A Case of Tracheal Carcinoma Diagnosed by Rigid Bronchoscopy in Lidocaine Anaphylaxis Patient (리도카인 아나필락시스 환자에서 경직 기관지내시경술을 통해 기도 암종을 진단한 1예)

  • The majority of flexible bronchoscopies are performed under topical anesthesia with lidocaine being the most commonly used agent. Anaphylaxis rarely occurs after local administration of lidocaine, but can be a fatal complication. We experienced a case of unexpected anaphylaxis. A 66-year-old woman was scheduled for flexible bronchoscopy to evaluate a tracheal mass and stenosis. The oral and nasal mucosa were pretreated with lidocaine. About 2~3 minutes later, the patient developed hypotension and we treated for anaphylaxis in the emergency room. Then, we decided to perform rigid bronchoscopy in this patient, under conditions of general anesthesia. A rigid bronchoscopy was performed in this patient, safely and successfully. The tracheal mass was determined to be squamous cell carcinoma.

Development of Bioavailability Enhancement System for the Skin Permeation Promotion of Psolarea corylifolia Extract (보골지 추출물의 피부 투과 촉진 시스템 개발)

  • Psolarea corylifolia extract that contains bakuchiol is known to have anti-microbial, anti-inflammatory and anti-scarring effects. In this study, a vesicles such as liposome, niosome, and transfersome were produced to encapsulate P. corylifolia extract and measured their stability and physiochemical property. The skin permeation and partitioning of P. corylifolia extract in the vesicles were elucidated in nude mouse skin by using Franz diffusion cells after topical application for 24 h. After storage at 25, 40, $70^{\circ}C$, and light, the stability of bakuchiol incorporated into the vesicles was maintained for 30 days. The optimal concentration of P. corylifolia extract entrapped into the vesicles was found to be 5~10%. From the physicochemical studies, after storage at 4, 25, and $40^{\circ}C$, the viscosity and particle size of the vesicles remained in 30~80 cP and the nanosize range for 6 months, respectively. From the permeation experiments, niosome showed a higher amount of bakuchiol permeated through the mouse skin compared to liposome and transfersome after 24 h. From these results, niosome and transfersome could be a good bioavailability enhancement system (BAES) for P. corylifolia extract to improve the skin permeation and stability.

Study on the development of polycaprolacton silica nanohybrid for bone substitutes (폴리카프로락톤 실리카 나노 복합체를 이용한 골이식대체재 개발에 관한 연구)

  • A bioactive and degradable poly(epsilon -caprolactone)/silica nanohybrid(PSH) was synthesized for the application as a bone substitute. PSH was manufactured by using silica and polycaprolacton. PSH was manufactured in some composition after low crystaline apatite had been formed in simulated body fluid and, was used this study. The safety of the PSH was established by test of acute, and subacute toxicity, sensitization cytotoxicity and sterility. In order to assess activity of osteoblast, the test for attaching osteoblast, proliferation test for osteoblast, differentiating gene expression test are performed in vitro. And bone substitutes were grafted in rabbit's calvarium, during 8 weeks for testing efficacy of bone substitutes. Degree of osteogenesis and absorption of substitutes were evaluated in microscopic level. In result, it was not appeared that acute and subacute toxicity, sensitization in intradermal induction phase, topical induction phase and challenge phase. It was shown that the test can not inhibit cell proliferation. adversely, it had some ability to accelerate cell proliferation. The result of sterility test described bacterial growth was not detected in most test tube. The attaching and proliferation test of osteoblast had good results. In the result of differentiating gene expression test for osteoblast, cbfa1 and, alkaline phosphatase, osteocalcin and GAPDH were detected with mRNA analysis. In the PSH bone formation test, ostgeoblastic activity would be different as material constitution but it had good new bone formation ability except group #218. futhermore, some material had been absorbed within 8 weeks. Above studies, PSH had bio-compatibility with human body, new bone formation ability and accelerate osteoblastic activity. So it would be the efficient bone substitute material with bio-active and biodegradable.

Anti-wrinkle Effect of Facial Mask Pack Containing Oinon (Allium cepa) Skin Extracts (양파 껍질 추출물을 함유한 안면 마스크 팩의 주름 개선 효과)

  • In this study, the domestic onion skin has been used to investigate the anti-wrinkle effect of the ethanolic extract. The onion skin has been known to contain a large amount of antioxidant including quercetin. Solvent extraction by 80% ethanol was carried out of the onion skin. The high amount of quercetin have been identified using HPLC as a bioactive compound. The extraction yield of the quercetin increased according to temperature up to $80^{\circ}C$. Two different antioxidant assays (DPPH and ABTS) were performed to determine the free-radical scavenging activity of onion skin extracts. The antioxidant activities ($EC_{50}$ of DPPH) of the vitamin C and the onion extracts were 2.2 and $5.09{\mu}g/mL$, respectively. The results of ABTS tests were 2.33 (of vitamin C) and $2.40{\mu}g/mL$ (of the onion extracts), respectively. In a clinical study, a panel of 10 female volunteers between the ages of 30 and 40 (mean age: 42 years) was given a mask pack containing 1% of this extracts for 4 weeks. It was shown that a highly significant reduction in the deep and moderate wrinkled was obtained with those treated with this onion extracts as compared to the control group. The reduction in deep wrinkles increased with time. Eye wrinkle were reduced statistically significant (p<0.05). The application of the proper topical cream and mask pack containing onion skin extracts has therefore shown to be effective enhancing anti-wrinkle activities.

Risk Assessment of Persicaria nepalensis Extract by Skin Irritation, Ocular Irritation, and Maximization Tests for Delayed Hypersensitivity (산여뀌 추출물의 피부자극, 안점막 자극 및 피부감작성에 대한 위해성 평가)

  • In this study, we evaluated the potential of 70% ethanol extract from Persicaria nepalensis (PNE) as a cosmetic ingredient by primary skin irritation, ocular irritation, and maximization tests for delayed hypersensitivity in New Zealand white rabbits and Hartley guinea pig. Skin safety study was performed to evaluate the potential toxicity of PNE using the primary irritation test. In the primary irritation test, 50% PNE was applied to the skin, and no adverse reactions such as erythema and edema were observed at the intact skin sites. Therefore, PNE was classified as a practically non-irritating material based on a primary irritation index of "0.0.". In the ocular irritation test, the 50% PNE applied did not show any adverse reactions in the different parts of rabbit eyes, including the cornea, iris, and conjunctiva. Thus, PNE was classified as a practically non-irritating material based on an acute ocular irritation index of "0.0.". Skin sensitization was tested by the Guinea Pig Maximization Test (GPMT) and Freund's Complete Adjuvant (FCA) using an intradermal injection of 10% PNE. Edema and erythema were not observed 24 and 48 h after the topical application of PNE in skin sensitization test, which exhibited a sensitization score of "0.0.". Therefore, it can be suggested that P. nepalensis could be used as potential candidates for cosmoceutical ingredients, without any major side effects.

Susceptibilities of German Cockroach, Blattella germanica to Insecticides According to Application Methods (바퀴에 대한 처리방법에 따른 살충제 감수성)

  • Susceptibility of Blattella germanica male adults to 61 commercial insecticides was evaluated by diet dipping method. Among them three insecticides of dichlorvos, fenitrothion, and fipronil showed over $90\%$ mortality. The insect was more susceptible to dichlorvos and fenitrothion in filter paper contact method than in diet dipping one, but fipronil was vice versa. Toxicities of the three chemicals were evaluated by application parts of the such as head, thorax, abdomen, and legs. Dichlorvos and fenitrothion were more toxic when applied to head, and flpronil was thorax. $LT_{50}$ values by diet dipping method showed that fenitrothion and fipronil acted more rapidly than boric acid and hydramethylnon. The values of the former two were 1.05 and 0.98 days, and those of the latter two were 3.92 and 2.26 days, respectively.

Insecticidal Activities and Repellent Effects of Plant Extracts against the Brown Planthopper (Nilaparvata lugens Stäl) (벼멸구에 대한 식물추출물의 살충활성과 기피효과)

  • Methanol extracts of 35 plant species in 20 families were tested for their insecticidal activities and repellent effects against Nilaparvata lugens female adult by topical application and spray methods. The insecticidal activities and repellent effects of variousplant species and parts were different. The methanol extracts from stem of Garcinia xanthochymus, Senecio scandens and Phytolacca americana, seedcoat of Ginkgo biloba, and leaf+stem of Ailanthus altissima and Catalpa ovata showed potent insecticidal activities against N. lugens. Specially, the G. biloba extract exhibited higher than 90% mortality against N. lugens at a concentration of 4,000 ppm. Repellent effects of plant extracts obtained from whole plant of Daucus carota, fruit of Semecarpus anacardium, leaf+stem of C. ovata and Wisteria sinensis were active, and potent. Also, the plant extracts that are potent in insecticidal activity not necessarily have high repellent activity showing no significant corelation between the two activities.

THE EFFECTS OF HYALURONIC ACID GEL ON THE HEALING OF ORAL MUCOSA (히알룬산젤이 구강 창상 치유에 미치는 영향)

  • Purpose: In spite of various advantages of hyaluronic acid in wound healing, there are few research about wound healing process and period in oral and maxillofacial surgery. So, We evaluated the effects of local application of hyaluronic acid gel on wound healing of the oral mucosa using this animal model. Materials and methods: Young adult New Zealand White rabbits, weighting between 2.5 and 3.0kg, were used. Almost uniform round ulcers could be created on the gingival of the rabbits by chemical injury with acetic acid. In experimental group, hyaluronic acid gel was applied daily until the wounds healed and in control group, ulcer lesion was not any treatment. The lengths of ulcers were measured the longest and shortest axes of the ulcers and calculated the areas of ulcer. For histological examination, specimens were made, and observed under a light microscope. Results: The results showed that the number of fibroblasts, new blood vessels and the epithelial thickness from experimental group was higher than from control group. Hyaluronic acid promoted proliferation of the fibroblast, keratinocytes isolated from gingival tissue of rabbits in vitro. Topical application of hyaluronic acid accelerated healing of ulcers created in rabbits. Conclusion: The hyaluronic acid may be effective for wound healing of oral mucosal lesions.

The effect of hyaluronic acid on anti-inflammatory action in mouse (마우스에서 히알우론산 나트륨이 항염효과에 미치는 영향에 관한 연구)

  • Purpose: The purpose of this study was not only to evaluate the relative mRNA expression of interleukin-$1{\beta}$(IL-$1{\beta}$), cyclooxygenase2 (COX-2) and prostaglandin E2 (PGE2) by RT-PCR analysis but to observe pattern of edema by light microscopic and electron microscope after topical apply of hyaluronic acid in inflammation-guided mouse. Material and methods: Mice of this study were devided into 4 groups: Control group (no inflammation guided), Positive control (inflammation guided + vaselin apply), Protopic group (inflammation guided + protopic apply), Hyaluronic group (inflammation guided + hyaluronic acid apply). Results: Hyaluronic group showed less expressions of IL-$1{\beta}$, COX-2, PGE2 than those of positive control & protopic group. Hyaluronic group revealed a decreased inflammation than positive control & protopic group in Light Microscope. Hyaluronic group appeared decreased edema of ear compare to positive control & protopic group in Elecron Microscope. Conclusion: It was considered that hyaluronic acid has an antiinflammatory effect for intercepting the gene expression of cytokines related to inflammation.

A Case of Docetaxel Induced Subacute Cutaneous Lupus Erythematosus (비소세포 폐암환자에서 Docetaxel 투여 중 발생한 아급성 피부 홍반루푸스 1예)

  • Drug-induced subacute cutaneous lupus erythematosus (SCLE) is associated with use of the following classes of medications: anti-hypertensives, anti-cholesterolemia, anti-psychotics, and anti-inflammatory drugs. Docetaxel is an anti-neoplastic agent, which is widely used for treatment of non-small cell lung cancer. Few cases of docetaxel-induced SCLE have been reported in the medical literature. Here, we report the case of a 58-year-old female patient who developed drug-induced SCLE after administration of docetaxel. After 4 cycles of chemotherapy with docetaxel and cisplatin, erythematous skin eruptions developed on the patient's face. Skin biopsies of the eruptions were remarkable for interfacing dermatitis with basement membrane thickening. Immunofluorescent study revealed characteristic features of SCLE, including granular deposition of IgM, C3, and apoptotic bodies along the basement membrane. The skin eruptions resolved gradually after cessation of drug and with the use of topical corticosteroids.
