Effectiveness of an extraoral cold and vibrating device in reducing pain perception during deposition of local anesthesia in pediatric patients aged 3-12 years: a split-mouth crossover study

  • Background: Local anesthetic injections may induce pain in children, leading to fear and anxiety during subsequent visits. Among the various approaches recommended to reduce pain, one is the use of a Buzzy BeeTM device that operates on the concept of gate control theory and distraction. The literature regarding its effectiveness during the deposition of local anesthesia remains limited; hence, the aim of the present study was to determine the efficacy of extraoral cold and vibrating devices in reducing pain perception during the deposition of local anesthesia. Methods: A split-mouth crossover study in which 40 children aged 3-12 years requiring maxillary infiltration or inferior alveolar nerve block for extractions or pulp therapy in the maxillary or mandibular posterior teeth were included. The control intervention involved the application of topical anesthetic gel for one minute (5% lignocaine gel), followed by the administration of local anesthetic (2% lignocaine with 1:80,000 adrenaline) at a rate of 1 ml/ minute. Along with the control protocol, the test intervention involved using the Buzzy BeeTM device for 2 minutes before and during the deposition of the local anesthetic injection. The heart rate and face, legs, arms, cry, and consolability revised (FLACC-R) scale scores were recorded by the dentist to assess the child's pain perception. Results: The mean age of the participants in Group A and Group B was 7.050 ± 3.12 years and 7.9 ± 2.65 years respectively. A reduction in the mean heart rate and FLACC-R score was observed during the deposition of local anesthetic solution in the tissues when the Buzzy BeeTM was used in both groups at different visits in the same subjects (P < 0.05) The Buzzy BeeTM device was effective in reducing the heart rate and FLACC-R scores when used during maxillary infiltration and inferior alveolar nerve block local anesthesia techniques (P < 0.05). Conclusion: The use of extraoral cold and vibrating devices significantly reduces pain perception during local anesthetic deposition in pediatric patients. Considering the results of this study, the device may be incorporated as an adjunct in routine dental practice while administering local anesthesia in children.

  • This study aims to serve as a critical comparison of the currently controversial 'new cold war' discourse. It took three triggers for the 'new cold war' discourse to emerge as a major issue in the media and academia and to have real political impact. With the launch of China's 'Belt and Road' project and Russia's annexation of Crimea leading to the 'Ukraine crisis,' the 'new cold war' discourse has begun to take shape. Trump's U.S.-China trade spat has brought the 'new cold war' debate to the forefront. The 'new cold war' debate is currently being intensified by the Biden administration's framing of "democracy versus authoritarianism" and Putin's invasion of Ukraine. Currently, there is no consensus among scholars on whether the controversial 'new cold war' is a new version, or a continuation of the historically defined concept of the Cold War. The term 'New Cold War' is less of an analytical concept and more of a topical term that has yet to achieve analytical status, let alone a theoretical validation and systematization, and the related debate remains at the level of assertion or discourse. Through this comparative analysis, I will argue that the ongoing discourse of the 'New Cold War' does not have the instrumental explanatory power to analyze the transitional phenomena of the world order today.

Experimental Studies on the Hair Growth Activity of Extracts of Pinelliae Rhizoma in Spontaneous Alopecia Model and Normal C57BL/6N Mice (반하추출물(半夏抽出物)이 자발적 원형탈모 생쥐와 정상 생쥐의 발모효과에 대한 실험적 연구)

  • This experiment was investigated the effects of fresh and ginger processed Pinelliae Rhizoma extracts on hair growth activity, and its fractions(chloroform, ethyl acetate and water fractions) obtained from fresh Pinelliae Rhizoma on hair growth activity of the normal and spontaneous alopecia areata model of C57BL/6N mice for 16 days. The results were as follows: In fresh Pinelliae Rhizoma extracts treated group, hair growth effect was observed in whole skin area(100%) all the normal mice in whose hair had been clipped on 16th days. In ginger processed Pinelliae Rhizoma extracts treated group, hair growth effect was observed in whole skin area in 25% of normal mice in whose hair had been clipped on 16th days. But in control group, hair growth effect was observed in a part of whole skin area in 25% of normal mice. In fresh Pinelliae Rhizoma extracts treated group, hair follicles of middle stage of anagen phase was observed and it were grown down to subcutaneous tissue of skin in all the mice on 10th day. But in ginger processed Pinelliae Rhizoma extracts treated group and control group, Most of hair follicles of telogen phase was observed in skin. The treatment of extracts of fresh Pinelliae Rhizoma increased the expression of TGF-$\beta$(146%), IGF(107%), and prolactin(115%) in the skin of normal C57BL/6N mice compared to control group(100%). But expression of placenta lactogen(93%) was decreased in the skin of normal C57BL/6N mice compared to control group(100%). In spontaneous alopecia model, The hair growth activity of fresh Pinelliae Rhizoma extracts treated group(100%) was observed to be strong compared with the control group(20%) on 15th day. Hair growth activity on chloroform fractions of fresh Pinelliae Rhizoma extracts was observed in whole skin area in 75% of normal mice on the 9th day. In water and ethyl acetate fractions, hair growth activity was observed in a part of whole skin in 75% and 25% of normal mice, respectively. but hair growth activity of control group was not observed. After application of fractions of fresh Pinelliae Rhizoma extracts for 10 days, hair follicles of chloroform fraction treated group was observed middle stage of anagen phase and hair follicle were grown down to subcutaneous tissue of skin in all the mice. But hair follicles of initial stage of anagen phase were observed in water and ethyl acetate fractions. Most of hair follicles of telogen phase was observed in skin of control group. These experiments suggest that extracts of fresh Pinelliae Rhizoma may stimulate the topical hair growth activity and its chloroform fractions can be useful for treatment of alopecia areata.

Effect of Green Tea Extract on Healing of Contaminated Wound in Dogs (개의 피부 감염창 치유에 대한 녹차 추출물의 국소도포 효과)

  • The purpose of this study is to assess healing effect of the green tea on Staphylococcus contaminated wound. On the back of 7 dogs, 6 full-thickness skin wounds($2cm{\times}2cm$) were made and Staphylococcus intermedius was inoculated. Each wound was applied with the 2%(experimental group I), 1%(experimental group II) extract of green tea and normal saline(control group I) after inoculation. The wound contraction rate of the experimental group I and II was higher than that of the control group in entire period. And from the $6^{th}\;to\;22^{nd}$ day, the wound contraction rate of the experimental group I and II was significantly higher than that of the control group(p<0.05). The occupation rate of wound healing in the experimental group I was higher than that of the control group from $0\;to\;9^{th}$ day except from $1^{st}\;to\;2^{nd}$ day and also higher than that of the experimental group II from $0\;to\;3^{rd}$ day except from $1^{st}\;to\;2^{nd}$ day. And the occupation rate of wound healing in the experimental group II was higher than that of the control group from $1^{st}\;to\;8^{th}$ day except from $2^{nd}\;to\;3^{rd}$ day. According to concentration of green tea extract increased, the wound contracted more vigorously in earlier period. The tensile strength of the experimental group I and II was significantly higher than that of the control group(p<0.05). And that of the experimental group I was also higher than in the experimental group II(p<0.05). The number of S. intermedius in the experimental group I and II was decreased more rapidly than that of the control group. From the $4^{th}$ to the $11^{th}$ day, the number of S. intermedius in the experimental group I was significantly lower than that of the experimental group II and the control group. And from the $4^{th}$ to the $7^{th}$ day, the number of S. intermedius in the experimental group was significantly lower than that of the control group. The histopathological findings of the experimental group I and II were similar to those of the control group on the $1^{st}\;and\;4^{th}$ day. On $14^{th}$ day, reepithelialization was completed in the experimental group I and II except the control group. Higher wound contraction rate, better bacteriocidal action, higher tensile strength and better histopathological findings were observed in the experimental group I and II than in the control group. These results suggest that the topical application of green tea extract can promote contaminated wound healing in dogs.

Degree of Interest for Dental Caries Prevention and Child's Oral Health Behaviors in Korean Adults: Gallup Survey (한국 성인의 우식예방 인식과 자녀의 우식예방 실천행위조사: 한국갤럽자료분석)

  • This study was to evaluate the influence of dental caries prevention-related concerned and perceived in Korean adults and oral health behavior of their first child if they had children. A cross-sectional study in 1,014 adults over 19 years old were conducted in Korean adults, asked to answer a computer-assisted telephone interview regarding their oral health-related knowledge, behavior, concerned of caries prevention and child's behaviors such as frequencies of tooth brushing, chewing the xylitol gum and dental checkup. The statistical analysis was done by using the SPSS 19.0 program (IBM Co., Armonk, NY, USA). Awareness of caries prevention in the female than male, 40~49 years old age group was higher. Parents of school-age children were higher level of awareness of the dental caries prevention than other group. As knowledge and behavior of dental caries prevention were higher, the child's conduct regular oral health checkups, fluoride topical application and dental sealant was significantly higher. The higher the concerned of caries preventive, the intention of oral health behaviors and child's oral health behaviors was more increased. Awareness of caries prevention had effect on the perception of the impact of the child's oral health behaviors.

Experimental brush wear pattern and cariostatic effect of Biscover (Biscover의 잇솔질에 따른 마모양상과 항우식 효과)

  • Objective: The aim of this study was to investigate the experimental brush wear pattern of a light cured surface sealant, Biscover (Bisco, Schaumburg, IL), and to evaluate its cariostatic effect. Methods: Caries-free human premolars were used for the Biscover coating group (n = 90) and the control group (n = 10). The Biscover coating group was randomly assigned to nine subgroups of 10 each and the control group was assigned to two subgroups of 5 each according to the number of brushing strokes. An experimental 3-body wear test was conducted under different strokes of wear test. Tooth-brushing was accomplished with movement of each brush head set at a frequency of 100 rpm under a force of 1.5N, Surface roughness was tested before, and after Biscover coating, and after brushing. Then, each of the 10 teeth of both groups were placed in artificial caries inducing solution for 7 days. All tooth surfaces were assessed using scanning electron microscopy. Results: Biscover coated surfaces showed a smoother texture than enamel surfaces. The roughness was increased after experimental brushing and after 10,800 brushing strokes, the whole layer of Biscover wore out. However, teeth in the Biscover coating group had a cariostatic effect in cariogenic conditions. Conclusions: We suggest that white lesions in orthodontic patients can be suppressed by topical applications of Biscover.

Effect of Short Term Treatment with Different Dosage of Inhaled Flucatisone Propionate on Basal Cortisol Concentration (단기간 Fluticasone Propionate 투여 용량에 따른 가저 코르티솔 농도의 변화)

  • Backgroung : The efficacy of oral corticosteroids in the treatment of chronic asthma is undisputed, but their long-term use is associated with adverse side-effects, including supression of the hypothalamic-pituitary adrenal axis function, osteoporosis, weight gain, hypertension and impaired glucose tolerance. The introduction of inhaled corticosteroids in the early 1970's represented a significant therapeutic advance in the management of asthma, since these compounds combined high topical potency with low systemic activity. Fluticasone propionate is a new topically active synthetic glucocorticosteroid that combinds a high degree of efficacy with negligible systemic bioavailability. This study was perfomed to determine the effect of inhaled fluticasone propionate on the adreocortical supression in patients with bronchial asthma or chronic obstructive pulmonary disease. Method : The adrenocortical function was assessed by measurement of plasma cortisol concentration at 8 o'clock in morning and free cortisol in 24 hour urine collection at interval. Absolutely, no steroid was taken during pretreatment period of 10days. There after each subject inhaled fluticasone aerosol, in daily doses of 500 or 1000micrograms for 12days. The dose was delivered by metered dose inhaler(MDI). Results : The serum cortisol and 24hour urinary free cortisol were not decreased during the treatment period in patients with inhaled fluticasone propionate in daily doses of 500 micrograms. In contrast, serum cortisol was significantly decreased on 9th and 12th day(p less than 0.05). And, 24hour urinary free cortisol was also significantly decreased on 3rd and 12th day of treatement period(p less than 0.05) in patients with inhaled fluticasone in daily doses of 1000 micrograms. Conclusion : These results suggested that endogenous cortisol secretion was not supressed after short-term inhalation of fluticasone in daily dose of 500 micrograms, but in daily dose of 1000 micrograms, the endogenous cortisol secretion was supressed.

Anti-Inflammatory Effect of Chondrus ocellatus Holmes Ethanol Extract on Lipopolysaccharide-induced Inflammatory Responses in RAW 264.7 Cells (Lipopolysaccharide로 유도된 RAW 264.7 세포와 마우스모델에 대한 진두발 에탄올 추출물의 항염증 효과)

  • This study aimed to investigate the anti-inflammatory effect of the ethanol extract from Chondrus ocellatus Holmes (COHEE) in RAW 264.7 cells and in a mouse ear edema model, by measuring the production of lipopolysaccharide-induced inflammatory response mediators. There were no cytotoxic effects on the proliferation of macrophages treated with COHEE compared with the control. COHEE inhibited the production of nitric oxide and pro-inflammatory cytokines [interleukin (IL)-6, tumor necrosis factor-α, and IL-1β]. The extract also reduced the expression of inducible nitric oxide synthase, cyclooxygenase-2, nuclear factor-κB p65, and phosphorylated mitogen-activated protein kinase in a dose-dependent manner. In the croton-oil-induced ear edema model, COHEE decreased the formation of mouse ear edema at the highest dose compared with the control, and histological analysis revealed that the epidermal/dermal tissue thickness and mast cell numbers were reduced. Therefore, these results suggest that COHEE may be a promising topical anti-inflammatory therapeutic material through its action of modulating NF-κB and the MAPK signaling pathway.

The Effect of the Local Anesthetic Cream in Alleviating Pain from Vaccination (영아의 예방 접종시의 국소 마취 연고의 통증 완화 효과)

  • To evaluate the effect of a new topical anesthetic cream (EMLA: Eutectic mixture of local anesthetics) on reducing pain associated with DPT vaccination, we conducted a clinical observation on eighty infants who were brought to well baby clinic of Yeungnam University Hospital for DPT vaccination. 80 Infants, who were between 2 months and 8 months in age, were divided into two groups. EMLA treated group and control group. Male to femle sex ratio was 1.4 to 1. EMLA cream was applied 60 minutes before DPT vaccination, the effect of reducing pain was assessed by using McGrath's face scale, Oucher pain scale and modified behavioral pain scale (MBPS) and also evaluated by measuring the duration and time of crying (the time of the first crying after injection, duration of the first crying, total duration of crying). The scores of those scales were lower in EMLA treated group than in control group significantly (P(0.01 in McGrath's face scale, MBPS and p<0.05 in Oucher pain scale). There was no difference in the time of the first crying after vaccination in both groups. The duration of the first crying was shorter in EMLA treated group than control group of crying was also shorter in EMLA treated group (EMLA treated group $9.0{\pm}6.0$ sec, control group $21.9{\pm}12.5$ sec, p<0.05). Transient skin erythema was noted in 5 infants after EMLA application, but no other adverse effects were observed. We conclude that the application of EMLA cream before vaccination seems to be an effective and safe way to reduce the pain from vaccination, but it takes usually 60 minutes to get the anesthetic effect of EMLA and it is expensive, so EMLA cream can not be recommended in routine vaccination in infants now.

Antioxidative and Antiaging Effects of Platycarya strobilacea Extract and Clinical Trial (굴피나무 추출물의 항산화, 항노화 효과 및 인체 시험)

  • In this study, we investigated the anti-oxidative, anti-wrinkle and whitening effects of Platycarya strobilacea bark extracts. The free radical (1,1-diphenyl-2-picrylhydrazyl, DPPH) scavenging activity ($FSC_{50}$) of extract / fractions of Platycarya strobilacea was in the order: 50% ethanol extract ($6.75{\mu}g/mL$) < deglycosylated aglycone fraction ($6.62{\mu}g/mL$) < ethyl acetate fraction ($4.15{\mu}g/mL$). Reactive oxygen species (ROS) scavenging activities ($OSC_{50}$) of some Platycarya strobilacea extracts on ROS generated in $Fe^{3+}$-EDTA/$H_2O_2$ system were investigated using the luminol-dependent chemiluminescence assay. The order of ROS scavenging activity was ethyl acetate fraction (OSC50, $0.56{\mu}g/mL$) < 50% ethanol extract ($0.02{\mu}g/mL$) < deglycosylated aglycone fraction ($0.01{\mu}g/mL$). The deglycosylated aglycone fraction showed the most prominent scavenging activity. The protective effects of extract / fractions of Platycarya strobilacea on the rose-bengal sensitized photohemolysis of human erythrocytes were investigated. The ethanol extract (50%) suppressed photohemolysis in a concentration dependent manner, particularly ethyl acetate fraction exhibited the most prominent cellular protective effect (${\tau}_{50}$, 717.27 min at $10{\mu}g/mL$). The inhibitory effect of Platycarya strobilacea extracts on tyrosinase were investigated to assess their whitening efficacy. Finally, their anti-elastase activities were measured to predict the anti-wrinkle efficacy in the human skin. The inhibitory effect ($IC_{50}$) on tyrosinase of some Platycarya strobilacea extracts was 50% ethanol extract ($243.98{\mu}g/mL$) < ethyl acetate fraction ($153.87{\mu}g/mL$) < deglycosylated aglycone fraction ($137.53{\mu}g/mL$). Also, The inhibitory effect of elastase ($IC_{50}$) of some Platycarya strobilacea extracts was 50% ethanol extract ($31.01{\mu}g/mL$) < ethyl acetate fraction ($14.42{\mu}g/mL$) < deglycosylated aglycone fraction ($1.48{\mu}g/mL$). The cream containing the ethyl acetate fraction of Platycarya strobilacea extracts was formulated. The skin hydration, transepidermal water loss, and the whitening effects were investigated after topical application of the cream. The skin hydration of cream containing extract was increased by $2{\sim}8%$ than the placebo cream, transepidermal water loss was decreased. The cream containing extract suppressed the melanogenesis of skin by 9.55% than the placebo cream. These results indicate that extract / fractions of Platycarya strobilacea can function as antioxidants in biological systems, particularly skin exposed to UV radiation by anti-oxidative activity and protect cellular membranes against ROS. The inhibitory effect on elastase and tyrosinase, and the increase of skin hydration and the whitening effect of the cream containing extract could be applicable to new functional cosmetics for antiaging.
