education improvement instead of meeting the standards of WFME. The deans' leadership and professors' ownership over their subjects need to be modified. A group of professionals that could lead change is required, while other factors such as full-time professorships, clinical practice hours, obligatory teaching hours and overall teaching environment need to be improved. Conclusions : It would be ideal if individual TKM colleges created success models of curricula change-despite the time and effort that is required-that could spread to other colleges. After the new curriculum has been accepted in more that half of the total eleven TKM colleges, an institutional isomorphism can be demanded.

국제보건에서 한의약 공적개발원조의 현재와 지속가능한 발전전략 (Current Status and Sustainable Development Strategies of Traditional Korean Medicine Official Development Assistance (ODA) in Global Health)

  • Objectives: Korea has a unique history of being both a recipient and a donor of Official Development Assistance (ODA), and the international community expects Korea to contribute to the development of developing countries by utilizing this experience. Traditional Korean medicine (TKM) seeks to contribute to global health, however the concept of ODA has been unclear and there has been no clear strategy and sustainable initiatives. Methods: This study examines the concept of ODA and its application in global health, including business objectives, scale, evaluation principles, and development strategies. Additionally, we reviewed the current status of Traditional Korean medicine globalization projects and conducted a SWOT analysis of the internal and external environment of the TKM sector. Based on these findings, we redefined the concept of ODA for TKM and proposed suggestions for its development. Results: The current study identified key ideas for TKM ODA. It should prioritize the improvement of primary healthcare in recipient countries, aligning with the international evaluation criteria of the SDGs. Secondly, TKM's 70 years of experience can be leveraged to enhance both the competence and economic benefits of recipient countries' medical systems. Based on these concepts, a TKM ODA development model was proposed, comprising two core visions, three development strategies and goals, and six core values. Conclusion: This study systematically examined the TKM in global health and suggested sustainable development strategies for TKM ODA. Through its expansion, TKM could contribute to the advancement of global traditional medicine and its overall engagement in healthcare.

한약의 독물동태학적 특성 (The Toxicokinetic Characteristics of Korean Traditional Medicines)

  • Toxicokinetics of Korean Traditional Medicines(TKM) is the description of what rate TKM will enter the body and what happens to it once it is in the body in terms of toxicology. However, it is not easy to understand TKM toxicokinetics because of various factors such as a mixture of 2-30 kinds of herbal materials containing thousands of chemicals, and complex chemical properties. For these reasons, little is known about toxicokinetics of TKM. This study was aimed to characterize and review the absorption, distribution and metabolism of korean traditional medicines in a view of toxicokinetics. For this aim, some of korean traditional medicines were reviewed on a basis of drug-drug interaction, biotransformation and intestinal metabolisms by bacteria. As the factors affecting mainly on toxicokinetics of TKM, individual herbal material's degree of lipophilicity and metabolic rate, and decoction components according to different kinds of herbal materials were considered. Other factors such as intestinal pH and bacterial activity for metabolism affecting on TKM toxicokinetics, especially in small intestine. It would be a better way for improving the adverse or poor effects caused by TCM if the factors affecting on toxicokinetics of TKM is considered.

  • Learning can be defined as its objects, main question for the objects, and its unique way to organize all the knowledge acquired as the results of the question. From the point of view like this, Traditional Korean Medicine(TKM) can be defined as learning for human body and its functions, health and diseases based on the theory of the Yin and Yang and of the five elements. Nowaday Many papers based on laboratory work publish for the name of scientization of TKM, but from the viewpoint of definition of learning, they have a problem that there is no basic theory. If TKM could be communicated with western natural science, it has to be solved. And oriental physiology has a same object and same questions with western physiology, so oriental physiology can be useful to make a bridge between TKM and western natural science.

알도즈 환원효소 활성억제와 소비톨 축적 억제로 인한 벌개미취 추출물의 당뇨병성 백내장 발병 지연 효능 (Slow Development of Diabetic Cataract in Streptozotocin-induced Diabetic Rats via Inhibition of Aldose Reductase Activity and Sorbitol Accumulation by Use of Aster koraiensis Extract)

  • Diabetic cataract is a major complication of diabetes mellitus. Excess accumulation of sorbitol plays an important role in the pathogenesis of diabetic complications such as cataract formation. In this study, we investigated the inhibitory effect of the extract of the aerial parts of Aster koraiensis (AK) on diabetic cataractogenesis. To examine this further, we evaluated sorbitol accumulation during cataract development using streptozotocin-induced diabetic rat, an animal model of type 1 diabetes. Diabetic rats were treated orally with AK (100 mg/kg and 200 mg/kg body weight) once a day orally for 9 weeks. In vehicle-treated diabetic rats, lens opacity was increased, and lens fiber swelling and membrane rupture were observed. In addition, sorbitol accumulation in diabetic lens was markedly enhanced. However, AK treatment delayed the progression of diabetic cataract through the inhibition of sorbitol accumulation, and prevented lens fiber degeneration in a dose-dependent manner. These observations suggest that AK treatment can delay the progression of lens opacification in the diabetic rats during the early diabetic cataractogenesis.

  • Yoon Gil-Young(1911-1987) was a frontier of the field of Traditional Korean Medical Physiology(TKMP) who was academically active since mid 1950s until early 1970s. He wrote several important papers and books. Among his writings, "A Study on the Methodology of Traditional Korean Medical Physiology", which was published serially in the "Korean Oriental Medical Journal" from June 1966 until March 1967, played an important role in establishing the contemporary TKMP and showed his academic stance on Traditional Korean Medicine(TKM). This review will evaluate this paper's contribution to the establishment of TKMP in historical context, summarize the unique characteristics of his understanding of TKMP by analyzing the text, and then based on those information, examine his view on TKMP and TKM. Historically, this paper was written in transition period from the traditional TKM knowledge system to the modernized one. Aim of this paper was to provide a methodology for establishing TKMP as a scientific knowledge system like that of the western medicine. Based on his study of Yin-Yang theory and theory of five circuits and six qi in Huangdi's Internal Classics, he suggested those theories as methods to observing life phenomena and systematizing the observations. And he regarded these methods as of great value in determining the unique characteristics of TKM compared to those of western medicine. Through re-reading this paper, it was found that he had pride and confidence in the methodology of TKM, and also that he thought in scientizing and modernizing TKM it was very important to understand and efficiently put in use the methodology of TKM. It was also found that his view on TKMP and TKM was pro modernization.

  • Objective: China is likely to nominate their traditional medicine (TCM) as a World Intangible Cultural Heritage. Regarding this movement, is important for Korea to clarify and reestablish the origin of traditional Korean medicine (TKM) to keep pace. Methods: Studied current cultural policy on TCM and revised the standpoint of Chinese scholars through papers. Conclusions: Though many researchers are aware of the necessity of cultural propagation of TKM, studies actually undertaken are scarce. Therefore, Korean researchers need to carry out more research on self distinguishing aspects of TKM and the propagation of its unique characteristics.

현대 한의학의 정체성 문제 연구 (Study on the Identity Issue of Current Traditional Korean Medicine)

  • This study focuses on seeking a desirable direction for the discussions about identity issue of the current Traditional Korean Medicine(TKM). The identity issue of the current TKM is about asking what exactly the current TKM is, what sets the TKM apart from the other medical systems, and what value the TKM has in the contemporary settings, and seeking suitable answers to all these questions. The most important factor that affects these answers is what kind of the concept of identity we have. Current discussions of the identity issue of the TKM take narrow and inflexible concepts of identity such as "Numerical Identity," or "Qualitative Identity", not suitable for TKM, which is always changing and open to other academic fields. This study suggests that Hannah Arendt's concept of identity is more desirable for the TKM, which highlights pluralism and relations, as well as defining identity as a creative, positive, and open-ended process. In addition, this study also suggests that taking a closer look at the "field", which has previously been hidden behind the discourse on the "modernization", will help in figuring out what exactly the identity of the current TKM is. Lastly, the current discussions about "unification of Eastern and Western medical system", and "Integrative Medicine" in Korea will affect the identity issue of TKM.

12개 미래 예측 한의약 정책 과제의 실현 평가 연구 (Assessment of the Forecasting Studies on 12 Traditional Korean Medicine Policy Realization)

  • Objectives : Aim of this study is to contribute to establishment of the Traditional Korean Medicine (TKM) policies in the future. Final assessment for 12 of the forecasting projects was carried out on the TKM policies that deduced by professionals in 1996 whether or not to realize in 2013. Methods : We investigated governmental and private research projects, reports and papers, and laws and systems on the forecasting projects. We reviewed them through the Traditional Korean Medicine Information Portal OASIS (http://oasis.kiom.re.kr), Korean studies Information Service System (KISS) (http://kiss.kstudy.com/) and DBpia (http://www.dbpia.co.kr/), Akomnews(http://www.akomnews.com/), THE MINJOK MEDICINE NEWS(http://www.mjmedi.com/), Ministry of Government Legislation(http://www.law.go.kr/). Results : Of the 12 forecasting projects, five were judged as 'realization', four were as 'partial realization' and three were as 'un-realization', The realization rate was 75.0%. Three un-realized projects included the TKM insurance coverage for various herbal medicines, leadership secure on medical technicians and commercialization of the TKM managing system on senior medicare policy. Realization of the future forecasting TKM policy projects was decided depending on conditions such as the importance, domestic capability levels, principal agents, methods and restrains. Conclusions : Continuous studies and new developed forecasting projects for the TKM policies will be required to realize the projects in the future.
